The employee was pushing a shovel when EE's hand began to burn and EE was unable to fully move EE's right wrist. The employee was taken to the ER where EE was examined and told to follow up with an orthopedic.

The injured employee was throwing away scrap metal (metal, rollers, etc.) into a dumpster. The employee had the truck parked next to the dumpster. When they went to throw something in, they said they "felt a pop in their back". The employee was taken to the hospital for examination. The doctor stated it was back spasms.
Employee went for annual hearing test, showed hearing loss, was re-tested and continued to show a hearing loss over 25 dB.
The injured employee was assisting on stemming a shot by raking material from an excavator bucket into the holes. EE claims EE's right shoulder popped. EE continued working until the job was complete. EE then got in a haul truck to move material and said while EE was being loaded, the truck shook and EE's shoulder began hurting more.
The injured employee was assisting to unstop the box above the 5 1/2 crusher. The material shifted and a rock spilled out of the box striking the employee on the hard hat and shoulder. The employee was taken to the hospital where they were examined. It was determined the employee had a concussion and shoulder contusion.
A contractor employee (NBD Industrial Services LLC - MSHA ID # C6519), was installing a wedge into the crusher by using a hammer to drive it in. The hammer head slipped off the wedge, causing EE's left index finger to get caught between the hammer handle and a beam. EE was taken to the ER where EE was treated for a finger fracture and laceration.

Employee was palletizing product, while picking up a bag of product EE's finger became caught between the belt and roller. The employee pulled hand free and removed the glove and saw the injury. The employee was treated on site and then taken to the hospital for further treatment and examinations. Left hand, middle finger fracture and cut.
Employee was working on a haul truck, once finished employee was carrying tools in one hand & pushing the oil pan with the other. The employee lost balance & tripped landing on right elbow. The employee was taken to the hospital where employee was examined. The employee was advised to rest, ice, & elevate. Employee was placed on light duty until 6/18/23. No broken bones from xray.

The employee was attempting to remove a feed hopper from a crusher. The employee was attempting to break it free by taping it with a hammer. The hopper broke free and sprung up pinching EE's hand between the feeder. The employee obtained lacerations to EE's left index finger and left palm.

Employee was working on a flatbed trailer in the garage. Employee was removing the air spring, it fell, employee reached to catch it, left ring finger was pinched between the air spring and a piece of metal. Cut/broke finger.
Employee was operating a battery powered drill when the bit stopped, and the drill twisted EE's right hand, hurting the wrist area.

Contract, over-the-road driver fell from their trailer while tarping the load. Driver was tied off, but as driver fell driver scraped right lower leg on truck or catwalk, had a puncture wound on the left lower leg possibly by truck, catwalk, or a bungee cord being used to secure tarp, and scratched left elbow on the truck or catwalk.

Employee was making sure that a tie-down strap was secure to truck. When ee pulled on the strap, it was loose, ee lost balance and fell. As ee fell the strap caught and pulled arm. This caused rotator cuff on right shoulder to tear.
Employee was painting outside of hopper when they lost balance and fell. The fall restraint lanyard expanded to slow the fall. Then the tie off point gave way allowing them to impact the ground causing contusion to right elbow, and sprain the right knee.
Employee placed hand between clamp on bagging machine. Clamp closed on finger, causing crush/laceration injury to right index finger.

Employee was leaving mill building to retrieve some tools to perform maintenance tasks. Employee stepped from walkway to temporary solid block unit step, the block rolled, employee twisted an ankle and fell.

Employee was opening a pallet of super bags by cutting the plastic band with EE's pocket knife. When the knife cut through the strap, it then punctured EE's left arm.
EE picked up a pallet and started to twist around and lay it on another pile when EE felt a pop in EE's back.
Employee was bagging dust and occasionally a bag will split. Employee said this particular bag burst and the dust blew up under EE's safety glasses.
EE began EE's shift at 3:30 p.m. on 1/11/16. Performing normal duties under normal conditions. EE started out shrink wrapping material. Then EE was going to begin loading rock dust into bags. EE bent over to pick up an empty bag, when EE said EE felt a pop/pull in EE's groin.
