Employee was using a piece of all-thread to move sand and got right pinky finger caught between the all-thread and metal flight causing a fracture to the tip of EE's finger.

Employee knelt down to look at parts on the floor and felt a twinge in their lower back. Placed on restrictions.

Working on #2 separator, the miner was installing a lower distributor table while putting in bolts, laying on back, and working overhead, a foreign object fell in EE's eye while wearing safety glasses. EE was prescribed eye drops.

Miner was welding utilizing an approved auto darkening lens welding helmet and ANSI approved safety glasses, when a piece of metal entered their left eye. It was not embedded, but prescription medications were provided.

Employee was picking up a piece of metal when EE strained back.

While loading the Rod Mill with 11'X4" steel rods, one of the rods fell out of the cradle and struck the miner on top of miners right foot.

The injured miner was placing an opened bag of Portland Cement next to the project, when a plume of the power like contents of the bag struck EE in EE's left eye. The miner washed EE's eye out at eye-wash station and received prescription pain medication at emergency room. The employee returned to work the next day.

Supervisor was checking operation of automatic sampler. While holding the retractable rod connecter to the sample container His hand was pulled towards the opening. Given the manner he was grasping the rod, thumb pointing towards opening the sampler pulled his hand into the side injuring his thumb of Rt. Hand. Initial first aid indicates possible stitches only.

Employee injured hand while working on dust collector fan

Employee experienced pain in left bicep while using a drill. Employee given light duty due to muscle strain.

Employee injured knee while walking as he served as a Miner's Rep on a regular MSHA inspection. Injury did not require modified duty until November 8, 2014.

Employee twisted to the left and experienced pain in his lower right back. Employee was put on restricted duty from 8/26/14 till 9/2/14 and returned to full duty on 9/3/14.

Employee stumbled over a floor drain and hit his left shoulder on the wall. It was later determined that he would need surgery for a rotator cuff repair. Note: surgery date of 8/1/14.

Employee stumbled while walking and smashed his little and ring fingers of his left hand.

Employee lifted a large wrench, twisted while lifting and felt a tightness in his back. Employee was put off of work until next evaluation.

Employee using a digging bar during clean up, impact to right ring finger, resulting in a fracture in the tip of the finger and lacerations. Employee is on restricted duty.

Employee slipped on snow/ice and twisted left knee and was given restrictive duties.

Possible occupational lung disease. No physical impairment. Employee retired 2/1/2014.

Black smoke observed coming from fluidizing fan of dryer. Dryer was not running. Due to unsuccessful attempts to start the dryer, fuel oil inside of combustion chamber was smoldering. Issue was contained inside of combustion chamber. Fire department called as a precautionary measure. MSHA was notified within 30 minutes of the unplanned event.

Using a wedge to remove liner and piece chipped off and flew up and cut through the pants and cut the knee cleaned area but after 2 days area got infected and to cut and cleaned out

Employee was assisting in the placement of a bundle of rods. The rods slowly swung toward the employee and he put his right hand up to steady them. The rods inside of the bundle shifted catching the tips of his middle and ring fingers of his right hand. Per employee, butterfly closures were used.

Employee mis-stepped off of ladder onto ground and over extended knee on 7/29/13. Employee opted to have torn meniscus repaired on 12/2/13. As of 12/2/13, employee is off work until next doctor appointment on 12/18/13.

Twisted wrong getting in or out of drill and strained muscle in lower back.

Employee was using a sledge hammer in a tight area and caught his right ring finger between the handle of the sledge hammer and a bolt stud. The impact resulted in a laceration/cut requiring 3 stitches.

Employee was walking down stairway and his foot slipped off the edge of the last step onto a landing. This caused him to fall against the handrail, and hit his left shoulder on the handrail. Employee was put on restrictive duty.

EE was handing welding rods to welder and received flash burn to eyes.

Employee was walking down the stairs, when she reported her ankle gave out on her. Her right ankle was swollen and was taken to the Berkeley Spring War Memorial Hospital. She ended up with a twisted right ankle, with a Fracture Lateral Malleolus-CL.

Employee was walking to the next room to continue cleaning and hit her hand on the door jam as she went through. She received a small cut on her finger that she received stitches for at War Memorial Hospital in Berkeley Springs.

Occupational Illness. No physical impairment. Possible occupational lung disease.

Employee was cleaning and painting a large hinge. The hinge was sticking out from the doorway and the employee stepped into the end of it. The hinge stabbed him in the leg below the right knee, causing a contusion.

Employee was replacing a top stand guide roller. He had the roller assembly in place on the conveyor frame and was pulling it to align bolt holes. Pulled muscle in right arm.

Employee was changing a blade in a utility knife. Knife slipped and cut his right arm, which required five stitches.

Employee was cutting a cable seal off of the bottom of a railcar and using a pry bar to remove the seal. When the seal released quickly, the employee fell against the car and injured his left shoulder. First aid until 8/26/10 when employee was sent to physical therapy by doctor.

Employee was carrying two pry bars and a chain pull up a set of stairs. When he got onto the 2nd floor, his right knee gave out. Employee had strained his knee approx. 4-6 weeks ago on a job. Diagnosed with a torn minicus. Minicus was repaired on 10/16/09. Employee did not miss work, but is on restrictive duty for two weeks.

Initial accident on 07/06/09-reportable as of 07/13/09. Employee was performing light shoveling along rail track, twisted to throw sand off of shovel and felt a pain in his left hip and lower back area. First aid until he was put on restrictive duty on 07/13/09.

Employee stepped down onto a rail car from the loading platform and twisted his left hip and knee. He did not report to operator until March 18, 2009. Considered a first aid until April 9, 2009, when the employee was prescribed physical therapy.

Employee changed out propane tank (approx 65 lbs) from forklift. He lifted the full bottle, turned to his left and set the bottle onto the forklift. Later that same day, he began to experience pain in the right shoulder area and right arm. He stated that the only heavy object he had lifted on this day was the propane bottle. Employee waited for 2 days before reporting.

Employee opening/closing bottom gates on rail car, right leg slipped, over extended knee, resulted in a torn miniscus. Classified as first aid with no days missed until surgery on 10/14/08.

Employee was attempting to retrieve a tool from the back of a 4x4 utility cart(Pug). As he reached into the bed of the Pug, the operator started to back up, striking the employees right foot. This resulted in a broken leg. The backup alarm was working on the utility cart. The employee stated that a lack of communication and inattention were to blame. Restrictive duty.

2 mechanics were hoisting an 8 in. flanged fitting of discharge hose to re-attach it to a pump that had been repaired. One bolt was put into the flanged fitting to and secured in place with a nut to hold it in place. Due to the tension on the hose, the bolt popped loose causing the hose to break free hitting the mechanic in the lower lip causing a laceration.

EE was walking down the steps at the crusher bldg.EE made a right turn at the landing,his right knee "popped" and ee felt pain.He was not carrying anything,there were no unsafe conditions or unsafe acts.Incident happened on 4/10/08 and was classified as a first aid until 5/2/08,when EE recieved knee surgery.

Employee slipped on stair while walking out of screen tower. He was holding onto the handrail and pulled his right shoulder. Diagnosed with shoulder sprain and then with a torn rotator cuff. Surgery on August 25, 2008.

Employee using a saw to cut trees and brush. Did not feel anything hit his face or eyes, but later felt an irritation in his left eye. He washed his eye out at 9:30 pm on 6/18 and at 5:00 am on 6/19. He the reported to plant management and was taken for med attn. It is possible that when he wiped his brow with his glove that he may have wiped the piece of rust into his eye.

On the first step of the ladder for the L330 Loader. Started the climb in a way that threw off center of gravity, using his left foot and left hand to "swing" his right foot and hand into a position. Employee's right hand was oily and slipped, he swung out, which put all of his weight on his left arm. He felt a pop in his arm.

Employee was removing a roller stand, cut his finger, requiring stitches.

Employee popped his right knee out of place doing maint. work on a loader. He indicated that the damage was from a pre-existing condition caused by a skiing accident and that the "popping" of his knee happened often. Injury was ruled a first aid at the time-no lost work days. Employee went for surgery on April 26, 2007 causing the first aid to become a LWC on April 27, 2007.

Mechanic cutting a 2" x 2" hole in the bottom of an airslide. Piece of metal flake went into eye.

EE was cutting an airline on a Microsizer project. He laid his utility knife down without closing it. When he flipped the hose around to straighten it, he ran his left hand into the open utility knife, cutting his hand. This required 4 stitches. He returned to work the same afternoon.

Employee was getting on a tow motor. When he pulled himself up with his left arm he felt pain in his left shoulder. He had an injury in the same place which resulted in surgery in 2002.

Employee had climbed a ladder to access a platform in order to unplug a chute. As the employee was looking up, he stepped through the access hole, slipped and caught himself on a piece of grating. By grabbing the grating, the employee cut his right index and middle fingers which required stitches. Employee returned to work, next day, no restrictions.

Employee was on top of a railcar trying to open a lid. When he bent down to open the lid he pulled something in his lower right back. Employee had experienced a non-work related back injury in Oct. 1989. He has pins in his back which cause his bending to be awkward. Started physical therapy 1/19/05.

EMPLOYEE WAS CUTTING A PLASTIC TIE TO REMOVE A BAG ATTACHED TO A DRAINPIPE COUPLING. POCKET KNIFE SLIPPED & EMPLOYEE CUT HIS LEFT WRIST, REQUIRING 4 STITCHES. NO RESTRICTIONS FOR WORK.

CLIMBING STAIRS TO 2nd STORY IN WAREHOUSE, HIT RIGHT KNEE ON STAIR POLE.

WALKING ALONG RAILROAD TRACKS & STEPPED IN A SMALL POTHOLE. TWISTED HIS LEFT KNEE.

EE WAS EXITING THE OFFICE BLDG WITH AN ARMLOAD OF IREMS. SHE PUSHED THE DOOR OPEN, IT LOCKED INT HE OPEN POSITION. SHE USED HER RIGHT FOOT TO CLOSE THE DOOR AND IN DOING SO, HER BRUISE ACROSS A SHARP PIECE OF METAL CUTTING HER RIGHT HEEL REQUIRING 4 SUTURES.

EMPLOYEE WAS ON 3RD OR 4TH STEP OF AN "A-FRAME" STEP LADDER, INSPECTING THE BEARING AT THE FEED END OF A SEPARATOR SCREW. AS HE STARTED DOWN THE LADDER HIS LEFT SLEEVE STUCK ON SOMETHING. AS H E TURNED TO FREE HIS SLEEVE, THE LADDER SLIPPED AND HE FELL 3 TO 4 FEET ONTO LADDER.

EE WAS ASSISTING WITH SCREEN CLOTH REPLACEMENT ON #4 ROTER SCREENER. WHEN SHE PREPARED TO EXIT THE SCREENER HER FOOT SLIPPED OFF THE GUARD SHE WAS STANDING ON. SHE GRABBED THE FRAME SUPPORT CA BLE WITH HER RIGHT ARM AND FELL 33" WHILE HOLDING CABLE, PULLING LIGAMENTS IN HER RIGHT ARM.

UNCOVERING A TRUCKLOAD OF FILTER SAND PREPAIRING TO LOAD THE TRUCK, WHILE REACHING AND STRAINING TO UNWRAP THE PLASTIC COVER, EMPLOYEE SLIPPED AND TWISTED HIS RIGHT KNEE.

SLIPPED ON WET SAND LAYING ON GROUND AT WAREHOUSE US SLIICA BERKELEY SPRING WV

EE WAS CARRYING A PIECE OF CHANNEL WITH ANOTHER EE, WITH ONE AT EACH END. THE OTHER EE BEGAN PLACING THE CHANNEL, CAUSING THE INJURED EE TO LOSE BALANCE & FALL AGAINST BUILDING WALL, WHERE HE CUT HIS ARM ON A PROTRUDING BOLT.

EE WAS TIGHTENING OIL FILTER ON TM400 TRACKMOBILE. AFTER HE GOT THE FILTER SNUG HE CLEANED HIS HANDS TO GET A GRIP TO APPLY FINAL TORQUE BY HAND. HE FELT A TEAR & HEARD A POP IN LEFT SHOULDER.

EE WAS MODIFYING BRACKETS FOR A RETURN ROLLER. HE HAD TO GRIND THE WELD OFF THE BRACKET, REPOSITION & REWELD. HE REMOVED HIS WELDING GLOVE TO GRIND. SWEAT WAS RUNNING DOWN HIS FOREHEAD & INTO HIS EYES. HE SWIPED HIS EYE WITH HIS HAND WHICH HAD METAL FRAGMENTS ON IT & DEPOSITED FRAGMENTS IN HIS LEFT EYE.

EE STEPPED OUT OF LEFT SIDE OF DOZER WITH HIS LEFT FOOT FIRST FACING THE BLADE. AS HIS LEFT FOOT SLIPPED OFF THE STEP HIS RIGHT FOOT HUNG UP ON THE STEP. HE FELL BETWEEN THE TRACK & PUSHER ARM WHILE HIS RIGHT FOOT REMAINED CAUGHT IN STEP. HE THEN ROLLED FORWARD TO FREE HIS RIGHT LEG.

EMPLOYEE WAS RESAMPLING RAILCAR CXST 228137. AFTER OBTAINING SAMPLES FROM THE TOP OF RAILCAR, HE WAS RETURNING ON THE GRATING WALKWAY ON THE OPPOSITE SIDE OF THE DOWN LADDER. AS HE CROSSED THE "A" END WALKWAY TO THE LADDER, HE LOST HIS BALANCE AND FELL 15' TO THE GRAVEL.

EE WAS GOING TO REPLACE ROLL OF STRETCH WRAP. IN HIS MOVEMENT TO PICK UP THE 32 LB ROLL, HE FELT A SNARP PAIN IN HIS LOWER BACK AT BELT LEVEL. HE HAD NOT PICKED UP ANYTHING, HE MERELY BENT OVE R.

EMPLOYEE WAS REPLACING THE BALL TRAY SCREEN, ATTACHING IT TO THE BALL TRAY OF #3 ROTER SCREENER, SITTING IN THE UPRIGHT POSITION. EMPLOYEE LAID THE SCREEN ON THE TRAY WHILE ANOTHER EMPLOYEE WAS SCREWING THE SCREEN ONTO THE TRAY, EMPLOYEE ALLOWED SCREEN TO FALL ABOUT 1/2 INCH AND CUT HIS FINGER TIPS.

EMPLOYEE WAS FUELING BOBCAT LOADER. UPON EXITING THE BOBCAT THE EMPLOYEE TRIPPED ON THE BACK EDGE OF THE BUCKET AND FELL FORWARD HITTING LEFT SHIN ON EDGE OF BUCKET AND HER RIGHT KNEE HIT THE GROUND.

GOT ON CONVEYOR BELT TO TURN SIZE 4 (PRODUCT) OFF, WENT TO GET BACK DOWN, FOOT SLIPPED OFF THE BOX (STEP) WHEN STEPPING DOWN TO THE FLOOR. SLIP RESULTED IN BACK SPRAIN.

EE WAS ASSIGNED TO DO REPAIRS TO THE #25 DUST COLLECTOR. IN ORDER TO DO THIS JOB, THE PLANT'S MANLIFT WAS USED TO GAIN ACCESS TO THE AREA. WITH EE IN THE MANLIFT, HE LIFTED A PIECE OF 10" DU ST LINE (WEIGHING APPROX 30#). AS HE RAISED ON THE END OF THE DUST LINE FROM THE BASKET HE FELT A SUDDEN PAIN IN HIS RIGHT THUMB. EE CONTINUED TO WROK HIS SCHEDULED SHIFT.
