He was taking a sack off of roller mill 2 and SC2 airlock started shooting out material and blacked out his area and he tripped and fell trying to find his way out of all of the dust. He tripped over a pallet and fell into a beam and the impact jolted his back.
The employee forgot to engage the parking brake on the forklift. He got off to untie a sack, and the lift started to roll towards him. The lift hit a beam which stopped it, but a sack of material fell on his right ankle.

The employee was bagging material at the auger packer. He placed a 55# bag on a pallet and when he stood up, he felt pain in the inner thigh, leg and lower stomach.
