Allen Powellton Mine Coal

Press bolt failed during belt splicing operations. Employee was struck in the left hand by press bolt or other unknown object and received a left hand contusion(No fractures) resulting in time off from work. Initial Dr. Release was estimated at 3 days.

While cutting a belt jumper, the knife slipped and employee cut right thigh resulting in 8 stitches.

Employee was diagnosed with pneumoconiosis.

Miner trammed rail mounted locomotive onto foot, pinching foot between frame of locomotive and ground.

While pulling the right middle shuttle car's trailing cable and anchor, employee felt pain between the shoulder blades.

While changing a female battery plug receptacle, employee's allen wrench came in contact with a battery post causing an arc resulting in a burn to employee's hand.

While installing roof bolts, miner's fingers became entangled around the drill chuck/roof bolt.

While loading roof bolt supplies from the scoop bucket, a bundle of roof bolts that was positioned on its side rolled over striking employee on the leg.

While attempting to load a hydraulic pump from a rail mounted personnel carrier to an underground four wheeler, employee pinched finger between pump and frame of four wheeler. Resulted in laceration requiring stitches and a fracture.

While operating roof drill, cap coal fell from roof striking miner's left hand fracturing the middle finger. Employee began missing work on the 02/17/2020 shift.

Employee strained back while loading supplies in scoop bucket.

Employee was installing a bolt for a shuttle car anchor when EE lost EE's footing and fell resulting in a bruise and soreness. EE continued to work until 11/03/2019 when EE was taken off work due to infection that resulted from the injury.

Employee exited scoop and rock fell from rib striking employee on the left knee causing them to fall forward. Incident was reported immediately to MSHA. Employee received full work release from medical facility.

While employee was carrying a bag of rock dust employee felt a pain in lower back.

Miner was diagnosed with pneumoconiosis.

Miner was cutting open a rock dust bag when knife slipped and cut miner's left hand. Resulted in four stitches.

While picking up cut ends from mine timbers, a timber fell over striking the employee on the right hand. Employee continued to work and sought no medical attention until 01/25/2019 due to continued discomfort. Diagnosed with fracture of the 5th finger on the right hand.

While raising a canopy on a JOY 10 S/C Shuttle Car the canopy slipped smashing employee's left thumb between the canopy and the frame of the shuttle car. Employee received 7 stitches. A hairline fracture was also found.

Miner picked up a rear end for a rubber tire four wheeler and felt a pop in their lower back.

Employee was diagnosed with pneumoconiosis.

Employee twisted back while entering scoop deck.

Employee was helping pick up trash. The scoop operator dropped scoop bucket to mine floor. Employees foot was caught between the scoop bucket and the mine floor.

Miner was operating the continuous mining machine in the No. 4 Entry when a rock fell from the top striking miner on left side.

While carrying a can of hydraulic oil, miner stepped in a hole in the mine floor causing the miner to twist EE's back.

While loading the supply tray on the roof bolter, miner felt a pop and pain in shoulder.

While loading cinder blocks in scoop bucket, miner twisted EE's back.

While unloading roof control supplies in a scoop bucket, injured slipped and fell twisting left knee and bumping head. While at hospital, employee had a severe allergic reaction to the contrast dye injected to perform a CT scan and was hospitalized.

Miner was walking outby the section load point in the #3 Entry when a slab of rock fell suddenly from the mine roof striking miner on top of shoulder.

Employee smashed finger while removing top pin from the head jack on the continuous miner. Resulted in multiple stitches.

Employee was loading roof bolt plates on roof bolting machine and struck arm against the plates resulting in a laceration. Received 6 sutures and a tetanus shot.

Employee was walking in the No. 8 Entry when they came in contact with a curtain hook striking them behind the left ear. This resulted in a laceration requiring 11 sutures.

While installing roof bolts in the first row of the No. 8 Right cut, employee was drilling when a roof bolt plate became lodged between the supply tray and the drill boom causing the roof bolt plate to be ejected striking the employee on left cheek. Resulted in a laceration requiring 5 sutures.

Employee was diagnosed with pneumoconiosis.

Employee was assisting in moving the miner cable when EE strained EE's back.

Employee has been diagnosed with Occupational Pneumoconiosis.

Employee was on knees, cutting zip ties from drill steel. Employee cut the tie nearest to self and as was reaching for the second tie, Lost balance, fell and cut left wrist with the Hawk Bill knife.

Employee had just put scoop on charge and was walking through intersection where the roof was high then lowered where EE ran into a fly board cutting forehead,just above the eye.

Employee has been diagnosed with Occupational Hearing Loss.

While traveling to the section, The roof bolt operator exited the man trip to open the doors on the track and twisted right ankle.

Employee was tramming shuttle car and hit a hole jamming head into the canopy of the shuttle car.

An unintentional roof fall occurred on the #1 Section #5 Entry 1 break outby spad 5818. The fall was approximately 25ft x 20ft x 6-7ft. There were no injuries reported. Two (2) mobile roof support jacks were involved in this roof fall.

Employee was taking a scoop off charge. As he was trying to lower the battery lids, the lid got caught on the fire suppression rack above the batteries. When he tried to lift the rack out of the way the lid fell on his right hand crushing his index finger.

Employee was walking bent over to check the charge on a four-wheeler when he felt a pain in his lower back. Employee had a compression fracture to the L4 vertebrae from a previous injury. After an internal investigation we feel this should be classified as a Degree 7 Occupational Illness. Medical notes to follow.

As Employee was walking around scoop a piece of rock fell striking Employee on the neck and back. This accident has been under internal investigation.

While Employee was moving an anchor back to bolt the anchor slipped striking Employee in the lip causing a laceration resulting in two (2) sutures.

Employee has been diagnosed with Occupational Pneumoconiosis.

Employee strained his back while helping hang 4" waterline which had fallen onto the track. Employee was treated at Raleigh General Hospital.

Employee has been diagnosed with Occupational Carpal Tunnel Syndrome

Due to repeated trauma Employee has experienced Occuaptional Illness is being filed for Degenerative Disc Disease. Attached medical notes will follow.

While bolting top the drill steel broke swinging back and striking Employee in the right hand thumb area. Employee did not seek medical attention until 6-19-14 and was diagnosed with a fracture.

Employee was using come-a longs to pull a top rail when the chain slipped on come-a longs causing rail to fall on Employee's left knee. Employee was treated at Raleigh General Hospital.

While Employee lifted lid to the shuttle car cable reel the SC operator pulled forward causing the lid to fall smashing Employee's left ring finger. Employee was transported to Raleigh General x-rays were taken and no broken bones. Employee was scheduled for surgery at CAMC Memorial Division for skin grafts to repair the finger.

Employee was bending over to attach fish plates to track rail when he experienced pain in right side of neck and shoulder. Employee was treated at Raleigh General Hospital. Employee continued working until 11/20/2013, at that time the physician instructed him to be off work until further notice.

Employee has been diagnosed with Occupational Pneumoconiosis.

As Employee walked past scoop batteries his shoulder hit the lid which was propped up. The lid fell striking Employee's right hand which caused a simple fracture to the 5th metacarpal bone.

Employee picked up a 40lb bag of rock dust to put in duster. While picking up bag employee felt sharp pain in lower back. Employee did not seek medical treatment until August 12, 2013 where he has undergone and will continue physical therapy for two weeks.

Employee has been diagnosed with Occupational Pneumoconiosis. Marfork Safety Department was notified on September 18, 2013.

He was driving shuttle car from the continuous miner toward feeder, attempted to readjust himself and tires hit hole in roadway causing him to hit canopy with hardhat jamming his back. He continued working 2 weeks, went on vacation for 1 week and began missing work on 7-8-2013. Company conducted investigation to confirm validity and found it valid on 7-18-2013

Employee was loading 12ft timbers in scoop bucket when he turned and his right foot got caught between 2 timbers. Employee felt pop in right knee and almost fell straining lower back.

Employee was loading 60lb rail off of a forklift onto the rail carrier. Forklift would not raise high enough to clear the standards on rail carrier due to height. Employee was lifting one end of rail at a time when rail fell smashing his left ring finger. Employee was treated at Raleigh General and was diagnosed with a slight fracture and received six (6) sutures.

Employee has been diagnosed with Occupational Hearing Loss. The Safety Department was notified of his diagnosis on March 31, 2014.

Employee was trying to pull protective rubber cover off of sharp edge of the cutting blade. Employee cut his right hand which required eleven (11) sutures in his right index finger. He was treated at Appalachian Regional Hospital.

Unplanned roof fall has occurred at Short stub mains #4 entry spad #3029. Roof fall was approximately 20'x 20'x 6'. No injuries were reported and no equipment was involved.

Employee attempted to hook chain on spreader bar. He grabbed the slack end of chain trying to pull chain to get fully hooked. He pulled chain and the chain slipped out of spreader and caught his left index finger in link of chain. Employee was treated at CAMC General Division where he underwent surgery to repair tip of finger. Employee was diagnosed with a crushing amputation.

An unplanned roof fall has occurred at Short Sub Mains #5 entry spad 3067. The roof fall was approximately 30' x 40' x 4'. No injuries or equipment involved.

Employee was pulling cable out of gob. As he bent over to pull cable again he felt his back pop and fell to the ground. He was transported to Raleigh General Hospital where he was diagnosed with muscle spasm in his back.

An unplanned roof fall occurred at survey spad 3106 #3 entry inby dumping point. The roof fall was approx. 40'x18'x7'. The initial 103(j) Order was modified to a 103(k) Order. Both Federal and State agencies approved a clean up plan to rebolt and clean roof fall. On June 17 the 103(k) Order was lifted to resume normal production. No injuries was reported or no equipment involved.

Employee was building a stopping in #6R across from feeder. A piece of rock broke loose from the rib striking employee on the right shoulder and right hand. Employee was transported to Raleigh General Hospital. Xrays were taken and was told to follow up with a specialist for further evaluation.

As employee was moving the rub rail on a 14CM15 Continuous Miner he felt a pulling sensation in the lower left abdominal area. Employee was diagnosed at Med Express with a hernia on 5-17-11. Employee returned to work on restricted duty from 5-18 to 5-24. The hernia was repaired on 7-27-11 which the Safety Department was not notified of his surgery until 8-3-11.

Employee and helper was removing a rock off of the roof bolter. As Employee pulled the rock it fell striking him on the right foot. Employee was transported to Raleigh General Hospital by Whitesville Ambulance. Employee received xrays and CT scan. He was diagnosed with a contusion on the top right foot. Employee was given restricted duty until the swelling went down in his foot.

Member walked up and asked the miner man how much more he lacked so he could come across and clean that side of the section and the cut where the miner was fell in on the miner and a piece of rock slid down and hit member on the ankle. He was taken by ambulance to Raleigh General where he met Analabs for a drug screen and was scheduled for surgery on 1/29/2011.

Member was putting a pump on the 488 scoop. The relief valve was tied back and it came loose and smashed his finger between the pump and relief valve. Member immediately ceased work to go to Whitesville Clinic and had 3 stitches put in his left hand, ring finger.

An EE was running the scoop and scooped up the pressure pump in order to move it. The door on the scoop deck was not shut and injured EE went to shut the door. Other EE thought that IE had completely moved out of the way of the scoop, but he had not. Other EE began backing up the scoop bucket and hit IE and pinned him against the right rib. He sustained injuries to his knee.

Member was walking toward the belt line and tripped over a water line buried in mud and hit his tooth on a wing nut on top of structure and broke one of his front teeth.

Roof fall at Spad # 522. No Injuries or equipment involved. Fall was 30' X18' X6'. Roof fall was 6000' from an active section.

Roof fall 40'x18'x5.5'. Between #5 and #6 entries Spad #644. No injuries or equipment involved.

Member was cutting a band on a pallet of block and slipped and his utility knife cut his left thumb requiring 8 stiches.

While moving roof bolts off of bolter to supply his tray for the next cut. When he grabbed the roof bolt a sliver of metal when into his finger.

EE was working on #3 belt at #4 head, belt started up & EE was dragged under crossover by the belt near #4 head. #3 belt was not turned off at tailpiece or locked out. #4 head was locked out.

EE was performing belt scraper maintenance on #4 belt head while standing on #3 belt, #4 belt was locked and tagged out, #3 belt was not locked and tagged out. #3 belt started up.

EE was taking outer jacket off right roof bolter to make a splice. Utility knfie slipped off jacket, cutting his thigh. Required 5 stitches to close the wound.

Employee was attempting to install a U joint in a shuttle car drive shaft. While using a large screw driver to pry it into the yolk he slipped causing his right hand to hit the frame resulting in a fracture to the tip of his right ring finger.

Roof fall in #3 entry, one crosscut inby spad #995 on old 1st right panel. Fall was 2900' from the active section. No injuries or airways blocked. Fall was 80'x 18'x 4'.

A roof fall occurred on 1st right panel #2 entry 900' outby the working section, spad #990 fall measured 100' long, 18' wide and 5' thick. No injuries or passage of men affected.

When attempting to raise the top cover of the belt tail piece, it slipped out of employee's hands causing it to fall on his left hand that resulted in an injury that required amputation of the distal phalange of his left ring finger.

9 different falls occured between spad #919 to spad 990. Spads 829, 934, 958, 965, 983, 998, 936, 993 and spad 960. No injuried and exact time of falls unknown.

Member was operating roof bolter and a piece of draw rock fell out of the top and hit him in the head, causing him to chip two of his teeth which will require dental work to fix.

A fall of the mine roof occurred in #5 entry @ spad #433 this entry was the primary escapeway, the escapeway will be rerouted around the fall. The fall measured approx. 30ftL x 18ftW x 54"T. There were no injuries.

A fall of the mine roof was discovered on 6/3/08 at approx 11:45 am. The size of the fall is 80 ft long x 19 ft wide x 10-15 ft high. There were no injuries and no airways blocked.

Member was picking up trash after building stoppings. He put some of the plastic bands that hold cinder blocks together in a plaster bucket. One of the bands popped out of the bucket and went behind his safety glasses and scratched his eye.

Miner was diagnosed with pneumoconiosis and exercised his right to be moved to an atmosphere with not more than 1.0 concentration of dust.

EE was taking roof jacks to the section when one of them fell off the forks of the fork lift. When he bent over to pick it up he felt a blunt pain in his back. He continued working until 2/8/08.

Fall in Room off of #1 Entry at spad #388. 20'W x 20'L x 15'H. No injuries, no airways blocked. Area was not on the active section area. Had been cribbed off prior to fall.
