Employee was doing maintenance in a primary crusher hopper. EE was installing new bolts in the structure under the hopper when EE stepped in a hole and twisted right knee.

While lifting a railroad tie to place in the rear of a maintenance truck, employee felt a shooting pain in right bicep with tingling in fingers.

Employee fell down steps trying to get feeder shut off to stop overloading conveyor belt.

Serious injury occurred to worker inspecting Dust Collector at the National Lime & Stone Plant. The worker was accessing the catwalk to inspect Dust Collector to see if there was any material in it before EE attempted to lift with the Crane. The worker fell about 20 feet to the level below through a catwalk.

Employee was dismounting the front end loader when EE slipped and fell injuring right shoulder and hip.

The injured employee and another employee were carrying a piece of wooden blocking when the injured employee tripped, causing him to fall. His fingers were pinched between the block and the floor.

Employee caught toe on metal plate laying on the floor as he turned to walk and felt pain in his right knee. Case was not reportable until Dec.20 when employee had surgery.

A refuse bank failure occurred at this site causing a front-end loader and articulated haul truck to be forced over a high wall berm and to fall to benches below the level they were working at. No one was injured. National has yet to complete its ongoing investigation & evaluation of the conditions contributing to incident

On February 2, 2012, contractor R. D. Jones Excavating Inc. reported to National Lime and stone that material that was being excavated had slid out from the toe of the excavation and engulfed the tracks of the excavator.

Employee was placing a guard on the back of a flatbed truck when he pinched his finger between the guard and truck, amputating the tip of his left long finger.

Employee was moving conveyor idlers and pinched his finger between two idlers. Note: This injury is reportable due to blood being drained from under his finger nail.

Employee was manually rolling the crusher reel over to check it. Pinched his finger between the reel and the crusher wall, lacertaing his finger. Wound required stitches to close.

Was holding a bolt with a wrench while another tightened it. The other employee's wrench slipped off and struck him in the mouth, chipping a tooth.

Employee was trimming a piece of skirt rubber when the knife slipped, cutting his left hand. Thw wound required 4 stitches to close.

Employee reached into a container of welding rods and cut his finger on the rim of the container. Employee received stitches in finger.

EMPLOYEE WAS LIFTING A BEARING OFF THE SCREEN SHAFT USING A CRANE. THE BEARING SWUNG UNEXPECTEDLY, PINCHING HIS RIGHT PINKY FINGER BETWEEN THE BEARING AND THE ADJACENT SHAFT.

May have come into contact with poison ivy while working at the plant. outcome of litigation determined that this case should have been reported.

WAS RE SPLICING A CONVEYOR BELT WHEN HE WENT TO REPOSITION HIMSELF, HE PUT HIS KNEE DOWN ON THE UTILITY KNIFE.

WHKLE ASSEMBLING CONVEYER BELT, PINCHED FINGER CONVEYER FRAME AND TROUGHING ROLLER.

EE WAS CLIMBING OU TOF BOBCAT & TWISTED ANKLE

EE STRUCK A CHIPPING HAMMER WITH ANOTHER HAMMER CAUSING A PIECE OF THE CHIPPING HAMMER TO BREAK OFF & LODGE IN LEFT HAND

EE WAS USING A CHIPPER HAMMER TO PUNCH HOLE IN CONVEYOR BELT WHEN A PIECE OF METAL FLEW INTO WRIT PUNCTURING THE SKIN

EE WAS RUNNING WIRES INTO FEEDER DISCONNECT. WHEN WIRE CONTACTED LEAD WIRE. AN ELECTRICK FLASH SET SHIFT ON FIRE BURNING RIGHT ARM & CHESYT

CLEANING DIRT OUT OF HOPPER WITH A HAMMER GUN. SMASHED LEFT RING FINGER BETWEEN HAMMER AND HOPPER. MEDICAL INTERVENTION INCLUDED STITCHES.

EE STATES: I WORKED ALL DAY WEARING BACK BRACE. RAN OUT WIRE & FELT SHARP PAIN IN LOWER BACK WHILE WALKING. COULD NOT STAND UP STRAIGHT. WENT TO HOSPITAL. OUR OFFICE WAS NOT NOTIFIED OF THE IN CIDENT UNTIL WE RECEIVED A COPY OF THE DOCTOR'S BILL WHICH HAD THE DATE OF INJURY.

WHILE CLEANING CONVEYOR, LEANED TOO FAR FORWARD, OVERCORRECTED AND FELL BACKWARD, SPRAINING LEFT ANKLE, BRUISING RIGHT SHIN.

AFTER CUTTING ANFO BAG TO DROP IN BLAST HOLE, PLACED KNIFE IN BACK POCKET WITH BLADE EXPOSED. WHILE WALKING LEFT ARM RUBBED AGAINST BLADE CUTTING LEFT FOREARM REQUIRING STITCHES.

WALING ON SNOW & ICE IN SLOPED AREA FELL JAMMING LEFT ARM INTO SHOULDER

BENT OVER TO PICK UP METAL PLATE TO PUT IN BOBCAT BUCKET. FELT PAIN IN LOWER BACK.

CHECKING FOR LEAK THAT HAD CUASED AN ELECTRICAL MALFUNCTION, FEEDER WAS TURNED ON BY OEPRATOR FRACTURING RING FINGER, RIGHT HAND.

EMPLOYEE WAS LEANING BACK ON A MOTOR BASE AND WHEN HE SAT DOWN THE MOTOR PIVOTED, PINCHING LEFT INDEX FINGER BETWEEN THE MOTOR BASE AND BRACKET.

WHILE SLEDGING THE HANDLE SLIPPED AND PINCHED FINGER AGAINST ANGLE IRON.

UNLOADING CONVEY FRAMWORK OFF TRAILER ON LOADER BUCKET WHEN COULDNT STAND UP STRAIGHT

USING UTILITY K3IFE TO CUT PIECE OF PLASTIC MATERIAL USED FOR BELT SCRAPER.PLASTIC LAYING ON SHOP FLOOR AND INJURED KNEEL DOWN PULLING KNIFE ALONG STRAIGHT EDGE TOWARDS HIM KNIFE SLIPPED AND C UT KNEE. 3

EMPLOYEE WAS CL9ANING A BELT PUNCH WHEN HAND SLIPPED AND PUNCH WENT INTO PALM OF RIGHT HAND.

OPERATOR WAS DI1GING MUCK PILE WITH 988 CAT LOADER.HE WAS WORKING ON A HIGH PILE AT THE CORNER OF THE FACE.WHEN HE DUG OUT THE PILE,PART OF THE PILE FELL ON CORNER OF LOADER BREAKING WINDSHIE SHIELD WHICH CU7 ARM.

INJ WAS BREAKIN1 CONCRETE OUT OF INSIDE OF MIXER DRUM WITH AIR POPERATED HAMMER WHEN A CHUNK FELL AND STRUCK HIM OON FRONT LEFT LEG

INJ WENT FROM L2ADER PLATFORM TO LOADER TIRE TO GAIN ACCESS TO PIN THAT WAS COMING OUT OF LOADER ARM.AS HE RETURNED TO PLATFORM NEAR CAB HIS FOOT SLIPPED AND HE FELL TO THE GROUND INJURING ANK

INJURED WAS HOLDING CONVEYOR WITH BOTH HANDS WHILE IT WAS BEING REMOVED FROM TRAILER WITH GROVE 28 TON CRANE. BOOM OF CRANE CONTACTED ONE OF THE 3 PHASE 4160 VOLT POWER LINES ELECTRICITY WAS T RANSFERED THROUGH THE INJUREDS BODY INTO THE GROUND

INJ UNHOOKING CONVEYOR THAT HAD BEEN BROUGHT FROM LIMA #1 TO LIMA #3 FOR REPAIRS WHEN HE THOUGHT FELLOW WORKER WAS HAVING HEART ATTACK.NOTICED I.NAME SLUMPED OVER CONVEYOR AND RAN OVER TO ASSI ST HIM.HE IMMED GOT SHOCKED BY 4160 VOLTS SINCE THE CRANE BEING USED TO LIFT CONV HAD ENGAGED 1 PHASE OF 4160 V LINE AND WAS SHOCKING I.NAME. ALMOST IMMED UPON BEING SHOCKED,CRANE OPERATOR I.

EMP WAS WALKING FROM SHOP TO OLD MILL CARRYING A PICK, WHEN HE SLIPPED ON THE ICE AND FELL AREA WAS COVERED WITH SNOW EARLIER BUT HAD BEEN SCRAPED OFF THE RECENT THAW CAUSED MELTING WATER WHIC H REFROZE MAKING THE AREA SLIPPERY

EMPLOYEE WAS STEPPING DOWN FROM A TRUCK WHEN HIS FOOT WENT INTO A HOLE IN THE PAVEMENT. TORE OR SPRAINED A LIGAMENT IN THE ANKLE.

EMPLOYEE WAS ATTEMPTING TO CLEAR A ROLLER WITH THE HANDLE PORTION OF A SHOVEL BY TURNING THE IDLER. THE IDLER BEGAN TO ROLL AND ALLOWED THE HANDLE TO MOVE AND PINNED THE EMPLOYEES FINGER UNDER THE SHOVEL AND THE FRAME OF THECONVEYOR.
