EE was cleaning the overchute above the cone crusher and was pulling the rock away and then felt a sharp pain starting at his shoulder and ran down his arm. This incident is considered an illness, due to the fact that a previous injury occurred to the same shoulder back in 1992.

Injury strained his back while climbing a ladder. He has had previous problems with lower back strains and inflamation prior to this incident. He did not have what would be considered an accident. It was considered a strain due to his physical condition and age. The original accident occured on 7/28/09, but was not diagnosed until 8/3/09.

During inspection of dust collector baghouse, employee got dust in left eye. Employee was wearing proper PPE provised for performing this procedure. Employee was wearing half mack respirator nstead of full face respirator provided. *Accident not reported until 8/19/08.*

While climbing ladder to install swing gate, employee had sharp pain starting in left ankle and going up left leg.

While installing a haul road warning sign, employee stepped onto berm to pack dirt around the sign when he felt a sharp shooting pain in right ankle. Diagnosed with soft tissue strain or sprain to right ankle.

During replacement of defective pump employee suffered muscle strain in left lower lumbar area of back. Conditions were wet and muddy and poor design by previous owners.

Employee cut finger tip of left index finger requiring stitches while installing bulk sack on air activated slide gate.

Employee was examining bad skirting and cut his hand on the conveyor belt. During inspection of guarding and skirting on conveyor #9 (BC-09) the injured employee was directing the maintenance man to a section that needed repair by pointing his finger. The employee's finger became caught between the skirting and the conveyor belt causing a cut to his finger that required sutures.

SAFETY SUPERVISOR NOTIFIED PLANT SUPERVISOR THATEE WAS COMPLAINING OF BACK PAIN. EE WAS CONTACTED AND HE STATED HE DIDN'T KNOW WHEN IT OCCURRED OR WHAT HE WAS DOING. THIS IS AN AGGRAVATION OF A PREVIOUS INJURY.

WHILE INVESTIGATING LOUD NOISE AND EXCESSIVE VIBRATION FROM USI CRUSHER. A STEEL PLATE WELDED ON CHUTE BROKE LOOSE AND FELL STRIKING EE ON HEAD. CALL PIPE WAS IN USE AT TIME OF ACCIDENT.

MATERIAL IN HOPPER FROZE WHILE PRYER WAS BEING REPAIRED DURING THE THAWING OF HOPPER. EE WAS USING T HANDLE BAR TO BREAK UP MATERIAL. BAR BECAME STUCK AND WHEN HE PULLED ON IT, IT CAME LOOSE A ND STRUCK HIM IN CITIEST.

WHILE OPERATING THE CUTTING TORCH TO REMOVE METAL FROM MIXER, THE HOSE TO THE TORCH STARTED LEAKING AND CAUGHT ON FIRE CAUSING MINOR BURN TO RIGHT ARM.

WHILE OFF LOADING PALLETS FROM TRUCK, A PALLET ON ADJACENT STACK WAS HUNG UP ON PALLET STACK BEING REMOVED AND FELL OFF STACK AS OPERATOR PULLED AWAY FROM TRUCK. PALLET STRUCK OPERATOR ON RIGH T ARM ELBOW AREA. FORKLIFT WAS PARRALE TO TRUCK WHEN PALLET HIT FORKLIFT AND OPERATOR.

EMPLOYEE TURNED TO WALK FROM ONE AREA TO ANOTHER IN FRONT OF THE BACK TIRE OF A FORKLIFT THAT WAS PASSING BY. THE FORKLIFT RAN OVER HIS RIGHT FOOT.

EE WAS PICKING UP SOME EXPANDED METAL WHEN IT CAUGHT ON A LADDER AND CUT HIS THUMB.

EE WAS WORKING ON A DRIVE BELT AND HAD HIS RIGHT INDEX FINGER TIP PINCHED AT THE SHEAVE WITH COMPRESSION RESULTING IN SURGICAL REPAIR.

EMPLOYEE WAS CLIMBING OFF THE FORKLIFT WHEN HE SLIPPED ON HYDRAULIC FLUID ON THE FLOOR OF THE FORKLIFT. HE FELL HEADFIRST TOWARDS THE CEMENT FLOOR, CATCHING HIMSELF WITH HIS HANDS AND INJUR ING HIS LEFT HAND.

EMPLOYEE WAS WALKING OUTSIDE TO CHECK ON A SURGE TANK WHEN HE SLIPPED ON AN ICY PATCH FALLING ON HIS LOWER BACK. HE REPORTED THE ACCIDENT THE FOLLOWING AFTERNOON (1/18/00) AND DID NOT REPORT I NJURIES REQUIRING MEDICAL ATTENTION UNTIL 1-21-00.
