the clamps on a 2" air line failed and the whip check did not properly arrest the hose and it struck a miner in the left ankle causing a broken leg.

While attaching an air line for a Drill the miner was tightening up the 2.5" line when they tapped the ears on the fitting with a small hammer the miner did not notice that the valve was cracked and the valve separated and struck the miner in the face neck and chest.
