Grand River Operation Metal/Non-Metal

Forklift operator from the facility was operating a forklift in excess speed and did not see our employee walking in the same direction of the forklift. The forklift struck the industry service employee.

No conditions contributed to the incident. Incident took place on a concrete walkway leading from the plant's parking area to the main office. Employee states that they tripped and fell. Employee was not carrying any items, or performing any work related activities that may have contributed to the incident.

Unknown at this time. Alternative (Alt) fuel supply ignited within the Alt fuel baghouse, damaging the baghouse internals and feeder belt within the equipment.

Employee was tarping a trailer at Carmeuse-Grand River when they slipped and fell, striking their head on the ground. Employee sustained a small laceration above the right eye, a concussion and vertigo. Employee was transported to health center via ambulance.

Residual grease laying in bottom of gear shroud. Although unknown at time of report, radiant heat from the kiln may have been the source of ignition. Minimal heat damage to a thermal coupler as a result of the incipient fire condition.

EE was removing a piece of foreign material from a head pulley with a crowbar when it came unjammed, EE's left arm came down on an edge of metal cutting wrist. ER Treatment - 6 stitches.

At 11:41 am on 9/27/13 the kiln system lost I/O communication causing an unplanned shutdown. The system was restarted @ 11:57 am. At 12:00 pm an ignition occurred in the primary air fan of the solid fuel handling system causing an explosion. The solid fuel handling system was shut down & inspected. No personal injuries occurred & limited damage to only the primary air fan housing.

Experienced a possible unplanned ignition of coal in the kiln 5 Raymond Bowl Mill Grinding system. No injuries or damages occurred.

Employee was in the stone tunnel when he grabbed a steering roller on the steering idler. His sweatshirt sleeve then became caught between conveyor belt and roll, pulling his hand in. He pulled his hand out of the pinch point. Off-Site Medical-Rx Drugs, Bone Fracture

Employee was placing brick on staging platform. Brick fell off platform striking employee left arm and left leg.

EE suffered an injury when he smashed the tip of this little finger between a hand tool and a concrete ceiling. He was attempting to re-engage the coal mill airlock clutch. While pulling down to engage the clutch, the airlock clutch coupler engaged causing the wheel and tool to spin up and smash the His finger. EE was given Rx Drugs, and Surgury

Employee was tightening, adjusting bolts on frame in an attempt to raise the frame. The frame would not raise so the employee used a hydraulic jack. The hydraulic jack popped out and struck the employee in the left leg.

Cleaning out the coal mill air lock, holding the air lock door with left hand and loosening nuts with right hand. After nuts were off the door slipped out of the left hand and fell open. The door caught his left index finger between the door and the drive motor housing. The employee recieved stitches on his finger

Their were high winds the day of the injury. The employee was working on the floor when a high wind blew lime dust into his eyes and face. The employee had safety glasses on at the time of the injury. The employee had his eye and face flushed immediately. The employee received medical treatment, Rx eye drops, and missed two days of work due to his injury.

EE CLEANING OUT EAST END OF #5 COOLING TOWER SCREW. HE HAD THE BAR IN CHUTE MATERIAL DROPPED DOWN, HIT END OF THE BAR CAUSING THE END OF THE BAR OUTSIDE OF THE CHUTE TO SLIP OUT OF HIS HANDS, STRIKING HIM IN THE RIGHT CHEEK.

WIND BLOWN FOREIGN BODY ENTERED EYE OF EE WHILE WALKING IN THE LIME KILN AREA. PHYSICIAN ADMINISTERED PRESCRIPTION MEDICATION AS PART OF TREATMENT.
