Wrightsville Quarry Metal/Non-Metal

Two employees were splicing a conveyor belt. While installing the clamps the wrench employee one was holding was moved and causing the middle finger of left hand to get caught between the wrench and the frame of the conveyor. Medically treated and currently on light duty.

Employee was working in a chute when they slipped and caught the outer shin of the left leg, cutting their leg. Employee required 15 small stitches to close.

Employee was draining water lines and blowing air through water line. All the air had not completely run out of the hose with additional pressure remaining and proceeded to hit the hand of the employee causing a contusion to the right hand.

Emp was working on the scalping screen on tower #1. EE was grinding the buildup off the screen. Later discovered that something was in left eye which progressively became more annoying. EE contacted superintendent and was told to go to the clinic for medical care. Foreign body was found in left cornea and referred to an ophthalmologist for removal and cleaning.

Employee cut his finger with an old screen during the process of removing it. He received stitches and the tip of the left index finger is broken. Employee is a plant maintenance person working the night shift; after treatment was given, a return to duty with no restrictions was put in the status summary.

Employee was removing belt from conveyor that had torn earlier that morning. He was working on the conveyor adjacent to the belt and when he was pulling belt off he was walking down conveyor and missed the three step ladder that led to the flat level of the plant causing him to fall and injure himself.

Employee is scale clerk who was removing some vegetation from around a CCTV camera at the scale. Employee did not see how close he was to the guardrail and struck his face against the rail causing a laceration to his face

Miner was dismounting from the back of his service truck when his wedding ring got caught in a hook. His wedding ring cut into the meat of his ring finger causing a laceration. Employee needed 4 stitches to close the wound.

Employee was washing underneath a conveyor and was bending down. When the employee stood up, he turned and felt a strain on his right knee. Employee was diagnosed with a strained/sprained right knee.

Employee was knocking grit off rails to install rubber components for a bucket. A piece of grit went into employee's right eye. Employee did not inform operator of injury until employee went to hospital to have grit removed from eye when eye became irritated. Employee suffered corneal abrasions.

Employee was changing belts on a motor. Employee was using a come-along to pull rubberized motor mount so that belts may be iinserted. Employee attached come-along to eye-bolt on motor housing. Bolt sheared while employee was installing belts. Motor housing pinched employee hand against angle iron causing laceration and fracture.

Employee is second shift maintenance. Employee alleges that while attempting to lift a 4x4 screen panel (wire cloth) onto a top-rail of a standard guardrail system, he sprained his left wrist. The employee states that he was attempting to toss the screens down to the ground below.

Loader operator stood up from off his seat to speak with a supervisor. Operator claims he felt a "POP" to his right knee. Returned to his cab when conversation was over and continued to operate his vehicle. Realized something was wrong when operator could no longer press down on the vehicle throttle controls.

Employee was attempting to clean hardened material from the inside corners of the loader bucket on Unit L107 (Cat 976) when he felt a pain in his lower back.

Employee was operating water truck. Dismounted to turn a valve. Caught finger in sharp edge on broken door handle. Laceration on right hand.

Employee was attaching a 6" hose on a 6" branch when hand slipped off hose. Right thumb slid across barb and cut right thumb. Cut was about 3/4 inch long.

Employee was walking to the truck door after putting antifreeze jugs into the bed of the truck. Overturned ankle on a clump of dirt causing left ankle sprain.

Employee was inventorying stockpiles. Walked on slope of stockpile. Left leg dropped into soft spot. Attempted to pull himself out when he felt a pop in his left lower calf. Diagnosed as muscle tear to lower left calf.

EE was shoveling stone and mud from under a conveyor. He felt pain in his left knee.

Employee stepped on a rock when cleaning a catwalk and injured his knee.

Employee was washing down screens with a fire hose. an Air bubble came through the hose and the hose whipped out of his hands. Throughout the day, his middle finger on his right hand became more sore.

EMPLOYEE WAS EXITING A LOADER. HE STEPPED TO THE GROUND AND OVERTURNED HIS ANKLE WITH HIS KNEE HITTING A ROCK WHEN HE FELL.

EMPLOYEE WAS WORKING ON STONE CRUSHER. HE WAS LIFTING A STEEL BEAM, IT SLIPPED AND FELL ON HIS FINGER.

EE WAS CHANGING SCREENS AT THE CRUSHING PLANT. THE EE SLIPPED AND THE SCREEN FELL ON HIM.

EMPLOYEE WAS PARKING HIS LOADER AT THE GARAGE. WHEN HE EXITED THE LOADER HE SLIPPED AND FELL FROM THE TOP STEP OF THE LADDER.

EE HAD DUMP TRUCK PARKED UNDER LOADING BINS. SHE CLIMBED UP ON TRUCK TO SEE IF IT WAS LOADED. SHE SLIPPED ON THE LADDER AND FELL TO THE GROUND.

EE WAS PRYING A STEEL PLATE THAT WAS WEDGED IN THE CRUSHER, THE STEEL PLATE GAVE WAY AND LANDED ON EE'S FINGER.

EE WAS MOVING A PIECE OF STEEL. HE TRIPPED WHILE WALKING AND THE STEEL FELL ON HIS FOOT.

EE WAS WORKING IN QUARRY, TRIPPED AND FELL ON RIGHT KNEE.

POSSIBLE RECURRING BACK INJURY FROM 1996 ACCIDENT LOWER BACK PAIN UNKNOWN CAUSE.
