Employee struck left hand with mallet while bricking the kiln.

Using a hammer to release some frozen coal when unloading on the Grizzly, EE misjudged EE's aim smashing EE's pinky finger with the hammer. The impact resulted in an open tuft fracture of the right hand 5th digit.

Production tech was completing preheater tower cleaning, tech was using a 6 foot poke bar with a T handle to remove buildup from the riser, as the tech pushed poke bar through the poke door, left middle finger was smashed between the T handle and the riser poke door frame.

Employee was tightening a rope seal using a pair of channel lock pliers. EE was pulling on the rope towards self. The pliers slipped off the rope and EE hit self in the mouth with the pliers chipping a couple of teeth.

Mechanic was sliding the forks on a forklift when mechanic felt a pop in elbow. Mechanic was treated for a strain by healthcare. Later, when minor pain was still persisting, mechanic went for an MRI. It was found that mechanic had a tear in left distal bicep. Mechanic went in for surgery repair 10/11/2021.

EE started feeling ill and went home just before midnight. EE tested positive for Covid 19 the next afternoon. EE had been working with an employee who also tested positive.

EE was changing out the bucket cutting edge on the 966 loader. While using an air gun to remove the nut from the carriage bolt, the partially loose bolt slid down just enough to turn, spun and the sharp edge sliced through EE's glove causing a small laceration. 3 stitches.

Employee was transferring 30lb belt calibration weights from a pickup bed to storage. One of the weights fell on the upper portion of left foot between the steel toe and the ankle. Employee felt it was just a bruise. Employee continued working the next few days. Finally convinced them to get an x ray. Fractured bone in foot.

Employee was removing steel plate from the raw mill chute. After cutting plate employee manually tried to remove it. The weight of the metal was more than expected. It fell 5 feet onto employee's right toe right behind the steel toe of the boot. It dislocated the tip of the pinky toe and required 11 stitches to close lacerations.

Employee was removing a drain plug from a hydraulic reservoir using a 1/2 Snap On breaker bar. The plug was in tight and EE gave it a little extra truck and felt a pop in EE's upper shoulder. EE was taken to a Work Med clinic where they prescribed Celebrex (allergic to ibuprofen) and then put EE on light duty.

Employee was jackhammering material through a poke hole on the tertiary elbow pipe. A chunk of material came loose and fell on the bit of the jackhammer forcing the hammering up, pinning employee's hand against the top of the door causing a fracture of the navicular bone of the left wrist.

A piece of refractory fell and hit the employee on the left side. The piece caused injuries to the employees shoulder, ribs, and leg.

Employee was cleaning the feedshelf in tower area and got dust in his eyes. He flushed immediately and reported. It was treated as a first aid. 2 days later, on his day off, it was bothering him. He went to doc and they removed a spec of grit from his eye. HE DID NOT REPORT TO MGMT THAT HE HAD GONE BACK TO THE DOC UNTIL TODAY 11/7/2011

Employees were trying to dislodge an oversize rock that was stuck in the crusher hopper. They had opened an 18" work door to jackhammer the rock. After 4 hours of work, a smaller baseball size rock fell on to the crusher reel and the spinning motion sent the rock flying out the access door striking the employee in the face/nose. INJURY WAS TREATED AS FIRST AID UNTIL 10/25

Employee was changingout ripper tooth on D11 dozer. He struck the tooth with a 4 lb sledge hammer. A piece of metal came off the tooth stricking him in the right calf. A small chunck of metal (1/4 size of a dime). Doctor removed from leg and put in 2 stitches.

Employees were removing liners from ball mill. Employees were using metal wedge and sledge hammer to break liners loose. Upon hitting liner a piece of metal broke off (1" X 1/4") and stuck in miners thigh. 5 stitches.

Employee were removing roll from coal mill. He working out of manlift inadvertently knocked a 1" drive socket out of the basket. It fell 15' striking employee below in safety glasses. Pressure from the strike, pushed glasses into cheek causing laceration. 5 stitches.

EE wsa breaking up material in bin with a poke bar. Struck hand on side of bin. Laceration between middle and index finger, 4 sutures.

Employee was gathering samples from different locations in the plant driving a Polaris Ranger. She bumped into a curb and fell out the side and sustained a laceration to the back of her head, requiring a few sutures.

Employee was cleaning buildup in the preheater tower feedshelf area with jackhammer. When the buildup fell off the jackhammer slid forward pinching finger between housing.

EE was changing cutting edge to loader bucket of 922 Cat. While loosening stove bolt, it slipped out of square hole, spun and cut EE's L index finger.

EE was barring plugged up material in a bin and smashed his finger between bar and bin.

Employee was cleaning buildup at 291 BC3 head pulley. Smashed finger between bar and concrete - 6 stitches.

Emplyee was working on window mounted AC unit. While repairing the fan blade and balancing it several times, it hit his L index finger causing small laceration.

Employee was cleaning the feed shelf with a 6' metal bar. He banged his hand against the feed pipe while cleaning and mashed his thumb.

EE was cleaning the feed shelf. He smashed his left little finger between the bar & housing, sustaining a laceration.

Employee was changing out blow pipes in baghouse. He was using a pipe wrench to loosen the bars. The wrench slipped smashing his left middle finger. 7 stitches.

EE was walking to area to work. When he got on the shady side of the bldg., he slipped in some mud. When he reached to catch himself he struck his hand on an I-beam causing hairline fracture requiring 2 stitches.

Employee and 2 coworkers were pulling and pushing steel plate. Employee's right hand was between the wall and plate. The 2 coworkers pushed the steel plate which placed employee's right index and middle fingers between the wall and plate, crushing his fingers. Tips of fingers cut, bones fractured, but no amputation of bone.

Employee was working in kiln and got some grit in his left eye.

Employee was changing out dust collector bags. The pipe wrench was used to loosen the holding bars. It slipped under pressure and mashed his left ring finger. Stitches were required.

EE was changing a proximity switch and did not lock out the flapper arm. got his thumb in between arm and bin. It pulled the thumb nail off.

EE WAS DRIVING A 1978 WABCO WATER TRUCK AND WAS UNABLE TO STOP WHEN THE ENGINE QUIT. THE UNIT TRAVELED 300 FEET AND STOPPED ON A BERM. THE WATER SHIFTED AND THE TRUCK ROLLED 3/4. EE RECEIVED 4 BROKEN AND 3 CRACKED RIBS. (EE WAS NOT THROWN FROM VEHICLE.)

EMPLOYEE WAS WALKING DOWN STAIRS, SLIPPED ON THE BOTTOM STEP AND TWISTED HIS FOOT - HAIRLINE FRACTURE, LITTLE TOE. THE EMPLOYEE RETURNED TO WORK FULL DUTY WITH NO DOCTOR RESTRICTIONS.

EMPLOYEE WAS CLEANING A BIN WITH A METAL BAR. MATERIAL FROM ABOVE FELL STRIKING THE BAR AND THE LEVERAGE PINCHED EMPLOYEE'S HAND AGAINST BIN. LACERATION AND TENDON DAMAGE TO RH MID FINGER.

LOWERING RAMP INTO THE KILN- COME ALONG HANDLE SWUNG AROUND AND STRUCK EE RIGHT HAND AND CRACKEDA BONE.

WHILE REPAIRING GUARD, EMPLOYEE CAUGHT HIS LEFT ELBOW ON EXPANDED METAL MATERIAL. REQUIRING 5 STITCHES.

WHILE REMOVING LARGE AIRSLIDE SECTION FROM BLENDER, AIRSLIDE SHIFTED (FELL) AND TORE-SLICED EE'SLEFT THUMB. REQUIRED APPROX 15 STITCHES TO REPAIR LACERATION.

PLUMBING: TIGHTENING A PIECE OF PIPE WHEN THE PIPE WRENCH SLIPPED AND STRUCK HIS HAND, CAUSINGA SMALL CUT.

WHILE CARRYING TRASH OUT TO DUMPSTER, EE SLIPPED ON THE ICE AND TWISTED HIS LEFT KNEE.

HE STEPPED ON SNOW COVERED STEEL, SLIPPED AND FELL, TWISTING HIS LEFT KNEE. HE CONTINUED TO WORK PERFORMING HIS NORMAL WORK UNTIL HE HAD ORTHOSCOPIC SURGERY ON 2-22-01.

LIFTING HINGED METAL DOOR 2 X 3' TO MEASURE MATERIAL IN BIN. DOOR SLIPPED AND PINCHED LH FINGERS CAUSING HAIRLINE CRACK TO LH MIDDLE FINGER.

DEMOLITION OF REINFORCED CONCRETE WALL. EE IN BOOM LIFT TO REACH AND TORCH CUT HANGING REBAR AND CONCRETE. HAND WAS FRACTURED WHEN IT WAS PINNED AFTER REBAR WAS TORCHED AND SWUNG BACK INTO THE BASKET STRIKING HAND.

WHILE LOADING FRONT TRAILER, THERE WAS A SPILL; DRIVER TOLD BULK LOADER HE WOULD CLEAN IT UP WHILE BULK LOADER FILLED THE PUP; LOADER DID NOTHEAR THE DRIVER. DRIVER PULLED AHEAD AND THE LOADER, WHO WAS IN THE SAFETY CAGE, FELL WHEN THE TRAILER PULLED AHEAD; HE LANDED ON FENDER, BREAKING & DISLOCATING HIS LEFT SHOULDER, THEN FELL TO THE CONCRETE FLOOR BUMPING HIS HEAD.

WHILE CLEANING THE ALKALI BYPASS WITH A BAR, THEEMPLOYEE HIT HIS HAND ON THE HOUSING COVER; THISCUT HIS RING FINGER.

REPLACING FAN BELTS. EQUIPMENT WAS LOCKED OUT. A 36" PIPE WRENCH WAS USED ON THE SHAFT. DRAFT CAUSED THE FAN TO ROTATE BACKWARDS, THE WRENCH SLIPPED OFF AND THE EMPLOYEE TRIED TO PULL THE WRE NCH OUT. IT CAUGHT ON THE SHAFT, FLIPPED OVER AND STRUCK HIS LEFT FOREARM.

EE READED IN TO AN OPEN WELDING ROD CONTAINER THAT HAD RAW EDGES. HE SLICED OPEN THE TOP OF HIS MIDDLE FINGER, REQUIRING 10 STITCHES.
