Employee was running a communication line from second floor to seventh floor. While performing this task, employee got lime in right eye. First Aid was administered and eye was flushed, then later in the shift employee said eye was still irritated. Taken to the doctor where the eye was examined and flushed again. Employee was prescribed anti-biotic as a precaution.

EMPLOYEE WAS CLEANING UP MATERIAL AND CARRYING A 5 GALLON BUCKET STRAINING THE RIGHT ELBOW.
Employee was cleaning the burn floor of the plant. As employee pulled the air hose to the next area to be cleaned, employee tripped over a 4" vacuum hose that had fallen down into the walkway. Employee fell to the concrete floor striking right shoulder.
EMPLOYEE WAS WALKING TO THE 52 BUCKET ELEVATOR AND LOOKED UP AND AT THAT MOMENT LIME WAS FALLING FROM AN OVERHEAD STRUCTURE. LIME GOT INTO RIGHT EYE. EMPLOYEE SOUGHT MEDICAL CARE AND RECEIVED MEDICATION.

Employee was assisting in opening the tarp on a truck to load by standing on the fuel tank. The tank was covered in ice causing the employee to slip and fall to the ground landing on both feet but spraining the right knee upon landing.
An electrician was injured while trouble shooting a controller bucket rotary disconnect for #1 dust collector fan. An arc flash resulted causing burns to employee. This is reportable because it is a lost time accident.

Employee was changing a bearing on a conveyor tail pulley. During repair employee received a metal sliver in the palm of his left hand. Medical treatment was required to remove the piece. Employee returned to work to complete the remainder of his regular scheduled shift with no work restrictions. This is a reportable accident because there was one suture.

Employee was raking coal ring from #1 Kiln, a large piece of coal ring fell and hit the rake which twisted/jammed left wrist.
Employee was beating on outside of metal coal hopper with sledge hammer due to coal being frozen. Sore arms, shoulders, thought pain would go away, it didn't. Investigation started 3/5/14. Seen by physician 3/6/14. Placed on restricted duty back to work and is currently working on restricted duty, no time lost.
Employee opened kiln door and dust blew into right eye while checking grates. As noted on follow-up form, Employee was wearing his P.P.E. along with face shield.
