Associate was using a handheld grinder and standing in a position where sparks were flying into EE's jeans. Jeans started on fire in the spot where the sparks were concentrated causing a burn to EE's leg.

Miner slammed the right index finger in a truck door resulting in a swollen finger. Miner sought medical treatment on 7/31/2018 for traumatic hematoma. Miner had a procedure done on 8/3/2018 for a cyst removal.

While descending the 972K loader, the miner missed the last step. EE's hand slipped off rail causing a fall on left shoulder. Refused to be taken to hospital and went home. EE said would see the doctor from home. On April 13, 2018 miner had rotator cuff surgery.

On 12/1/2017, miner told coach that EE went to the doctor for tennis elbow. Miner commented it was from splitting sand in the dry lab. Miner was given options to do therapy or go on Worker's Compensation and EE elected therapy and kept working. On 1/15/2018, miner was given a hand brace with restrictions.

Associate was sitting in a chair in the jet shack. When beginning to stand up, the associate indicated he was unable to stand up as his lower back was hurting. Associate did not hear or feel any "pop" in his back, but felt pain. Associate was sitting for a couple hours prior to standing up or getting out of the chair.

An associate was working at the Mobile Equipment Shop, changing out the waste oil tote (platic tote in a cage) when he cut his left hand resulting in medical attention (6 sutures). The associate used his left fist to pound an aluminum tag from the tote cage and was not wearing gloves.

EE was cleaning the north cooler with a ompressed air tool. EE inserted the tool into a cooler tube and when the tube reached the bottom of insertion, the EE's finger got pinched between a 90 degree union and the seal block. The EE was not wearing gloves at the time of the incident. The injury to his right hand pointer finger required four stitches.

The 345D excavator was low on hydraulic oil and needed to be topped off. While attempting to remove the Hyd. oil cap (which was on very tight) with a ratchet wrench the ratchet wrench malunctioned. The momentum of the slipped wrench caused the associate to fall backwards to the ground on a rocky surface. This resulted in the asociate breaking the bone in his left arm

HE OPENED TH EWATER VALUE TOAPPROX 80 PSI, HE STEPPED TOWARDS THE 8" PIPE, THE HOSE REJECTED BACKWARDS OUT OF THE PIPE HIHTTIN GEE IN THE MOUTH.
