#7 Prep Plant Coal

EE was on ladder against I beam that was attached to structure to be demo. EE was attaching a choker chain from crane when the beam/ladder began to fall. EE grabbed the chain and left little finger was caught between the chain and the beam. EE was approx 4' high, structure and ladder was at ground level. Finger tip has linear fracture, 2 sutures to hold nail in place.

A contracted coal truck mechanic broke a bone in right foot when the truck that the mechanic parked at the new refuse bin rolled back in to the mechanic and knocked the mechanic down. The brakes were checked by both a company inspector and an MSHA inspector with no deficiencies found. The driver apparently did not have the vehicle in "Park".

The R4 Refuse bin collapsed on contractor truck while it was being loaded from above, which prevented the driver from exiting the vehicle for approximately twenty minutes.

The employee was wiping off a glass beaker after having washed it out. Employee cut right hand ring finger on the top of the beaker due to an unnoticed crack in the glass. The cut required stitches.

Tightening wheel on truck and ratchet slipped hitting employee in the eye causing employee to need 6 stitches.

As employee was exiting a 980G loader he slipped on the bottom step and fell to the ground (approximately 2 feet) and landed on his right shoulder. Employee continued to work and due to complications he started missing work on 11-18- 2009.

Stocked coal was observed smoking. While moving the smoking coal it started blazing.

Employee got the index finger on the left hand cut when he stuck it in a hole in a coal chute as a co-worker was prying with a 3/4 inch pipe trying to unstop the chute. The pipe caught his finger between the edge of the hole and the pipe. It took 5 suters to close the cut.

Employee was getting a coal sample from a coal pile by shoveling coal into a sample bag when he felt a sharp pain in his lower back.

Employee was cutting a rubber water hose to hook to a water spray bar and the knife slipped and the blade hit him on the left thumb of his hand.

EE HAD HELPED HEAT A SHAFT TO REMOVE A STRUCK GEAR ONT HE CRUSHER ON THE FLOOR BETWEEN THE 1ST AND 2ND FLOORS OF THE PLANT. EE WAS RETURNING TO THE WORK SITE WITH A GRINDER WHEN HE LOST HIS BA LANCE AND FELL HITTING THE HEATED SHAFT WITH HISRIGHT HAND.

EE WAS WORKING ON A GUARD ON A BELT TAIL ROLLER WHEN HE CAME IN CONTACT WITH A PIECE OF METAL AND CUT HIS FOREARM.

EE WAS AT THE ROTARY BREAKER WHEN HE NOTICED ROCK ON A BELT. THE ROCK WAS BEHIND THE TAILPIECE HE WENT INTO AREA & REMOVED ROCK AND WHEN HE RAIESED UP FROM BENT POSITION, HE STRUCK A IBEAM WITH HIS FOREHEAD Y
