EE was in the process of cleaning TK96/97 bulk scale with water hose when he stumbled and felt a slight muscle pull in his lower back. After task was completed he parked the water truck and started to exit the truck when he experienced pain in his lower back that restricted further movement. The process of twisting to exit truck caused the resulting sensation of pain.

Employee was climbing ladder at TK13 with a 45 degree angle pipe on shoulder when employee lost contact with 4th or 5th rung causing him to fall to ground level about 4 to 5 feet. Employee received a laceration to right arm at the elbow and minor abrasions to his back. Employee also received a minor cut to crown of head.

Employee knee gave out as he was navigating around a pallet. Employee had a pre-existing condition that may have caused his knee to fail .
Employee was splicing telephone wire and employee cut his left hand and required 4 sutures. Employee was wearing gloves and was using the right knife for the job a splicing knife. Employee stated that the knife came loose of the splice and contacted his lift hand.

Employee was doing pre-shift inspection on the Road Grader. The employee climbed backward down (normal routine) off the Grader, placed his right foot first on the ground, turned to place his left foot down and twisted the ankle on the right foot.The Grader was parked on level ground, but the ground surface was somewhat uneven. No lost time from work.
While employee was bagging 50 # bags of material at bagging station he strain stomach muscle.

Laceration of wrist - small cut.
Loader tripped over open hatch while in process of retrieving load out ramp. Injury to right knee and lower back.
Dump truck was crossing a railroad track inside the plant. A heavy rain had washed out the grade crossing site. The site was covered with standing water. It was rough enough to jerk the steering wheel from the driver's hands, causing the driver to sprain her left thumb. Driver placed on light duty - no driving.

EE was running the M200 bag station and was also training a new employee on the bagging process. EE had placed about 12 bags on the pallet and was up to layer #5 of the pallet when he felt a sharp pain in his chest just below his sternum. (Note the bags weigh 50 lbs. each). (Physician's diagnosis was back strain.)

Employee was loading bulk trailer when driver removed chocks and stop sign. Employee jumped so he would not get crushed by ramp and hit the safety rail on trailer, causing a puncture wound to his right temple area and right thigh area. Employee received 3 to 4 stitches on each wound.

EE WAS CLEANING THE MEN'S ROOM WHEN SOMEONE CAME TO THE FRONT WINDOW AND RANG THE BELL. EE HURRIED OUT OF THE MEN'S ROOM INTO THE HALLWAY, MAKING HER WAY TO THE FRONT OFFICE TO ANSWER THE BELL . HER FEET GOT TANGLED AMONG SEVERAL PLASTIC TRASH BAGS THAT SHE WAS CARRYING AS SHE WAS BACKING OUT OF REST ROOM, CAUSING HER TO TRIP OVER TRASH BAGS AND TO FALL TO THE FLOOR.
EE HAD CLEANED OUT ELEVATOR ROCK CHUTE AND TURNED THE ELEVATOR BACK ON THE ENSURE THE PIPE WAS CLEAR. ONCE THE CHUTE WAS CONFIRMED TO BE CLEAN, THE COVER WAS LOWERED BACK INTO PLACE AND APPREN TLY THE CORNER OF THE LID CAME IN CONTACT WITH AN ELEVATOR BUCKET CAUSING THE COVER TO KICK BACK OPEN. THE KICKBACK OF THE LID STRUCK THE EE ON THE JAW AREA.
EE ALLEGES HE HURT HIMSELF WHEN HE WAS TRANSPORTING BAGS FROM UNIT 2 TO UNIT 3 ON A FORKLIFT ON FRIDAY, 2-21-03. DURING TRANSPORT, THE EE ALLEGES HE HIT A POTHOLE THAT JARRED HIM, AGGRAVATING HIS LOWER BACK. ONCE HE TRANSPORTED THE BAGS, HEHELPED A FELLOW EE SACK 1 1/2 PALLETS AND THEN HELPED AN OPERATOR LOAD A TRUCK. HE THEN COMPLETED HIS SHIFT DOING SUPER SAKS, WHICH REQUIRES LIF

AFTER DUMPING A LOAD OF ROCK IN HOPPER A ROCK WAS BETWEEN TAILGATE AND BED. THE DRIVER TRIED TO KNOCK THE ROCK OUT. WHEN IT FELL OUT HIS FINGER TOP WAS CAUGHT BETWEEN TAILGATE AND BED, BREAKIN G BONE IN TIP OF FINGER.
