Employee was sorting and stacking Pump hoses and fittings onto a pallet. Employee lost their balance and fell backwards onto tongue/hitch area of a cargo trailer. Employee hit their back on the tongue/hitch area of the trailer.

EE1 put bolt-inner bearing housing. EE2 loosened outer fastener;bearing house pulled out.EE1 said stop;EE2 re-tightened fastener. Bolt in-place for alignment of shaft/flange. Vibration/movement from impact wrench may have let shaft spin on bearing. Pendulum effect occurred;paddles contacted EE1. EE exited tub w/o help; walked 300' to truck. EE1 stated legs hurt;driven to hospital.

I was traveling west on the haul road with a load of track pads for the dragline in the bucket of the loader. The pads fell out and I ran over them bouncing me up and into the ROP cutting my left eye brow and the back of my head. I was taken to the ER and the cut above my eye was glued shut. The one on the back of my head was ok.

The operator was feeding the plant with the loader and was stung twice by a bee.

The employee was changing a screen on a portable shaker plant. He lifted a screen, the screen slid on the frame of the plant. He tried to catch it and twisted his body injuring his lower back.

The employee and a contractor were bradding a bolt that holds the track pin in on the dragline. The employee was holding a hammer on the pin and the contractor was hitting it with another hammer. The contractor hit the side of the machine and struck the employee on the left little finger breaking it.

Employee was lifting a converter chain for a dragline. He felt a sharp pain in his back and left leg. We took the him to the hospital. He recieved two shots Demerol, and Phenergan. He was also given two percriptions Cyclobenzaprine, and Hydrocodone. The employee was placed on light duty.

Employee was in the proces of cleaning off mud from the conveyor when he slipped and fell against a troughing roller causing injury to his right side of ribs.

Employee was walking down stairs at the river pump. He slipped on ice causing him to fall on his right hip.

Employee was stung by a bee on his right arm and had to go to the doctor to get a shot of benedryl.

Employee was in the process of changing a drag cable on the dragline #160. He was working between the two drums installing a wedge. When he went to climb out from between the two drums he pressed down on the lagging to lift himself out and a sharp edge on the lagging cut through his gloved hand and made a one inch laceration on his right hand. He had to receive stitches.

Employee was walking on the sidewalk going to the QC lab when he tripped over some metal edging on the corner of the sidewalk. This caused the employee to land on his left hip. The employee broke his left hip.

Employee was stepping over the 902 freshwater pump in order to get to the 901 freshwater pump to get it started. He stepped over the pump and shifted his weight onto his left leg. He lost his footing, his left foot slipped out from under him. When this happened he lost his balance and fell to the ground striking his right knee.

EMPLOYEE GOT OFF OF HIS LOADER TO RETRIEVE A PIECE OF METAL THAT HE SAW IN THE MATERIAL BEING LOADED. AFTER HE RETRIEVED THE PIECE OF METAL HE WAS DESCENDING DOWN THE PILE OF MATERIAL. HE LOST HIS FOOTING AND HIS FOOT FOLDED UP BEHIND HIS LEG THUS CAUSING HIS KNEE TO POP. PAIN WAS FELT IN THE KNEE WHILE WALKING.

Employee was removing and sorting screen cloths on a secondary tower structure for the purpose of housekeeping when he lost control of a screen and smashed the tip of his middle finger on his right hand between the screen and top rail of a nearby hand rail.

Employee recieved metal in his eye while tapping on a pump with hammer. Employee was wearing PPE.

Employee was doing a walk-thru inspection when he came upon a recent gravel spill on a catwalk. He began cleaning the catwalk and then decided to finish his inspection and return later to finish cleaning the spill. As he continued up the catwalk he twisted his knee and ankle on a rock that was on the catwalk. He sustained a sprained right knee and ankle.

EE WAS WALKING OUT TO THE BOAT WHEN HIS RT LEG SLIPPED AND HE ROLLED INTO THE WATER. UPON STANDING HE FELT A STINGING IN HIS LEFT KNEE.

REMOVED CROWN BAR RUBBER OFF SCREEN-PULLED LOOSE AND CUT HAND.

WHILE SPLICING A CONVEYOR BELT THE UTILITY KNIFESLIPPED AND CUT LEFT LEG BELOW KNEE.

TURNED TO GET TOOL OFF OF TRUCK AND KNEE POPPED.

UNLOADING BALES OF PEAT MOSS FROM BACK OF 18 WHEEL DELIVERY TRUCK WHEN A BALE OF PEAT FELL OFF OF PALET KNOCKING EE TO GROUND FRACTURING RT. ARM.

EE WAS REMOVING A CONVEYOR TAIL PULLEY GUARD WHEN IT SLIPPED DOWN IT CAUGHT THE LITTLE FINGER ON LEFT HAND, INJURING IT THE FINGER WAS CUT BUT NO SITICHES WERE REQUIRED.

STRAINED BACK WHILE UNLOADING 50# BAGS OF FERTILIZER FROM A FLATBED TRUCK.

EE WAS CHECKING FOR MATERIAL LEAKAGE IN A ROCK CHUTE AND WHEN HE ATTEMPTED TO PULL HIMSELF OUT OF THE CHUTE HE STRAINED HIS LEFT SHOULDER. HE CONTINUED TO PERFORM HIS REGULAR JOB UNTIL 11-22-0 0 WHEN OTHROSCOPIC SURGERY WAS REQUIRED TO REPAIR HIS SHOULDER.

WHILE REPLACING BRAKE PADS ON DRAGLINE DRUM BRAKES THE PRY BAR THAT WAS BEING USED SLIPPED CAUSING EE TO HIT SHOULDER ON BRAKE FLANGE.

EE WAS WORKING ON A DRAGLINE FAIRLEAD ASSEMBLY ON THE SHOP FLOOR. WHEN HE REACHED UNDER THE FAILEAD TO ALIGN A BEARING THE SHEAVE SHAFT FELL THROUGHT THE SHEAVE AND PINCH HIS HAND BETWEEN THE SHAFT AND FLOOR, CUTTING HIS FINGER.

USING A CABLE COMEALONG TO PULL CONVEYOR BELTING TOGETHER TO SPLICE BELT. WHEN PULLED ON LEVER TO RELEASE THE LATCH ON COMEALONG THE CABLE PULLED LOOSER FROM SPOOL AND CUT REIGHT HAND REQUIRIN G STITCHES.

ON JUNE 3RD EE BENT OVER TO CLEAN OUT DUST PAN ON THE DRAGLINE HE HAD BEEN OPERATING AND FELT A PAIN IN LOWER BACK. HE DID NOT FEEL THE NEED TO SEE A DR UNTIL 6-21-99. DR PUT HIM ON LIGHT DUTY . BEGINNING 6-21-99.

EE WAS STANDING ON THE CATWALK OF A LOGWASHER LEANING OVER TO WELD AN OVERHEAD FLANGE AND STRAINED HIS LOWER BACK AND RIGHT SHOULDER.

EE WAS CHANGING OUT A SHAFT ON THE GARTRY OF A DRAGLINE WHEN HE STRAINED HIS LOWER BACK ATTEMPTING TO LIFT THE SHAFT. HE STRAINED HIS BACK ON 9-21 BUT DID NOT SEEK MEDICAL ATTENTION OR REPORT ACCIDENT UNTIL 9-28-98.

INJURED WAS ATTEMPTING TO ALIGN A GEAR CASE FOR INSTALLATION ON A DRAGLINE WHEN THE CASE SHIFTED AND CAUGHT HIS RIGHT RING FINGER BETWEEN THE PRY BAR HE WAS USING AND A PART OF THE DRAGLINE. H IS RIGHT RING FINGER WAS LACERATED REQUIRING STITCHES.

BURNED LEFT HAND & FOREARM WHEN ATTEMPTED TO CUT OFF FLOW OF ACETYLENE FROM A CUTTING TORCH HOSE THAT CAUGHT ON FIRE FRFOM BEING TOO CLOSE TO WELDING MACHINE EXHAUST.

HIT SHAKER SCREEN TENSION PIN WEDGE WITH HAMMER TO REMOVE IT AND IT STRUCK EMLOYEE BETWEEN EYES.

EE SLIPPED & FELL WHILE WALKING DOWN STAIRS ON DRAGLINE. HE FELT SOME PAIN IN LEFT SHOULDER BUT DID NOT FEEL NECESSARY TO GO TO DOCTOR. SHOULDER WAS A LITTLE SORE FOR A FEW DAYS BUT THEN GOT B ETTER. SHOULDER HAS SINCE GOTTEN SORE AGAIN & EEFELT A VISIT TO DOCTOR NECESSARY. VISITED DR. ON 3/21/97. NOTE: EE DID NOT SEEK MEDICAL ATTENTION UNTIL 3/21/97.

EMPLOYEE WAS MOVING 6-FOOT STEP LADDER TO GAIN ACESS TO STORAGE BUILDING. FELT SHARP PAIN IN RIGHT ARM.

EE CLEANING SPILLAGE ON FIELD CONVEYOR SECTION TO PROCESSING PLANT. FELT PAIN IN LOWER RIGHT BACK WHEN STOOD US.

EE PLACING USED PIECES OF SCREEN CLOTH INTO LOADER BUCKET ON BACKHOE LOADER. SCREEN CLOTH SLIPPED CATCHING LEFT RING FINGER BETWEEN SCREEN CLOTH AND LEAD EDGE OF LOADER BUCKET CUTTING FINGER.

WHILE ATTEMPTING TO PULL A BENT PIECE OF 1/2IN PLATE STEEL BACK INTO PLACE WITH A BOLT THE BOLT PULLED THRU THE I BEAM BEHIND THE PLATE & THE BELT HIT HIM IN THE LOWER LIP

EE WAS LYING UNDER A CONVEYOR WHEN HE DROVE THE WEDGES OUT OF A TROUGHING WHEN HE STRUCK THE WEDGE WITH HAMMER A PIECE OF FOREIGN MATTER FELL IN HIS RT EYE. THE MATTER SAND DIRT IMBEDDED ITSEL F INTO THE CORNEA.

EMPLOYEE ATTEMPTING TO PUT A BAND AROUND A DISCHARGE HOSE. WHILE REACHING AROUND HOSE, EMPLOYEE CUT RIGHT INDEX FINGER ON SHARPE EDGE OF BANDING MATERIAL.

EMPLOYEE WAS CU7TING A PIECE OF BELTING WITH A HOOK KNIFE WHEN THE KNIFE SLIPPED CAUSING LACERATION OF LITTLE FINGER ON LEFT HAND REQUIRING STITSCHES.
