TIGHTENING A BOLT ON THE 125C CLARIC LOADER AND USING A CHEATER PIPE ON THE WRENCH, THE PIPE SLIPPED AND HIT EE BELOW THE LEFT EYE.

HE WAS DOWN FILLING THE WATER TRUCK, HE WENT TO STEP DOWN OFF OF TRUCK AND SLIPPED IN THE MUD.

He was lifting 100 lb. bags and stacking them on a pallet.
