During plant maintenance, EE tripped and fell on open grate.
Plant operator was prying a rock loose with pry bar. Pry bar slipped off rock and pry bar struck plant operator in face. Plant Operator went to doctor and received 2 stitches. Operator returned to work next day.
Victim, an unknown male, trespassed onto property that adjoins to the mine property after hours of operation. Parked car within 50' of public road right of way. Placed hose into car tail pipe and into car cab. Victim died inside of car in back seat. Not an EE, contractor or customer. Unknow victim to Weidle Corp.
Haul truck driver complained his back hurt at end of shift on Thursday 10/11/07. Driver did work Friday as a labor but not on mine property. Sat. driver went to doctor and driver reported to employer on MONDAY that he would be off this week due to back. On Friday 10/19 driver reported he would return Monday 10/22/07. OFF work 5 days.
