Rum Creek Preparation Plant Coal

A non injury fire occured after a 1/4 Inch piece of metal had been cut out for the cyclone media box , the fire occurred after replacing the 1/4 metal and reinserting the 14 inch piece of poly piping .Logan Fire Dept responded and helped extinguish the fire ,and monitored it for hot spots afterwards .

Employee operating telehauler was attempting to extend forks. Forks was hitting structure. EE went to adjust forks using left hand. Forks slid and caught EE's left index finger.

Injured employee was walking on the opposite side of the newly installed refuse belt from a telehandler that was picking up new belt lying on the ground to place on the belt rollers. The belt slid off the forks and side of belt structure bumping the employee enough to cause employee to lose balance in the mud, twisting a knee.

Removing a guard that covers the drive line between the 2 exciters, when EE's left index finger got caught between the guard and the exciter, resulting in a fracture and 7 stitches to repair the laceration.

When installing a water clamp on a wash down hose EE's screwdriver slipped out of the clamp cutting left hand in between index and middle finger resulting in a laceration requiring 7 stitches

The belt person was walking back from the raw coal stacker belt. EE fell into a hole where a piece of grating had been removed, to retrieve a piece of curtain out of the flume. EE fell striking EE's elbow and EE's left foot struck a piece of 4-inch channel causing fracture to EE's toes on left foot

Was going to hook up the belt trailer to the pickup truck, EE pulled the pin on the wheel that holds up the trailer and it fell onto EE's left foot fracturing two bones. Nothing was wrong with the trailer nor the jack on the trailer both was new.

Stepping off last step of step ladder when EE's right foot touched the ground EE's knee buckled. Dr thinks may have a patella tendon injury.

Employee was helping to get cribs to stack under new R8 Belt head, when employee felt pain in left shoulder around rotator cuff. Employee was taken off work by the ER Doctor.

Employee was picking up 26" roller felt pop in neck/rt shoulder worked until 10-20-20 and was taken off work. This is 3rd incident with this shoulder we ask this be coded as occ illnes due to repeated movements and total mine exp.

Employee was running belt to check it and a lot of dust came from belt getting into EE's eyes causing redness and irritation, this belt had just been changed out is the reason for the dust in the raw coal side, EE sought treatment on 7-30-20, returned to work on 7-31-20

Drilling a hole fixing wires that were damaged, drill slipped out of EE's hand and spun around. The battery hit tooth, chipping corner of tooth.

The Employee and two others were lowering a feed box to the basement floor. The feed box was setting on its edge, while there was slack in the hoist rope the employee bent over to unhook the load when it shifted causing the hook/sheave to hit them in the head. Employee went to the Dr. on 1/17/2020 and began physical therapy on 1/20/2020.

Employee was helping another employee load a 48" top belt structure into a pickup when employee's feet slipped down an embankment causing employee to fall, landing on butt.

4-17-19 EE reported incident to foreman at end of shift. No witness to incident. EE didn't seek medical attention until 4-23-19 when they began missing work. EE was backing off the met coal pile toward steam coal pile when neck began to hurt. Part of normal duties for operating dozer. Believe this is occ. Illness. No instantaneous event described that initiated neck pain.

Employee was in the process of installing new skirt boards in the R-4 tailpiece, after unhooking the chain that was used to place the new skirt boards on the belt, employee was walking on the belt back toward the tailpiece and slipped and fell backwards. They attempted to catch themselves, causing a small "chip" fracture in their wrist.

Employee removed a top belt roller to make a splice in the conveyor belt. When ee turned to drop the roller ee felt a pain in right upper arm. EE received medical treatment on 3/29/2018 and returned to work on next scheduled shift on 4/3/2018 and begin physical therapy on 4/5/2018.

Employee was using a sledge hammer and a slate bar to knock the pins out of a D10-R bulldozer's right tilt cylinder and felt a pain in neck and shoulder. Employee continued to work but started physical therapy on 1/30/2018.

The employee was diagnosed with Occupational Pneumoconiosis on 09/24/2017, we received notification on 12/07/2017.

Employee was putting an impact roller in a tailpiece, when employee lifted on the roller employee felt a pull and burn in employee's lower back. Employee returned to work next shift but received physical therapy beginning 7/31/17.

Employee was removing rock from the #2 raw coal belt and felt a pain in left lower back.

Employee was cleaning up the back of the warehouse around a separator drum. While cleaning up employee reached down to pull a piece of metal from under the drum when the magnet in the drum pulled the piece of metal to the drum mashing employee's thumb between the drum and the metal.

This employee has not been working since 5/13/2009. On 04/11/2016 this Plant was notified that employee was diagnosed with pneumoconiosis.

Employee was lifting a 12" X 24" screen panel and felt pain in back as EE twisted to set it down. On 06/16/2016 EE informed this mine that EE had been receiving physical therapy since the incident.

Employee was helping move a 3' X 10' X 3/8" steel plate into place in a vessel and it slipped, coming down on the steel toe of his left foot.

Employee was closing metal gate, gate hit a guardrail & bounced back toward him, hitting his left arm. Bruised arm.

Claim filed for pneumoconiosis. Employee retirement date of 12/29/2014. Diagnosis date of 3/26/2015.

Employee was on ladder and lost footing and fell hitting the back of his left calf

Employee was changing flights in a vessel when he felt pain in his lower back. He finished his shift and went to the doctor the following day. He was released to return to work following this doctor visit. Afterwards he continued to have pain and sought additional treatment on July 6 where he was given one day off work. He has also began seeking chiropractic treatment.

Employee was lying on a creeper while spraying belt structure with a water hose. The high pressure stream hit a bottom roller causing the stream to bounce back and hit him in the face, allowing material into his eye.

Employee was placing the lid back on a feeder and when the lid dropped into position, it pinched the side of his left index finger.

Dozer fell through a void in the stockpile trapping the operator for approximately 2 hour before rescuer could uncover and free the operator from the dozer.

We were notified on 7-19-13 that employee has been diagnoised with occupational pneumoconiosis

Employee stepped off a platform onto an oil bucket lid causing him to slip and fall hurting his right shoulder. He continued to work until 5/24/2013 when he was taken off due to increase pain in his shoulder.

Employee was running a Caterpillar D10R Dozer. As he was backing up, he struck a 3' high pile of coal which gave him a jolt. Employee claims he began to see a black spot in his right eye.

Employee was reaching out to catch a ladder that was shifting toward him and strained his left shoulder.

Employee was assisting with the removal of loose concrete that was busted up and was injured in a fall that resulted in a bruised leg and a cut requiring sutures.

Employee was oiling a wrench when he felt something bite him on his left leg and looked down and saw a spider. Treatment required a prescription antibiotic.

Employee was dragging rebar into place when his elbow gradually began to hurt. He continued to work until 06/20/2010 at which time he sought medical treatment.

EE was using a handheld grinder in a tight position and got something in his eye. EE was wearing safety glasses with side shields at the time.

Employee was operating a mobile crane looking up often to watch the loads, and over the course of time, his neck and shoulders began to hurt.

The employee slipped and fell on icy conditions, causing him to twist his ankle.

Employee was walking toward manbasket & tripped on a piece of conduit. He tried to catch himself and injured his hand & shoulder. Also twisted his knee.

Installing a chain dog on chain. Chain dog struck lip resulting in minor cut to lip.

Employee was removing a piece of sheeting from a screen deck when he slipeed and fell on the slippery decking, landing on a screen lifting thing in the area of the groin.

Employee was assisting in pulling a vessel chain out of a coal washery vessel. The chain slipped trapping his left hand against the side of the vessel resulting in a laceration and open fracture to the middle finger of his left hand. He received 11 stitches adn a splint from Logan medical center.

Coworker was shifting load on truck. Load swung around and hit injured employee on left thigh.

Member was changing a 100 hp motor on the direct ship cruiser located on the 2nd floor of the plant. The spring loaded base was in the up position & when the motor was removed it released the motor base. The base came down and hit him on the left hand.

EE was removing master truck pad off dozer. A chain hooked to the pad connected to the boom of a maintenance truck, truck slipped striking EE in the mouth.

Member was in the process of putting oil in hyd. tank of D-10 dozer. He was standing on tracks of dozer lifting a 5 gallon can of hyd. oil on to the landing for the operator's compartment. His feet slipped causing him to fall backwards against the grab handle.

Member was unplugging chute with a pipe and a hammer. As he was pounding on pipe, the hammer struck his hand. Injury resulted in laceration and fracture.

EE was helping other ee an old dryer screen toward the hoist well so they could take it out of the plant, in the process of rolling the old dryer screen he tripped and fell causing laceration to his left hand and leg.

while torch cutting a section of 8" pipe to be installed ee was attempting to reposition the pipe when it fell off the blocking fracturing his left middle finger.

Member was washing under tail roller of #2 belt. He had his hand on the belt structure. A rock bounced off the belt striking him on his right hand.

Member was pulling a piece of conduit off of the conduit rack when the end he was holding became unbalanced and went up pinching his right thumb between the conduit and the support for the rack.

While changing rollers on refuse belt, the member said he felt a pain in Rt shoulder. This accident was not determined to be reportable until 8-10-07.

Member was removing a piece of belt with a mechanical splice from the vessel. Feed chute during this process his hand slipped and his forearm raked against a splice cleat, causing a laceration to the top of his forearm.

Fracture to right index finger: Employee was handing a piece of metal to co-worker near magnet. Finger was smashed between metal and magnet.

Member was picking up scrap metal raised up hitting head on a pipe hurting neck & back. First day off work was 8/21/06.

Member was standing on clean coal chute while attempting to fix a leak in a 6 inch magnetite return. He lost his balance and fell to floor hurting his right arm and back.

Member was putting hydraulic oil in dozer when he slipped hurting his right leg.

EMPLOYEE WAS STRUCK IN THE EYE WITH A PIECE OF ROCK (EMBEDDED OBJECT IN LEFT EYE).

Member was shoveling loose coal, while doing so members foot came in contact with the guarding causing member to fall, hurting neck.

Employee was cleaning coal on 5th floor when he turned around & hit his head on a drain pipe.

Employee was coming off the crane by the steps located at the right rear outrigger beam. It was a wet, cold day with some snow. When he stepped onto the beam, his foot slipped and he fell striking his tailbone on the outrigger box.

EE WAS USING A HAMMER TO KNOCK COAL AND ICE OFF RAILROAD CAR DOORS WHEN EE TWISTED AND FELT A SHARP PAIN IN BACK

EE was setting a ladder up to tape a leak on the vessel separator pipe. He slipped and fell to the floor striking his back and shoulder. He continued to work until 11/22/04. He went to a chiropractor on that date who told him to stay off work until 12/2/04.

EE WAS PULLING CHAIN ACROSS A GRATED FLOOR. THE CHAIN BECAME STUCK AND EE PULLED IT. ON 11/11/04 EE WENT TO THE DOCTOR WHO INSTRUCTED EE TO STAY OFF WORK.

Ee was bent over swinging a sledge hammer busting a conrete floor. He felt a sharp pain in his lower back. he continued to work and finished working his shift. he experienced numbers in his left foot and pain in his left leg the morning of 10-14-04. he went to the dr whom told him to take two weeks off work.

EE BENT OVER TO WALK UNDER THE SAMPLE BELT. EE'S HARD HAT FELL OFF AS HE WAS BENDING OVER. AS EE RAISED UP HE STRUCK HIS HEAD ON THE BELT STRUCTURE, CAUSING A LACERATION TO HIS SCALP. EE RECEIVED 6 STAPLES AT THE POINT OF THE LACERATION.

EE WAS BOLTING UP SKIRT RUBBER IN THE BACK OF A CHUTE IN A BENT OVER POSITION. WHEN RISING UP, HE STATED HE BUMPED HIS LOWER BACK ON THE BOTTOM OF A HANDRAIL. HE WAS SCHEDULED OFF FOR SIX DAYS & DURING THIS TIME HE WENT TO SEE THE DOCTOR. HE WAS DIAGNOSED WITH A HERNIATED DISC. RECOVERY TIME IS UNKNOWN. NOTE: HE DID NOT MISS WORK UNTIL 8-9-04.

Ee was operating a water truck to fill decant pipe. he was backing water touch up to fill decant pipe, back truck off road into ditch and turned loaded water truck onto side.

Employee climbing off dozer, slipped and fell approx. 18" into a sand refuse pile, landing on his back, climbed back onto dozer and operatoed for approx. 30 minutes then complained of numbness in his feet

MEMBER WAS USING A UTILITY KNIFE TO TRIM A BELT. WHILE HOLDING THE KNIFE IN HIS RIGHT HAND, HE PULLED HIS LEFT HAND ACROSS THE KNIFE CAUSING A LACERATION TO HIS LEFT SMALL FINGER.

EE AND ONE COWORKER WERE INSTALLING PIPE. A MONORAIL HAD BEEN CUT OFF TO ACCOMMODATE THE ROUTE THE PIPE HAD TO TAKE. EE AND COWORKER LEFT TO RETRIEVE A 1-TON CHAIN FROM THE OTHER END OF THE MONORAIL. UPON THEIR RETURN, THEY PASSED UNDER THE CUT OFF END OF THE MONORAIL, AND A SECOND CHAINFALL ATTACHED TO A DOLLY CAME OFF, STRIKING THE EE IN THE BRIDGE OF THE NOSE.

EE stated his right leg muscle started hurting and hurt worse as the shift went on.

EE was unplugging the #3 feeder at the bottom of the raw coal silo when coal and water suddenly rushed out of the feeder. ee jumped approx. 8' off a catwalk in order. to get out of the way. this resulted in ee fracturing his right foot.

EMPLOYEE WAS WALKING DOWN A CATWALK WHEN HIS HEAD (HARDHAT) STRUCK A STRUCTURAL I-BEAM. EMPLOYEE IS SCHEDULED FOR A CAT-SCAN ON 6/20 TO FIND OUT WHAT HIS INJURIES ARE.

Employee was struck by jib being installed on a Grove 65 ton Chase to add reach. as it was swinging into position, he grabbed it to slow it down with another ee. It struck something and bounced back, hitting rim in the left side of the chest.

EMPLOYEE WAS CLEANING THE PLANT FEED BELT AT THE TIME OF THE ACCIDENT. HE WAS ABOUT 20 FT. ALONG THE WALKWAY FROM THE BOTTOM OF THE BELT SHOVELING ON THE CATWALK. DUE TO AN UNDETERMINED REASON THE BELTING FELL TO THE GROUND. HE WAS THROWN TO THE GROUND LANDING ON A PILE OF COAL AND MUD.

EMPLOYEE WAS HELPING RAISE A COUNTERWEIGHT TAKE-UP ON THE RAW COAL PLANT FEED BELT. DURING THIS TIME, DUE TO AN UNDETERMINED CAUSE, THE BELT FELL TO THE GROUND. WHEN THIS OCCURRED, EMPLOYEE WA S STRUCK ON THE SHOULDER WITH AN UNKNOWN OBJECT.

EMPLOYEE WAS CUTTING THE TAPE FROM A ROLL OF PIEZOMETER CABLE WITH A KNIFE, USING HIS LEFT HAND. THE KNIFE SLIPPED AND CUT THE TOP OF HIS RIGHT HAND BEHIND THE THUMB AND FOREFINGER, NECESSITAT ING FOUR STITCHES.

EE WAS PERFORMING A CLEANING DUTY ON THE BYPASS BELT WITH A WATER HOSE. HE HAD THE WATERHOSE STRUNG ACROSS THE ROADWAY & SPRAYING WHEN A TOAL TRUCK DRIVER BY HEATH ROWES WENT BY & CAUGHT HIS WATER HOSE AND JERKED EE'S BACKWARDS CAUSING PAIN TO HIS RIGHT ARM, SHOULDER & RIBS

EE WAS CHANGING AN IDLER SLIPED AND FELL PINCHING HIS RIGHT MIDDLE FINGER BETWEEN THE IDLER AND THE BELT STRUCTURE

MEMBER WAS DISMOUNTING HIS DOZER WHEN HE SLIPPED ON THE STEP ON THE PUSH ARM & INJURED HIS LEFT KNEE.

SUBJECT WAS TRAMMING AN EXCAVATOR THROUGH WOODS ON ACCESS ROAD WHEN IT CONTACTED A HORNETS NEST. THE NEST FELL ON TOP OF THE ENGINE COMPARTMENT. THE HORNETS SWARMED INTO THE OPERATORS CAB. SUB JECT FELL WHEN ATTEMPTING TO EXIT THE CAB TO GET AWAY FROM THE STINGING HORNETS.

WHILE VACUUMING COAL AROUND #3 FEEDER, THE FENCE GUARD SLID DOWN ON EE S LEFT HAND, CUTTING MIDDLE FINGER REQIREING 10 STITCHES EE WAS WEARING ALL REQUIRED P.E.

UNLOADING TRANSMISSION AND TORQUE CONVERTER FROM A DELIVERY TRUCK, CLIMBING OUT OF BACK OF DELIVERY TRUCK, PULLED GROIN, PAIN AND KNOT ON RIGHT TESTICLE.

HAND CAUGHT IN ELEVATOR DOOR, FRACTURE OF MIDDLE FINGER ON RIGHT HAND. HE WAS TRYING TO KEEP THE ELEVATOR DOOR FROM CLOSING.

EE WAS WALKING BELT LINE WHEN HE SLIPPED & FELL STRAINING HIS GROIN MUSCLE.

EE AND TWO OTHER MEMBERS WERE IN THE PROCESS OF INSTALLING FLOOR STEEL. IN THIS PROCESS EE AND ONE OTHER WERE ROLLING AN "I" BEAM ON SMALL ROLLERS. AS EE BEGAN TO PULL ON THE BEAM HE EXPERIENC ED A SHARP PAIN IN HIS LOWER BACK AREA.

EE WAS IN THE PROCESS OF WALKING ACROSS A FUELING PLATFORM FROM A CAT DOZER WHEN HE FELL STRIKING HIS HEAD AGAINST THE HAND RAINING LANDING ON THE WALKWAY OF THE FUELING PLATFORM.

EE RECEIVED AN AWARD FROM WORKERS COMP. IN THE AMOUNT OF 5% RECOMMENDED BY THE OCCUPATIONAL PNEUMOCONIOSIS BOARD.

EE WAS SETTING UP CRANE, WHEN A SKID PLATE FELL FROM OUTRIGGER JACK. SKID PLATE STRUCK HIS MIDDLE FINGER ON HIS LEFT HAND. EE SUFFERS FROM A BROKEN FINGER.

EE WAS ATTEMPTING TO STEP ON HIS DOZER FROM THE LANDING AT THE FUEL AREA. AS EE MADE THE STEP HE SLIPPED STRAINING LOWER BACK. EE WORKED THE NEXT TWO WORK DAYS 2-1-03 EE DID NOT REPORT TO WORK ON 2-3-03.

EE WAS REMOVING COVER OF A FLUID COUPLING TO ACCESS THE COUPLING. AS HE REMOVED THE COVER, IT FELL SMASHING HIS LEFT POINTER FINGER. HE SUFFERED LACERATIONS TO THE TOP & BOTTOM OF HIS FINGER.

MEMBER STATED THAT HE FELT DISCOMFORT IN HIS LOWER BACK AFTER HELPING SPLICE TOGETHER A 48" BELTHE STATED THAT AT NO CERTAIN TIME CAN HE SAY WHEN THE INCIDENT OCCURRED. THIS HAS BEEN CLASSIFIE D AN OCCUPATIONAL ILLNESS DUE TO THE INFO BY DR.

EMPLOYEE WAS IN THE PROCESS OF SLIDING A STEEL PLATE OFF OF A BUSTED WATE RLINE WHEN HE FELT PAIN IN HIS LOWER BACK. EE WAS INVOLVED IN A LTA ON 11-9-01 WHERE HE RECEIVED WHIPLASH TO HIS NE CK. THE E.R. DOCTOR STATED THAT THIS IS A RE-OCCURRENCE. EE WAS ALSO TREATED FOR DEHYDRATION.

WHILE STEPPING ON A STEP LADDER, APPROX. 4'OFF THE GROUND, THE INJURED HAD ONE FOOT ON THE LADDER AND ONE FOOT ON A HAND RAIL. IN CHANGING POSITIONS HE SHIFTED HIS WEIGHT FROM HIS RIGHT FOOT TO HIS LEFT CAUSING HIS LT. KNEE TO BUCKLE AND STRAINING HIS LT. KNEE.

EE WAS USING A SLATE BAR TO UNSTOP A RAW COAL CHUTE ON THE 2ND FLOOR OF THE BREAKER BUILDING. WHILE HITTING MATERIAL IT ALL BROKE FREE AT ONCE. HOT MATERIAL LANDED ON HIM CAUSING MINOR BURNS T O HIS LEFT ARM AND NECK AREA.

EE WAS TIGHTENING BOLT ATTEMPTING TO FIX A FEEDER. AS HE WAS PULLING ON BOLT HE FELT A SHARP PAIN IN LEFT SHOULDER. THIS INCIDENT ORIGINALLY WAS RECORDED AS A FILE ONLY ACCIDENT DUE TO THE FAC T EE NEVER CEASED WORK & CONTINUED TO WORK UNTIL4/2/02.

EE WAS MOVING A SCREEN PANEL FROM A VIBRATING SCREEN HE WAS WALKING BACKWARDS WHILE MOVING THE PANEL, TRIPPED AND FELL LANDING ON HIS BACK WITH THE PANEL ON TOP OF HIM. BRUISING HIS BACK.

EE WAS WORKING ON A LADDER PATCHING CHUTES. HE CLIMBED DOWN THE LADDER, FELT A "CATCH" IN HIS BACK. PAIN BECAME WORSE DURING THE DAY. HE WAS WEARING HELMET, BOOTS, GOGGLES/SAFETY GLASSES.

MEMBER WAS IN THE PROCESS OF CLEANING AROUND THERAILROAD TRACKS WITH A BOBCAT ENDLOADER. WHILE HE WAS CLEANING AROUND THE RAILS, THE LIP OF THE BUCKET GOT CAUGHT ON A RAIL CAUSING IT TO SUDDEN LY STOP. THIS LED TO WHIPLASH OF THE NECK.

ON 6/2/01 EE BEGAN FEELING PAIN IN HIS NECK & SHOULDER UNABLE TO WORK FOR THE PAIN HE SOUGHT MEDICAL TREATMENT. THIS HAS BEEN CLASSIFIED AS AN OCCUPATIONAL ILLNES DUE TO HIS JOB & PAST MEDICAL RECORDS

EE WAS STRADDLING AN 8" PIPE, CUTTING IT WITH A TORCH. WHEN HE FINISHED THE CUT, IT SHIFTED AND PINNED HIS LEFT FOOT AGAINST A DIRT BANK.

EE WAS CARRYING A COME ALONG ON HIS SHOULDER HE HAD RETRIEVED FROM THE BACK OF A SPICK UP TRUCK. HE WALKED AROUND THE BACK OF A BOOM TRUCK AND DID NOT SEE A PIECE OF 3"X3" ANGLE LYING ON THE G ROUND. HE STEPPED ON THE ANGLE, WHICH CAUSED HIMTO SLIP AND FALL, STRAINING HIS LOWER BACK.

CARRYING CONDUIT UP STEPS-GOT SHARP PAIN IN BACK. PAIN CONTINUED, DID NOT GO AWAY.

MEMBER WAS STRIKING A STUCK TOP ROLLER WITH A HAMMER WHEN HE SLIPPED CAUSING HIS HAND TO STRIKE THE ROLLER. THIS RESULTED IN A BROKEN MIDDLE FINGER ON HIS RIGHT HAND.

EE WAS DESCENDING STAIRS & HAD TO GO UNDER AN ELECTRICIAN'S SCAFFOLDING BRACE. HE SLIPPED ON STEP AND FELL.

EMPLOYEE WAS REMOVING A NEW CHUTE FOR ADDITIONALFABRICATION. HE TRIED TO PRY IT LOOSE WITH A 5' LONG PRY BAR. HE HAD HIS RIGHT HAND OVER THE SHARP POINT OF THE BAR. THE CHUTE BROKE LOOSE AND THE BAR GOUGED HIS HAND, CAUSING A SEVERE LACER ATION TO THE PALM.

THE EE WAS ASSISTING IN THE WEST OF A CHUTE FOR THE FEED ON THE MAGNETIC SEPARATOR. WHEN THE CHUTE WAS BEING MANUEVERED INTO POSITION BY USING AN ELECTRIC CHAIN HOIST. THE STOP FOULED ON THE CHUTE CAUSING THE CHUTE TO SHIFT WHILE TRYING TOCONTROL THE CHUTE HE TWISTED HIS BACK RESULTING IN A SHARP PAIN

EE WAS STEPPING ONTO A CROSS BRACE ON A PIECE OF BELT STRUCTURE AT GROUND LEVEL. HE HAD MUD ON HIS BOOTS CAUSING HIM TO SLIP. HE STRUCK HIS LEFT ELBOW ON THE STRUCTURE.

EMPLOYEE WAS STANDING ON A PALLET AT GROUND LEVEL PUMPING FUEL INTO A 60 TON CRANE. THE TANK FROM UNDER WHICH THE FUEL WAS BEING PUMPED WAS ON THE SAME PALLET. HIS FOOT SLIPPED CAUSING HIM TO FALL, STRIKING HIS LEFT HIP ON THE EDGE OF THE PALLET. DID NOT SEEK MEDICAL TREATMENT UNTIL 5-3-00.
