Employee was operating a small front-end loader. Employee said while operating the unit, EE received a Perianal Abscess.

Employee prying a cylinder pin and the pry bar slipped and struck their wrist, hand and forearm.

While doing maintenance on the undercutter, EE and another miner were driving a cylinder pin out and the hammer EE was holding while another miner was hitting it with another hammer, and their arm was forced down onto the cylinder, cutting their forearm requiring 3 stitches

While driving a kubota utv, some salt dust fell in eyes and when they went to wipe the dust from their eyes, they swerved to the right and drove into a chunk of salt and rolled the utv onto it's side. When they rolled the UTV, their mine belt had a harness ring on the back and that's what they landed on, fracturing their L2 vertebrae.

Employee was picking up a chunk of salt to place on a conveyor. Chunk of salt fell on left foot, causing bruise.

While roof bolting, the chair in the roof bolter collapsed. Employee reached to stop the movement of the chair and fractured finger on left hand.

The employee was using a utility knife to cut a plastic pail. While cutting the pail the employee slipped with the knife and cut their left forearm resulting in getting stitches.

Employee was hand scaling the mine roof when EE stepped into a hole in the mine floor. Employee sprained right knee.

Maintenance pickup truck caught on fire. No one was underground at the time the fire was discovered. The Company was in the middle of a maintenance shutdown. All vehicles had not been used underground for two weeks prior to the fire. No one had been underground except the personnel working on the Production Shaft rope change for two weeks prior.

Employee was installing frame of air door from a man basket attached to a telehandler. The basket came off of the forks of the telehandler. When to employee jumped down, EE bruised EE's left heel and left hip.

Employee was stacking 35-pound calcium bucket bags on a pallet. When the employee twisted around to stack the bag, they strained their lower bag.

Employee was troubleshooting a non-working light switch in a tight area. Employee stood up and hit head on an overhead beam. Employee started to fall and caught self, and doing so, twisted lower back causing a back strain. Employee missed three days of work.

Employee was lifting an under-cutter cable to make a repair on the cable. When EE was lifting the cable, EE felt EE's right shoulder pop. Possible rotator cuff injury. Employee started losing time on 09/19/2023.

Employee was using a down comer pipe on a piece of equipment when it slipped. Employee tried to catch it and fractured right thumb.

Employee was lining up a jack pad on undercutter #2. The steel slipped when attempting to line up the jack pad and pinched EE's middle-left finger between steel frame and pad.

Employee was stepping out of a Company Truck. When the employee placed weight on left leg, left knee started hurting badly.

Employee was stepping on a ladder to place a hooking strap to remove stored mafia blocks, the ladder slipped causing the employee to lose footing. Employee fell forward striking the mafia block, causing an abrasion on left elbow and also a sore midsection.

The auxiliary cage in the service shaft was removed and a maintenance inspection took place on the cage. Several items were repaired. Being that this unit is the 2nd conveyance coming into and out of the mine, all personnel were removed from the mine. The cage was prepared and placed back in service on Friday, April 14, 2023.

Employee was changing the ride control actuator on a frontend loader. The actuator slipped and slid, striking the employee's right hand. EE's middle finger on EE's right hand received a fracture. Placed on light duty for 14 days.

Employee was spreading forks on a forklift and strained back.

Employee was stepping out of the cab of a machine scaler. Employee slipped and hit shoulder and back on the tracks of the unit and then the mine floor. Employee did not start losing time from work until Monday, April 3, 2023.

Employee was hand scaling with a hand bar in a lift basket. The hand bar slipped and caused the employee to pinch the right little finger between the bar and the basket railing. Laceration required stitches to close wound. Placed on light duty for 10 days.

While striking a wrench to break loose a bolt on the final drive assembly of a LHD, the bolt broke free and pinched EE's left index finger between the wrench and another bolt, causing a laceration. Five (5) sutures were required to close the wound.

Employee was grinding a weld on a plate of steel. The grinder caught the plate and bounced up and cut employee's face under their face shield. Small cut on face required stitches.

Employee was trying to empty a wheel barrel of salt into a bucket of a frontend loader. Operator of frontend loader rolled the bucket of loader to be flat on the ground. Injured employee did not realize that their foot would be under the bucket. Bucket pinched foot, causing a contusion. Employee started losing time on 10/11/2022.

Employee was removing a fuel line. EE cut wrist on a holding clamp. Employee receives sutures for repair of injury. No lost time from work.

The Production Hoist motor would not operate. The motor had to be replaced for the hoist. The hoist became operational at 7:30 a.m. on March 1, 2022. The Production Shaft is the second way out of the mine.

Employee was attempting to plug coupling of undercutter trailing cable into a circuit center. While walking over to plug the cable in, employee stepped on the trailing cable and twisted EE's right ankle/knee and fell to the ground.

Employee bumped left knee when entering personnel carrier. Knee began to swell. Employee went to doctor on 2/17/2022. Employee began losing time from work on 2/17/2022.

Employee was turning to sit down on the wire truck. When EE turned, EE twisted knee and dislocated knee cap. Lost Time Injury.

Employee was exiting a Company Polaris Ranger. When exiting, employee twisted EE's back, causing a strain/sprain to lower back. Employee began losing time from work on 1/4/2022.

Employee was traveling to a work area in a Polaris ranger with their foot outside of the cab. Employee's foot hit the rib causing a broken ankle.

Employee was cutting cable ties with a utility knife. Knife slipped and cut and stabbed left hand. Employee received sutures for laceration.

Employee was filling oil in KB-09. Employee fell off of the maintenance truck while getting more oil, striking knees and left arm. Employee began losing time from work on July 2, 2021.

An employee was removing the spreader bar for the cables of the south skip of the Production shaft. The spreader bar slipped and struck another employee in the rib cage. Injured employee received a fracture of the left rib below the breast.

Employee was sliding a grating into a location. When sliding the grating, the grating slipped and pinched fingers on both hands. Lost tip of left little finger and had lacerations on the others.

LHD Unit Number 15 caught on fire. The mine was evacuated and no injuries were reported. Unit was left to burn out. Mine Rescue Teams entered the mine and made sure fire was out on the LHD. After verification that fire was out, the Teams explored the rest of the Mine, with authorization from MSHA.

Employee was climbing ladder to the second deck of the surge bin to grease the feeders. When climbing, employee felt pain in right arm bicep.

Employee was picking up hoses to clean the floor. While picking up the hoses strained their back.

Employee was tightening down bolts with a 4" ratchet on a new conveyor drive unit. Employee felt a strain in employee's back.

The brake on the service hoist became out of adjustment when the hoisting engineer was doing the hoisting checks. The brake was adjusted after trouble shooting and placed back in service.

Employee was loading 20 foot cable bolts onto a truck. When turning, employee felt something pop in lower back.

Employee was replacing light bulbs. Employee was inserting old bulb into cardboard casing, causing bulb to break. A piece of shattered bulb went into right thumb. Employee needed stitches.

Undercutter cable was being pulled to new circuit center. Cable tightened and took employee's feet out from under them, causing them to fall on their face.

Miner was dragging concrete out from the chute of a concrete mixer and got in too far. Got EE's fingers caught in auger.

Lifting steel parts and strain back

JLG Forklift caught on fire. Mine was evacuated. Mine air was monitored until mine rescue teams were able to go into the mine and check on fire. Fire was finished and unit destroyed by fire when mine rescue teams got to fire location. Mine was checked for safety and determined safe for workers to return.

Employee was changing bits on an under cutter. EE's hand slipped down onto bit shard while changing bits. Employee had a laceration on left hand. Laceration required stitches.

Employee had a hearing test. Test indicated that employee had a recordable shift in hearing in left ear. Test has to be reported to MSHA.

Employee was cutting open a box with a folding box cutter. When EE closed the knife, it snapped back and cut EE's left thumb. Incision required sutures. No lost time.

An output board on the PLC for the Production Hoist went bad causing the hoist to shutdown. The output board was replaced and the hoist became operational again.

Employee was cutting part of a conveyor belt with a razor knife. Knife slipped and cut left index finger. Stiches were required to repair wound.

Removing shock off of MV-22 Maintenance Truck, employee pinched finger. Finger tip was fractured and stiches were required to replace nail

New drive unit was installed on production hoist. Glitches were discovered in the new system. Engineers had to work through the problems on the hoist drive system and remove the glitches. Hoist became operational on Wednesday, May 23, 2018 at 2:30 p.m.

Employee was hand scaling mine roof at face. When hand scaling, a small chunk of salt fell and struck employee in mouth. Injury caused a loss of a tooth.

Employee was repairing overhead door. When working on the door, the spring shaft came apart and the bracket struck the employee's left forearm. Stitches were required and employee was placed on light duty.

Employees in the Service Shaft were cutting out an old 6" diameter air line when a piece of hot slag started a small methane fire in the shaft approximately at the 689.5 level. Methane was coming out of a joint in the concrete in the shaft. When the employees discovered the fire, they went to the level of the ignition and extinguished the fire with a fire extinguisher.

When driving Maintenance Vehicle back from mining section, vehicle hit a chunk of salt in roadway. Steering wheel turned fast and caused strain on right shoulder and arm. Arm kept bothering employee. Went to doctor, and doctor wants to do surgery. Started losing time from work on 11/13/2017.

Employee was walking and tripped over a cable bolt that was left on the ground. Employee fell to ground and fractured employee's right collar bone.

Employee was wiring up starting in frontend Loader. When performing task, ee felt pain in right arm above elbow. Employee started losing time from work on September 27, 2016.

The Production Hoist kicked out. Received a false signal that the skip dumped into the 200 ton bin on surface. Lowered loaded skip to the bottom of the shaft. Skip went into over speed and over travel and went into crash beams at bottom of shaft.

While employee was running the roof bolter located in 6 North, employee grabbed the wrong lever while bolting. This bent the bolt that wasn't completely installed correctly and at that time the bent bolt struck the employee on the left side of the head temple area.

Employee was departing Front-End Loader on South side of Retsof Salt Storage Building. He felt dizzy when traveling down ladder of loader and fell approximately 6 feet. When striking the ground, employee fractured left arm.

Employee was changing tires for a Polaris Ranger. He was swinging a hammer to break bead on tire. He felt a pain in right arm above elbow.

Employee was exiting a forklift when his left knee buckled.

Employee was walking to an under cutter to perform maintenance work. Employee tripped over a chunk of salt and fell, spraining right knee. Employee lost time from work.

Employee was driving LHD Loader from 3 Yard East to 4 Yard North when he was jarred in the loader. Employee injured neck. Lost time.

Employee was hand scaling ribs. When employee pried down a scale, the chunk of salt broke and split when it hit the floor. The broken chunk of salt rolled and hit the top of the employee's right foot. Salt chunk caused a fracture.

Employee was repairing loader lost footing when getting off the loader a fell approximately 4 feet to the ground. Right knee, right elbow, lower back, left shoulder and left calf were body parts injured.

A roof fall took place on the east side of 3 Yard North conveyor belt between 18 Left and 19 Left roadway. The fall hit the conveyor and took down three channels. Part of the fall rolled and blocked the roadway on west side of the conveyor belt. The area was cleaned up and repaired and the roadway re-opened.

Employee was hand scaling. He slipped and fell backwards. Put his arm out to catch himself and strained right shoulder. Did not lose any time from work until April 15, 2014.

The northeast was engulfed in a winter blizzard. All personnel were leaving the mine early. There was a power outage causing the service hoist to kick out and the high speed breaker on the hoist not to function. It took one and one half hours to get the high speed breaker to function again causing the hoist to be down that long. The service shaft is one of the two escapeways.

Employee was hand scaling. The company assumes that when the loose was barred down, it struck an electric cable hanging underneath it. Striking the cable caused the loose to spin and struck the employee.

Employee was holding a punch while his partner was striking it with a hammer. The punch shifted on impact, hurting his right wrist. Employee began missing time from work on March 07, 2014.

The main bearing on the east side of production hoist began to fail. The hoist was shutdown, and the bearing replaced. Hoist was placed back in operation at 2:00 p.m. on 01/08/2014.

There was an electrical storm in the area. The Plant had a power surge and the DC high speed breaker kicked out on the Service Hoist. The time that the sequence was completed for re-setting the breaker was one hour and twenty minutes. The Hoist became operational at 8:20 a.m.

While riding in a maintenance vehicle, the unit hit a uneven undercut on the mine floor jarring the pick-up truck. The jarring action caused employee to have a pain in the lower back.

Employee was operating water wagon and tractor employee slipped off of the seat of tractor and landed on his back.

Employee stepped off of deck of tractor he was operating and slipped, twisting his left foot and ankle, causing sprain/strain. Employee did not start missing time from work until April 2, 2012.

The contactor on Number 2 Drive of the Production Hoist went bad at 1:00 p.m. It had to be replaced. This caused the Production Hoist not to operate. It was replaced by 3:00 p.m. The Production Hoist and Shaft is the second escape way out of the mine. The mine had to be evacuated until the hoist was repaired.

When employee was running the under cutter the bug duster shear pin broke. When employee was clean around the bug duster the auger fell out and struck the employee on the left hand.

Employee was hand scaling when scaling his sixth room he felt a sharp pain in his right arm. At this point he contacted the Supervisor. After contacting the Supervisor he was transported to the surface and taken to his Doctor.

The main drive contactors on the General Electric 1,500 Horse Power electric motor for the Production Hoist went bad. The contactors were replaced and the hoist became operational again. The Production Hoist and Shaft is the second escapeway out of the mine.

Miner was operating loader Underground on 11/29/2012 he told the supervisor he wanted to go home two hours into his shift when asked if he was sick or hurt he replied "No". The next day the employee (Miner) returned to work with a Doctors note saying he had a Strain, Neck Spinal Cord injury.

Employee attempting to straighten a steal pin cut the palm of his hand. The end result was two stitches on left palm. The employees stitches will be removed in ten days and until than he will be working on little duty.

Employee was oiling chains on booms of Fletcher Drill Number 7. While stepping down off of drill employee tripped on bed hose causing him to fall against frame of drill, striking left shoulder and elbow. Possible Rotator Cuff tear in shoulder.

Employee was helping install new overhead air door behind the Production Office. While working he hit his right wrist on the track of the door, causing a laceration. Laceration required three staples to close. Was given light duty.

Employee was putting ANFO into the ANFO Pot on explosives rig. He picked up a bag of ANFO (50 Lbs.) and when he did so felt something pull on his lower right abdomen side. Doctor believes that it is a hernia.

Employee was dismounting LHD Number 11 when he slipped because of a dusty hand rail and steps. Employee struck head and back when he slipped. Employee refused medical treatment at first. After, he decided to go to the doctor because of the bruises. Doctor placed employee off work on June 11, 2012.

Employee working on an overhead door underground, when the overhead door spring came loose the cable struck the employee in the hand and bicep. This employee was treated for stitches and released the same day.

Underground Cutter Operator was changing out the cutter bits when picking up the old bits off of the floor he twisted his back.

There was a air valve problem with the right brake on the auxiliary hoist. The valves were removed and cleaned and the hoist was put back into service.

While lifting a wooden pallet, employee caught his right wrist in the pallet. When pallet fell, it twisted his right wrist, causing a sprain.

THE PRODUCTION HOIST WENT DOWN ON 12/14/2011. A MINE EVACUATION WAS STARTED AT 12:09 P.M. BECAUSE THE PRODUCTION SHAFT IS THE SECOND WAY OUT OF THE MINE. IT WAS DISCOVERED THAT THERE WAS A BLOWN FUSE ON THE NUMBER 0NE DRIVE FOR THE HOIST. THE FUSE WAS REPLACED AND THE HOIST BECAME OPERATIONAL AGAIN AT 12:31 P.M.

A 1000 volt fuse blew on the Production Hoist. The Production shaft is the 2nd way out of the mine. Hoist was not operational until 4:35 pm. so the miners had to be evacuated out of the mine until the hoist was running again.

Employee was moving a crusher segment into place. A weld on the segment broke, causing the segment to move and in struck the employee in the right leg.

Employee was operating John Deere Gator. Gator was overheating. Employee was troubleshooting the problem. Put his hand too close to the fan blade while it was running. Blade struck tip of middle finger, right hand, causing bad laceration. Laceration required 25 stitches to close.

The production hoist became non operational. The production hoist and production shaft are the second escape way out of the mine. It was discovered that there was a bad ribbon cable and bad processing board on drive number 1 for the hoist. They were replaced and hoist became operational again.

There was a fault in the number 2 drive for the production hoist. Replaced circuit board and hoist began to operate again. The production shaft is the second escapeway out of the mine.

The master control board on drive #1 of the production hoist became damaged. The problem was trouble shooted and the control board was replaced. The production hoist is the second escape way out of the mine. After the master control board was replaced, the hoist became operational again.

Alleged. Employee states that he bent over to pick up a piece of steel and hurt his back. He had been heard to state by several employees that he was not going to work on Thursday and Friday, July 21, 2011 and July 22, 2011 because it was too hot. Employee was given light duty for these two days.

Production hoist kicked out. The production hoist and Production Shaft are the second escapeway from the mine. Mine was evacuated. A limit switch on the south side of the Production Shaft shorted out. It was replaced and the hoist became operational again.

Employee was untangling trailing cable of Fletcher Drill. His hand slipped and struck back of the Drill. Small piece of steel entered hand and had to be removed by a Doctor. No Lost Time.

Main Fuse blew on the Production Hoist. Production Hoist and Production Shaft are the second escape way out of the mine. Mine had to be evacuated. Replaced fuse and Production Hoist was put back into operation.

Employee was hand scaling in Main Heading in 2 Yard North, a scale came down the scale bar and struck employee in left hand, causing a fracture to left ring finger. No lost time from work.

Employee was installing bits onto an undercutter bar. When he was striking the bits with a hammer to place them into their holders on the bar, he smashed his right index finger into a cutter bit. He fractured his right index finger.

Employee tripped over bolts that were mounted in the ground that held the tugger for the skip replacement job. Employee wrenched his neck and shoulder.

Employee was hand scaling loose material in the mine roof when he felt a pop in his right shoulder and the shoulder started to ache. Employee did not start losing time from work until Tuesday, November 16, 2010.

The main transformer that operates the Production Hoist blew. The Production Hoist and Shaft are the second escapeway from the mine. The mine was evacuated. A new transformer was installed and the Production Hoist was placed back in operation.

A fuse blew on the drive for the number one motor on the Production Hoist. The Production Shaft is the second escapeway from the mine. The mine was evacuated. The fuse was replaced and the hoist was placed back in operation, and employees returned to work.

Blocking up forklift to change right front tire, when operator lowered boom. It pinched employees left index finger between boom and block. Index finger was fractured, employee lost time.

The B80 Bio Diesel fuel transfer line to the underground mine ruptured in the production shaft. This resulted in a fuel spill of 1500 gallons of fuel and forcing an evacuation of the mine due to slippage of the production friction hoist.

Employee was standing on the bed of the drill repairing a feed chain when he lost his balance and fell onto the axle of the front tire, injuring his left rib cage area.

Employee lifted his drill cable up and while lifting felt pain in his back. Employee started to miss work on Monday, May 3, 2010.

Employee was servicing Roof Scaler Number 4 in 2 Yard Sout Service Area. He stepped off of platform and fell on his side, bruising his rib cage. Employee lost two (2) days of work.

Employee was in the process of pulling out the belly pan from underneath a LHD. Had a rope tied to the belly pan to pull out from underneath machine, in doing so rope slipped off of belly pan and Employee slipped and fell onto his right hip, injuring it. Employee began to miss time from work on May 10, 2010.

Fuse blew on drive of Number 1 motor of Production Hoist. Had to replace fuse. Hoist was down for 1 hour and 15 minutes so mine had to be evacuated because Production Hoist is second escapeway.

Employee entered bulk storage tank at Retsof Bagging Facility to dislodge a blockage of salt in the bin bottom. Employee hooked up to fall protection but not a life line. When salt dislodged, employee was engulfed with salt up to his arm pits. Employee was removed from the salt safely. Employee was not injured.

While walking in the Main Heading while running a shot line, employee tripped on an undercut and fell on to his knee.

Employee was shoveling salt, felt a slight strain in his left shoulder. Did not start hurting until the next day. Did not go to the doctor until four days later.

Fuse on Number 1 hoist motor for the Production Hoist blew. Fuse had to be replaced. Had to evacuate the mine while the fuse was being replaced, because the Production Shaft is the secondary escapeway for the Mine.

While employee was lifting 50 Lb. bags of ANFO and filling ANFO pot on explosives trailer, felt pain in lower back.

Employee was helping lift some steel into a pick up truck and felt a pain in his back.

Employee dropped tool box cover on right thumb, fracturing thumb.

Employee slipped on top of railcars and fell between cars. Fall limiter fall protection stopped him from falling to the ground. Employee banged his ankle and shoulder.

Employee was roof bolting mine roof for conveyor advance. Drill steel got caught in mine roof. Employee reached for hammer with right hand to use to knock steel loose. Employee's left hand was on drill chuck. Drill steel fell out of mine roof and struck employee's left ring finger by nail joint, crushing finger.

Employee was lifting steel and strained back. Employee did not lose time from work until 6/03/2009.

Employee was walking in front of tractor in 3 Yard East. He slipped on an undercut and fell to the ground, striking his back on his self rescuer. Employee was given light duty.

Leaving work, employee slipped on black ice in parking lot. Fell to the ground and fractured two ribs.

Main Fuse on Number 1 Drive for the Production Hoist blew out and had to be replaced. Production Shaft is second escapeway and personnel had to be evacuated from the mine until fuse was replaced.

Employee slipped on a pile of salt while walking and strained back.

The Production Hoist kicked out and would not restart. It was determined that the lower over travel switch in the shaft had shorted out because of water. The switch was replaced and the hoist started again. The mine had to be evacuated when the hoist went down, because the Production Shaft is the second escapeway out of the mine. The hoist started again at 5:45 p.m.

Employee was wiping off of a piece of steel for fabrication with his bare hand, cut right hand on a piece of welding slag. Cut required sutures.

Employee was driving John Deere Gator, went through strip door too fast. Strip of door caught front of gator rolling over gator. Employee bruised hip & back, causing lost time.

Lifting ANFO Bags on to Powder Rig Trailer, employee felt a pain in his back.

Production hoist tripped out at 6:20 p.m. The cause of the hoist trip was a Drive 2 Fault. A fuse was replaced and the hoist started again at 7:35 p.m.

Production Hoist went down because of Drive 1 Fault, loose connection. Loose connection was fixed and the Hoist began to operate again.

Employee states that he pushed on a door and this caused him to sprain wrist. Wrist did not swell up or hurt until he woke up from sleep the following day.

When striking a keeper pin on an undercutter chain, a piece of metal flew off of the pin and struck employee in right thigh. Needed sutures to close wound.

Employee struck himself in the face with the end of a scale bar, when he was hand scaling.

Employee was putting a jack stand under front end loader. Heard his shoulder pop and felt a lot of pain in shoulder.

The overtravel limit switch went bad in the shaft for the Production Hoist. The hoist had to be stopped for one hour while the switch was replaced. The Production Shaft is the second escapeway out of the mine so the mine had to be evacuated. The switch was replaced within one hour and the personnel returned underground.

Employee was hauling trailer with tractor. Draw arm from trailer came off of the hitch of the tractor. Draw arm went forward striking employee's right leg behind knee causing leg to hit instrument panel of tractor. Injury was contusion with sutures required.

Employee was operating Fletcher Roof Bolter. A bearing plate for a bolt slid and struck his in left arm causing a laceration. He needed sutures.

Employee stepped onto running board of pickup truck and slipped. Twisted his right knee, causing a sprain. Employee missed three (3) days of work.

Employee was cutting conveyor belting material with a razor knife when he accidently cut his right index finger. Cut required sutures.

Employee pinched finger between trailer hitch and the hitch of a pick up truck, cutting finger. Employee needed one suture to close wound.

Employee slipped off of catwalk of scaling machine straining back. Employee was given light duty.

Employee was removing a broken drill steel from the Fletcher Drill. He received a steel sliver in his right index finger. The finger became infected and he had to go to the doctor on 01/17/2008 for treatment.

The powdermen were loading 22 Right Cross Cut in 2 Yard South when the compressor running the ANFO Pot caught on fire. The fire engulfed the truck, compressor, and ANFO Trailer. The mine was evacuated. The exhaust was monitored until the air reached safe limits and then the mine rescue team went into the mine and examined the fire scene to determine that the area was safe.

Employee was using hand drill. Drill caught in wood and snapped back, causing a sprained right wrist. Employee was placed on light duty until sprain heals.

Employee was prying with pry bar on head shaft of crusher feeder, the bar slipped causing the employee to fall backwards. Employee struck middle of his back against a cross member of crusher.

Employee stepped on rock and twisted right ankle while putting engine oil in locomotive.

Employee walked between a forklift and 3 1/2 yard loader. Forklift rolled back a little bit and pinched employee's left thight between rear of forklift and rear of loader, causing a contusion.

While employee was hand scaling the roof, a scale struck the scale bar, causing the scale bar to strike the employee's right knee. Laceration of knee needed sutures.

Closing gate on a railcar, the bar slipped and employee fell to the ground (crushed stone). Injured right shoulder and right elbow.

The Production Hoist stopped operating. It could not be re-started. It was discovered that an overtravel switch at the bottom of the Production Shaft was damaged. The switch was replaced and the hoist began to operate again.

Production Hoist stopped operating. Discovered that there was a blown fuse in the drive unit. Fuse was replaced and Production Hoist began to operate again.

Employee was lifting copier paper box. When she lifted the box she felt a sharp pain in her lower back.

Employee tripped and fell while moving undercutter cable out of the roadway, tripped over cable.

The west brake potentiometer failed on the Production Hoist. The Production hoist is the second escapeway out of the mine. The potentiometer was replaced.

Employee was walking from Shop to YPS Building, felt a spasm in his back. Lost three days of work.

The gear box that contols the resolver for the HSS (Hoist Safety Supervisor) and the computer programmer on the Service Hoist started to make noises. It was felt that a bearing was going bad in the gear box. The hoist was shut down and the gear box was replaced.

There was a problem with the Production Hoist so it would not operate. Discovered a blown fuse on main number 1 that was replaced. Changed the mother board in drive number 1 and repaired an air leak on drive number one cabinet. Hoist became opertional again at 3:00 p.m. on 4/17/2007.

Employee stepped out of the cab of the ANFO Truck. Not knowing there was a drop off, he lost his footing and fell forward. EE said his back started hurting later in the shift.

Employee was standing on top of Pepco Chute Car helping to move it. When car was moved, employee pinched his left ankle between a steel I-Beam and a steel channel.

Employee was servicing Number 1 Drill. When he stepped off of the drill he slipped, fell to ground and hurt his left knee. Was given three days of light duty.

Employee was installing left side boom swing cylinder on Undercutter 5.Cylinder dropped down on his left middle finger crushing the end of his finger.

He was a passenger in MV-12 (Ford Pick-up Truck), operator lost control and vehicle hit rib.

The employee was loading a crane on a truck. While putting chains and binders on the truck, the employee must have twisted wrong and pulled something in his back. The Doctor states that it is a lumbar strain.

Employee has chronic bad right shoulder (surgery in past). Employee was putting flag back into proper position on emergency stop cord. When flag was placed back, the emergency cord snapped tight, pulling on employee's shoulder. Motion pulled shoulder out of socket, dislocating shoulder. Employee went to hospital to repair dislocation.

The employee was scaling the ribs with a hand scaling bar when a section of scale fell hitting the scaling bar. This caused the bar to bend his right wrist back. A bone was fractured in his wrist. Employee was placed on light duty until wrist heals.

Employee was placing roof bolts on to his truck. He felt as though he pulled a muscle in his stomach. X-rays showed a hernia. He had surgery on 8/23/06 for the repair. He will be off work for approximately 30 days.

Employee stated he was loading face with explosives when he got dizzy and felt sick. It is felt that he received elevated exposure of CO2 and low oxygen when he took out explosives from the magazine at the start of the shift.

The employee stepped off of a cut in the floor at the face of the mine (approximately 6"), slipped and sprained his back.

Injured employee tossed an empty Gatorade bottle at General Mine Foreman to get his attention. Bottle struck and bounced off of Foreman's leg and landed at the feet of another employee. The employee picked up the bottle and tossed it back at first employee, striking him in the mouth. Broke off front eye tooth at gumline. Employee will need Dental attention.

Employee was working on undercutter Number One. He stated that during the shift he felt a pain in his knee. The employee did not go to the doctor until three days later. Doctor recommended that he be placed on light duty.

The employee was moving the trailing cable on a 15RU Joy Undercutter. During the procedure, the employee slipped. He caught himself and doing so he hurt his neck.

Employee was drilling in 3 Yard East, 9LXC, with Fletcher #3 twin boom face drill. After drilling a test hole in the roof, the employee went up to check the depth of the drill hole. He slipped and caught hold of the drill steel to stop his fall. He felt a sharp pain in his left shoulder.

The employee was lifting a 50 Horsepower electric motor out of the dust collector in the Process Gallery. While lifting he felt a strain in his right shoulder. No lost time from job but given light duty.

Employee step out of cockpit of LHD. When he dismounted he stepped on a chunk of salt causing him to twist his left knee. Employee felt slight pain in knee but went back to work. Employee came to work on 3/20/06 with Doctor's excuse putting him off work and stating that he needed knee surgery.

Employee was lifting steel covers on 500 KVA power center. Pulled muscles in lower abdomen. Employee was refered to a specialist after diagnosis by personal physician of a hernia. Employee went for surgical repair of hernia on 3/22/06.

Employee was removing the hydraulic pump from underneath the fuel truck when it slipped out of his hands and struck him in the face, cutting his face between his nose and upper lip. He received four stiches to close the cut.

A roof fall took place on the west escape roadway going into 1 Yard North, apprximately 1,000 feet east and 200 feet north of the Service Shaft. The roadway was partically covered. The fall was approximately 50 feet long, 30 feet wide, and three feet thick. No one was in the area when the fall occured. The area has been bolted and meshed and is now safe for travel.

Employee was directing another employee to dump a bucket of salt from the LHD into the stamler crusher-feeder. When the bucket was dumped, a chunk of salt rolled down the pile next to the stamler towards the employee. The employee tried to get out of the way but slipped. The chunk of salt struck the employee in the right ankle, breaking ankle..

EE was helping install channels for an extension onto a conveyor with a forklift. One channel slipped off of the forks of the forklift and pinched three fingers of EE's right hand, causing fractures. EE has been given light/restricted duty.

Main Production hoist kicked out and would not restart. Electrical system on hoist was checked and discovered that 1100 amp, 1000 volt fuse had blown. A replacement fuse was installed after delivered to mine site at 10:00 p.m. Mine was evacuated because hoist is secondary escapeway.

Employee was cutting a zip tie with a knife. When he cut through the tie the knife stabbed into his left hand between left thumb and index finger. Employee needed sutures to close the wound.

Employee was helping another employee sand blast a head shaft for a stamler flight chain. The shaft was sitting on a cart and the employee was rolling the shaft for the sand blasting. The cart flipped up causing the employee to pinch his right middle and ring fingers between the shaft and the ground. Employee fractured the tips of both fingers.

Employee was operating Elphinstone LHD #5 when the LHD went over a jump in the floor causing the operator to strike his head on the roof of the canopy causing an injury to the neck. Employee was given light duty.

Employee received electrical burns when checking voltage on 995 vac receptacle. Burns were on both hands. Second degree burns. Received light duty.

Employee was taking off transmission cover off of LHD #4, the cover slipped out of the employee's hand and struck him in the right shoulder. Employee was given light duty.
