Screen slipped during installation and cut employee
Worker using cutting torches to build a guard had a flashback, burning his glove and hand. Hand had blisters, and medications were prescribed.
Worker slipped and fell walking in the plant following an ice storm, landing on his shoulder and elbow. Work continued, but subsequent discomfort and pain led to an MRI that revealed damage requiring surgery. Orthroscopic surgery was performed on June 16, 2008.
Worker shoveling felt upper back discomfort. Went to medical facility and was told it was a simple strain of upper back. Subsequently felt more discomfort and was given prescription, causing a recordable.
