LONGVIEW MINE Coal

Employee set up a 5' step ladder to assist with hanging a bag. When they stepped up onto the first rung the left leg shifted causing them to lose balance and fall. They used their left arm to brace for the fall. Their hand struck the ground spraining their thumb. Injury was not the result of a training or compliance issue.

The emergency hoist was damaged during routine testing resulting in the hoist being removed from service for a period of greater than 30 minutes.

Injured employee was handling block while building a stopping when EE felt pain in EE's left knee. Employee started physical therapy on 11/24/2025. No scheduled shifts were missed as a result of this incident.

Employee was dragging mine spray with a crew when another employee unexpectedly dropped the end causing a right shoulder strain. This incident did not involve any aspect of compliance with rules and regulations; mine equipment or mining system; job skills and miner proficiency, training and attitude; or protective items relating to clothing, or protective devices on equipment.

Injured employee was attempting to separate drill steels when the steels came apart and struck their upper lip, chipping their front tooth requiring dental repair. No time was missed as a result of this incident.

Employee was a passenger in a rail mounted mantrip that ran into another mantrip. Employee came out and sought medical attention. Injury was not the result of a training or compliance issue.

Injured employee felt pain in lower abdomen during shift. A hernia related to a weakened abdominal wall from birth was later diagnosed by a physician. Employee was taken off work on 5/29/2025.

Injured employee was a passenger in a diesel powered mantrip being operated on the mine bottom when the mantrip struck a supply car resulting in a simple fracture to a transverse process bone in their lower back area.

The personnel hoist was down for a period of time greater than 30 minutes due to a door switch/wiring issue at the shaft bottom.

Injured employee was carrying blocks up a ladder at the return regulator on the Northeast Mains. While descending back down the ladder employee felt pain in right hip and leg. Employee has not missed work but started physical therapy on 2/3/25.

An ignition occurred on the Headgate #3 working section. The ignition occurred in the #2 entry, 40-41 block, when the #1331 roof bolter was installing a roof bolt in the mine roof.

Injured employee was assisting in roof bolting on a roof fall clean-up project when a piece of rock fell from the mine roof, struck the roof bolting machine, then the top of EE's right foot resulting in a laceration requiring three sutures to close.

Injured employee was helping pull continuous miner cable when the cable tightened and took EE's feet out from under EE, causing EE to fall and strain EE's right shoulder. Employee continued to work until 2/14/2025.

An unplanned fall of the mine roof above the anchorage zone occurred at 4 block on the mine bottom, 3-4 crosscut measuring approximately 10' high x 30' long x 17' wide.

Injured employee was removing built-up material from the rollers along the slope belt when the employee sustained a fracture to right forearm.

Faults involving the limit switches on the mine personnel hoist resulted in the hoist being down for a period of time greater than 30 minutes. No miners were entrapped and no injuries resulted.

Injured employee was assisting in moving an auxiliary fan utilizing a scoop. After loading the fan and hooking the scoop's winch rope to the fan employee stepped down from the scoop bucket a distance of approximately 1' and stepped on a rock, twisting left ankle.

An unplanned fall of the mine roof above the anchorage zone occurred outby on the First Northeast Mains between 30 and 31 block in the #5 entry.

During the daily inspection of the vertical personnel hoist, broken strands were discovered on the wire rope. The hoist was removed from service by the operator.

An unplanned fall of the mine roof above the anchorage horizon occurred outby on the #2 Headgate section at 5 block in the #4 entry. The fall was approximately 14' high x 18' wide x 20' long.

A heating event occurred involving the Strata foam that was pumped above a set of steel set arches to fill a roof fall void from 47 to 49 block in the #1 entry of the #1 HG Longwall section.

An inundation of carbon monoxide occurred on 6/28/24 at the void fill area from 47 to 49 crosscut on the HG #1 Longwall section.

Injured employee was loading trash onto a mine supply car when they slipped and struck their right elbow.

The vertical hoist servicing the underground mine was down for more than 30 minutes due to a faulty solenoid.

An unplanned fall of the mine roof occurred at 48 block on the #1 HG Longwall section in the #1 entry.

Employee stated that after picking up a battery backup and a fixed mesh node for the wireless tracking/communication system employee felt a pain in left shoulder. Employee continued to work until 8/7/24 when employee was taken off work for surgery.

Injured employee stated that while walking on the HG #2 Section in the #2 entry between 55 and 56 block EE twisted EE's right knee. Employee continued to work until 5/24/2024 when EE was taken off work by a physician.

An ignition occurred on the #1 HG longwall face at 88 shield.

An unplanned fall of the mine roof occurred at 68 block, #1 entry of the #1 Tailgate section.

An unintentional roof fall occurred on the #1 HG Longwall section between 55 and 56 block in the #1 entry.

Injured employee was performing maintenance on the longwall shearer when a piece of rock fell from the mine roof and struck EE's left leg resulting in a fracture.

A face ignition occurred on the #1 HG Longwall face at #163 shield.

Employee was checking the sanding devices on a locomotive on the mine bottom. As they walked around the locomotive and stepped over a track rail, they twisted their right knee.

An unplanned fall of the mine roof above the anchorage zone occurred in the #3 entry intake air course at 48 block on the #1 HG Longwall section.

Injured employee tripped and fell while examining the slope belt resulting in a laceration to their right thumb requiring four sutures to close.

An unplanned fall of the mine roof preventing passage occurred at 58 block of the belt entry of the HG #1 longwall.

An unplanned roof fall that impeded passage occurred at 48 block in the #1 entry of the HG #1 section.

Injured employee was operating the longwall shearer when the employee was struck on the right index finger by a piece of rock resulting in a laceration requiring four sutures to close.

Employee was cleaning up spillage on the longwall face and picked up a chunk of coal to throw back into the panline and got finger caught between the panline and chunk of coal causing laceration.

Injured employee tripped and fell while walking de-icing rollers along the Met #3 conveyor belt.

Injured employee was checking the suction on a roof bolting machine when a piece of rock fell from the roof/rib and struck left calf resulting in a laceration that required sutures to close.

Employee was putting down material to dry up a roadway. They reached down to pick up a pick hammer when a piece of rock fell out of the top striking them on the back of the head while they were bent over causing injury.

Injured employee was attempting to change bit on drill steel. While holding the steel with their left hand they lowered the mast on the roof bolter causing it to catch the steel and push it onto their left foot just above the metatarsal guard resulting in a fracture.

Injured employee was placing a roof bolt in the chuck of the roof bolter when they bumped the rotation lever and caught their left hand resulting in a sprain/strain type injury. Employee began missing work on 1/2/24.

Injured employee was attempting to roll a track rail into place when it struck EE's lower left leg resulting in a fracture.

The slope hoist car sustained damage while hoisting a continuous miner out of the mine.

Employee was doing belt work and EE was struck in the back of the neck by the clamp of a come-along when it accidentally broke loose causing injury. Employee did not miss work from the incident until December 11, 2023.

Employee was installing roof screen when a piece of rock slid off the screen and struck the back of their left leg causing their knee to strike the roof bolting machine resulting in a fractured patella.

An unplanned fall of the mine roof above the anchorage horizon occurred outby the Headgate Section, #36 block in the #2 entry.

Injured employee was walking down the #2 entry of the 1st Northeast Mains working section dropping bolter cable when they stepped on a rock and fell to the mine floor, fracturing their right wrist.

Injured employee bent down to move the continuous miner cable and felt pain in lower back. The injured employee continued to work until 10/16/23 when the employee was taken off work by a physician.

An unplanned fall of the mine roof above the anchorage horizon occurred at 13 block, track entry of the 1st Northeast Mains. The fall measured approximately 18' wide x 20' high.

Injured employee sustained a fracture to EE's lower leg when a piece of rock rolled out from low on the rib and struck EE as EE was positioning the continuous miner on the #1 Headgate section.

Injured employee was assisting a co-worker with positioning a steel plate on a supply car when the plate shifted and caught their right third finger resulting in a fracture and laceration. No time was missed as a result of this incident.

Injured employee was struck on the left hand by a piece of rock that fell from the mine roof resulting in a laceration requiring sutures to close. No time was missed as a result of this incident.

Injured employee was attempting to move a piece of roof screen when they stepped on a rock and felt pain in their right knee.

An unplanned roof fall above the anchorage horizon of the roof supports occurred at 36 crosscut, #3 entry of the #1 Headgate Section.

Injured employee slipped off the platform of a roof bolter and caught left hand between the ATRS jack and the frame of the ATRS resulting in a fracture to hand.

Injured employee stated that while lifting crib blocks they felt a pain in groin area. Employee was diagnosed with a hernia.

Injured employee sustained a laceration to forehead when the personnel carrier on which the employee was a passenger overturned. The injury required sutures to close.

Injured employee sustained bruises to chest/abdominal area when the personnel carrier on which the employee was a passenger overturned.

Injured employee was walking along the right side of a scoop on the #1 Headgate section in the #3 entry when a section of rib slid down and rolled onto the employee's right foot resulting in a fracture.

Injured employee was operating the company #1035 scoop on the #1 Headgate section removing trash from the section. While unloading the trash from the scoop employee felt pain in left knee and was unable to complete the shift.

Injured employee stated that while walking off the bottom area into the mine they slipped off a section of track rail and felt pain in their right calf.

An unintentional roof fall above the anchorage horizon occurred on the 1st Northeast Mains section in the #6 entry at 22 crosscut.

Injured employee was constructing the footing of a stopping at 19 block on the #1 Tailgate section when the employee was contacted by a shuttle car resulting in a bruise to hip. Employee started physical therapy on 1/30/2023. This incident is being reported as a no lost time, medical treatment injury.

Employee was cutting a piece of all thread rod with a chop saw, when a piece of debris hit employee's cheek and went under the safety glasses, into left eye.

An unintentional roof fall above the anchorage zone occurred in the #3 entry, 12 block of the Northeast Mains section.

Injured employee was walking just inby the section power center in #3 entry of 8 block when the employee stepped on a rock and twisted right ankle. No work was missed as a result of the incident, however, the employee received medical treatment.

Connecting steel brace to column at Head House #3. EE signaled crane operator to 'cable up', brace came up, slammed against the brace clip and pinched the palm-thumb of left hand. Lacerated the derma, requiring 6 stitches.

Ironworker connecting Bridge Steel on prep. plant thickener: Spudded a hole, while holding the spud wrench, bridge shifted and spud wrench came back and jammed a finger against the steel.

Setting steel Hood/Cover over the chute on the Head House Frame.

Injured employee was attempting to adjust roof mesh when IE mistakenly hit the drill rotation catching left hand on the drill steel and roof mesh.

Employee stated that while unloading a steel beam off a diesel forklift EE caught left ring finger between the beam and the forks resulting in a laceration requiring sutures to close. Protective gloves were being worn by the employee at the time of the incident.

Injured employee twisted left knee while attempting to hang a ventilation tube in the face of the #2 entry of #1 Mains. Employee continued to work until 9/20/22 when employee was taken off work by a physician.

Moving a full sheet of grating across the top of steel to set in place using a separate lanyard when grating fell into an opening. EE hooked the grating and wrapped the lanyard around EE's arm. It fell through an opening, pulling EE to the edge and arm through the floor opening. Upper Right Arm broken w/transverse fracture.

I.E. was driving a concrete mixer truck to deliver a load of batched concrete to support tube #2. As the driver was making a sharp right turn EE stated that EE felt the truck start tipping to the drivers side. It could not be corrected and the concrete mixer truck rolled over on coming to rest on the drivers side.

Crossing from ground to concrete pad, injured did not get their entire foot on the pad (only the front part of the foot) and fell off the pad into a small open ditch, breaking both bones in the lower right leg.

The headache ball on the crane caught on steel structure with tension. Upon the ball dislodging, an attached angle brace "bounced" and struck the ironworker in the mouth/chin area.

Employee was stung in throat/neck area by a bee. EE is allergic to bee stings and was taken to Broaddus ER by co-worker. NOTE: EMPLOYEE WAS RELEASED TO COME BACK TO WORK (FULL RELEASE) ON 09/15/2021 WHICH WAS EE'S NEXT SCHEDULED DAY DUE TO EE'S WORK SCHEDULE ROTATION. EE WAS QUARANTINED ON 09/15/2021 DUE TO COVID-19 PROTOCOLS.

Employee was stung by bee and they had reaction to sting.

While employee was pushing on concrete form to get in position, EE's hand slipped off and got caught between form and bolt smashing finger.

During concrete invert pour, the concrete car was being pulled to surface, the screed cable supposedly got on the concrete car, which pulled the screed up hill, causing screed to come into contact with employee

Employee was in the process of drilling hole for roof support. EE had laid EE's roof bolt against drill mast and realized the drill bit was stuck in hole, EE went to retrieve bill using drill steel and as EE was lowering the drill mast it came in contact with roof bolt causing it to strike employee in the mouth.

Employee just installed glue and roof bolt into hole and was helping operator put bolt into drill pot, the operator started to push bolt up into hole unaware that other employee had hand on drill pot, as EE raise boom, employee got pinched between mast and steel clamp

A roof fall occurred approximate 15' wide by 20' long and 5-6' in height, Just before raise drill pilot hole punched through, water migrated through rock strata and settle in swag of roof, and started draining before roof fell in. Just outby raise bore area.

Employee was in the process of changing steel for jack Leg Drill, when a piece of rock broke loose in front of support jacks and fell from top striking employee in left arm, causing laceration

Employees were removing damaged fan line. During the process of hooking up rigging, an employee hooked rigging on loose tail end of cable. Once the cable took weight of the rigging, it pulled the assumed connection point, allowing rigging to fall to work deck hitting employee.

Employee was in the process of bolting up wire mesh in the slope, when a piece of rock fell out and hit canopy causing rock to bounce off and hit bolter operator in back of head, causing laceration to rt. ear.

Employee reached up to spin 630 mucker to align to fit through work deck. They placed a hand on top of workdeck post, when the crosshead set down on the work deck smashed the finger.

Employee was reaching to unchain shaft jumbo drill steel, when EE unchained the steels, EE's foot slipped off edge of Jumbo and fell to the GRD and landed on EE's hands. A drill steel also fell and hit employee in back of head
