Employee was taking the cap off the radiator on a 12" pump when the water overflowed, causing a burn to his chest.
Employee was getting off loader. His foot slipped and this places all his weight on his arm. This caused a pull in his shoulder.
EMPLOYEE WAS PUTTING FUEL IN PUMP, HE TURNED & HIS BACK WENT OUT.
DRIVER GOT OUT OF HIS TRUCK AND FELL FROM THE RUNNINGBOARD. HE BROKE HIS HEEL. THIS DRIVER DROVE FOR AN INDEPENDENT TRUCK CO. HE WAS NOT ANEMPLOYEE OF THE PIT, ROSE CITY SAND CORPORATION.
EE WAS OPENING THE CAB DOOR ON A FRT END LOADER MACHINE WHILE OPENING THE DOOR, HE RUPUTRED THE TENDON IN HIS LEFT SHOULDER
2 MEN WERE ATTEMPTING TO REPRIME A 6" WATER PUMP THAT HAD LOST ITS PRIME AND GOTTEN HOT. WHEN THEY OPENED THE PRIMING PORT HOT WATER SPRAYED OUT RESULTING IN 1 ST AND 2 ND DEGREE BURNS TO ONE MANS RIGHT LEG.
EMPLOYEE INFORMED MANAGEMENT HE WAS HAVING SURGERY JAN 13, 2000. THIS WAS THE FIRST TIME ANYONE WAS AWARE HE INJURED HIMSELF. NOBODY KNEW WHEN HE INJURED HIMSELF.
