Holding a bolt head while another guy was using an impact gun to loosen the nut. The bolt shifted down enough to come loose and spun in my hand cutting though my leather glove and cutting my hand.

Contractor was using a chain hoist through the eyebolt of 30 hp motor to position the motor into place. A mechanical failure caused the hook of the hoist to swing back into the face of the contractor. He was taken to the emergency room, where he was treated and released that evening. Contractor returned to his normal duties 2 days later.
EE was using a torch to help install gates on the pit feeder. He was holding the torch with left hand when the hoses moved and caused the torch head to swing around forcing the flame over the back of his right hand.

Employeee was tightening bolts on jaw crusher. Wrench slipped off and he smashed finger between wrench and walkway.
Employee was walking away from loader after adding antifreeze and tripped on the garage floor and fell.
