A miner was changing the bucket teeth on a loader. When the miner was striking the tooth for removal, a piece of metal broke off and embedded in the right leg, which required removal.

A miner reported back pain the day after was tasked with shoveling material. First aid treatment was administered on site and EE declined to see a medical professional or go to the clinic. *On 6/15/2022, the EE called off work and said was going to the ER. The employee did not return to work.

A miner received a minor burn from slag during welding activities. First aid treatment was administered on site and EE declined to see a medical professional or go to the clinic. Based upon information communicated by the inspector 07/07/2022, medical treatment may have been rendered. This incident remains under investigation.

Employee's finger was caught between the reaction arm of a hydraulic torque wrench and the frame of the jaw crusher causing a laceration.

Working to dislodge blocked material miner injured back.

At approximately 7:20 a.m. on Tuesday, May 13, 2014, an electrician was in the process of pulling wire through a 3/4" electrical conduit from a junction box to the belt scales. As he pulled, his left foot slipped and he started to fall but was able to catch himself and regain his footing, but he twisted as he did, and he felt a pop in his back.

Employee reported severe pains in upper stomach / lower chest area. He had been operating a skid steer prior to feeling bad. He was taken to the hospital by ambulance. It was determined that he had a bad reaction to medication, Predazone, a medicine his family doctor had given him for poison ivy/oak. Employee felt he got the poison ivy/oak from his dog.

While on the rock breaker in the pit breaking rock, the employee broke through a large rock and in doing so the machine shook some dust off the ceiling of the excavator and it drifted down behind his safety glasses and into his eye.

While replacing the oil back into the lube tank on the 4 1/4 crusher with an air pump, the discharge end slipped out of the tank and splashed oil on his face which got into his left eye.

Holes loaded and pit was cleared for blasting. When connection was made to the blasting machine and prior to pressing the firing button, holes fired prematurely. No injuries to any miners.

Air grease gun had pin hole occur in rubber grease line, injecting grease into right index finger.

Employee was operating his haul truck and parked at muck pile. A 114 inch rock rolled down face of the muck pile striking the back of the truck jolting the driver and causing his back to tighten up.

WHILE OPERATING SKID STEER, EE HIT HIS ARM ON THE WINDOW BRACE.
