The employee and a co-worker were moving the metal lid of the #2 Slurry detection box by hand to check the flow meter. The lid slipped and the employee's hand was pinched between the lid and box frame, cutting a finger.

Individual was bent over, cutting a piece of metal out from between crusher rolls, when a piece of mud that was stuck to the side of the chute fell, struck the employee, knocking EE into the side of the chute.

The #8 raw coal feeder became blocked and a void was created. The dozer operator was in the process of cutting to the void when he trammed too close to the void's edge and the dozer he was operating slipped into the void.

A pump repair was in progress. The flywheel came off and struck the employee in the abdomen, chest, and face. The abdomen and chest were bruised, the chin was cut, and two teeth were chipped.

A fire occurred in the engine compartment of the Caterpillar D10 dozer as it was pushing coal on the raw coal stockpile. The fire was quickly extinguished (under 5 minutes) as the fire suppression system functioned as designed. (*No personal injuries. Both the Federal and State agencies were notified)

New contract EE being trained on dozer on the raw coal stock pile by supervisor. After several hours, supervisor thought he was ok to work alone. EE pushing coal into large void and trammed too close to edge of angle, slide sideways and could not get out. EE pushed STOP control/all activites were shut down. A dozer brought to fill void, allowing stuck dozer to tram free.

The employee was using a pipewrench to tighten a 3/4" diameter pipe. The pipe was overhead - the employee was pulling down - a "pop" was heard and pain was felt in the right shoulder.

The employee was preparing to hoist a 10' peice of polyethylene pipe that included a ceramic fitting on one end, the pipe was resting on a hand rail, the fitting caused the pipe to roll falling off the hand rail. The "plastic" pipe bounced striking employee on both the bounce and the fall
