Powellton #1 Mine Coal

EE stated was working to remove a spray block on the head of a C/M machine. While prying the block back and forth the EE stated that a finger got caught between the spray block and frame of the miner pinching the left middle finger.

Employee was walking along overland beltline spotting rollers and accidentally tripped over a piece of rock on the grating and started to fall. EE tried to catch self and got their hand caught between the belt and the roller causing injury to hand.

A non-injury roof fall occurred on 09/27/2022 and was discovered approximately at 12:50pm. The fall is located in the No.9 return at crosscut No.143 on the Toney Fork Mains Panel. The fall is approximately 25'Long X 18'Wide X 4'Thick. This fall will not be cleaned up. The fall has been dangered off.

EE noticed EE's cable wasn't taking up on EE's way back to the feeder. EE got out of 73 S/C and was going to pull on EE's cable while the S/C was still running. Before EE could start pulling on the cable, the cable started taking up and it pulled the EE's feet out from under EE. When EE landed EE felt a pop in EE's right foot.

EE was pulling S/C cable with other employees at the new section set-up at brk 133. As EE was pulling, EE got tripped up on the rail which resulted in a fracture to the ankle.

EE was loading belt structure when EE's foot slid under the bucket of a scoop, the bucket was being lowered at the time of the incident.

An unplanned roof fall occurred in the No. 2 bleeder entry of the Coal Branch No. 5 panel just in-by survey station No. 17961 near brake No. 36. The fall measured approximately 40' Long X 5' Thick.

EE stated was driving inserts into the head of a miner with a sledge hammer. EE stated EE felt a pull in the right bicep. EE continued to work through April 17, 2021 and started missing work on April 18, 2021.

EE was moving crib blocks in place to build a crib under the feeder. EE caught left pinky between a crib block and the feeder. EE received a fracture to the tip part of the left pinky. EE also received sutures to close the wound. EE was placed on restricted duty.

EE was operating a personnel man bus when the man bus wrecked. EE was attempting to put the wrecked man bus back on the rail. The EE fell striking a leg to the ground in the process. The EE is currently working with restrictions.

The number 1 section move crew was attempting to clean up a rib roll. During this process an additional piece of coal/rib fell out striking the EE on the left leg.

EE was walking when EE fell falling on EE's left hand. The EE landed on left hand causing a boxer fracture.

A roof fall measuring 40'L X 18'W X 7'H was found in the No.3 left X-cut over to the No.2 intersection In-By survey station No. 17530. All approaches to the roof fall were immediately dangered off.

A roof fall measuring 40'L X 18'W X 4'H was found in the No.8 entry of the Coal Branch No.4 Panel Bleeder Return Panel in-by survey station No.17290. Approaches were dangered off.

The Section Pressure Pump Breaker dropped out. EE attempted to put the power back in on the pressure pump and received an electrical shock in right hand. EE is currently working on restricted duty.

Two related roof falls were found on 7/1/2020 in the bleeder entries of the CB#4 Panel. One roof fall occurred in the #6 entry inby spad 17394 and measured 20'Lx18'Wx4.5'H. The other roof fall occurred thru the crosscut directly inby between #6 entry & #7 entry (spad 17435) and measured 30'Lx18'Wx4.5'H.

EE was setting a timber in the No.5 belt entry. EE was in the process of tightening the timber with the use of a cap wedge. During this process the EE took a step back twisting the left knee.

A fall in the return air course on Toney Fork #2 mains at X-Cut #148 was found on 3-30-2020. The fall was between the #4 entry and #2 entry measuring 5'Hx19'Wx80'L. The fall is located out-by survey station #2189 and in-by survey station #2188.

EE was loading highline in sled. EE was flipping the cable over in the sled and twisted back. EE was put on restricted duty.

EE and co-worker were working to free up miner from being stuck and bumped into each other, resulting in EE being knocked into miner striking their left arm against boom.

EE was prying on planetary with a slate bar when the planetary came out. The weight from planetary shifted to the slate bar smashing EE's hand to the ground causing a laceration to the left pinky finger. EE received four sutures to close the laceration.

EE was dropping strands of trailing cable for slack. EE tried to hand the cable back and felt sharp pain in lower back.

EE was in the process of installing roof support "Jacks" EE was moving some rock out of the way when EE smashed the right middle finger. This is reportable due to 4 sutures.

A roof fall was found measuring 4 1/2'high X 14'wide x 30'long. The fall has been dangered off at all approaches. The fall does not impede travel or ventilation.

EE was bolting in No.5 entry when a piece of draw rock fell between the bolt and rib. The rock bounced off the canopy striking the EE in the lower back and lower left leg resulting in a hair line fracture in the lower left leg. This employee is currently working on restricted duty.

A roof fall was found in the No. 1 entry extending over to the No.2 Entry "Continuation of fall found on 06/10/2019" The fall measured 9'H x 19'W x 70'L. The fall is located 1bk outby survey station 16087 at 19brk on Laurel Branch Submains. The fall has been Dangered off.

A roof fall measuring approximately 95'long X 18'Wide X 8'Thick was found on 06/10/2019 in the Powellton No.1 Laurel Branch Submains. This fall is located between survey station No.16062 and survey station No.16028. This fall does not impede travel or ventilation. This fall was immediately cribbed, timbered, and dangered off.

EE was removing a scoop from the charger. EE was trying to flip the charger cable up to get it loose from the side of the batteries. When EE made motion to throw the charger cable EE felt a pain in the groin area. The incident became reportable when the EE went to a specialist on 04/12/2019 and was diagnosed with a hernia.

EE was attempting to push a brake off of the shaft of a discharge roller. A piece of 4X4 metal shot out from between a porta power and the brake and striking the EE in the face.

During removal of an old B bonded stopping EE was attempting to recover the mandoor, EE's hand was overtop the sharp edge of the mandoor. An overhanging block free fell hitting EE's hand, forcing right wrist to come into contact with the sharp edge of the mandoor resulting in an avulsion type wound. Sutures were required to close the wound.

Individual cutting strap on a hammer pouch and cut palm of left hand requiring 8 stitches. No LTA necessary.

A roof fall measuring approximately 60'long X 20'wide X 4' thick was found in the Powellton No.1 Mains Intake, entry #2 @ 101 cross cut. This fall is located 1 cross cut outby survey station #970. This fall does not impede travel or affect the ventilation. The fall has been cribbed and dangered off.

Occupational Illness

EE was loading R/B machine with supplies. Another employee threw a bundle of R/B plates. The banding material broke allowing one of the plates to strike the EE in the upper lip resulting in a laceration requiring six sutures.

EE was walking around 4B Belt Head and slipped in wet rock dust. EE tried to catch self and reached for the tailpiece resulting in a dislocated right shoulder.

EE was installing the last roof bolt in the No.5 entry. EE claims to have felt a pop in the right shoulder.

Notified of Occupational Illness on 08/15/2018. Miner is currently on leave and unavailable to discuss transfer to low-dust area.

EE was walking around back of roof bolting machine when EE tripped over EE's own feet and fell to the ground landing on EE's right shoulder resulting in a dislocated shoulder. No visible tripping hazards were found. Returned to work on EE's next scheduled shift.

EE was backing left side continuous miner out of #8 Entry, moving it to #9 Entry. EE had hooked up third rope and was turning around and twisted EE's left knee when EE's foot became caught between miner cables and water line. This incident became reportable when EE sought medical attention on 8/6/18 and was taken off work.

Occupational Illness

EE was bolting area for boom hole. While bolting a piece of rock fell striking EE on the top of the foot causing a laceration that required 3 stitches.

Occupational Illness

Injured backed the scoop up to the charger hole to put scoop on charge. The jumper cables that was hung from the roof fell and struck the injured on the middle finger on the left hand cutting the finger

EE was racking highline in old works onto highline sled. EE stated EE felt right shoulder pop. EE continued to work. This incident was not reportable until EE began to miss work on June 7, 2018 due to having surgery on shoulder.

EE was walking through an intersection. EE walked under a damaged roof support pan striking the back of EE's neck resulting in a laceration that required 6 stitches. EE was released to full duty for the next oncoming shift.

A non injury roof fall measuring approximately 7'Hx18'Wx120'L was found near survey station 129 in the right return in an area that had been roomed off. This fall does not obstruct travel or ventilation. All entries to this fall has been dangered off.

EE was shoveling 1B Head when ee felt a pop in lower back. EE has returned to work on modified duty.

EE was loading belt on a belt sled in the mine yard and the nylon rope being used broke striking the EE in the face.

EE knocked a stopping. EE was cleaning up the debris out of the roadway. A block fell from the top and struck EE on the left lower portion of the back.

EE bent over to pick up miner cable and waterline to hang it. EE states EE felt pop in lower back. EE continued to work until January 8, 2018. EE was diagnosed with disc desiccation.

EE was walking to the air lock doors at the bottom of the slope. EE stepped in a whole twisting EE's right ankle. This is being reported due to the EE having corrective surgery on February 21, 2018. The EE worked up until EE's surgery and now is being placed on restricted duty.

EE bent over to move the continuous miner cable, during this process a section of the rib rolled out striking EE on the inside of left knee area.

EE was tramming a scoop thru a set of double doors @ 81 Brk Intake toward the belt line to rock dust. After pulling thru 1st set of doors, EE arrived at the 2nd set of doors and started to open them when scoop started rolling back toward 1st set of doors which the scoop hit causing door to go across EE's body in the deck of the scoop scraping back and catching arm against scoop.

Tug Valley Surveying - Contractor, Land Surveying Services employee suffered a heart attack.

Employee was in the process of finishing a stopping and was chipping the edge off of a block with a hammer; striking their right hand against a metal band, resulting in a laceration requiring 5 stitches.

EE was putting up Fly-Board. EE was trying to adjust roof bolt plate with the Fly-Board. EE caught a finger between the board and plate resulting in a fracture to the left middle finger.

EE was operating Roof Bolting Machine backing out of the #4 entry. EE had hand out of the operator's compartment when hand was caught between the rib and the roof bolting machine.

EE was on EE's knees cutting conveyor belt. The knife slipped causing a laceration to the upper part of EE's right leg. The laceration required 3 stitches.

EE was rolling up curtain when a piece of draw rock fell striking EE on the Neck and shoulder area.

At approximately 13:00hrs a non-injury roof fall occurred on the #2 section - Castle Branch No.1 Panel 007 active MMU.

A fall has taken place above anchorage in the #2 entry at survey station #12442 of the Castle Branch #1 Panel. The fall measures approximately 10' x 14' x 8' high.

Pulling track; stepped over track to get dinner bucket; tripped on rail causing fall which resulted in injured right shoulder.

EE was bolting top when the drill steel bent and broke hitting EE in the right arm.

EE was installing a curtain board to be in compliance with ventilation plan, during this process a piece of draw rock fell from between bolts striking the EE on EE's left foot/ankle.

EE was taking Structure apart. When EE picked up bottom roller, the rail fell out and pinched EE's finger causing a laceration that required five stitches.

EE was bolting in return - took slate bar to pull loose rock. The rock fell on canopy & as EE was trying to get rock off of canopy another piece of rock fell out between bolts striking EE on the back.

EE was chain ratcheting scrubber duct work into place. The chain ratchet came loose striking the EE in the chin causing a laceration that required stitches.

On 12-14-16 the operator was notified that a salaried foreman had slipped & struck their head against a scoop. During the operator's investigation, it was found that the employee was caught between the scoop & mine surroundings. The operator's investigation is on-going.

EE was running a continuous miner on a Pillar section. EE saw the top dripping over the miner and was starting to reposition self and was struck by a piece of draw rock.

Employee was diagnosed with Congenital Spinal Canal Narrowing, Degenerative disc disease including disc osteophyte complex and uncovertebral/facet hypertrophy. The operator received this diagnosis 11/28/2016.

Employee was helping another employee to try to break the bell plug loose on a set of batteries in order to change it. The leads inside the bell plug arced as they were trying to get it to break free and the employee received a flash burn.

Employee was adjusting the forks on the Caterpillar 930H Loader. One of the forks fell off the forklift portion of the loader and landed on employee's right foot. The fork fell approximately 1 1/2 feet. The employee had on metatarsal boots at the time of the accident.

Employee was struck by a piece of slate measuring approximately 7"W x 10"L that fell from the mine roof. The mining height in this area is approximately 62".

Employee was walking and stepped in the mud and this caused EE to twist EE's right ankle. ***Employee continued to work until 9/23/2016 when EE was removed from work by EE's medical provider.

Employee experienced pain in lower back when ee was bending to remove a piece a metal from the mine floor. The operator received the results of an MRI on 8/10/16 which noted (1) Degenerative changed noted above at L3-L4 and L4-L5. (2) Degenerative facet disease.

Employee was treated for flash burn to eyes.

Employee was pulling cable when EE slipped and fell and strained neck.

Employee was diagnosed with tri compartmental osteoarthritis.

Employee was helping to change out batteries on scoop when lid became stuck against the fire suppression system and EE smashed hand while trying to catch self from falling.

Employee was moving cable when hand was pinched which resulted in a fracture.

Employee was operating a continuous mining machine when a rib rolled and struck employee in the back and on the foot.

Employee was diagnosed with occupational pneumoconiosis. The employer received notification on April 6, 2016.

Employee was diagnosed with anterolateral spondylosis. Documentation is attached.

Employee states EE was unhooking chain from scoop when some draw rock fell and hit finger which resulted in a fracture to the finger.

Employee states that EE was building cribs when EE slipped on some rock, twisted knee and felt pain. EE's first day away from work was 01/25/16.

Employee was clearing rock off of miner and lost grip of the slate bar and as the slate bar fell, it hit his left thumb which resulted in a fracture.

A non-injury accident occurred at this operation involving an unplanned roof fall. This fall was discovered on December 01. 2015 at approximately 06:00 p.m. The fall occurred in the area of spad # 10085.

Employee was loading a roof bolter drill pot onto a flat car when he lost his grip. As it fell to the ground he received a cut to his finger that required stitches.

A non-injury accident occurred at this operation involving an unplanned roof fall. This fall was discovered on November 19, 2015 at approximately at 6:35am. The fall occurred in the area of spad #9701.

Employee was attempting to make a belt splice and while pulling out a top piece of belt structure he felt a pain in his shoulder. Employee's first day away from work was November 30, 2015.

A non-injury accident occurred at this operation involving an unplanned roof fall. The fall was discovered approximately 17:35 in the No. 2 entry of the Buffalo Creek No. 1 Panel 120 feet inby survey spad #10098. A 103(k) order was issued. Please see citation # 9064714.

Employee underwent a comprehensive audiological evaluation after an abnormal hearing test. Operator was notified on November 3, 2015 that the employees work history and test results are consistent with noise-induced hearing loss with the workplace being a significant contributing factor

Employee was riding down roadway in rubber tire ride and there was an incline and a bump in the roadway which caused a 10 ton jack to dislodge and fall on his hand (right) smashing middle and ring fingers.

Employee placed his hand on side of scoop bucket to remove the excess chain slack when the scoop operator pulled the ram lever, therefore, pinching his finger which resulted in a cut that required stitches.

An unplanned roof fall occurred at this operation on the 8th SE Panel near spad #9994 at break #38 in the #1 entry on 10/12/15.

Employee was on knees working when a piece of coal fell and hit employee. Employee never sought treatment until his 10/14/15 shift where he was placed on restricted work duty.

Employee was operating a continuous mining machine when a piece of rock fell and hit him on his forearm which resulted in the employee receiving stitches.

A non-injury (unplanned) roof fall occurred at this mine. The roof fall occurred on the 2D Belt, at crosscut 17 in the #6 entry through crosscut #21 at approximately 8:40 p.m. A 103(k) order was issued. Please see citation # 7278196.

A non-injury (unplanned) roof fall occurred at this mine. The roof fall occurred on the Main Line #4 Belt at crosscut #189 near spad #3121 at approximately 6:30 p.m. on 8/15/2015. A 103(k) order was issued. Please see citation # 7278195.

Employee was standing on operator's side of roof bolter when he grabbed a bundle of glue to toss to the other side when he suddenly felt pain in his right side. NOTE: Operator was notified on 8/27/2015 that the injured worker had a fractured rib.

Employee was separating drill steel when a piece came loose suddenly and hit his glasses and they broke which caused a cut and he received stitches.

The fork of the end loader that the employee was operating fell into the dumpster as he was unloading some trash into the dumpster. Employee climbed into the dumpster to loosen the fork and while doing so he jarred his neck and back. NOTE: Employee continued working his normal work duties until 8/24/15 when he began working restricted work duty.

Miner was diagnosed with bilateral spondylolysis on 6/26/2015 and continued working until 08/17/2015.

Employee was getting ready to change out scoop batteries when he slipped and fell to the ground.

Employee was hooking a rope hanger to a rock near the feeder so it could be moved when it shifted and landed on his left foot.

Employee was crossing underneath belt at cross guard when he lost his footing and fell in a hole.

Employee was putting derailed ride back on track when ride shifted causing jackbar to hit employee resulting in a laceration.

Employee was operating a continuous miner when a piece of rock fell and struck employee.

Employee was loading airlock doors when he began to experience pain in his back.

Employee was cutting leaky feeder cable with a knife when it slipped and cut his hand that resulted in stitches.

Employee was installing roof bolts when he began to experience pain in his back.

Employee was installing fly boards when a piece of rock fell and hit employee's foot which resulted in a fracture.

A non-injury roof fall accident occurred at this mine at approximately 09:30 a.m. The roof fall occurred on the active 007 & 008 MMU's in the number 9 entry across from survey spad #8862 two breaks outby the section dumping point and a second fall occurred in the bleeder inby blocking both bleeder entries. A 103 (k) order was issued #8295131.

Employee fell while getting out of ride (Mac 8) and landed on the mine floor. * Employee never missed work until his 03/23/15 shift.

Employee was cutting cable off cathead when he cut his knuckle with a knife which resulted in stitches.

Employee went to his tool box to get chain ratchet out of the box when the lid fell down on his hand.

Employee stepped off motor into some mud when he slipped and fell and hit his arm. NOTE: Employee never missed work until his May 13, 2015 scheduled work shift.

Employee was performing work on roof bolter when a piece of rock fell and hit the employee.

Employee was taking bottom roller out of belt when he began to experience pain in his shoulder. Note: Employee never missed work until his June 05, 2015 shift.

Employee was pulling line curtain down in 5 entry when he began to experience pain in his lower back.

Employee was walking inby from the powerbox when he slipped and fell. While raising himself up, he bumped into a flyboard

Employee was moving rock off of miner when his hand was caught and resulted in the employee receiving stitches.

Employee was cutting a piece of belt with a utility knife when it slipped and cut his hand requiring stitches.

Employee was fire-bossing track and power centers. As he was walking to box, he stepped on a small unseen ledge and slipped when he fell to the ground and began to experience pain in his knee.

Crew was spot bolting 5R on #2 section. Both operators had finished putting up bolts, and operator went to inch tram controls. Offside bolt man walked back to get bolts for his side. The operator started the bolter and began tramming forward, that is when the bolter slid off a small ledge, on top of employee's foot.

Employee was cleaning area and tried to catch a tire that was moving. While trying to catch the moving tire, the tire fell over and caught his wrist between two tires. Employee was put off work on 10/01/2014.

Employee was moving waterline and miner cable out of roadway against rib when he began to experience pain in his back.

Miner was diagnosed with evidence of pneumoconiosis and was placed in a Part 90 Miner status in accordance with 30 CFR 50.20-6(b)(7).

Employee was preparing to bolt top when a piece of rock fell from the top and struck him in the upper right leg area.

An unplanned roof fall occurred at this operation on 5/31/2014 pm the #2 section at approximately 08:00 p.m. Please refer to citation #7278162. The fall occurred inby Spad #7709.

Miner was diagnosed with evidence of pneumoconiosis and was placed in a Part 90 Miner status in accordance with 30 CFR 50.20-6 (b)(7).

A roof fall occurred at this mine at approximately 8:45 pm at crosscut 146 on the #4 belt. Please refer to citation #9011798.

Employee was attempting to make ends on a belt splice. Before he could begin, he noticed that one end was trapped beneath the rest of the belt. The employee tried to free the end by pulling with channel locks when he began to experience pain in his back.

Employee was making his normal fireboss run on a ride and was coming through the outby airlock doors. He got off the rail ride to close the doors. As he went to close the doors, he noticed the ride to start moving and he ran and jumped onto the ride when his foot was caught underneath the ride which resulted in a fracture.

Employee was attempting to change skirt rubber on tailpiece when the chain ratchet slipped causing the apron to fall on employee's hand fracturing two of his fingers.

Employee was performing maintenance on a roof bolter. He removed the bottom pin from the ATRS cylinder causing the bottom framing to fall on his left foot.

Employee was pulling on a cat head and cable when he suddenly started complaining of back pain.

Employee was operating a rubber tired ride when the front tire hit the rib causing the wheel to spin around and hit his hand resulting in a fracture.

Employee was loading roof bolt plates from back of bolter onto tray when the roof bolt plates fell off the bolter and hit his left knee cap. The employee continued to work until 7/24/13.

EE was moving two rolls of curtain when he felt pain in his back. Employee never missed work until his 07/15/13 shift.

Employee was moving forks on forklift when he smashed his thumb between fork and frame.

Employee was taking preventative measures by inspecting roof bolter canopy for broken center pins. While inspecting they had trouble getting the canopy to free and the canopy came loose and swung on the chain ratchet it was secured by and hit him on the face.

Employee was preparing to bolt top when a piece of rock fell and struck employee in the shoulder area.

Employee was operating a shuttle car going around a corner when the canopy of the Shuttle Car hit the corner and knocked down the brow which hit the employee's left thumb resulting in a fracture.

Employee was operating a rubber tired ride when the steering wheel whipped around and caught his hand causing a fracture.

Employee was fire bossing when he bent over to pick up supply line and felt pain in his back.

EE was setting timbers when a rib rolled into a timber. The timber then hit EE's hard hat which caused a laceration to his face. The EE received stitches.

Employee was preparing to bolt top when his hand got caught between pusher steel and wrench causing a laceration. The employee received 8 stitches.

Employee was working on shuttle car and walked around machinery and felt a sharp pain in his leg. The employee never missed work until his 01/21/13 shift.

Employee was pulling slack up in shuttle car cable when he felt a pain in his shoulder.

C/M operator was loading a shuttle car when a rock came over the side of the boom striking another rock that was sitting on the machine sending it airborne and struck him on the hip.

EE was trying to open tailpiece doors when the door's "batwings" slipped and closed on his finger.

EE was bolting top when a piece of rock fell from the top and struck one of his fingers. The EE received three stitches to his left index finger.

EE was trying to lift a timber from gob the scoop had just cleaned up at the edge of the scoop bucket when he felt a sharp pain in his back and down his leg.

Employee was cleaning a piece of metal with a pick hammer when it struck and hit his hand.

Employee moved a block while building a stopping when he twisted his knee and felt a pain in his back.

EE was tightening up motor mount on belt head when ratchet slipped and he hit his elbow on the corner of a I-Beam. EE completed the rest of shift. He never missed work until 6/19/12.

Employee was assisting in hanging MRS cable when a rib rolled onto his right leg during retreat mining.

EE was shoveling out from 1A take up. He put contents into wheelbarrow and while trying to lift wheelbarrow up, he felt a pain in his back.

EE was cutting rope hanger off MRS cable when he cut his wrist during retreat mining.

Employee was trying to open outby set of doors when he felt pain in his left leg.

Employee was cleaning 3C Belt Head with fire hose when nozzle blew off and hit employee in mouth knocking two teeth out. Employee twisted his knee.

Employee was lifting structure when he felt a pain in his shoulder. Employee worked until 6/26/12. Employee is scheduled for surgery on 7/20/12.

Employee was shoveling belt. Employee picked up a rock to throw on belt when he felt a burning sensation in his back.

Employee was in #7 entry going to get on Shuttle Car when a rib rolled and struck employee.

Employee was bending bolts when he felt something pop in his shoulder.

EE was traveling on #1 Belt Entry and was turning to go into track entry when a piece of rock fell from the corner and struck him in the leg.

EE was bolting on operator side when finisher became lodged. EE took pusher by hand trying to dislodge the finisher from the top & was unsuccessful. He then held the pusher with his left hand and operated the drill pot with his right hand running the controls. When the drill pot was raised to the top, he tried to hold it over with his left hand & mashed his finger receiving a cut.

Employee was working on water spray when he cut his arm with a knife.

EE stated he was driving outby when he was exposed to smoke from friction on a nearby belt. This 7000-1 form is being submitted in reference to citation No. 8142583.

EE was traveling inby 5 Belt Head when he was exposed to smoke due to belt friction. This 7000-1 form is being submitted in reference to citation No. 8142583.

Employee stated he was shoveling belt. When he went to turn, he twisted his knee and felt a sharp pain in his knee.

Employee was making a splice on feeder when hand slipped off knife and blade cut his hand requiring stitches.
