Employee was locked in the stone silo elevator between floors due to an overload condition on the motor

Employee got finger pinched by lever arm of shotgun. On 7/24 it was confirmed that the employee fractured the tip of their lt ring finger resulting in modified work.

The employee was working on the TDF dock, where they experienced a foreign body sensation in left eye while they were offloading tires. The employee did not report the incident until 6/23 until after visiting the ER where they were given an RX antibiotic.

Employee was loading a tire on a monorail hook and felt a pop above their belly button.

Employee pinched finger between concrete ledge and jackhammer resulting in a bruised nail bed

The employee assisted with maintenance on kiln. The employee then approached a lid to inspect for damage. While doing so, the lid quickly became top-heavy tipping over and landing on employee's left foot resulting in a fracture of metatarsal.

Pursuant to a request from MSHA, we are submitting this first report of injury. The operator cannot determine when this alleged injury occurred so the date of accident and time are when it was reported to us. A temporary employee alleges that EE was exposed to carbon monoxide while working at the site. There is no evidence to support this claim.

Incident date and time noted is when we were informed. The site cannot pinpoint an actual incident time or date. Employee informed our 3rd party triage company of a potential alleged CO overexposure. There was no report of an overexposure to Lafarge management. The operator disputes that an injury or illness occurred at the site.

On April 12, 2022, approximately 4:15 PM noticed a fire at the alternative fuels system. At the same time, the alarm sounded in the control room, the CO2 fire suppression system was activated automatically. The Fire Department was called and immediately responded and the fire was under control shortly thereafter.

Employee was working in the packing area of the Whitehall Plant. While troubleshooting some packer issues, employee was found unresponsive by other team members. Ambulance services responded and were able to stabilize the employee for transport to a local hospital where the employee passed the following day.

The electrical instrumentation technician knelt down to grab a #14 GA wire from a reel to tape it and felt pain in EE's lower back.

Employee was conducting a cement silo change on top of the bulk silos. The employee was tightening the valve by pulling down on a steel bar that was inserted into the bottom knuckle when the bar slipped out causing the employee to lose balance and fracture the right thumb.

Production Utility Operator (PUO) was walking through the plant headed to the kiln area. Employee was near the equipment storage building when they slipped on snow and fell on left side. This incident was upgraded to a modified work duty on 02/19/2021.

An electrical Employee received a cut / laceration to left knee while reinstalling a cover on a guard.

While clearing a choke above the impactor, the material flushed pulling the bar that the employee was using downward rapidly causing a strain in the employees, elbow, shoulder and neck.

Employee experienced low back while doing normal job duties this morning. Employee denies a specific injury which caused the back pain stating "I was just lifting tires like I always do at my job". Employee reports the pain started in the center of the low back and progressively got worse.

While a Production Utility Operator was lancing at the finish mill, EE stopped what EE was doing to talk to another employee and lifted the face shield. When EE lifted the shield there was cement on the brim of the shield and some dust got into EE's eye. EE was wearing all appropriate PPE, safety glasses, face shield etc.

While addressing a slide gate that would not go into position at the top of the clinker tank, the employee started cleaning the clinker in the area to access the job site. The employee was shoveling when ee lost footing on the clinker that ee was cleaning and felt a strain in lower to middle back.

A 23 year employee-electrician, was replacing the reverse air fan on the Kiln 2 main baghouse. The fan had been locked out but the motor drive belt to the fan wheel was still rotating. EE attempted to stop it by grabbing the belt. Their right hand pulled by a drive belt into a sheave pinching their pointer finger.

Multiple (3) tractor trailers on fire with tires in the trailers. Suspected vandalism.

Cleaning lumps from the B11 trap before the blending pod. Wrench slipped and EE felt a sudden jar forward. Initially the operator reported only some slight stiffness. Following day EE was performing housekeeping and reported pain and stiffness.

Employee was inspecting the tower power shot port for a stage choke and build up. EE was wearing all required PPE. There was a brief period when the system went positive & EE breathed in some hot gasses causing EE to have some shortness of breath.

Employee was using a 10# sledge hammer to force a slide gate into position at the west clinker silo elevator. The elevation of the slide gate required the employee to swing the sledge hammer overhead to strike the desired position. It was from this movement that the employee felt a strain in back

Employee was sweeping the floor for approx 45 minutes then began shoveling a pile of cement dust in the finish mill area. EE felt a pain in EE's back, attempted to switch sides and body position but pain continued.

Employee was working on tire dock loading tires from the trailers to the feed system. Reports that as ee was walking into the trailer that ee had half of left foot on the trailer and the other on the air dock. As ee moved forward, foot slipped off the dock into the trailer causing all of weight to go onto the back of left heel rolling left ankle.

Tire dock employee got EE's right hand caught in the hook used to break down truck tires while moving them out of the trailer. EE was using the proper tool and wearing gloves but the tires in the back shifted forward unexpectedly causing EE's right hand to get stuck in the handle of the tool

LEFT SHOULDER INJURED WHILE PULLING TIRES DOWN FROM A STACK.

While working on the tire dock, the attendant hurt his shoulder. Per the tire dock attendant, he wasn't doing anything specific to injure himself.

Dust went into his eye while removing his PPE.

Hurt his shoulder offloading tires.

Twisted his ankle while working on the tire dock.

EE reports that he began to experience muscle cramping in his legs, arms and hands on 07/07/2015 but did not report the condition until the following morning (07/08/2015) when the cramping continued with additional symptoms of clammy skin and dizziness. EE was treated for symptoms of heat related illness.

Employee struck across both legs by high pressure water spray.

EE was unloading tires from trailers at the TDF Dock when he stepped in the crack between the dock and a parked trailer. He fell to the floor striking his right knee.

EE was checking DAA flow at the inspection port on Kiln 3 line. The DAA flow was OK but went positive as the EE closed the inspection port. EE was struck on left side of face and left eye. EE made his way to nearby coworker who took him to an eyewash station and then the control room. Transported to local clinic and then eye specialist. Multiple flushes applied.

EE removing build up from K2 stage four vessel. After several power shots a quantity of material fell into the stage four cone and became lodged in the discharge. EE was able to shift the piece of material with steel rod. As the material end of the bar EE was holding on to was forced up striking EE on the right side of the jaw. Laceration, fractured jaw and chipped teeth.

A fire was discovered in the pulverized fuel bin at ~8:30AM. An employee extinguished the fire with a fire extinguisher. The local fire department was called. There are no injuries as a result of this incident. MSHA was called at ~9:00AM. Inspector arrived on site and conducted an investigation.

EE production utility operator, was air-lancing a vessel in the prehater tower when he wrenched his right elbow. He did experence some swelling at the joint and was taken to a local clinic for examination. The physician aspirated the joint, removing a small amount of fluid. He returned to work without complication.

Employee & coworker were removing 4 pieces of siding,(each approximately 4' x 4')from the south side of our #1 Pachhouse building. In removing & lowering the bottom piece, the piece above fell & hit the employee on the left forearm, resulting in the need for 7 stitches.

Employee was pushing a low, wheel mounted cart when he felt a sharp pain on his left foot. He reported the incident "just in case" & returned to work. Healthbridge was contacted & home care was recommended. Next day, the employee felt a sharp pain walking. Healthbridge was contacted & the NCM recommended a local health clinic exam, which revealed a closed fracture of a small bone.

Employee was stacking tires in his truck, when stepping over the tires his foot slipped spraining his l knee.

Employee was assisting in removing a ceramic lined jet assembly from a cement blender. This required him to be on his knees in an area containing numerous 1/2" plastic hoses. When leaving the space he was in, it is assumed he bumped the assembly which freed it to fall & hit his finger, requiring medical treatment & 7 stitches.

Millwright was fabricating an oil pump base in the machine shop. He was working on a table, welding & grinding the base. When he rotated the base, the leg on one side slid off the table & the inside of the base fell about 3", pinching his left hand small finger. He was wearing all his PPE. X-rays showed no broken bones. He was prescribed pain medication.

Employee was stacking tires in trailer, after driving to another site noticed numbness in the fingers and pain in trapezius muscle.

Employee was moving a portable heater from the East side of Packhouse #4 to the West side where he would be working. As he wheeled the heater out of tunnel, the wheel got caught on the tap box and his shoulder jerked back, causing a great deal of discomfort. He then reported to supervisor.

Employee had removed wheel chocks from vehicle and moved the truck. When walking back through the area, the employee tripped over the chocks and fell to the ground. Incident became reportable on 12/9/11 when Dr placed him on restricted duty.

Employee was breaking down a barrel stack of truck tires in order to organize tires on the conveyor system for use as fuel for the kiln. One truck tire fell from the stack and pinched his right hand against the tire he was rolling. Employee received medical treatment but intense swelling delayed diagnosis of injury. On 7/28, MRI results indicated 3 bone contusions in right hand.

While unloading a truck trailer full of tires, employee reached for a tire at the top of the pile and felt a burning pain in his left shoulder. He said he felt minor discomfort, but during the weekend he stated that he needed to see a doctor as the pain worsened.

Employee was leaving the main office training room to complete a field exercise. His wedding ring caught the inside of the door push bar and cut his finger as he removed his hand from the door.

Employee was in the stone silo to clear a feeder choke in CR5. Upon exiting the silo door, the employee fell to the ground coming down on his right knee. His knee hit the containment berm surrounding the door, an area which was muddy. This was reported as a First Aid but the level of follow-up treatment elevated this to a Medical, explaining the delay in reporting.

Employee slipped on a step going to the Washhouse lunchroom. He did not report the incident. Friday morning he reported off sick. At 4:00pm that day, he reported to the Plant Admin that he injured left hand and shoulder. He has gone to his family doctor that day & doctor determined that he chipped a bone in his left wrist.

Victim was emptying an aurora hopper, which was on a forklift. When returning the aurora hopper to the locked position, his left index finger got pinched between the hopper and forklift cage. Pressure was applied immediately and dressing was put on the finger to control bleeding. The victim was taken to the health clinic for medical attention, where he received stitches.

operator was transferring waste tires from one truck onto a tractor trailer. The two vehicles were butted against each other, however there was a small 3 inch gap between the vehicles that allowed the operator's foot to get caught in the gap.

Employee was opening the mill, using the crossbar to break the hatch loose. He felt pain after he struck the hatch with the crossbar. Crossbar weighs about 100 lbs, and is bounced on the hatch to break it loose from the mill.

EE and other were fixing a probe on the drive end of 406 Drag. They realized they would need a bigger tool to complete the job. other went to the electric shop to get a bigger pipewrench and he proceeded to work on union with 12" pipe wrench. He pulled on the wrench and went down on one knee and claimed he pulled something in his back. Note: Injury not reported until 9/5/08

EE was a ladder for the purpose of adjusting the tracking of the belt on the feed conveyor. The belt needed to be adjust. He was leaning against a pipe and held onto the machine to reposition himself when his glove got caught an the return roller and pulled his arm into the conveyor. The other man on the floor hit the emergency stop button. The bar prevented accessibility beyond

EE was leaning into a 52" tall box, to remove a remaining scrap of material. the sideflap was standing straight up and there were 2 boxes directly in front of the box. The forklift driver put the forks into the skid and upon lifting it, shifted the box next to it, shifting the box. Because of the flap, the driver did not see EE in the box. He jumped and fell against railing.

A length of pipe 116" fell from a 30" high pipe stand and landed on the workers right foot.

He fell on ice - broke right ankle in 2 places.

EE attempting to clean build up from the K2 preheater tower using the cardox system. EE filled and stored the shot tube in the hand truck that is supplied to hold and transport the tubes. EE was lifting a power shot tube from the cart to fire the tube when EE felt pain in his right uper arm.

Walking around water puddle, slipped in muddy area, extended right arm to break backward fall.

Driver pulled into tarping station climbed up on tarping station to roll tarp out to cover flat. Driver was placing foot from station on to trailer slipped fell b/w station & trailer. Driver hit head (wearing hard hat) bruised side, cut hand (palm)

Employee was lifting truck tires while loading his truck (lower back strain).

EMPLOYEE REPORTS SLIPPING ON A STEP WHILE EXITING A BUILDING BETWEEN SILOS & A-FRAME. EE TWISTED HIS LEFT ANKLE. EE WAS LEAVING PREHEATING AREA THROUGH UNCOMMONLY USED DOORWAY, STEPPED ONTO UNFASTENED STEP, STEP SHIFTED. EE STATES HE HAD A CRACKING NOISE COME FROM HIS ANKLE & REPORTS STIFFNESS, SWELLING & SHARP PAIN.

EMPLOYEE ATTEMPTED TO REMOVE MATERIAL WITHOUT TURNING THE BELT OFF.

WORKING ON #2 RAW MILL SEPERATOR, WELDING. HOT SLAG GOT INTO HIS RIGHT SHOE.

Large lump through K2 clinker breaker created dusty condition. Dust got in right eye.

Ee used his fingers to remove build up cement from the rollers of packer.

EMPLOYEE WAS CUTTING ELEC.LINE; KNIFE SLIPPED, CUT HIS LEFT THUMB.

Tire fell on ee's right foot.

REMOVING THE INNER BEARING RACE FROM THE COAL MILL SHAFT, THE WELD BROKE OFF THE TWO ALL-THREAD RODS. THE ROD FAILED AND SPRUNG UP INTO THE FACE OF THE EMPLOYEE.

EE HAS BEEN EXPERIENCING PAIN IN THE FINGERS OF HIS LEFT HAND AND ARM OVER THE PAST FEW WEEKS.

EE ATTEMPTED TO REMOVE A TIRE FROM THE #2 BELT. BELT WAS NOT SHUT OFF. THE OPERATOR WAS NOT AWARE THAT THIS EE WAS REMOVING THE DISLODGED TIRE FROM THE SYSTEM

EMPLOYEE WAS HANDING AIR ARCING RODS TO FELLOW EMPLOYEES WHO WERE ENGAGED IN OPENING THE SLOTS IN THE MILL PARTITION. EE ACCIDENTLY LOOKED IN DIRECTION OF THE PARTITION WHEN THE ARC WAS STRUC K.

EE HAS NOTICED A GRADUAL HEARING LOSS IN BOTH EARS OVER THE LAST FEW YEARS.

EMPLOYEE WAS USING A JACKHAMMER WHEN THE THREADS BROKE ON THE FITTING AND THE LOOSE END OF THE HOSE STRUCK EMPLOYEE ON THE RIGHT LEG ABOVE THE ANKLE.

EE PICKED UP A TRUCK TIRE WHEN HE FELT A SHARP PAIN ON HIS CHEST

EE WAS REPORTING TO WORK WHEN HE SLIPPED AND FELL ON ICE.

HEART ATTACK.

EMPLOYEE WAS VIEWING WORK THAT WAS DONE ON THE 101 DRAG CONVEYER. AS THE COVER WAS REMOVED, DUST AND DIRT WAS BLOWN AROUND BY THE HEAT EXCHANGER FANS AND A FOREIGN BODY LODGED IN RIGHT EYE.

PALLET OF CEMENT BAGS STOPPED ON CONVEYOR. EMPLOYEE MANUALLY PUSHED THE PALLET TO GET IT STARTED AND STRAINED HIS KNEE. INCIDENT REPORTED 2/14/03. INJURY NOT EVIDENT UNTIL LATER.

EE WAS USING AN AIR HOSE TO CLEAN OUT BULLDOZER CAB. HE SLIPPED AND FELL WHEN TRYING TO DESCEND FROM EQUIPMENT.

ROLL DOOR LEFT OPEN; DRIVER NEEDED TO CLOSE DOOR TO TAKE TRAILER ON RUN. MAY HAVE BEEN DEW ON TAIL END OF TRAILER; DRIVER LOST FOOTING AND FELL TO THE GROUND.

EMPLOYEE IS EXPERIENCING A PARTIAL TEAR OF THE RIGHT ACHILLES TENDON CAUSED BY REPETIVE MOTION.

EE LIFTED HEAVY PACKAGE FOR DISTRIBUTION OUT OF MAIL BIN AND FELT PAIN IN WRIST.

EE PACKING TYPE III CEMENT WHEN BAG BLEW UP IN HIS FACE.

EE WAS REMOVING QUARRY FEEDER LUBE PUMP AND FELT PAIN IN HIS RIGHT WRIST.

EE WAS WORKING ON THE TIRE DOCK INSIDE OF A TRAILER TRYING TO FREE A TIRE THAT WAS STUCK, WHILE PULLING ON THE TIRE HE AGGRAVATED HIS SHOULDERS, CAUSING BILATERAL PAIN.

EMPLOYEE WAS DRILLING HOLES IN METAL FUNNEL SECTION OF LOAD OUT SPOUT USING A 1/2" DRIVE DRILL WHEN DRILL BIT CAUGHT AND DRILL TURNED IN HIS HAND.

EE WAS REMOVING RETURN IDLER ON #9 RAW FEED BELT WHEN IT ROLLED IN HIS LEFT HAND CAUSING A BLOW TO HIS THUMB.

EE WAS SHOVELING RAW MATERIAL (STONES) INTO BUCKET OF BOBCAT WHEN HE DEVELOPED PAIN IN HIS LEFT FOREARM.

INDIVIDUAL WAS SHOVELING STONE, FELT PAIN IN HISBACK THAT GOT PROGRESSIVELY WORSE THROUGHOUT THEDAY.

EE POKING THRU A POWERSHOT SHOT PORT WHEN HE STRUCK HIS LEFT ELBOW

EMPLOYEE WAS REPAIRING AIR SLIDE ON H-6 SILO WHEN HE STRAINED HIS LEFT KNEE.

EMPLOYEE WAS CLEANING THE A-FRAME ELEVATOR PIT PAN FEEDERS WHEN HE PINCHED HIS FINGERS BETWEEN THE PAN FEEDER AND THE CHIPPER.

EMPLOYEE WAS BREAKING DOWN A STACK OF TRUCK TIRES HOUSED IN A TRAILER. THE STACK OF TIRES FELL AND STRUCK THE EMPLOYEE ON HIS RIGHT SIDE.

EMPLOYEE WAS TRYING TO FREE TRUCK TIRES FROM INSIDE TRAILER WHEN HE FELT A SHARP PAIN IN HIS GROIN AREA.

EMPLOYEE SHOOTING CLINKER ACCUMULATION IN COOLER#2 WHEN A BURST OF HOT MATERIAL CAME BACK AT EMPLOYEE BURNING HIS RIGHT FOREARM.

EMPLOYEE WAS LOOSENING IDLER BASE BOLT TO ADJUST TRACKING OF BELT WHILE FEED BELT WAS IN MOTION. EMPLOYEE BRUISED HIS RIGHT HAND AND BROKE HIS MIDDLE FINGER.

EE WAS REMOVING AN UPRIGHT TIRE FROM A TRAILER WHEN A TIRE FELL FROM THE TOP OF THE STACK HITTING THE EE ON THE LOWER LEFT SIDE OF HIS BACK.

EE WAS LIFTING BOXES OF SHOTGUN SLUGS AND PLACING THEM IN A STORAGE LOCKER WHEN HE FELT PAIN IN HIS LOWER BACK.
