While working in the Air Separator (GSAS-01) at the Grinding Mill, a maintenance EE's hands slipped of a Whizzer bar EE was removing. There was a sharp piece of metal protruding which lacerated the EE's right thumb between the 2 knuckles. EE received 4 stitches to their thumb.

At roughly 3:30 PM on Tuesday December 1 the 2nd shift Wet Process Operator stepped on a rock getting out of vehicle and sprained left knee. Employee was taken to the ER. The result was a sprained left knee and ankle. Employee is to return to work in 3 days.

Thursday April 30th while installing dryer flights at the rotary dryer (DR-01) the maintenance employee strained back. EE did not want to go to the doctor and finished the shift. Friday May 1st the employee was able to come to work, stretch it out and make it thru the day. Thursday May 28th the employee stated EE needed to go to the doctor because back was injured.

At approximately 8:45 am an employee slipped on algae injuring left shoulder on the concrete pad at PU-42 while working on a guard. Employee reported the incident at 10:00 am and finished the shift. Monday afternoon the employee reported needing to go get checked out. The doctor put the employee on lost time for 2 days, after which no restrictions.

Maintenance employee was working at the BC-08 slide handle near the cement tank. Employee moved an 8-10 lb guard and felt a pop in right shoulder.

Employee was dismounting a John Deere 400D Water Wagon. When ee took the final step down to the ground, ee felt a pain in right foot. Employee was taken to the medical center where they diagnosed ee with a Contusion of the Right Foot.

Maintenance EE was replacing a flow divider for the mill hydraulics. EE loosened the hose for the flow divider and hydraulic fluid splashed ee in the face and went under safety glasses and into eyes. EE flushed eyes and was taken to the ER. ER flushed eyes again and prescribed prescription eye drops. EE returned to work with no modified duty or lost time.

Ee went to check on the 510 air valve in the tunnel. Ee rested EE's hand on the upper ledge of the tunnel entrance. A small amount of sand fell from the ledge getting into the EE's right eye. Eye was flushed. EE went to the emergency room the next day and received a prescription for eye drops. Ee returned to work. No modified duty.

Employee was carrying a 5 gallon diesel can about to enter a door to the mill. The employee set the can on the ground to go inspect a noise at another location. When they started to walk away they slipped on a patch of ice with the left foot. They fell on their right side injuring a shoulder.

Employee was attempting to remove a bushing from a sheave positioned on the ground. While repositioning to its side the sheave began to slip and the employee got right index finger caught between the sheave and adjacent structural steel support. The right index finger was pinched and caused a laceration that required 10 stitches. EE was released to returned to work.

EE was attempting to loosen a nut with a wrench. Nut was diffucut to break loose EE felt pain in the back. EE was taken to ER X-rays were taken. ER reported Muscle strain of the upper and mid back with possibility of rib factures.

While performing maintenance on rotary dryer wind blew sand up under EE's safety glasses into right eye. EE was taken to local hospital where eye was flushed out and prescription was given.

Employee was using a pipe wrench and small sledge hammer to remove an inspection plug on the mill drum to measure media level when the hammer glancing blow off the wrench and over rotated employee's arm

The employee was attempting to turn left on the southbound side of the haul road, when the 1/2 ton pickup ee was driving was struck on the driver's side by a CAT 773 truck that was coming up from behind ee. Employee was taken to the hospital and checked out. Employee was released to light duty. Damage to pickup was substantial on drivers side. Minimal damage to CAT 773 truck.

Employee was working on replacing a conveyor belt with 2 other employees. A few days later EE felt a crook in neck but did not report it. EE went to personal doctor on 22 June 2016 (a chiropractor) and received treatment. The mine operator was notified on 29 June 2016 that EE went to the doctor and wanted to fill out a injury report. Dr diagnosis pinched nerve in back.

Back is hurting but no accident nor incident reported.

EE had finished shift start brief and had exited the breakroom door to travel to his work area. He looked up to check the height of a material pile and next thing he remembers is he is holding his neck leaning against handrail. Sutures to neck 15 sutures

Employee exited the dozer, tripped on small windrow/rut on the way to personal vehicle to go home.

Employee was operating a Volvo 220F front end loader in the stockpile when he felt a pain in his lower left back due to the jarring motion of the loader.

EE was using a bobcat loader to stand up a pipe secured to the loader bucket. Once set the rigging was removed, EE was backing the loader away, the pipe tipped toward the loader, EE reached out to catch the pipe and the tip of his right little finger was trapped between the pipe and loader frame causing a small laceration requiring stitches and fracture to the tip of the finger.

EE was helping oiler change the cutting edges on a motor grader he had been operating. As EE unscrewed the last nut (edges come bolted together) the cutting edges fell faster than expected, pinching his left pointer finger, resulting in the loss of a small part of finger tip (approx. 1/4" X 1/2"). All PPE was being worn/used at the time, including gloves.

EE states his left wrist began hurting and he believed it to be a muscle ache. It continued on & off Sat, becoming constant on Sun. and began to swell. EE did not wish to seek medical care, but on 2/19/14 he went to clinic-diagnosed w/tendonitis. He has been on conservative treatment until 6/18/14. On 6/19/14 he had outpatient surgery to correct pain. Lite duty 6/20/14.

This incident did not immediately involve either modified duty or lost time. For more than three years EE has had incidents on this job with his left ankle. Yesterday(05/06/2014) he came in with a notice from his doctor stating that he would be out of work for at least 30 days following surgery on his left ankle to correct the problem.

EE was retrieving a come along and belt brackets from the tail section of a elevator he reached down to pick them up and felt that he may have pulled muscle in his lower stomach. EE did not report the alleged incident until 11/18/2013 and did not seek medical attention until 09/05/2014, restrictions were prescribed for a possible hernia at which time the incident became reportable.

The truck driver (NOT a Unimin employee but a driver picking up material) was found non-responsive sitting in his truck. Paramedics were called. Driver was transported to local hospital where he was declared dead of natural causes. Found nothing to indicate that it was anything but natural causes.

Employee was installing a new mechanical splice in a belt conveyor, when the chain in the lever hoist ("come-a-long")suddenly froze. This sudden unexpected stop caused the employee to sprain his right elbow.

the front end loader had a faulty seat that was bottoming out, and the operator received muscle spasms as a result

Employee was found non responsive sitting in his personal vehicle. Paramedics were called. Employee was transported to local hospital where he was declared dead of natural causes. Found nothing to indicate that it was anything but natural causes.

In the process of removing bucket tips on a 385. Retainers were removed and tips were stuck to adapters. While using 10 lb sledge hammer to loosen a sliver of metal split off and struck employee in left leg above knee.

Employee was stepping out of his shack and stepped onto a rock, spraining his right knee in the process.

While watching the sky for tornados in area, employee stumbled down a truck ramp and fell forward, hitting the concrete with his head and right hand which was stretched out to catch his fall. His hand was cut and needed 11 stitches to repair.

Sprained left ankle while climbing over GSPM-01 motor to repair wiring.

Employee slipped on icy sidewalk and fell on left knee.

During the morning safety meeting the EE and his supervisor had a disagreement during the disagreement the EE was pushed into his chair to sit and the action resulted in a bulged disk at C6-C7.

While removing access door to GSBE-03 to check elevator buckets, employee got dust or debris in his right eye.

Employee's chair collapsed, pinning his left knee between the seat and the base, spraining his left knee.

Employee was reinstalling the south access door on GSBE-03, when he strained his right elbow.

EE strained right shoulder when using a cheater pipe on a ratchet to tighten VS-01 gearbox

Employee fell while climbing out of a manbasket, tripping on his safety harness / lanyard and landed on his right shoulder.

While trying to position a broken Drisco line for weld repair, another weld downstream broke, causing both lines to come together, and pinching the employee's right hand between. The result was a laceration which required stitches.

Got blisters on his feet while working on hot sand in the screenhouse storage bldg. This will amend the 7000-1 filed on 8/1/08.

Employee cut his left thumb while using a shovel with a split handle. He was taken to the emergency room and was given nine stitches to close the wound.

Climbing off 16G motor grader, ladder collapsed, EE fell 5' to the ground. EE received fracture to left ankle/heel.

Employee walked into a 4" metal drain line under a belt conveyor on the way to work, suffering a minor injury to the forehead.

Alleged occupational illness

An over the road truck driver found unconscious in a porta-john.

employee attempted to step out of office, stumbled and fell, fracturing a bone in his foot.

DRIVING DUMP TRUCK IN PIT, TRUCK SEAT COLLAPSED, EMPLOYEE BOUNCED UP AND BACK DOWN CAUSING HEADACHE AND BACKACHE.

WHILE PARTNER HELD ONE END OF 1/4"X4"X10' PLATE, EMPLOYEE ATTEMPTED TO WELD THE OTHER ENT TO THE VS-01 SUMP. WHEN THE PARTNER DROPPED HIS ENT, EMPLOYEES THUMB WAS CAUGHT BETWEEN THE PLATE AND THE SUMP, PINCHINGT IT AT THE KNUCKLE

EMPLOYEE ATTEMPTED TO TURN THE WET PROCESSING MONITOR FROM ONE PILE TO ANOTHER WHEN THE MONITOR TIPPED OVER TOWARDS HIM, CAUSING HIM TO LOSE HIS BALANCE & FALL.

Employee (wet processing operator) had shut a pump off and proceeded to the employee's breakroom when he stumbled on a concrete step leading to the breakroom. This slip-trip-fall resulted in a soreness in employee's right shoulder, right arm and right knee.

Employee was wearing safety glasses and spraying a chemical substance/penetrant in order to loosen bolting material. the wind blew the chemical into employee's face and irritated the right eye. per msds the eye was flushed thoroughly with water and the employee was sent to the emergency room for treatment.

CLIMBING INTO HEAVY EQUIPMENT BLADE, GRABBED HANDRAIL TO PULL HIMSELF UP & HE SUFFERED A SHARP PAIN IN HIS LEFT HAND & FOREARM.

EE WAS CLEARING BRUSH AND VEGETATION AROUND DIESEL TANKS AND ALTHOUGH WEARING PROTECTIVE COVERALLS. CONTRACTED POISON IVY ON NECK AND FOREARMS.

EMPLOYEE WAS WORKING WITH A CO-WORKER ON THE HANDRAILS BY THE BC02 HEAD PULLEY ON A WINDY DAY. ALTHOUGH HE WAS WEARING HIS SAFETY GLASSES, DUST OR DEBRIS BLEW INTO HIS RIGHT EYE (CORNEAL ABRASION). NO DEBRIS WAS FOUND BUT HIS EYE WAS SCRATCHED.

EMPLOYEE SLIPPED AND FELL SPRAINING THE MIDDLE FINGER ON HIS RIGHT HAND.

EE SLIPPED AND FELL WHILE WORKING AROUND THE WET SUMP. LEG AND BACK PAIN OF UNKNOWN SEVERITY DUE TO INJURY.

EE WAS WALKING ON EARTH EMBANKMENT SURROUNDING DIESEL TANKS. EE SLIPPED, AND FELL DUE TO THE SLOPE AND ICE ON THE GROUND. HE DID NOT GO TO THE DR UNTIL 3-3-03. AT THAT TIME HE WAS DIAGNOSED WI TH A HERNIATED DISK.

EE WAS DIGONSOED WITH CARPAL TUNNEL SYNDROME. HE WAS INFORMED THAT SURGERY WAS THE ONLY TREATMENT. THE DATE OF THE ACCIDENT REFERS TO THE DATE OF HIS SURGERY AND THE BEGINNING OF THE LOST TIME . CONTIBITING CONSITION AND REPETITIVE MOTION. QUANTIFY IMPAIRMENT AND UNCERTAIN.

WHILE CHANGING TIRE ON HAUL TRUCK, THE RIM FELL ON RIGHT THUMB & CUT IT.

EE WAS STEPPING DOWN FROM THE PAD AROUND THE 800TON TANK. HE WAS CARRYING AN ARMLOAD OF TOOLS AT THE TIME. THE EXTRA WEIGHT EE HAD IN HIS ARMS CAUSED A STRAIN ON HIS LOWER BACK. THIS WAS REPOR TED, BUT EE FELT THERE WAS NO NEED TO SEE A DR.

EMPLOYEE WAS TRANSFERRING GREASE FROM ONE 55 GALLON DRUM TO ANOTHER. HE STRAINED HIS RIGHT ELBOWWHILE PERFORMING THIS TASK.

EE WAS REMOVING A PIECE OF UNUSED ELECTRICAL WIRE STUCK BETWEEN 2 GUARDS HE WAS INSTALLING. THE WIRE WAS STUCK SO THAT HE CONTINUED TO PULL HARDER EACH TIME UNTIL THE WIRE WAS DISLODGED. AS IT BECAME DISLODGED, HIS SAFETY GLASSES FELL OFF OFHIS FACE. THAT TIME HE PULLED BACK TOWARD HIS FACE PUSHING THE WIRE IN HIS EYE.

WHILE WORKING IN A GREASY WORK AREA WITH GREASY TOOLS, EMPLOYEE WAS ATTEMPTING TO REMOVE A DRIVE CHAIN PIN WHEN HIS HAMMER SLIPPED STRIKING HIS FINGERS ON LEFT HAND.

EMPLOYEE WAS LIFTED UP IN A MANLIFT BASKET TO WORK ON A BUCKET ELEVATOR, HE WAS CLIMBING OUT OF BASKET OVER THE HANDRAILS TO ACCESS PLATFORM TO BUCKET ELEVATOR WHEN HE FELT A POP IN HIS RIG HT WRIST.
