Employee slipped and fell hitting their elbow and arm resulting a sprain/sprain on 1/15/25. The employee continued to work normally until they were later taken out of work by a physician on 1/30/25.

On 9/25/24 employee was lifting a 55-pound bag of flobeads (Dry Cat) when EE felt a sharp pain in their back. Employee continued to work until EE was taken out of work by a physician on 10/22/24.
Employee was conducting repairs on the bird cage/agitator section of the centrifuge when the bird cage rotated and pinched the employee's fingers resulting in lacerations. The employee received sutures to 2 fingers as a result of the injury.

Employee was using a handheld chain ratchet set to hold one side of a pre-wet shaker screen as EE was changing a feed end spring on the shaker. In the process of changing the spring, the hook broke on the ratchet set resulting in the employee's hand to get caught between the spring.

Employee took the first step from the 5th floor to the 4th and his right knee popped. This resulted in pain on the inside of his right knee area. He continued to work up until he was taken off by a doctor on 8/6/13.

Employee was bringing a 5 gallon bucket from lower steps to set on steps above. When he was doing so he felt pain in his lower back area. He continued to work until he went to the doctor on 4/17/12 and was taken off work.

Climbing up ladder to access wier plates to change out wier plates. Ladder was tied off at top, but employee's boots were slippery due to conditions. Ladder shifted slightly and boots slipped causing him to fall against the inside edge of a wier plate at same elevation.

Three minutes after the shift started, EE was putting his lunch bucket and a set of torch hoses and gauges into the back of a pick-up truck to take to the worksite and felt a sudden pain in his back, about the middle.

A surface belt fire occurred October 12, 2006 involving a conveyor belt located on the Coal Creek Prep Plant refuse area. The fire was discovered at approximately 6:15 am when the dozer operator entered his work area. The number 8 refuse belt sustained 220' of damage and the number 7 refuse belt sustained 410' of damage. The cause of the fire is unknown at this time.

WHILE REPLACING A RETURN ROLLER ON THE #3 REFUSE BELT, THE EMPLOYEE FELT PAIN IN HIS KNEE & IT LOCKED UP. THIS HAS HAPPENED ON NUMEROUS OCCASIONS WITHIN THE PAST YEAR. EACH TIME, HE WAS ABLE T O POP HIS KNEE BACK IN PLACE. THIS TIME HE COULD NOT, REQUIRING MEDICAL ATTENTION.

EMPLOYEE WAS PULLING A CABLE DOWN THE CATWALK OF STACKER BELT AND STEPPED OFF THE END OF THE WALKWAY AND FELL ON HIS BACK. HE FELL APPROX. 5'. EMPLOYEE WENT TO A SPECIALIST ON MONDAY AND RETUR NED TO WORK.
