Clear morning start of shift, after the pre-shift of equipment was completed, the operator started to mount the haul truck steps with three points of contact, but the step strap broke. This caused the operator to fall from the step and lacerate EE's lower right leg. This required stitches.

A maintenance worker lost control of a small hammer. The hammer slid down the screen deck and bounced off the deck, striking a second employee in the nose, causing a laceration that needed sutures to close.
