Employee picked up a water jug approx. quarter full and felt a strain in back. Employee went to employee's chiropractor and was placed on modified duty.

Employee was walking towards the de-watering pumps and knee buckled. EE felt pain later in the evening and went to the clinic the next day. EE was placed on modified duty for a knee strain.

Employee went to the clinic due to shoulder irritation. EE returned to work on modified duty, the doctor thinks the irritation may have been caused by an incident that occurred in 2011.

Employee was grinding with hand held grinder wearing safety glasses and face shield. The next day employee's eye felt irritated employee went to the clinic and they removed a small piece of metal, employee received ointment prescription for eye. Employee did not miss any time.

Employee was in a haul truck and hit a rut in the road, felt a strain in neck. Employee was referred to the clinic and placed on modified duty.

Employees were changing teeth on an excavator bucket. One employee was using a hammer to adjust the position of the teeth, a piece of metal hit the other employee in the head causing a small cut that received staples.

Employee complained of aching in his shoulder and the doctor put him on modified duty.

Employee was in a kneeling/squatting position while applying rail car seals while in this position employee took two steps causing a strain to his right knee which resulted in modified duty.

THIS IS NOT A UNIMIN EMPLOYEE. This report is being filed on behave of All Pro Door Company (507)546-3667. While loading equipment on their trailer, an All Pro Door EE was pushing a scissors lift forward on the trailer when his finger got pinched between the lift & trailer. He received a 3/8" cut on his pinky finger. He received 4 stitches. AGAIN THIS IS NOT A UNIMIN EE.

Employee tried to clean shavings off of a drill bit that was in a running drill press. The drill bit then got tangled with his glove and wrapped his left hand up in the drill bit thus, dislocating his left index finger.

Employee was tightening a nut with a wrench on a screen the wrench slipped off the nut causing the employee to project forward and hit his head on the frame of the screen causing a small cut on his forehead. He was taken to the clinic and received stitches.

Employee fell while climbing down from the catwalk which resulted in a compression fracture.

Employee complained about shoulder strain. Employee went to the doctor and has to attend physical therapy.

Employee was climbing onto the Shuttle wagon, as he reached the top platform area his foot slipped and he struck his left shin on the steel edge causing a laceration which resulted in stitches.

Employee was tightening a chain and was unable to turn the ratchet with his hands and instead of trying a different ratchet he attempted to use his foot to gain power as he did this he felt a strain in his back. Employee did not report incident until the next day and he then received chiropractic treatment.

Employee was driving a lull and turned a corner too sharp tipping it onto its side, employee jumped out of the cab as it was tipping which resulted in a laceration to his head and a dislocated shoulder.

Employee lifted a bucket over his head to hand to another employee and he felt a strain in his back. Employee received chiropractic treatments which included diathermy treatment.

Employee picked up a worn belt conveyor roller off of the quarry floor and the sharp edge cut through his glove cutting his thumb which resulted in stitches.

Employee used his hand to prop up the cutting edge bolt while removing the nuts and cut his finger which resulted in stitches.

Employee was using a grinder to cut a 12" piece of angled steel, when the angle started to move he removed his left hand from the handle of the grinder and proceeded to use his hand to hold the angle in place instead of a vise or clamp. At this point the grinder got bound up causing the grinder to jump and hit the employee on his left hand/thumb area leaving a laceration.

EE was welding and a piece of metal got into his eye.

Employee was removing a chain that was stuck on the mast of a lull. Employee pulled the chain out from between the mast and backstop and the mast swung back to the backstop catching employees fingers in between which resulted in 2 fractured fingers.

EE was separating girts and pulled one girt off of the top of the pile and slid it forward onto the forks of the fork lift. The girt bounced and hit the top of the EE's left forearm causing the laceration.

Employee strained his back on 8/15/2012. Employee had an MRI done 11/9/2012 and had a follow up appointment on 1/29/2013 which he returned with lifting restrictions and was placed on modified duty.

On 4/1/12 employee twisted his knee while climbing down the ladder on the front end loader. On 7/16/12 an MRI was positive showing a meniscus tear.

Employee complained of pain in elbows, forearms, and hands but was unable to pinpoint a specific incident. Dr. diagnosis is tendonitis. Employee placed on modified duty with lifting, gripping, squeezing, and push/pull restrictions.

On 2/13/12 employee slipped and twisted left knee. On 5/3/12 an MRI was positive showing a meniscus tear.

Attempting to swat a wasp the employee broke through a window cutting his right wrist requiring 6 stitches. Doctor assessment was employee was fit to return to pre-injury, regular work. No lost time.

Employee complained of lower back pain, but was unable to pin point a specific incident. Dr. Diagnosis is acute exasperation chronic low back pain. Employee placed on modified duty with lifting restrictions.

Employee sprained rib while lifting pipe clamp. Employee saw doctor on 10/9/2010 and was placed on modified duty with lifting restrictions.

Employee was shoveling sand, when he pulled the shovel back he felt a pain in his lower back. Employee went to the clinic and received a prescription and was placed on modified duty.

Employee was disassembling a guard with the help of another employee he was pulling on the guard with a jerking motion and felt a strain in his lower back. Employee went to the clinic and was placed on modified duty.

Employee was performing maintenance on a pump and he felt a strain in his back, he went to the clinic and was placed on modified duty and given prescriptions.

Employee was using a sledge hammer to loosen rock and felt strain in his lower back. Employee went to clinic and was placed on modified duty.

Employee was closing a discharge hopper door. While bending down he felt a strain in his upper groin. Employee went to Urgent Care and was placed on modified duty.

Employee lifted a torch tank into the back of a maintenance truck and strained his lower right back. Employee was seen by chiropractor and placed on modified duty for 2 days.

Employee was stepping off of platform and stepped in to I beam, rolling his ankle, which resulted in a sprain. He is placed on modified duty.

Employee was pulling pins on a bucket of a machine which requires hammering and lifting.

Employee was opening a top rail car hatch and felt a sharp pain in his back. Employee was seen by a doctor and placed on modified duty.

Employee sprained/strained right knee while performing his regular duties. Employee was taken to hospital and received a knee immobilizer, prescription and was placed on modified duty.

Employee was pressure washing 777 haul truck in preparation for scheduled maintenance. Water stream swung across employee's right foot, cutting through the leather and allowing high pressure hot water under steel toe. This resulted in the employee sustaining 2nd degree burn on right big toe.

Empoyee was servicing mobile equipment, while on his back he was using a ratchet and felt strain in his right shoulder. Employee was seen by his doctor 9/16/08 and placed on modified duty.

Employee was bending over to pick up a piece of equipment and felt a strain in his lower back.

Employee was cutting a strap with a utility knife; employee cut the 2nd knuckle of his right thumb with the blade of the utility knife. Employee received 3 stiches.

While trying to unplug a drill hole using loading poles, employee felt pain in back.

Employee sprained right ankle stepping off a step onto the screenroom floor. Reporting due to modified duty.

While driving front end loader back to plant, it fell into a hole jarring employee and bumping his head on cab. He was diagnosed as having a neck strain. Reporting due to presription medication.

Employee was removing a cutting edge bolt. He was holding the bolt head with his hand and when the impact wrench turned the nut it spun the bolt in his fingers, cutting his fingers open. Reported due to modified duty and stitches.

As EE was closing a leaky R.R. hopper door, the cooper bar slipped out of the door mechanism, dropping to the concrete, pinching his finger. Reporting due to modified duty.

As ee was preparing to lift a blowbar, it slipped off the edge, of the crusher, hitting his left foot. As a result ee sustained a contusion to his left foot. Reporting due to mod duty.

Employee strained his back moving pipe onto forklift. Reporting due to modified duty.

WHILE TRYING TO REMOVE ICE FROM THE SNUBBER PULLEY, EMPLOYEE HIT HIS FINGER WITH A HAMMER. HE DID NOT GO TO A DR. FOR TREATMENT UNTIL 7/6/05, WHEN THE FINGERNAIL REQUIRED REMOVAL.

EMPLOYEE INJURED THUMB WHILE PERFORMING MAINTENANCE ON FRONT-END LOADER. EMPLOYEE'S THUMB WAS DISLOCATED AND REQUIRED MEDICAL ATTENTION.

While looking for a handrailing in the scrap area employee stepped on unlevel ground and twisted his right ankle.

EMPLOYEE WAS SHOVELING SAND OUT OF THE DRYER WHEN HE GOT HOT SAND TRAPPED INSIDE HIS GLOVE, BURNING HIS WRIST.

While entering quarry with loader/ scraper the employee hit a hole, and bounced him hard causing a sharp pain in lower back.

Employee was in change room. he turned to wash his hands and when he turned he shifted his weight from left to right leg. when he did this his knee buckled, causing a popping noise and sharp pain. employee is scheduled for an mri to determine extent of injury.

LEFT ARM HAS BEEN GETTING STIFF AND SORE WHENEVER EMPLOYEE USES IT. (SHOVELING & STEERING FRONT-END LOADER). NOTE: ON 5-7-04 DR ADVISED NOT RUNNING FRONT-END LOADER BY EMPLOYEE.

While unloading a head pulley drive assem with forklift employee grabbed a corner to straighten it on forks and strained his back.

Employee slipped and fell while stepping off a catwalk onto concrete. employee hit his left wrist on a rock.

AS EE WAS ATTEMPTING TO MOVE A ROCK IN THE GRIZZLY BY HAND ANOTHER ROCK FROM ABOVE SUDDENLY CAME DOWN FROM FEEDER, STRIKING HIS LEFT RING FINGER. EE S FINGER WAS BROKEN AND REQUIRED STITCHES.

WHILE USING A UTILITY KNIFE TO OPEN A TUBE OF SILICONE, EMPLOYEE CUT LEFT INDEX FINGER. TWO DOCTOR VISITS - REACTION TO TETNUS SHOT.

EE'S LEFT THUMB WAS PINCHED BETWEEN THE TAILINGS LINE & A ROCK, WHILE TRYING TO MOVE ROCK.

EE WAS CHANGING OUT SCREENS, WHEN HE STARTED NOTICING SOME PAIN IN HIS KNEES. EE WAITED TO GO TO DR UNTIL 11-11-03. REPORTING DUE TO PHYSICAL THERAPY.

WHILE SHOVELING SAND IN BASEMENT OF PLANT EE STRAINED LOWER BACK.

EE HAD BEEN HAMMERING ON A PIPE TO CLEAR A PLUG AND BEGAN FEELING SORENESS IN HIS RIGHT SHOULDERWHICH BECAME WORSE OVER TIME. REPORTING DUE TO MEDICATIONS & PHYSICAL THERAPY

EMPLOYEES RIGHT KNEE BECAME SORE AND SWOLLEN AFTER WORKING ON HIS KNEES FOR 3 HOURS, CUTTING ANDREMOVING GRATING AND MOVING A SECTION OF 12" PIPE. REPORTING DUE TO PHYSICAL THERAPY AND X-RAYS.

EE WAS CUTTING BOLTS OFF LOADER WITH CUTTING TORCH WHEN HIS PANTS CAUGHT ON FIRE BURNING @ 2"DIAAREA OF SKIN ON LEFT CALF. DR TREATED IT APPLIEDOINTMENT AND BANDAGE . WILL RECHECK ON 4-14-03 TWO MEDS. PRESCRIBED AND FILLED.

WHILE EE AND CO WORKER WERE TRYING TO GET THE OLD LINER OUT OF THE VALVE, HE STRAINED HIS LEFT ELBOW. HE DEFERNED TREATMENT UNTIL 8-1-02.

THE EXCESS SHAIN FROM A CHAIN HOIST DROPPED FROM THE CATWALK. EE WAS HOLDING THE LOOSE END OF THE CHAIN AND DID NOT SEE OR HEAR THE CHAIN DROPPING. WHEN IT REACHED THE END IT PULLED HIM FORWAR D, JERKING HIS ARM AND BACK. REPORTED DUE TO MODIFIED DUTY.

EE WAS OPENING A R.R. HOPPER BOTTOM HATCH WITH A STEEL BAR WHEN HE FELT SOMETHING POP IN HIS LEFT RIBCAGE, AS HE WAS PUTTING DOWNWARD PRESSURE ON THE BAR. THIS FRACTURED A RIB. REPORTING DUE T O FRACTURED RIB & MODIFIED DUTY.

WHILE OPENING BOTTOM HOPPER DOOR (TO A R.R. CAR)WITH A 5' BAR, THE BAR SLIPPED OUT OF THE DOOR &KNUCKLE ON LEFT MIDDLE FINGER WAS SMASHED WHEN CONTACT WAS MADE WITH CAR. KNUCKLE CONTUSION ONLY BUT REPORTED DUE TO MODIFIED DUTY.

AFTER USING LOADER IN QUARRY, EMPLOYEE STARTED GETTING A SORE BACK. SEEKING MEDICAL ATTENTION; PHYSICAL THERAPY, AND A PAIN RELIEVER WAS PRESCRIBED.

EE WAS OPENING TOP HATCH DOOR OF A RAILCAR WHEN HIS RIGHT LITTLE FINGER GOT CAUGHT ON A SHARP PIECE OF METAL REQUIRING 6 STITCHES.

EE STARTED TO REPLACE FLOOR GRATING WHEN ANOTHEREE REQUESTED HIS HELP. EE WENT TO HELP OTHER EE & STEPPED THROUGH THE HOLE THAT WAS STILL IN THEFLOOR, CUTTING HIS LEFT LEG NEEDING 5 SUTURES.

EE AND A CO-WORKER SET A PLATE OF STEEL AGAISNT A SAW HORSE ON A FLAT BED OF THE MAINTENANCE TRUCK. STEEL BEGAN TO SLIDE. WHEN EE TRIED TO STOP IT, HIS FINGER GOT SMASHED BETWEEN THE STEEL AND FLATBED OF THE PICK-UP. REQUIRED 2 STITCHES/ MODIFIED DUTY.

WHILE MOVING A ROCK OFF THE HAUL ROAD, HE STEPPED ON THE BERM, SLIPPED AND FELL, FRACTURING HIS RIGHT ANKLE.

WHILE WALKING TO HAUL TRUCK HE SLIPPED ON PATCH OF ICE, FELL WITH HIS LUNCH BOX ON HIS RIGHT SIDE. THIS RESULTED IN A HAIRLINE FRACTURE TO THE RIB.

WHILE PERFORMING MAINTENANCE ON A TAIL PULLEY OF PLANT CONVEYER BELT, EMPLOYEE WAS LIFTING THE T PULLEY WITH A STEEL BAR. BAR SLIPPED AND EMPLOYEE STRAINED LOWER BACK. -12-01.

WEATHER: BLIZZARD CONDITIONS, WIND, SNOW, SLIPPERY. EMPLOYEE SLIPPED OFF LOADER LADDER LANDING ON LEFT HEEL. BRUISED LEFT HEEL - MIDIFIED DUTY.

EE WAS CUTTING A CONVEYOR BELT WITH A UTILITY KNIFE WHEN THE KNIFE SLIPPED & CUT THRU BOTH THETIP OF HIS GLOVE & HIS LEFT INDEX FINGER RESULTING IN 5 STITCHES

WHILE EMPLOYEE WAS OPERATING THE FRONT END LOADER (ME-12), HIS LEFT ELBOW RECEIVED A SMALL PIN-HEAD SIZE CUT FROM A TORN/CRACKED VINYL COVERING ON THE ARM REST. CUT BECAME INFECTED AND EMPLOYE E WAS PUT ON MODIFIED DUTY.

EMPLOYEE WAS LIFTING A TROUGH IDLER FROM A PALLET (NOT BENDING OVER), HE TURNED AND WAS WALKING TOWARDS THE CONVEYOR FRAME WHEN HE FELT A SHARP PAIN IN LOWER BACK.

EMPLOYEE WAS CARRYING IDLERS TO BE INSTALLED ON THE NEW QUARRY CONVEYOR. WHILE PERFORMING THIS WORK HE TWISTED HIS LEFT KNEE IN THE LOOSE SAND ON THE QUARRY FLOOR. WAS NOT EXAMINED BY A DOCTOR UNTIL 6-7-97. WILL BE GOING THROUGH THERAPY.

TIGHTENING THE FLEXABLE EXHAUST PIPING ON THE 75C UTILITY LOADER, THE 3/8" DRIVE RATCHET & 9/16" DEEP WELL SOCKET SLIPPED OFF OF THE NUT STRIKING HIM IN THE FOREHEAD. EE RECEIVED 4 SUTURES ABO VE HIS RIGHT EYEBROW.

AFTER GETTING DOWN FROM LOADER EE NOTICED SHARP PAIN IN HIP. THOUGHT PAIN WOULD PASS BUT GREW STEADILY WORSE AS TIME WENT ON. EE WENT TO DOCTOR ON 12/17 AND WAS DIAGNOSED WITH PULLED MUSCLE. HAS RETURNED TO LIGHT DUTY AFTER 3 DAYS AWAY FROM WORK.

EMPLOYEE CAME UP FROM THE QUARRY TO RE-FUEL AND TAKE A SHORT BREAK. WHEN HE WENT BACK TO THE LOADER, HE SAID VISION IN HIS LEFT EYE BECAME POORT AND HE COULDN'T SEE PERIPHERALLY. HE ALSO REP ORTED PAIN IN THE RIGHT SIDE OF HIS HEAD. HE WAS TAKEN TO THE EMERGENCY ROOM, EXAMINED, AND SENT HOME TO REST.

TWO EES WERE INSTALLING A DEWATERING PUMP. WHILELIFTING PUMP, THIS EE SLIPPED ON ICY AREA & FELTPAIN IN HIS LOWER BACK. BECAME MSHA REPORTABLE ON MARCH 4, 1996. EE IS TAKING A PRESCRIPTION MED ICATION.

WHILE CLEANING IN THE PARTS WAREHOUSE, EE WAS PLACING SOME DENVER PUMP IMPELLERS ON A PALLET. HE FELT A STRAIN IN HIS LOWER BACK.

WHILE OPERATING THE LOADER IN THE QUARRY, THE BELTS KICKED OUT. THE OPERATOR FOUND THE CHUTE TO BC-07 WAS PLUGGED AND THERE WERE ROCKS ON THE CONVEYOR. HE GOT OUT OF THE LOADER TO SEE IF THE OTHER OPERATOR NEEDED HELP. HE SLIPPED ON A LOOSE ROCK AND LANDED ON HIS RIGHT ELBOW. HAD TO HAVE A CORTIZONE SHOT ON 2-7-96.

EE WAS INVOLVED IN CONSTRUCTING HOPPERS FOR A CONVEYOR SYSTEM. DURING THE DAY HE WAS LIFTING STEEL AND WELDING ON HIS SIDE, ABOVE HIS HEAD, AND BENDING OVER. THE NEX DAY EE REPORTED HIS BACK W AS SORE. HE WILL BE SEEING A SPECIALIST.

EMPLOYEE WAS DISMOUNTING THE 75 LOADER AND HIS LLEFT BOOT HEEL GOT CAUGHT ON THE BOTTOM RUNG OF THE LADDER. EMPLOYEE TRIPPED AND CAME DOWN SIDEWAYS ON HIS RIGHT FOOT.

WHILE INSPECTING THE QUARRY CONVEYOR SYSTEM EMPLOYEE NOTICED A ROCK HUNG UP INSIDE FRAMEWORK OF BC-03 WHILE TRYING TO REMOVE THE ROCK HIS HAND WAS PINCHED BETWEEN THE ROCK & CONVEYOR FRAME

EMPLOYEE WAS USING RAILROAD TIES TO BLOCK UPAND LEVEL QUARRY CONVEYOR. AFTER MOVING APPROX EIGHT RAILROAD TIES BY HIMSELF, SORENESS DEVELOPED IN LOWER BACK. SORENESS DID NOT IMPROVE IN TWO DA YS AND EMPLOYEE WENT TO THE CHIROPRACTOR.

SUPERVISOR WAS CLEARING PIT PRIOR TO BLASTING. HE INSTRUCTED DRIVER OF A HAULMASTER TRUCK TO BLOCK THE ACCESS ROAD OF THE STRIPPING CONTRACTOR WITH THE TRUCK. THE HAULMASTER WAS NOT IN THE ROA D WHEN THE SUPERVISOR REACHED THE TOP OF THE RAMP. THE SUPERVISOR STOPPED BEHIND THE HAULMASTER AND WAITED FOR THE DRIVER TO PULL OUT TO BLOCK THE ACCESS ROAD. THE HAULMASTER BACKED INTO SUP.

EMPLOYEE WAS CRAWLING DOWN INTO RAIL CAR USING A CHAIN LADDER. A RIVET BROKE LOOSE FROM ONE LADDER RUNG. THE EMPLOYEE'S LEG HIT THE NEXT RUNG DOWN AND WHILE MAINTAINING HAND HOLDS ON THE ABOVE RUNG, JARRED HIS RIGHT SHOULDER.

WHILE OPERATING A FRONT END LOADER IN QUARRY,EMPLOYEE WAS BACKING UP WITH A FEULL BUCKET OF SAND UP IN THE AIR.A TIRE DROPPED INTO A LOW SPOT CAUSING THE LOADER TO TIP TO THE SIDE.THE EMPLOYEE BRACED HIMSELF WITH A TIGHT GRIP ON THE STEERING WHEEL.THE JERKING MOTION CAUSED A STRAIN OF THE EMPLOYEES NECK.

EMP WAS TRYING 7OW ROK A ROCK FREE WHICH WAS WEDGED IN THE JAW CRUSHER INSIDE THE PLANT HE WAS USING STEEL HOOK AND PRY BAR WHEN ROCK BROKE LOSE HOOK CAUGHT ON EDGE OF CRUSHER JERKING EMPLOYE LEFT ARM AND S2OULDER DOWNWARD AGAINST JAW CRUSHER FRAME DOCTORS DIAGNOSIS IS A ROTATOR CUFF INJURY

BOB WAS WALKING THROUGH A STEEL STORAGE AREA, WHEN HE STEPPED IN A HOLE. HE TWISTED HIS LEFT KNEE TEARING CATILAGE THE HOLE WAS A WASH OUT FROM RAW. A DESIGNATED AREA IS BEING POSTED FOR TRA VEL WHEN LOOKING FOR ITEMS NEEDED IN THIS AREA.
