55061S LOADING STEEL RAIL ONTO TRACK BUS WHEN HE LIFTED SECOND RAIL ONTO THE TRACK BUS HE SAID HE PULLED SOMETHING IN THE LOWER PART OF HIS BACK. DR SAID HE HAD STRAINED MUSCLE AND LOCKED MUSC 35?00
EE WAS BOLTING TOP IN THE NO. 1 ENTRY FACE AREA OF THE 001 UNIT WHEN A SMALL SCALE OF ROCK FELL BETWEEN THE LAST BOLT AND ATRS JACK HIT HIM ON LEFT SIDE OF THE HEAD THE ROCK WAS 1 1/2 X 1 1/2 DIMAETER AND 1 1/2 THICK DR SUTHER LAND SAID HE HAD A LIGHT CONCUSSION AND NERVE PINCHED IN NECK LEFT SIDE OF HEAD.
A ROCK WHICH IS CALLED A SANDSTONE HORSEBACK, WHICH WAS 20' WIDE, 5' THICK AND 65 TO 70 FEET LONG, FELL ON THE NO.3 BELTLINE 13 BREAKS ABOVE SPAD NO.1910. THE AREA HAD BEEN CHECKED 3 HAS PRIOR & DID NOT DETECTED ANY FAULTY ROOF.
I.NAME UNLOADING A BELT ROLLER OFF 3-WHEELER CAUGHT LEFT HEEL ON THE EDGE OF THE TRACK RAIL CAUSING STUMBLE WITH THE BELT ROLLER AND STRAIN LIGAMENT IN LEFT KNEE.THIS HAPPENED AT THE END OF TH E TRACK ON 001 UNIT.
INJURED WAS BOLTING BAD TOP IN THE NO 3 ENTRY OF THE 001 SECTION WHEN HIS CAP LIGHT GOT DIM HE MOVED BACK ON BOLTER AND REACHED ACROSS CONTROLS LOOKING TOWARD FACE TO PUT UP BOLT.HE HAD HIS RI GHT HAND AT BOOM AND LEFT HAND ON CONTROLS.HE LET BOOM DOWN ON HIS RIGHT HAND BRUISING THROUGH RIGHT FINGERS.

INJURED WAS UNLOADING A TOOL BOX OFF OF HIS TRUCK ONTO A MANTRIP BUS IN THE SHOP WHEN HIS HAND HOLD SLIPPED CAUSING HIM TO MASH HIS RIGHT RING FINGER.
RIDING OUT OF MANTRIP WENT AROUND CURVE PULLE D MUSCLE LYING ON BACK
EE SAID A RIB FELLIN THE FACE OF THE 001 SECTION AN NO. 1 EWNTRY THE PIECE OF COAL ROLLED OFF HITTING HIM ON RIGHT HIPP. HE WAS OPERATING A SHUTTLE CAR.
ABOUT 10:00 PM WAS HELPING FOREMAN MOVE RAIL BY HAND. WHEN THEY MOVED RAIL HE MASHED HIT RIGHT HAND MAYBE FRACTURING THAT HAND. THIS HAPPENED ON AREA NEAR 001-002 SECTION ON TRACK.
HE WAS WORKING ON THE BELT AT THE NO.6 TAILPIECE. WHEN THE TWO WERE WORKING ON A SPLICE THAT WAS TORN. IT CAME ON HITTING I.NAME ON THE LEFT LEG AND HITTING HIM ON THE LEFT LEG AND HIP. I.NAME SAID HE THOUGHT THE BELT WAS OFF. WE ASSUMED THE GOBB OFF SWITCH WAS CUT OFF BY A ROCK AT THE TAILPIECE ON THE NEXT BELT NO. 5. THE ROCK JARRED OFF LETTING THE BELT COME ON NO. 6.
EE WENT TO THE SECTION TAIL PIECE WHICH IS THE NO.6 BELT LINE. TO CHECK ON ARSPLICE TO BE MADE. HE THOUGHT HE HAD THE BELT OFF AT THE SWITCH WHEN HE AND EE WAS MAKING ARSPLICIAN IT. IT CAME ON CATCHING HIM ON THE LEFT KNEE & OTHER EMPLOYEE ON LEFT LEG. THEY FOUND THE BELT TO BE FOULED IT BROKE ITSELF LOOSE & FOUND THE BELT SWITCH TO STILL BE ON THE GOFF OFF ON THE DRIVE DISLODGED T
EMPL WAS RUNNING ROOF BOLTER ON 002 SECTION WHEN HE GOT UP FROM SIDE OF PINER TRAMMING IT TO FACE. HE WENT TO FRONT TO OPERATE IT & BUMPTED KNEE AS HE WALKED TOWARD FRONT HIT RIGHT KNEE.
EMPL WAS PRIZING ON BAR TO TIGHTEN BEARING ON OUTSIDE BELT DRIVE & TWISTED KNEE.
PRESSING DWON ON BELT DRIVE WITH SCOOP IT SLIPPED OFF TRHOWING HIS HEAD AGAINST THE TOP BUMPING HIS HEAD

HE CALLED ME AT HOME ON THE 10TH OF OCT. & SAID HE MAY HAVE TO FILE COMPENSATION HE THOUGHT HIS BACK MAY GIVE HIM TROUBLE. HIS WIFE CALLE

HE HAD NO MINER BLOCK UP WHEN HE STARTED FEEDING WEDGES AND BOUNDS UNDER CATS TO BLOCK IT UP WHILE TRAMING IT AND CAUGHT FINGERS BETWEEN BOARDS AND CAT MASHING MIDDLE AND INDEX FINGERED.
EMPL STUCK ARM UNDER PINER BOOM TO TIE UP HOSE OPERATOR DIDN'T SEE HIM & LET BOOM DOWN ON ARM. LEFT WRIST & ARM WERE CAUGHT MAY HAVE MUSCLE OR LIGAMENT DAMAGE TO WRIST OR FOREARM.
THE TOP POPPED WHEN HE REACHED TO GET A GREASE GUN TO SERVICE PIECE OF RIB FELL STRIKING EE ON LEFT UPPER SHOULDER. THIS MAN COULDNT TELL ME EXACTLY HOW ACCIDENT HAPPENED CONTUSION BRUISE AND MABYE A LIGHT SPRAIN.

MINER HELPER WAS MOVING BACK BECAUSE A PIECE OF ROCK WAS WORKING AND A SLAB OF ROCK FELL SLIDEING DOWN RIGHT LEG HITTING ON TOP OF RIGHT FT DAMAGING SOME LIGAMENT AND SMALL BONES.
BOLTER OPERATOR STARTED PINNER STEEL AND LOOSE KETTLE BOTTOM FELL OUT HITTING OPERATOR ON LEFT WRIST CAUSING LIGHT FRACTURE TO WRIST.
FOREMAN WAS MOVING SHUTTLE CAR TO LET PINNER CHANGE PLACES. PINNER OPERATOR WAS TRAMMING FORWARD AND SHUTTLE CAR OPERATOR ACCIDENTLY HIT WRONG LEVER AND MOVED FORWARD AND PINNER OPERATOR MOVED AT THE SAME TIME. THE PINNER OPERATOR WAS CAUGHT BETWEEN SHUTTLE CAR AND PINNER CAUSING LEG INJURY.
