Mill Creek Limestone Quarry Metal/Non-Metal

Employee was getting into the back of a truck using the step and felt a pop in their foot.

Crew was in process of removing counterweights from a Grove GMK6350 crane. During this process, the crane tipped over onto its side while attempting to set two counterweights onto a trailer. This resulted in the crane cab falling onto the side of the first trailer adjacent to the crane pinning the employee's legs. Employee's injury details are described in section 7.

Lowering torque converter, it hung on hoses, went to free it, it fell onto the employee's hand causing a fracture

EE was using hand & a metal ruler to remove a broken soap dispenser mounted with adhesive to a glass mirror. While prying the dispenser away from the mirror, the glass broke & cut through the EE's Level 5 cut resistant gloves ee was wearing. This resulted in a laceration to the two middle knuckles. EE received 4 stitches & was released to full duty with no restrictions.

EE was tying down & securing the train. As the EE was ready to dismount, the EE faced the ladder and placed right foot to pivot. The soles on EE's new boots gripped the catwalk not allowing the foot to pivot. Later EE felt soreness in knee. The EE was released to full duty on 10/1/18. On 11/19/18 an MRI determined a torn meniscus and EE was released to restricted duty.

EE's were replacing feeder pads on the apron feeder. They were lining up the bolt holes and bolting in place. While in the process of setting the ninth feeder pad in place, the EE caught the tip of finger in the pinch point resulting in a displaced fracture and laceration. EE received stitches and was released to restricted duty.

Equipment Operator was mounting the first step (21" from flat surface ground) on the CAT 769D Water Truck at the fresh water lake fill. As EE began to mount the step EE felt right knee pop. EE was diagnosed the following day with a knee strain and recommendation of sitting activities at work.

It was a hot humid day with temperatures around 100 degrees. EE was not feeling well and went back to the truck to cool off in the Ac and drink water. EE rested in the truck for awhile but was out of breath. An employee immediately rushed EE to the nearest hospital.

EE was pulling motor wires through conduit; wire became stuck; EE tugged on the wire and it became free. EE's hand slipped and hit the fan shroud, cutting area where hand and wrist meet. This area is always in motion, so 2 sutures were needed to aid in healing. EE was released and returned to work immediately. No medications and no restrictions.

EE was in the process of beginning to refuel the gas powered air compressor when the vapors from the metal safety can ignited. EE suffered 2nd degree burn to the left hand, arm and a small portion to left abdomen.

EE's were conducting routine maintenance tasks on rail car. While changing a brake shoe on the rail car, EE heard air and felt his right ring finger become caught between the brake shoe and the brake beam. The second EE was able to use a pry bar to free the EE's finger. EE suffered an avulsion fracture to the right ring finger and was released to light duty restrictions.

EE was in bottom deck of shaker removing a knock-in strip with a 6lb hammer. With hammer in right hand, he swung to hit the knock-in strip. His (R)hand slipped down the handle causing him to miss his target and strike his (L)thumb that was placed approx 6" above target. This caused a laceration requiring stitches. EE returned to work with restrictions of no gripping with thumb.

EE was in training class when he experienced labored breathing. First Aid Responders suspected an apparent heart attack or other unknown health problems. 911 was notified. EE became unresponsive & CPR was administered. An AED was activated. Following 30CFR50.10, MSHA was notified. EMS arrived & EE had a pulse. EE was transported by ambulance to hospital where he later passed away.

EE was assisting in change out of skid steer bucket. EE was having difficulty getting the bucket locking pins to release. EE was standing in front of skid steer bucket when locking pin finally released. When the bucket came free from the machine it landed on the employees left foot resulting in dislocation and multiple fractures.

Employee was replacing cord for a water pressure transduceron SS7 screen,he had set down so he could reach the LB to take the cover off. When he got up he felt a sharp pain in his lower back. He was taken to Mercy Occupational clinic and diag. with a lower back strain. He was prescribed two weeks of PT. He has been placed on light duty with follow up exams.

Employee Strained right arm/shoulder while cleaning rail cars with a shovel. Employee has been placed on light duty.

EE was adjusting the tension on a 1.5 Ton come-a-long from a gravity take-up system, when the pin that holds the latch broke releasing the chain, allowing the chain to swing back striking EE on the arm. He may possibly have a hairline fracture near his elbow.

On October 29, 2012 employee (Sr. Electrician) was changing out a size three starter bucket. He bent over to pick the starter bucket off from the floor of the MCC room and in the process of raising back up felt a pop/pain in his lower back. He was taken to the doctor and diagnosed with a lower lumbar strain, he returned to work the same day on light duty.

Employee was pulling on a 1.5" wash hose and strained his back.

While removing a urethane screen, using a pry bar, it slipped and struck EE on the left side of his upper lip.

EE was removing flooring out of the primary dust control building on the morning of July 20, 2011. while lifting and pulling on a piece of plywood that was in the floor of the building he strained his back.

On 8-20-2010, employee was working on a vertical turbine water pump to free the clutch. He was using a 24" pipe wrench to break free the back stop. The shaft broke free and spun the pipe wrench and struck his hand. He was taken to the doctor for x-rays to check for fractures. The x-rays were negative for fractures but the small finger was bruised severely.

Working in 100+ temperatures with high humidity - Heat exhaustion.

Employee was assisting others in cleaning out open top gondola rail cars. The outside temperature was approximately 85 degrees and the employee got hot and passed out. He was taken to the hospital by ambulance where he was treated for heat exhaustion and released.

Employee operates a 992G loader which is operated with Joysticks. Repetitive motion of the left arm causing "tendonitis" in the left elbow.

Employee was performing cleanup activities and while cutting brush and tree limbs, came into contact with poison ivy or poison oak. Employee was treated by a Dr the following day with an injection and again 4 days later prescribed medication. Employee was allergic and broke out with a severe rash on his arms neck and torso.

Employee was replacing Troughing rollers on a 48" conveyor. While lifting one of the rollers, employee twisted and strained his back. Employee went to lunch and felt stiff after sitting and then reported the incident to the supervisor. Employee was seen by a Dr. and placed on light duty.

On 2/18/2009 at 3:00pm Equipment operator, was helping to pull electrical cable because the MCC building was being moved to another location. He was pulling on the wire when the slack pulled back the opposite direction. EE had his feet planted in the ground and it pulled his upper body backward causing him to twist dislocating his right knee.

Employee was helping with clean up efforts after a storm came through the area. While in a manlift, removing bent up sheet metal from the roof of a building, he was pulling against a piece of metal and it cut through his glove into his little finger. The cut required 7 stiches. (Surgery 5/5/09-work off Misc Mail 6/2/09 #2)

On Wednesday 3-12-2008 employee was stepping over a twenty inch poly pipe that is located on the west side of the concrete plant. When he stepped over the poly pipe, his right foot slipped causing a discomfort in the groin area. He was taken to the doctor and diagnosis was a right groin strain and received two days light duty.

The employee was fueling equipment near the Quarry. The fuel line became tangled and when the employee pulled on it the hose jerked his left wrist. This resulted in a light spring to the left wrist.

Employee was splicing a conveyor belt. He was climbing on and off the belt and his lower back started getting sore. He notified his supervisor at 4:15 pm on 1/9/2007 and was taken to the Doctor to be checked out.

EE was removeing his work gloves in the secondary breakroom. A piece of sand got in his right eye causing irriation to the right eye.

Employee was at the sand plant working and got sand in his right eye. He reported this on 10-16-2007 and received treatment on 10-16-2007.

On 7-9-07 employee was doing a screen change on the Asphalt shaker. His left foot slipped off the screen and he hit his shin on a beam. He stated he did not need medical attention. On 7-16-07 his leg was swollen and he was taken to the doctor. Diagnosis was cellulitus in his left leg.

Employee was operating a 992G ( LD-8 ) wheel loader in the Quarry loading haul trucks. The employee started feeling pain in his left knee and was taken off the loader and to the Doctor. The employee was placed on light duty and scheduled for a MRI.

Employee was working on the sandplant and got some sand in his right eye. When he got up the next morning his eye was irritated. He was taken to the Doctor and had an infection in his eye. He was given a presription but no restrictions.

EE was changing a gear box out on L-3 conveyor in the load out. When EE woke up the next morning he was sore and stiff in the lower back. He reported this as soon as he got to work and he was taken to the Doctor and diagnosis was lower lumbar strain.

Employee was climbing down the ladder on a 988B wheel loader using three point contact. The employee felt his right shoulder pop and started having pain in it shortly after.

Employee was changing troughing rollers on P-10 in the primary. When the troughing roller turned up, he got his left middle finger pinched. Employee was wearing leather gloves but received a laceration that required seven stiches.

Employee was operating a small 463 bobcat near the fueling station by the quarry. He was dumping a bucket of gravel when his right front tire went up on the berm causing the bobcat to turn over. He received a laceration on the left elbow. He was taken to the Doctor and received six stitches in the left elbow. Their was no restrictions or lost time.

Employee received damage to his middle finger and ring finger on his right hand. He was attempting to open a roll back chute on the shaker in the primary. He received stitches and is on light duty.

On Tuesday, 5/2/06 at 8:30am, EE squatted down to pick up a ramp liner and felt a sharp pain in his back. EE had not lifted anything yet. EE was taken to DR. and the diagnosis was a lumbar stain to his lower back.

The ee was helping to change rollers on a 60 inch conveyor and after he finished with the seventh one, he complained of his lower back hurting. He was taken to the clinic for evaluation.

The ee was changing rollers on a 60 inch conveyor. While trying to maneuver the roller through the frame to drop it to the ground the roller pinched his finger between the rollers and the skirt board causing a laceration on his right index finger. The employee was taken to the ER for treatment.

On 11-15-05 Employee was operating a 992 C pit loader in the pit. He strained his left knee using the D-clutch. The loader was shut down and repaired the next morning. Employee is working on light duty in the control room.

The employee was using a hammer to knock screens into place when the hammer slipped causing the employee to hit left index finger. The doctor's diagnosis was contusion to left index finger and a splint was placed on it for precautionary measures until 10/28/05.

Employee was climbing onto his CAT 777 Haul Truck and strained his back.

Employee was opening an inspection door on a VSI crusher when he strained his tendons in his right shoulder.

THE EMPLOYEE WAS HELPING TO MOVE A ROLL BACK CHUTE AS WE WERE PREPARING TO DO A SCREEN CHANGE AT THE CONCRETE PLANT. MOVING FORWARD THE EMPLOYEE STEPPED OFF OF THE FLOOR INTO A SIDE CHUTE CAUSING A TWISTING MOTION TO HIS RIGHT HIP.

EMPLOYEE WAS STEPPING DOWN OFF A WALKWAY ONTO THE TOP DEC OF THE SHAKER SCREENS. EE TURN HIS LEFT ANKLE RESULTING IN AVULSION TO HIS LEFT FIBILA.

EMPLOYEE WAS CLEANING AROUND THE SECONDARY CRUSHER. A ROCK CAME OUT OF OR OFF OF THE CRUSHER AND STRUCK THE EMPLOYEE IN THE RIGHT EYE. THE EMPLOYEE WAS WEARING THE PROPER PPE. THE EMPLOYEE SUFFERED FROM HYPERLLA IN RIGHT EYE. SAFETY GLASSES PREVENTED SERIOUS INJURY.

Employee lifted then tossed a piece of scrap metal that weighed approximately 50 pounds, on to a trash pile. This act resulted in the employee straining his left buttock muscle.

Excess bending and the lifting of heavy weight manually resulted in a strained lower back.

Employee was driving through the plant in the plant pickup. A sharp pain hit him in the back between the shoulders. Employee reported the incident to the supervisor. Employee was then taken to the Dr. for examination. The Dr. stated that Employee had a muscle spasm and placed him on light duty and physical therapy.

Contributing conditions: Employee was using his personal pocket knife rather than prescribed tool. Damage: Employee received a small cut on his middle finger which required 3 stitches to close the wound.

Employee received second degree burns on 4 small places on his right shoulder after he leaned against a hot piece of metal.

EE WAS GETTING OUT OF A CRANE. HE TURNED TO GO DOWN THE LADDER AND WAS IN A TWISTED POSITION WHEN HE FELT A SHARP PAIN IN HIS LOWER BACK. HE COULD NOT STRAIGHTEN UP WITHOUT PAIN.

EMPLOYEE WAS KNEELING ON THE FLOOR PREPARING TO SLIDE A SHEET OF METAL. WHEN HE LEANED FORWARD HE FELT A CATCH IN HIS BACK AND HAD DIFFICUTLY STANDING UP.

EE WAS USING A GRINDER TO CUT AWAY A BEARING AND A METAL SHAVING GOT IN HIS EYE. HE WAS WEARING SAFETY GLASSES AT THE TIME OF THE INCIDENT.

EE WAS PUSHING A ROLLBACK CHUTE WHEN HE FELT A STRAIN IN HIS RIGHT SHOULDER AND NECK.

EE WAS WELDING DURING THE DAY AS IS NORMAL COURSE OF DUTIES REQUIRED. DURING HIS SHIFT HE HAD A FRAGMENT OF METAL ENTER HIS RT. EYE. HE DID NOT FEEL DISCOMFORT UNTIL AFTER HIS SHIFT. AT THE BE GINNING OF HIS NEXT SHIFT THE INCIDENT WAS REPORTED & EE WAS TAKEN FOR MEDICAL TREATMENT.

EE WAS STANDING ON THE TRACKS OF A CAT 345 EXCAVATOR REPLACING A WINDSHIELD. HE TURNED TO ASK A FELLOW EE FOR A TOOL WHEN HE SLIPPED AND STARTED TO FALL. HE CAUGHT HIMSELF WITH HANDHOLD. TWIST ING AND PULLING HIS BACK. HE WAS TAKEN TO THE DR OFFICE AND DIAGNOSED WITH SPASM IN THE LEFT LUMBAR REGION AND BACK ON LIGHT DUTY UNTIL HEALER.

EMPLOYEE WAS CHANGING CROWN BAR RUBBER. HE WAS USING HIS POCKET KNIFT TO CUT THE RUBBER WHEN HIS KNIFE SLIPPED CUTTING HIS LEFT THUMB ACROXX THE KNUCKLE - STITCHES REQUIRED.

EE WAS TYING A FILTER SCREEN TO THE END OF A PUMP SUCTION HOSE TO PREVENT IT FROM SUCKING ROCKS INTO THE PUMP. WHEN HE PULLED ON THE ROPE TO TIGHTEN IT, HIS SHOULDER POPPED.

CREW WAS IN THE PROCESS OF CHANGING HAMMERS. HELPER HAD HOLDING WRENCH BOLTED UP AGAINST THE MAST. WHILE CONNECTING, DRILL STEEL CROSS THREADED ONTO HAMMER. DRILLER REVERSED ROTATION TO UNTHRE AD. WHEN HE DID THIS IT CAUSED THE HOLDING WRENCH TO MOVE. HELPER RE-GUIDED DRILL STEEL ONTO HAMMER, THE DRILLER HIT THE ROTATION TO SCREW ONTO THE HAMMER, CAUSING THE WRENCH TO HIT HIS ANKLE

DURING THE COURSE OF THE EMPLOYEE'S SHIFT, HE WAS ATTEMPTING TO MOVE A GATE TO CHANGE THE FLOW OF MATERIAL. THE FLOP GATE THAT HE WAS ATTEMPTING TO MOVE WAS STUCK AND HE USED HIS SHOULDER TO P USH ON IT. HE REPORTED THAT HE STRAINED HIS SHOULDER DOING THIS. AT THAT TIME HE FELT HE DIDN'T NEED TREATMENT. 8 WKS LATER HE AGGRIVATED HIS SHOULDER, COMPLAINED OF PAIN, WAS TAKEN TO THE DR

WORKING ON LOADER, REPAIRING HYDRAULIC LEAK IN A CRAMPED SPACE, BUILD UP OF OIL AND DIRT PREVENTED GOOD FOOTING, STRAINED BACK.

EMPLOYEE WAS CHANGING FINGER GATES IN THE CONCRETE SHAKER SCREENS. HE WAS WORKING ON ONE OF THE GATE THAT WAS STUCK, WORKING IT BACK AND FORTH WHEN IT BROKE FREE AND MASHED HIS RIGHT THUMB. TH E EMPLOYEE WAS TAKEN TO THE DOCTOR & RECEIVED STTCHES IN HIS THUMB.

EMPLOYEE TYING FOR CONCRETE PAD, SLIPPED AND FOOT FELL THROUGH REBAR MAT INJURING INGUINAL TENDON. SURGERY REQUIRED TO REPAIR DAMAGE (TEAR).
