Miner Operator was tramming into cut. When he positioned the miner onto sights, the mining machine was on a knuckle on the bottom causing it to swing in his direction, pinning him against the rib with the tail of the mining machine.
Roof fall in intersection of #7 Entry of 3#2 Intake. Fall was 20'x 25'x 7'-8' high. Fall does not affect travel or air movement.
While on weekly run, examiner found a fall of roof above the anchorage point. This fall is located 80' inby survey spad 1317, #1 entry, 1 crosscut outby black mains inlet. The area does not affect air or travel through the return area. 7'H x 40'L.
Miner was tramming golf cart in belt line, he struck belt structure which caused him to depress tram pedal. He caught his left arm between cart and post, stretching his shoulders. He continued to work his regular duties until 8/27/15.
Miner was inserting a wrench into the chuck of the drill head when a rock fell from the roof striking his right hand. After being x-rayed, it was learned that he had a fracture of his right middle finger. He was released modified duty.
Bolter was adding steels to his set when he rotated his drill head causing his hand to wrap around the steels.
Section Foreman was walking bent over and caught his ear on a broken strap. He received a laceration on his ear and temple resulting in 17 stitches.
Employee was bolting in #5 entry when some cap rock fell out striking him on the shoulder. He was installing the outside bolt at the time. He went to ER and was put on restricted duty for a deep muscle bruise.
Miner was bolting on the operator side of the roof bolting machine. He noticed a rock loose in the roof and when he moved, he hit the fast feed lever and joystick, dropping the drill head onto his left knee.
Employee was traveling into section on a mantrip. The trip hit a hole on the travel way and bottomed out, jarring the employee. He was transported outside.
Miner was getting bolts for next row, when a rock fell from the roof, striking him on the left leg, just above the knee. He went to Dr, and was released modified duty with a deep bruise.
Miner was changing a bottom roller on the belts. The roller slipped off the pry bar he was using striking his right wrist.
Miner was loading resin onto roof bolting machine when he felt a pop in his right shoulder.
Employee was removing rock from roof strap. Rock broke apart, causing employee to punch roof. Employee received 8 stitches to the back of his left hand. He was released without restrictions from ER.
Miner was tramming scoop along #5 entry (travelway) of Black Mains approx. 1800 feet outby the section. A brow of rock popped free of the rib and struck a 5"x7" post that was 8 feet in length, causing it to fall into the kitchen of the scoop, striking the miner on the left side of his head.
miner was splicing water line for continuous miner when his hand slipped and hit against steel clamp causing a cut on his right ring finger. Miner went to ER and received 2 stitches to the injury.
Employee was setting posts. He bent over to lift post from the scoop tractor bucket. As he straightened up, he felt a twinge in his lower back. He worked the rest of his shift.
Employee caught his finger between T3 channel and plate while inserting his bolt into the roof. Employee finished his shift and drove himself to the hospital where he was x-rayed, and received 4 stitches to his left index finger. Employee did not miss work.
Roof fall occurred in Gold Mains intake #8 entry at survey spad 967. Fall was approx. 20'x20' in size. Fall did not affect ventilation and is not in an area normally traveled by men.
Employee was sorting trash. He picked up a bucket of used miner bits and threw it underhand. He felt a popping sensation. He was seen by Corporate Care and released to regular duty with restrictions. Employee worked within his restrictions until 11/18/2013 on which date he had surgery to repair a torn rotator cuff.
Employee was moving a rock out of his way when he smashed his right pinky finger between two rocks.
Employee was moving his snub, when a rock fell out of the roof striking his left hand. This resulted in him receiving two stitches to the middle finger.
Employee was loading straps onto roof bolter when he felt a pain in his back/ribs. He was transported to the hospital for x-rays and diagnosed as having a muscle strain.
Upon entering the section, maintenance crew observed a roof fall occurred. Section Foreman called Mine Foreman who notified MSHA. Fall occurred 1 cross cut inby the loading point of entry #5 in Gold Mains Section. 40' outby survey station 753. Measured 30'L x 24'W x 14' H.
Employee was leveling belt structure. He felt pain in right elbow to hand while using bumper jack.
Employee was removing rock from continuous mining machine. He twisted and felt a sharp pain in his back. He stood up quickly and struck his head on the roof, then fell and hit his head on the mining machine.
While picking up front of fire hose cart employee felt pain in lower back.
Employee was striking suspended radiant heater with shovel handle to dislodge any float dust possibly on top. He knocked heater off suspended hooks. Heater fell striking his hard hat forcing metal clip of hat liner suspension to cut his forehead requiring 8 stitches.
Employee was starting drill hole when a piece of loose roof rock approx 1 1/2" x 2' came down striking left middle finger causing contusion to end of finger requiring three stitches.
While pounding on belt splice with pick hammer piece of hammer peened off striking employee in chest causing impaled object.
Miner was pulling miner cable and pulled groin, miner worked till 10-3-12 light duty till he went to specialist may be medical condition.
While installing inside bolt, piece of slicker sided rock fell behind bolter operator striking him on foot causing bruise.
EE was putting in the inside roof bolt in the #2 entry when a piece of rock fell striking him in the left ankle requiring 2 stitches.
Employee was kneeling along cont miner 3 rows of bolts back from edge of cut through in xcut 5 to 6 entry when sharp piece of shale roof fell cutting right ear and tip of right hand.
While using post to push rock from atop continuous miner, employee got finger pinched between post and roof support causing injury requiring stitches.
