Employee was climbing onto a 773D Rock Truck and struck knee cap on the ladder.
The employee and a co-worker were moving the metal lid of the #2 Slurry detection box by hand to check the flow meter. The lid slipped and the employee's hand was pinched between the lid and box frame, cutting a finger.

Individual was bent over, cutting a piece of metal out from between crusher rolls, when a piece of mud that was stuck to the side of the chute fell, struck the employee, knocking EE into the side of the chute.

Unhooking trailer from truck, when he used fender as a step causing him to lose his footing and fall. Should have used steps on truck and 3 points of contact to get down from vehicle.

The #8 raw coal feeder became blocked and a void was created. The dozer operator was in the process of cutting to the void when he trammed too close to the void's edge and the dozer he was operating slipped into the void.
A pump repair was in progress. The flywheel came off and struck the employee in the abdomen, chest, and face. The abdomen and chest were bruised, the chin was cut, and two teeth were chipped.

A fire occurred in the engine compartment of the Caterpillar D10 dozer as it was pushing coal on the raw coal stockpile. The fire was quickly extinguished (under 5 minutes) as the fire suppression system functioned as designed. (*No personal injuries. Both the Federal and State agencies were notified)

New contract EE being trained on dozer on the raw coal stock pile by supervisor. After several hours, supervisor thought he was ok to work alone. EE pushing coal into large void and trammed too close to edge of angle, slide sideways and could not get out. EE pushed STOP control/all activites were shut down. A dozer brought to fill void, allowing stuck dozer to tram free.
Climbing back on truck after lunch, missed hand rail and fell to the ground backwards.

The EE was cutting a 2" X 4" board with a curcular saw, when the saw kicked back and cut the EE on the right leg, requiring sutures.

Employee was carrying piece of steel angle and tripped on an extension cord. Falling into a piece of equipment. Received contusion to the right side of chest and shoulder.

The employee was disassembling a truss, a sleaving bar was inserted to pry the sections apart. When the sections seperated, the bar forced the employee's left hand into the steel structure.
The employee was rigging a 6' long piece of 6" dia. PVC (plastic) pipe with chain hoists, the pipe slipped and fell app. 15" - striking the employee's left hand.
Carrying an Acetylene cylinder with a coworker, the coworker dropped his end, the impact caused the injued employee, to drop his end striking him in the right lower leg (calf).
Installing bolts in structural steel at the loadout system and hit his finger with his own hammer.
While kneeling on his left knee the employee was hammering a concrete expansion bolt into a hole when he felt a "sharp" pain between his shoulders.
The employee was using a pipewrench to tighten a 3/4" diameter pipe. The pipe was overhead - the employee was pulling down - a "pop" was heard and pain was felt in the right shoulder.
The employee was preparing to hoist a 10' peice of polyethylene pipe that included a ceramic fitting on one end, the pipe was resting on a hand rail, the fitting caused the pipe to roll falling off the hand rail. The "plastic" pipe bounced striking employee on both the bounce and the fall
Employee was using a cutting torch - a piece of slag entered the employee's LEFT eye. The employee was wearing eye protection.

The employee unhooked a load that was placed by a crane, a coworker did not maintain control of the headache ball allowing it to swing into the injured employee - striking him in the head.
Employee felt a pull in his right hip/lower back while pushing on a steel chute that was being installed.
Employee began feeling pain in groin and low back. Dr. initially diagnosed kidney stones, after off work for several days it was determined to be low back muscle strain. Began missing work on 8/8/2005.
Incident reported on 5/5/05, no medical treatment sought until 5/16/05. Employee was loosening a bolt with a boxed end wrench. He jerked on the bolt to loosen it - pain was felt in his RIGHT shoulder.
