Miner Operator was tramming into cut. When he positioned the miner onto sights, the mining machine was on a knuckle on the bottom causing it to swing in his direction, pinning him against the rib with the tail of the mining machine.

While on weekly run, examiner found a fall of roof above the anchorage point. This fall is located 80' inby survey spad 1317, #1 entry, 1 crosscut outby black mains inlet. The area does not affect air or travel through the return area. 7'H x 40'L.

Miner was tramming golf cart in belt line, he struck belt structure which caused him to depress tram pedal. He caught his left arm between cart and post, stretching his shoulders. He continued to work his regular duties until 8/27/15.

Miner was inserting a wrench into the chuck of the drill head when a rock fell from the roof striking his right hand. After being x-rayed, it was learned that he had a fracture of his right middle finger. He was released modified duty.

Miner was bolting on the operator side of the roof bolting machine. He noticed a rock loose in the roof and when he moved, he hit the fast feed lever and joystick, dropping the drill head onto his left knee.

Miner was tramming scoop along #5 entry (travelway) of Black Mains approx. 1800 feet outby the section. A brow of rock popped free of the rib and struck a 5"x7" post that was 8 feet in length, causing it to fall into the kitchen of the scoop, striking the miner on the left side of his head.

Employee caught his finger between T3 channel and plate while inserting his bolt into the roof. Employee finished his shift and drove himself to the hospital where he was x-rayed, and received 4 stitches to his left index finger. Employee did not miss work.

Employee was sorting trash. He picked up a bucket of used miner bits and threw it underhand. He felt a popping sensation. He was seen by Corporate Care and released to regular duty with restrictions. Employee worked within his restrictions until 11/18/2013 on which date he had surgery to repair a torn rotator cuff.
