Clear morning start of shift, after the pre-shift of equipment was completed, the operator started to mount the haul truck steps with three points of contact, but the step strap broke. This caused the operator to fall from the step and lacerate EE's lower right leg. This required stitches.

A maintenance worker lost control of a small hammer. The hammer slid down the screen deck and bounced off the deck, striking a second employee in the nose, causing a laceration that needed sutures to close.

Employee was cleaning the C-2 box with a digging bar and hammer when he missed the bar and struck the edge of the box with his left forearm.

Employee slipped on some ice while working around the plant assisting maintenance with a broken conveyor belt and fractured his right ankle.

Walking from Scale house to his vehicle, slipped on snow, causing him to fall. Had hip surgery a number of years ago, fall caused pin to twist bone.

While shoveling mud and stones from walkway, employee heard a pop and started developing pain in his left shoulder.

WHILE COMING DOWN OFF STACKER EMPLOYEE'S FOOT SLIPPED CAUSING HIM TO TWIST HIS LEFT ANKLE.
