A FOREMAN FOR SELECT ERECTING WAS DISMANTLING A HOPPER. AFTER CUTTING A 42" X 120" SHEET OF 3?16" STEEL FROM THE TOP OF THE HOPPER, HE ATTACHED A SHACKLE AND STEEL CHOKER. HE WAS WORKING FROM AN AERIAL BOOM LIFT. HE THEN DISCONNECTED HIS LANYARD FROM HIS FULL BODY HARNESS AND LEFT THE LIFT BASKET. HE WALKED OUT ON AN I-BEAM AND TURNED TO SIGNAL THE CRANE OPERATOR TO PICK UP THE PIE

Miner was helping lower a chute on the secondary plant, EE and two other employees were using come-alongs to lower the chute into place. They let the chute drop halfway down, and it landed on the Miner's right hand, fracturing it.

We were not notified of the injury until 07.22.2024. Miner was lifting a hammer out of our VSI when miner's foot slipped, and miner then pushed up to keep the hammer from falling, causing a sprain/strain to miner's right shoulder rotator cuff capsule.

Employee was walking down a hill and turned abruptly to talk with someone behind them and states that the pivoting motion caused a "pop" in their left knee. Employee describes lost range of motion but no pain. Employee had the knee wrapped and went on light duty for the rest of the day, at which time they would go to a local clinic.

During the previous 3 days (June 16-18), EE had been working on a project pulling a lot of hose. He felt discomfort and pain from the pulling in his groin and behind the belt area. He though it might just have been indigestion, but the pain increased and was reported on 6/19/14.

Employee attempted to repair a piece of 3 1/2" drill steel (used as grizzly bars) that broke off the static screen. Employee failed to secure the broken drill steel after placing it in its receiving saddle. The drill steel fell from its cradle resulting in a fractured and lacerated left index finger.

At the chip stacker headpulley area, the employee was welding a wiper assembly in place. He was standing inside the transfer box. He had taken off his gloves and continued to work. As he tried to reposition himself, he grabbed both sides of the box and struck his left thumb against a burr on the top of the box. This caused him to cease work and contact his supervisor.
