Miner was working to clear an obstruction on a wash down hose. The line was pressurized causing the end of the hose to fly up striking the miner in left eye. Eye drops were prescribed.

While backing the roof bolting machine down the no. 3 entry the cable reel was taking up cable causing the trailing cable that was hung to be jerked down striking the miner on the head.

Miner was assisting in changing conveyor boom on a Joy 14/15 continuous miner when the boom unexpectedly shifted. The miner was caught between the boom and the mine roof injuring miners back. Injury resulted in paralysis below miners waist.

As employee was entering the maintenance office, EE jammed finger into door frame. Surgery was conducted on 12/19/2017 to repair tendon.

Picking up trash and back popped causing pain.

Miner was tramming roof bolting machine out of the face of the No. 2 entry from the operators deck and smashed pinky finger between the coal rib and the frame of the machine. Resulted in lacerations and fractures of the pinky finger.

An unintentional roof fall occurred approximately 2 breaks inby spad 16356 on the Low Gap Mains at 15 break along the old 8A belt conveyor in an adjacent entry. The area was supported with 48 inch fully grouted roof bolts. The fall measured 40ftL X 20FtW X 4ftH. All approaches to the fall have been supported. The fall will not be cleaned up.

Employee was helping co-worker replace a 6 inch plastic "T" waterline in the conveyor belt entry. After replacing the old "T" they turned the water back on and was examining the "T" for leaks when the 6 inch "T" burst causing pieces of the pipe to strike EE in the face.

Employee was installing a bucket jack on scoop. The jack slipped cutting employees right index finger. Employee was treated at Raleigh General Hospital where EE received seven (7) sutures.

As Employee was walking to the shuttle car he struck a bolt hanging from the top resulting in a laceration to his right ear. Employee was treated at Princeton Community Hospital where he received nine (9) sutures.

An unintentional roof fall occurred at Squealer Knob panel 3 entry spad 7569. The fall was approximately 12ft x 12ft x 3ft. There were no injuries and no equipment involved.

Employee injured his right hand/fingers while cleaning the pre-dump on the roof bolter. When he started the suction the flapper valve closed on his right hand. Employee was transported to Raleigh General Hospital for treatment where he received thirty-one (31) sutures.

While bolting, employee was struck in left hand by a piece of draw rock that fell from roof.

Employee has been diagnosed with Occupational Pneumoconiosis. The Safety Department received notification of his diagnosis on Oct 14, 2015.

An unintentional roof fall occurred on the #3 Section spad 16486. The fall was approximately 20ft x 20ft x 3-5ft. There has been a roof fall clean up planned developed and approved. A Joy 14/15 continuous miner was involved in the roof fall. No injuries were reported.

Employee has been diagnosed with Occupational Pneumoconiosis. Employee's employment terminated on November 1, 2014. The safety department received notification of his diagnosis on November 30, 2015.

Employee was loading belt structure in an insert when he felt a pain in his lower back, down his right hip, and into his thigh. Employee was treated at Oceana Medical facility.

After finishing the cut, employee was struck and injured by a rock.

As Employee was kneeled down cleaning out to set a timber in the #4 Entry, scoop operator (with a load of timbers) struck employee in the back. Employee was transported to Raleigh General Hospital for treatment.

An unintentional roof fall has occurred at the intersection of spad 12666 #7 entry Ellis Creek Sub Mains. The roof fall was approximately 20ft x 20ft x 4ft. There were no injuries or equipment involved.

Employee has been diagnosed with occupational pneumoconiosis. Employee retired July 31,2014 but was diagnosed January 21, 2015

While lifting sand jacks and pushing against top to let sand drain down, Employee felt a sharp pain in his lower back. Employee was treated at Raleigh General Hospital.

Employee was picking up rub rail to put on CM when he strained his back. Employee was treated at Raleigh General Hospital.

While shoveling belt on his knees Employee twisted to put coal on belt and felt a sharp pain in his back. He was treated Prime Care 12 in Princeton where xrays were taken.

Employee was moving a rock off the top of the continuous miner when the rock slipped striking Employee's finger. Employees finger was infected and has been treated with antibiotics. Accident was not reported timely by Employee to the Mine Management. Investigation has been pending.

An unintentional roof fall has occurred on the #2 Section #5 Belt #4-5 Entry Intake. The fall was approximately 40ft x 19ft x 4 to 5ft. No injuries were reported and no equipment was involved.

Employee was pulling a timber across the track when he felt a pull in his groin.

Employee was carrying a bucket of miner bits when he felt a pull in his back. Employee tried stretching his back to get some comfort and when he did he felt a pop. He immediately came outside and was transported by EMS to Raleigh General Hospital for treatment.

As Employee was horse playing with another employee his finger got caught on the belt strap of the other employee causing a fracture to his pinky finger.

Employee has been diagnosed with Occupational Pneumoconiosis. Employees Date of Last Exposure was 5-21-07 and was diagnosed October 1, 2013.

An unintentional roof fall has occurred on the #2 Section Left Return break 15 spad 6677. The roof fall was approximately 40ft x 18ft x 4ft. There were no injuries reported and no equipment involved.

An unintentional roof fall has occurred on the #3 Section #2 Entry spad 14837. The fall was approximately 90' x 20' x 8'-12'. No injuries were reported. The roof fall fell above anchorage point which covered up the continuous miner. A recovery/clean up plan has been submitted.

As Employee was taking down a stopping he picked up a mandoor and a jack fell over pinching his hand between the jack and mandoor. Employee received treatment at Raleigh General Hospital where he received three (3) sutures.

Employee was cutting off a belt flapper and attempting to remove a damaged clip with channel locks. The channel locks slipped striking Employee in the mouth breaking a tooth. Employee was seen by a dentist and had his tooth repaired.

An unintentional roof fall has occurred at the old 7A Section #3 #6 Belt Entry outby spad 13633. The roof fall was approximately 44ft x 19ft x 5ft to 6ft. There were no injuries reported and no equipment involved.

As Employee was moving Feeder in outby area of the scoop roadway the Feeder was stuck in a water hole. Employee placed a crib block under the cat to free up Feeder. As the cats were being raised Employees right ring finger was pinched between the crib and the lifting jack. Employee received treatment at CAMC General Division where he received 13 sutures.

As Employee was pulling cables he stepped into a hole and twisted his right knee. Employee was treated at Raleigh General Hospital. Xrays were negative and Employee was instructed to wear a knee brace for three days.

As Employee was pushing a loaded wheelbarrow it started to turn over. Employee attempted to counter balance the load when he strained his back.

As Employee was bolting a piece of drill steel broke off striking him on the forehead causing a laceration. Employee was treated at Beckley Appalachian Regional Hospital and received four (4) sutures.

Employee was cutting two (2) 4" to 6" pieces of belt. Employee was flipping belt over and making last cut when the utility knife slipped causing a laceration to right thigh. Employee was treated at Beckley Appalachian Regional Hospital where he received 6 staples.

An unplanned roof fall occurred on the #3 Section spad 13020 at 6A Belt Head tailpiece. The fall was approximately 30' x 20' x 4' to 6'. The 6A Belt Head, Tailpiece was covered by the fall. No injuries were reported.

As Employee stepped over continuous miner cable he felt his left ankle pop. He was treated at Raleigh General Hospital on January 8, 2013. Employee did not miss any work until he was seen by a specialist on February 8, 2013 and was instructed to remain off work until further notice.

Employee lifted a lid on a rock duster in an attempt to unplug it. He had his left hand resting on the opening when a mechanic hit the side of the machine with a hammer causing the lid to fall and strike the injured's left hand.

CMO bowed heads down against rock to push it over and out of the way to turn CM around when a bit broke off and flew backwards striking operator in the head above left eye.

Employee was using a strap hooked to a man trip to pull water line. The strap broke striking employee on the neck and face causing a contusion to his neck and broke his front tooth.

As Employee was attempting to pull a loose piece of rock off the top, several pieces of rock fell striking Employee on the head. The largest piece of rock was approximately 9.5" x 6" x 1'. Employee complained of neck pain and was transported outside and taken by EMS to CAMC General Division for further evaluation. All xrays and scans were negative.

Employee was watching the fork lift as they were pulling track rails back for the next belt. He was standing on the belt car and when he stepped off the fork lift he stepped onto a piece of belt and his foot went between the car and rib and the fork lift struck the belt car causing it to smash the employees foot between the rib and the belt car.

Employee sat down in the scoop roadway and turned cap light off and fell asleep. The dayshift scoop operator came down the roadway and ran over his lower legs primarily his ankles. He was also on the offside of the dayshift scoop operator.

As Employee was holding a piece of belt down to cut a flapper off the knife slipped and cut his left forearm. Employee was treated at Raleigh General Hospital and received four (4) staples.

an unplanned roof fall occurred in the #4 entry head end of 4 right panel 1 break inby the 13404 spad. This fall is a continuation of a previous fall that occurred on 3/9/12 e-document number 1535764.

While employee was loosing a bolt with a wrench the wrench slipped striking employee in the mouth knocking front tooth out. Employee continued his shift. Employee seen by his family dentist for treatment.

Employee has been diagnosed with Occupational Pneumoconiosis - During a Part 50 Audit it was determined a 7000-1 was not submitted for this Employee

Employee was moving cribs on top of scoop and caught right middle finger between crib and eye of scoop. Employee was treated at Beckley Appalachian Regional Hospital and was diagnosed with a fracture at the tip of his finger.

Employee was spot bolting cable bolts in #7 entry and a rock fell between bolts striking employee on his left thigh. Employee was treated at MedExpress.

An unplanned roof fall has occurred on the #3 Section #4 Entry 1 break inby spad 13439. The roof fall was approximately 20' x 40' x 8'. No equipment was involved or no injuries were reported.

Employee was loading material into dumpster when he smashed his finger between scraper and dumpster. Employee received treatment at Raleigh General Hospital. Xrays were negative. During a Part 50 Audit it was discovered this incident was reportable due to the Employee receiving a splint for his finger.

An unplanned roof fall occurred outby at spad #4441. The roof fall was discovered by examiner and MSHA inspector.

A unplanned roof fall occurred 1 break inby of spad #13404 on the #3 section in #4 entry. The fall was 15' wide 16' in length. The fall was investigated by mine Foreman and MSHA inspector . The section has been stopped and turned left-handed 400 feet back.

A unplannned roof fall occurred at 3A belt break 114. The roof fall was discovered by examiner and MSHA inspector.

While backing bolter out #6 entry, there was a rock in the roadway in the intersection. The bolter was trammed onto the rock and when the machine dropped off the rock it jolted the employee's lower back. Employee was transported to CAMC General Division for evaluation.

Employee was loading rock dust on scoop. As he turned he felt pain in lower back. Employee was treated at MedExpress.

Employee was pulling on rail with a slate bar when he slipped and fell striking a set of comalongs which were laying beside the track injuring his back.

While Employee was moving belt he hooked a set of comalongs to the scoop and began to tighten the chains when the comalongs broke. The chain struck Employee inside the operators deck of the scoop striking him on the head behind ear. Employee was transported to Raleigh General Hospital for further evaluation.

As employee was checking faces he stepped on a ledge in an intersection causing his ankle to roll. Employee was transported to CAMC General Hospital for treatment. Employee was diagnosed with a fracture to his ankle.

Employee was walking outside to get welding supplies. As he was walking under the canopy he struck his head on an 8in I-Beam causing swelling to his face. Employee was treated at the scene by EMTs and transported to Raleigh General Hospital for further evaluation.

While Employee was bolting in #3 entry a loose piece of draw rock fell striking the ATRS on the roof bolter. A small piece of draw rock struck Employee on the left thigh. Employee was transported by EMS to Raleigh General Hospital. X-rays were negative and was instructed to follow-up with a specialist.

As Employee was attempting to pull water manifold out of gob he felt a sharp pain in his left shoulder and back area. Employee drove to Raleigh General Hospital for evaluation.

A rock measuring 4' x 7' x 1"-4" fell striking Employee on his right foot. Employee was transported to Raleigh General Hospital and was diagnosed with a fractured big toe. Employee was scheduled for out patient surgery but instead was admitted on 9-23-11 for surgery to correct his fractured toe.

As Employee was bolting in #7 entry the drill chuck was not lined up with the bolt head. Employee hit the rotation lever causing his hand to twist in the bolt. Employee was treated at the scene by EMTs and drove himself to Raleigh General Hospital for further evaluation.

While Employee was pulling miner cable out of roadway in the #2 entry he felt extreme pain in his lower back and left upper thigh. Employee went to MedExpress for treatment on Aug 10, 2011 and was put on modified duty due to his injury.

Employee was building a stopping approximately 7 1/2 feet high. Employee stood on a cinder block which overturned causing him to stumble twisting his left knee. Employee was taken to Raleigh General Hospital and was diagnosed with a knee strain.

Member was reaching for drill steel when a rock 36" wide and 4" thick fell on his left forearm and shoulder. It cut his forearm with a 4" gash. Member received 11 stitches at Raleigh General Hospital.

Contractor was told to plaster stoppings but had left his plastering gloves on the mantrip, which had already returned outside, so his boss informed him to plaster the stoppings with metacarpal gloves instead. Contractor later went to Raleigh General seeking medical attention, where he was told he had sustained first and second degree chemical burns to his hands and arms.

Member stated he used forklift to try and pick up a piece of belt and turned sharp, causing the forklift to pivot up in the middle and slam his head into the top. He felt a pop in his neck on the left side. The area was 48" high and 18 1/2 feet wide. Member was taken by ambulance to the hospital where he was diagnosed with a fractured vertebrae.

EE was making a splice on some cable. While skinning the cable, his utility knife slipped and cut his right hand between the thumb and index finger.

EE was watching the scoop to push the rail on rail trucks, the rail got caught on Fender of scoop. EE and another ee decided to push rail over with slate bar. EE put pressure on back of the rail while ee took bar and attempted to slide rail over. The rail slid off the side of the scoop and caught his leg underneath it.

The belt had already been cut and skived. EE was going to trim a little piece of the belt off, when he did the knife slipped and cut his left hand index finger.

Member bent over to remove cap coal that had fallen onto his neck, while an approaching shuttle car was turning toward right handed entry, the shuttle cars rearend left side bumped the member.

An unplanned roof fall has occurred at or near spads 4307 & 4442. These are worked out ares of the mine and the falls were found during weekley examinations of airways. No injuries reported. Area developed in 2003.

Member was in the process of removing a rock measuring 2'Long X 18"wide X 8"thick from inbetween the cats of the miner and the pan, when he bent over to remove the rock he felt a pain in his lower back.

An unplanned roof fall occurred in the track entry, one crosscut inby spad # 0404, in the # 6 break on 2a belt line. The fall measured 20'Long X 12'Wide X 1.5' in Height. No accidents were reported.

EE was walking down the belt entry when his left shoulder hit a belt hanger causing him to fall backwards onto his SR100 SCSR. EE continued to work until 06-17-2010. Due to continued pain he visited Med Express and was instructed to remain off work.

Employee was making a belt splice, hammering nails into splice when he felt pain in his right shoulder. He said it was probably where he had a steel plate in it from a previous accident. Didn't report it at first because I thought it a reoccurence of an old injury. Then Dr. said it came loose due to work he was performing.

Roof fall in the worked out panel of 1st Left off Pug Mains in the #2 Entry at Spad #7399 Roof fall was 60' X 18' X 4'. No injuries or equipment involved.

Using a knife to remove a board on belt roller. Knife slipped and went thur glove, cutting the left little finger.

While tramming a battery powered Fairchild 35C Injury's middle finger of his left hand became tangled in a nylon cable hanger causing it to pull the distal phlange off.

While making a vulcanize splice, EE cut the top of his left thumb, requiring stitches.

Employee was carrying a roll of fly pad(brattice)material, preparing to hang them in #6 entry. As he was walking he fell, cutting his hand on a piece of rock which required 5 stitches.

While making a belt splice on the #4 belt, EE attempted to throw a belt clamp across the belt, causing it to hit a water spray bar. It bounced back, hitting him in the left ear, causing a laceration that required 10 stitches.

EE stated when traveling #2 entry in 21 Joy Shuttle Car on #1 section, the miner cable fell down across his roadway. He stopped the car, attempted to hang cable by himself, when he hurt his back. Requested transportation to ER, where diagnosis was negative and released to RTW the following day(no restrictions). He continued to work until 10/31/08.

While attempting to let down a miner cable that was hung up, employee's hand was punctured by a cable hanger requiring 4 stitches.

Putting top rollers on belt structure.

Member was pushing cable into an entrance gland of the starting box. His foot slipped in some mud causing him to twist his left knee which resulted in a dislocation of his left knee.

Member was lifting a piece of metal for building a rock box and felt pain in his stomach. A small knot came up. He went to the doctor and was diagnosed with a hernia. No surgery has been done at this time.

While riding in the front of a 5 ton supply locomotive that attempted to stop on a slight grade, but started sliding & slid off the end of the track, hitting a parked roof bolter. He refused emergency care & transportation. When he visited the hospital he was referred to a specialist. On 07-30-08 Marfork Safety was notified he had been diagnosed with a concussion.

EE was in the process of helping electrician change canopy jack on roof bolter. The jack slipped and pinned his thumb between the jack and the ATRS on the machine causing a laceration that required 24 stitches.

While crawling in a area that was wet and muddy causing employee's knee pad to be pulled off in the mud, his left knee became twisted resulting in the meniscus. He continued to work until 09/24/2008. Marfork Safety was not informed by employee that he was missing work due to this injury until 10/06/2008.

EE was crawling across section when he put his weight on his right arm and it buckled under him and caused him to fall onto his arm and dislocate his elbow.

Member was picking up trash in supply yard while trying to throw a piece of crib block into the dumpster he struck his left arm on the dumpster causing a fracture to the lower arm in the area of the elbow.

On 2-25-08 employee twisted his right knee while walking in the intake entry. He continued to work until 4-9-08 without losing any time. Due to continued pain he was informed by his doctor that surgery had to be performed on 4-9-08. Recovery time is expected to be six weeks.

While loading belt rubber onto a car ee stepped on a rock and twisted his left ankle.

While making up a belt splice, employee was cutting belt. His knife slipped and cut through his glove causing a laceration to his left pinky finger that required 11 stitches.

Due to repeated trauma, employee has been diagnosed with severe degenerative arthritis of the spine due to occupational illness. This was not due to a single event and is substantiated by medical records.

Bent over to pick up gloves, hit left elbow on a roof bolt plate causing a laceration that required 4 stitches.

While attempting to cut tape off a lever of a Joy 21 shuttle, the knife slipped causing a laceration to his ring finger of his left hand. The laceration required 3 stitches.

While handling belt structure, EE's right middle finger became pinched between the belt tail piece and structure.

When employee turned to exit the deck of an Eimco scoop he felt his right knee pop. He continued to work until 8-30-07.

While loading belt structure, employee's hand became pinched between two pieces of structure.

Employee stated that on 6-21-07 when attempting to hang a miner cable he felt a pain in his back. This accident was not reported until 8/27/07. Employee continued to work until 8-30-07 when instructed to remain off work.

Individual was attempting to pull a matrip to motor barn with another mantrip. The mantrip being towed derailed in a curve and landed on his right foot resulting in fractures. Individual was communicating with operator of the other mantrip while standing beside the mantrip.

Individual was tramming bolter for new set up and ran over 8" ledge causing him to hit his head and ear on canopy resulting in a laceration requiring 7 stitches to ear.

A roof fall measuring 20' X 20' X 5' was discovered by a fireboss approx 3500' from working section. No injuries occurred. The fall will be cleaned up and supported per roof control plan.

Employee was moving the continuous miner cable from the roadway when he twisted his right knee. He was seen at the health place on 1-8-07 and was sent for an MRI. On 1-17-07 the results of the MRI were: a torn meniscus.

While raising tailpiece, guard dropped smashing middle finger.

While sawing a timber for use in pillaring, EE was hit in the back with another timber that had been set when another EE bumped it as he was attempting to lay another timber down.

While Building a brattice in the x cut between #7 & #8 entry EE was working 18" away from a check curtain. A 488 Long airdex scoop after stopping and sounding and his bell started through the curtain and hit him.

A roof fall occurred on # 2 section in # 3 entry at appromixately 4:50 PM. The roof fall measured 80' long 20' wide and 4' high. The area was supported with 48" fully grouted resin bolts. There were no injuries. Fall was immediately timbered or cribbed off at all entrances.

Member was setting 8' square 6 x 6 timbers. He knelt down to get a wedge when the timber he was setting fell and struck him in the back. He had not wedged the timber prior to turning it loose to pick up another wedge.

Employee was entering parts building on surface. When entering he turned and twisted right knee. Investigation revealed that there was no stumbling or tripping hazards found.

WHILE ATTEMPTING TO PUSH THE LOW STRUCTURE BACK FOR A BELT MOVE, EMPLOYEE GOT ON TOP OF BELT. THE STRUCTURE BOWED UP CRUSHING HIM AGAINST THE TOP.

EE was hanging the cable of a joy 21 shuttle car and came into contact with electricity at a splice.

Roof fall in #2 entry at spad #6938. 60' x 20' x 5'. No injuries. #1 section.

EMPLOYEE LIFTED A 40 Lb. BAG OF ROCKDUST TO PUT IT INTO A ROCKDUSTER. AS HE TWISTED TO PLACE IT INTO THE DUSTER, HE FELT A PULLING SENSATION IN HIS LOWER BACK.

AFTER COMPLETING THE MINING OF #2 ENTRY, WHILE BACKING THE BRIDGES DOWN THE BELT ENTRY INJ EE PLACED HIS HAND ON THE LOWER REAR EDGE OF THE OPERATORS DECK DUE TO GETTING TOO CLOSE TO THE LOW STRUCTURE EE'S HAND BECAME CAUGHT BETWEEN THE LOW STRUCTURE AND THE BRIDGE DECK.

EE was carring 2-5 gal cane of iso 68 hyd. oil around a water hole. He slipped and fell one of the cans of oil hit on his little finger right hand lacerating the bottom flesh part off.

INJURED EE FELL STRIKING THE BACK OF HIS NECK ON A 6"X6" TIMBER. HE WAS TRANSPORTED TO CHARLESTON CAMC GENERAL DIVISION. WAS REFERRED TO A SPECIALIST. ON 8/19/05 EE VISITED SPECIALIST, BUT A DIAGNOSIS COULD NOT BE GIVEN DUE TO SWELLING. MARFOLK SAFETY WAS UNABLE TO CONTACT INJ EE UNTIL 8/25/05 TO GET RESULTS.

Cleaning in #5 entry with scoop, undercut coal rib causing brow to fall and hit member in back and neck. Member had fractures to neck and back.

Member had completed installing an 8' cable bolt. As he started around the roof bolter he struck his head on the bolt causing a pain in lower back.

Roof fall in #4 entry. 100' x 20' x 5'. No injuries. 48" fully grouted resin bolts were being used. Inby spad #6621.

Member was scooping roadway holding cable up that was hung across roadway got finger cut between canopy and roof plate.

EMPLOYEE WAS HANGING CABLE WITH COATED TIE WIRE. CABLE SLIPPED & FINGER WAS CUT & PUNCTURED BY THE WIRE, REQUIRING 5 STITCHES.

EE WAS CRAWLING BESIDE LOW LOW STRUCTURE WHEN EE GOT TO TAIL PIECE A SMALL PIECE OF ROCK FLEW OUT OF TAIL PIECE AND HIT EE IN MOUTH BREAKING TOOTH

EE WAS TIGHTENING LUG BOLTS ON SCOOP. TIRE RATCHET SLIPPED AND HIT EE IN THE CHIN AND MOUTH.

#8 belt head ee was installing a 150 hp belt head motor while hoisting the motor in place it suddenly swung toward the belt head frame. While trying to stop it EE felt a sudden pain in his lower back.

Employee was attempting to adjust the hyd pressure on a cont miner. He raised a shield app 3" and slid it to the side, when he felt pain in his lower back. The shield size was 26' by 30" by 1/2".

EE WAS STANDING IN THE INTERSECTIONS OF 3XR. A PIECE OF DRAW ROCK FELL MEASURING 24" X 15" 3" STRIKING HIM ON THE RIGHT FOOT CAUSING A CONTUSION.

MEMBER WAS REMOVING CRIB BLOCKS FROM UNDER A SCOOP. WHEN REACHING DOWN TO GET ONE, ANOTHER CRIB BLOCK FELL FROM THE TOP OF THE SCOOP, STRIKING HIS FINGER, LACERATING IT. IT REQUIRED 13 STITCHES.

EE WAS ATTEMPTING TO LIFT A ROCK OFF OF A CONTINUOUS MINER AND STRAINED HIS LOWER BACK.

EE WAS INSTALLING A 48"RESIN ROOF BOLT IN THE #2 ENTRY. HE ATTEMPTED TO TURN THE PLATE ON IT WHILE PUSHING BOLT UP & GOT THE END OF HIS MIDDLE FINGER ON LEFT HAND PINCHED. REQUIRING 10 STITCH ES. EE WAS NOT SCHEDULED TO WORK AGAIN, NOT UNTIL 10/20/03 DUE TO A RELIGNMENT OF THE WORK FORCE. EE WAS NOT SCHEDULED TO WORK AGAIN UNTIL 10/20/03 DUE TO A REALIGNMENT OF THE WORK FORCE

EE WAS UNLOADING BELT STRUCTURE FROM A CAR. HE REACHED INTO CAR & STARTED TO PICK UP A PIECE AND FELT A PAIN IN HIS GROIN ON THE LEFT SIDE.

EE WAS CROSSING THE CONVEYOR BELT WHEN THE BELT CAME ON THROWING HIM OFF, CAUSING TORN LIGAMENTS IN HIS LEFT KNEE.

EE WAS HANGING A CURTAIN IN A CROSSCUT TO THE LEFT OF THE BELT ENTRY WHEN A PIECE OF ROCK FELL OUT OF THE RIB, STRIKING HIM ON THE RIGHT KNEE CAUSING A SPRAIN. HE CONTINUED TO WORK UNTIL 9-8-0 3.

EE WAS LIFTING THE AX RAIL OF A VULCANIZING PRESS UP ONTO A FLAT CAR, WHEN HE FELT A PAIN IN HIS LOWER BACK. HE CONTINUED TO WORK UNTIL 9/11/03. HE WAS DIAGNOSED WITH A POSSIBLE RUPTURED DISK.

EE WAS OPERATING A 482 S&S SCOOP HE WAS CLEANINGUP MUD WHEN HE HIT THE RIB WITH SCOOP DECK A PRICE OF ROCK CAME OF STRIKING HIM IN THE BACK

EE WAS OPERATING A 482 SCOOP WHEN HE RAN OVER A ROCK JARRING HIM & CAUSING A LUMBAR STRAIN

EE WAS ROOF BOLTING IN #3 CROSSCUT TO THE RIGHT HE HAD INSTALLED THE BOLT AGAINST RIB AND WHEN HE MOVED THE ROOF BOLTER HEAD OVER A PIECE OF ROCK MEASURING 4'X4'X3" FELL OUT HITTING HIS LEFT LEG.

EE was helping to cut out a peice of belt that was wrapped around the take up. he was standing on the take-up roller when he slipped and fell app 2'

WHILE OPERATING A 21 JOY SLEAR EE RAN OVER A LEDGE IN THE BOTTOM CAUSING HIM TO STRIKE THE UNDERSIDE OF THE CANOPY HE CONTINUED TO WORK UNTILL 08-15-03 HE WAS DIAGNOSED WITH A CERVICAL STRAIN.

WHILE OPERATING A 18 TON BROOKVILLE MOTOR EE STARTED UP A GRADE. THE LOCOMOTIVE LOST TRACTION AND BEGAN TO SLIDE BACKWARDS, EE JUMPED OUT, HE FELL STRIKING HIS RIGHT HAND ON A ROCK LACERATIN G HIS MIDDLE FINGER.

EE DROPPED AN OPEN BUCKET OF STOPPING PLATER CAUSING TO THE SPLASH UP UNDER HIS SAFETY GLASSES, IRRITATING BOTH EYES.

EE WAS LOADING 1000' OF BELT RUBBER ON A CAR WITH A SHEIVE ROPE & DUCK BILL BELT CLAMP. THE DUCKBILL SLIPPED OFF HITTING HIM IN THE MOUTH CAUSING A LACERATION TO HIS CHIN

ROOF FALL ON MAINLINE BELT AT SPAD #2487. 90'L X 20'W X 5'H.

CONTINIOUS MINER HAD COMPLETED THE LAST LIFT IN THE PILLAR BROCK BETWEEN # 2 & 3 ENTRIES AND WAS BACKING OUT WHEN A PIECE OF ROCK 12'X?X14" THICK FELL FOULING CONTINOUS MINER.

INTERSECTION TWO BREAKS OUTBY PILLAR LINE #3 ENTRY. ROOF FALL. 4 1/2' THICK 20' X 30' AT SPAD #3812.

EMPLOYEE WAS CHANGING A BUSTED HYDRAULIC HOSE ON THE SECTION SCOOP. HE HAD A SHIELD RAISED UP & NOT BLOCKED. IT FELL ON THE MIDDLE FINGER OF HIS RIGHT HAND. HE RECEIVED 3 STITCHES.

MEMBER WAS OPERATING THE MINER IN THE FACE OF #2 ENTRY WHEN A PIECE OF ROCK FELL OUT ON TOP OF IT. HE BACKED MINER TO SUPPORTED ROOF. THE CABLE ROLLED NEAR CATS. HE STOPPED MINER, BENT OVER TO MOVE CABLE. ROCK SLID OFF MINER STRIKING HIM ON RT KNEE.

EE HAD POSITIONED A ROOF BOLTER FOR THE 1ST ROW OF BOLTS IN THE 4 RIGHT CROSSCUT. AS HE RAISED THE ATRS A PIECE OF ROCK APPROX 120" LONG X 23" WIDE X 2-3" THICK FELL FROM BETWEEN THE BOLTS. A PIECE OF THE ROCK MEASURING 22" LONG X 24" WIDE X 3" THICK STRUCK HIM ON THE RIGHT FOREARM RESULTING IN A LACERATION REQUIRING SUTURES.

EE WAS LOADING A SHUTTLE CAR IN THE FIVE RIGHT XCUT WHEN A PIECE OF ROCK MEASURING (9' LONG X 47" WIDE X1-3.5' THICK) FELL FROM THE MINE ROOF FROM BETWEEN THE FIRST AND SECOND BOLT OUT FROM TH E RIB STRIKING HIM ON THE LEFT LEG AND ANKLE. A FRACTURE WAS SUSTAINED FROM THIS INJURY.

WHILE PERFORMING RETREAT MINING IN #3 ENTRY OF #1 SECTION 014-0MMU, THE INTERSECTION FELL WHILE TAKING #4 LIFT TO THE RIGHT OF #3 ENTRY. THE FALL WAS APPROX. 5'H X 60'L X 20'W. NO INJURIES.

SITTING ON BOTTOM WITH FEET AGAINST BELT, PULLING OUT BELT PIN WITH CHANNEL LOCKS. FELT A BURNING SENSATION IN BACK WAS TREATED AND RELEASED WITH TORN MUSCLE IN BACK AND TOLD TO REMIND OFF WOR K 1 WEEK. RETURNED TO WORK 4-22-02. WORKED 2 DAYS AND EXPERIENCED PAIN IN BACK. OFF FROM WORK AGAIN 4-25-02.

EE WAS OPERATING A CONTINUOUS MINER IN THE #1 ENTRY WHEN A RIB ROLL OCCURRED STRIKING HIM ON THE LEFT LEG. THE ROLL MEASURED APPROX 3' LONG BY 2' WIDE BY 3-6' THICK. HE WORKED A TOTAL OF 6 SHIFTS BEFORE SEEKING MEDICAL ATTENTION FOR THIS ACCIDENT. ON 4-18-02 HE VISITED THE DOCTOR,TOLD TO STAY OFF WORK, AND RESCHEDULED. ON 4-22,DIAGNOSED WITH POSSIBLE TORN LIGAMENTS.

EE WAS DISMOUNTING A SHUTTLE CAR IN AN ATTEMPT TO REPOSITION A PIECE OF VENTILATION CURTAIN AND A SECTION OF ROOF BOLTER CABLE WHEN HE STUMBLED ON THE METAL RAILING SURROUNDING THE FLOOR OF TH E DECK AND FELL TO THE GROUND. HE RECEIVED A FRACTURED BIG TOE FROM THIS OCCURRANCE.

EE WAS WALKING FROM THE SOUTH PORTAL BATHHOUSE TO HIS VEHICLE IN A GRAVELED LOT WHEN HIS RIGHT ANKLE SUDDENTLY LOST ITS STABILITY RESULTING IN HIS FALLING ON HIS LEFT ARM.

EMPLOYEE WAS REMOVING THE OPERATOR SIDE REAR WHEEL FROM 21SC. AFTER TAKING OFF THE LUG NUTS, EMPLOYEE TOOK A LADDY SLIPPER TO HELP PRY THE TIRE LOOSE FROM THE WHEEL UNIT. THE WHEEL FELL OVER, SMASHING HIS RIGHT FOOT AGAINST A CRIB BLOCK. HE WAS DIAGNOSED WITH A CONTUSION.

EE WAS WALKING ACROSS THE TRACK TO THE SCOOP BATTERY CHARGER WHEN HE STEPPED IN A HOLE TWISTING HIS LEFT ANKLE.

EE REAGGRAVATED HIS KNEE FROM AN INJURY ON 11/27/01 AT THE WHITE QUEEN #2 MINE. HE REINJURED THE KNEE WHILE BUILDING A STOPPING AT THE BRUSHY EAGLE/ELLIS MINE ON CHRISTMAS VACATION.

EE WAS REMOVING A CHECK CURTAIN WHEN A PIECE OF ROOF ROCK FELL STRIKING EE ON HEAD, THEN STRIKING EE ON RT. ANKLE. EMPLOYER WAS NOT NOTIFIED UNTIL 12-22-01. EE IS ON LIGHT DUTY UNTIL CAST IS REMOVED. MINE WAS IDLE ON THE DEC. 23-25,2001.

ALL APPROACHES EXCEPT ONE HAVE BEEN BREAKERED OFF AND CLEAN UP PLAN POSTED. 20' X 20' X 5'.

EE WAS DROPPING A PIN THROUGH A TONGUE IN THE ACT OF HOOKING UP A RAIL CAR TO A MOTOR. THE MOTORMAN PULLED FORWARD RESULTING IN THE PIN KICKING FORWARD, MASHING / PINCHING HIS FINGER.
