Associate took off hard hat to fit in small area and when EE moved, EE cut EE's head on an edge of steel which required stitches.

Associate was using an electric rail car opener when the reaction arm slipped from the rail car body and struck the associate in the head. The associate was confused and had brief memory loss. Associate suffered a concussion.

We were removing pipe from the clients well. Crew was using a dong tong (large pipe wrench) utilizing a sling on a ram to help break the coupling loose. The sling slipped off the wrench causing the wrench to move back toward employee. Employee twisted EE's knee when this happened. Employee has been seen and is currently on light duty working in our shop.

Associate was changing a tooth on a bucket when they struck the tip of the tooth with a hammer causing a sliver of metal to break off. The sliver of metal was embedded in their cheek.

While cleaning a car at the rail clean-out, the associate used a bar instead of a car opener on a stubborn hard rail door. While doing this, the associate struck EE's right leg/shin with the bar and suffered a puncture wound to the right leg/shin area. Associate went to clinic and received 2 x-rays, (1) stitch and a tetanus shot.

Miner was cutting tape off a wire lead to change a motor. Miner cut a slit in the top of the tape/lead and the knife cut through quicker than usual and the knife cut miner's forearm.

Associate was working on a pump in the primary wash area. Coworker noticed the associate sweating a lot and not responding to questions that the coworker was asking the associate. Temperature was 55 degrees outside. Associate seemed confused and not acting right.

Technician was adjusting blocking, load shifted causing 4x4 board to come up and hit technicians mouth/lip.

Associate took a bar to close the bottom gate of a rail car and associate felt a tinge in their back. Stopped what they were doing and asked another associate to close the gate on the car.

Employee had a an audiogram that showed a Standard Threshold Shift. EE also had a second audiogram to confirm the findings of the first. The Associate Farms, Hunts and uses a Chainsaw in personal life without proper hearing protection, so this hearing loss is most likely due to high non-occupational noise exposure.

Changing out face shield from clear to tinted using a leatherman multitool knife to pry it off. The blade wasn't locked open so it flipped shut causing a laceration on little finger of left hand.

Associate was unloading pieces of scrap wire from work truck when a piece of wire flipped up and went under safety glasses and poked right eye.

Associate caught fingertip between drive V-belt and driver skave while removing impeller from pump. This caused fracture to the fingertip on left hand ring finger.

Miner was moving a rock near a culvert when the rock pinched EE's left hand, ring finger, between the rock and culvert causing a cut to the finger resulting in three stitches.

Miner was in a crouching position to clean out under a railcar. When miner came out under the railcar, miner could not straighten knee that felt locked up in a bent position. Miner had surgery on 6/10/2019.

At 7:30 AM an articulated haul truck crossed a dike which caused the road bed to fall into a hole under the road which is located on the top of the dike. At 10 AM the secondary wash plant shut down to reduce water entering pond 2. At 2:15 PM the diameter of the hole changed so was reported to MSHA as an unstable impoundment.

While unbolting two pipes and taking the last bolt out, one of the pipe flanges popped up striking the miner by the right eye.

Miner was using a ¾ inch ratchet to operate a gate on the S3 structure. When the miner pushed down on the ratchet, their pinky finger was around the end of the ratchet handle and the ratchet slipped out of the modified shaft end causing their finger to be caught between the guard and the handle.

The miner was fueling the haul truck from the fuel tank. The miner turned around to talk with another associate and tripped over the large orange wheel chock falling on left knee cap. The miner had surgery on December 7, 2017 and is expected to be off work two to four weeks.

When stepping down from a conveyor, the miner miss stepped and fell striking against another conveyor.

Miner was standing on a ladder cleaning the grizzly in dry 2. The ladder twisted causing miner to loose balance and fall approximately 5 feet to the ground.

While changing an elevator bearing, the miner went to assist the other miner on the opposite side of the elevator. EE's body was then out of position and they noticed a shoulder problem when upper range of motion was attempted. Miner had surgery on 06/25/2018 since shoulder did not heal properly over time.

Miner was checking the proper sequence of rail cars. The miner did not realize that ee had traveled past the end of fall protection lifeline. This stopped the miner and caused body to turn and fall onto the side of the rail car. Miner did not miss work but was sore so went to doctor on day off 2/15/17. X-rays showed cracked ribs. No other medical treatment was given.

The miner was using a Leatherman to cut plastic pipe. The Leatherman slipped and cut the inside of the miner's left thigh. The miner received a cut resulting in five stitches.

Miner was crimping a hydraulic hose fitting. The metal plate shifted causing a metal sliver to become imbedded into his finger.

Associate was removing the tip assembly from the 988H loader bucket. Associate was using a 2 pound hammer and pry bar. Part of the tip assembly broke and hit the associate's left forearm.

Associate was removing a short piece of hose off a barbed end on a floc skid. He was using a utility knife to cut the hose loose from the barb, and the knife slipped and cut his finger.

Associate stepped in front of a moving train and was struck from behind.

The miner's foot slipped off or missed a rail tie causing the miner to fall into a rail car resulting in a cut forearm.

While welding the weld lead splice connection came apart and the extra lead struck the miner.

Employee was eating lunch, a hornet flew into his mouth stinging him on the inside of his mouth. Employee is allergic to hornet stings and was taken to a clinic for treatment.

Employee was removing metal banding. During removal process, individul was struck in arm by banding causing laceration

The associate was using an utility knife to remove the outer coating of an electrical cable. The knife slipped and it cut his left index finger.

Miner was removing a plastic pipe that was supported by a metal ring. The ring slid down the pipe about 20 feet and hit his hand.

Miner was getting out of a truck when he fell hitting his back on the running board.

At the bi-annual occupational health physical, the physician noticed problems with the miner's wrists. Upon further medical evaluation on 10/1/2012 the miner was diagnosed with carpal tunnel syndrome in both wrists.

The miner bent over to close a 6 inch butterfly valve on a discharge line and while bending and twisting to close the valve felt pain in his lower left back area.

Miner was loading railcars and at the time of the incident crossing from one railcar to the next. The lanyard from the fall protection system is rolled out ahead of the miner. In this case the rollers attached to the lanyard which rolls on the overhead cable caught on a splice. When this happened the associate fell backward on the catwalk of the car behind.

Associate was filling the water truck. The valve to fill the truck was over grown with weeds and he had to clear them out. He thinks he touched poison ivy on his arms and it spread to his stomach.

Associate called work from his home residence after his work shift. Associate was working outdoors two days in the warm rail yard. He reported not feeling well. Upon the phone call we encouraged him to seek medical attention. He saw a doctor that afternoon and learned he had another medical issue going on.

Associate was running the 834 dozer for extended periods of time the past few months. The control level for the blade has not been working the way it should. This exposed the associate to repeated actions with his fingers to accomplish what should be a smooth continuous movement.

Installing a fender on haul truck box assembly. Associate was standing on an elevated platform to position fender. Fender was swung into position prior to him mounting the platform stairway. As he was aligning bolt holes to fasten the fender it became wedged in the assembly. When fender unstuck it hit associate causing him to overcenter his weight and platform to tip over.

Annual hearing conservation audiogram showed a positive threshold shift in the associate's left ear.

Cutting handrail with Porta band saw, took one hand off to catch handrail, saw came of handrail and struck his forearm resulting in stitches.

Hand hoisting a 24-ft piece of sheeting: pulled on the rope line and looked away for brief moment and the cut end of sheet struck left elbow.

EE and co-worker were bolting a beam in place using the wrong size of bolt, they tapped it out and it flew and hit EE in the mouth fracturing his bottom right tooth.

Replacing fasteners on 980G loader bucket cutting edge assembly. One of the fasteners fell down not in contact with blade anymore so associate had to reach through and push the fastener in place to remove and replace it. The worn sharp edge of this fastener cut through the glove and left middle finger.

No specific event. Working at rail loadout car cleaning site installing 136 pound grating sheets and 24 inch belting in between rails. Utilized a material handler to do heavy work and physical work consisted of positioning the grating sheets in place with two associates.

Working in a scissor lift, got off of it ot carry angle iron. As he was walking with the angle, his safety harness lanyard got caught on the heal of his boot, which caused him to fall foward over some pipe and land on the ground.

Associate was assisting to move and lift a 32 foot fiberglass ladder from the scrubber building. The ladder was located in a tight area and was entangled in an upper unistrut. While one associate was working to release the upper portion, the associate on the lower side twisted and injured his lower back.

Associate was using a 3"x3"x8' long post to pry over an 8 inch plastic pipe line in soft muddy conditions when he hit his finger on the clamp of the pipe when jamming the post under the pipe.

A large portion on material sloughed from the outside slope of the downstream embankment requiring the pond to be pumped down to minimize failure.

Excavator had been removing material from the toe of the slope which surrounds Pond 7. While taking a break, material began to crumble off the slope. The operator attempted to move the excavator away from the slope, but while attempting to do this, the excavator was engulfed with material and flipped partially on it's side.

Mine associates left the main office area and traveled to the mine work area at 6:20 a.m. One EE notified another EE at 6:25 a.m. by FM radio the mine office trailer had been destroyed by fire. The fire occurred sometime over the weekend. The mine was not in operation and no injuries. MSHA was contacted at 6:34 a.m. to report the fire. Cause has not been determined.

Associates were closing a difficult discharge gate on a rail car. Associate reached in to adjust the lock at the same time his team mate gave leverage to a bar to close the door. The associate's right index finger got pinched between the lock and rebar.

Associate twisted his lower back while adjusting the weight of a wheel assembly.

Associate was getting a production sample at the secondary wash and tripped on the grating and fell into the derrick and jammed his pinkie finger.

Shirt sleeve contacted grinding wheel & pulled arm into grinding wheel. Caused laceration on right forearm. Butterfly stitches used to close the wound.

Co-worker was passing a wood 2x4 up to injured EE. It struck his chin, resulting in stitches.

Associate was installing right steering cylinder on 988G loader. When positioning the front end of the cylinder, the rear portion came off the mount and cylinder shifted pinching his left middle finger between the chain and the frame of the loader.

Associate was replacing a length of rail. The temperature was in the high 80's with high humidity. Associate had not been drinking fluids and became overheated.

While operator was running the dozer, the air conditioner vent came loose and fell down. The operator attempted to replace the vent cover without turning off the air conditioner and his hand hit the fan blade which is about five inches inside the unit.

Repetitive motion from closing and tieing bags.

Loading tote bags with sand. Loading tote bags into a rail boxcar with a forklift. Felt pain in neck and shoulders.

Associates were welding pipe. A tension problem for pipe clamped into position. The associate got out of crane to assist the ground man. The associate was unbolting tension using his left hand which was near pinch point. At the same time other associate made adustment with hydraulic level to remove tension. The level spiked up then down pinching thumb between metals.

EE REPORTED SORENESS IN RIGHT ELBOW WHICH BEGAN ABOUT 9 A.M. & CONTINUED TO WORSEN THROUGHOUT THE DAY. EARLIER THAT MORNING HE WAS WORKING ON THE EAST OUTSIDE ELEVATOR TRYING TO TIGHTEN BELT & HAD PROBLEM TIGHTENING FROZEN TAKE UP SLIDES. HE HAD BEEN USING AN IMPACT WRENCH AND RATCHET. DOCTOR DIAGNOSED TENNIS ELBOW ON 12/31 & THOUGHT HIGH IMPACT WRENCH MAY HAVE CONTRIBUTED.

EE BENT OVER TO PICK UP A PIECE OF THREE INCH TUBING IN THE WELD SHOP FOR A MAINTENANCE PROJECT HE WAS WORKING ON AND FELT SOMETHING IN HIS BACK. HE WENT TO THE CHIROPRACTOR AT 11:45 A.M. AFTE R THE APPOINTMENT HE RETURNED TO WORK BUT THEN WENT HOME. THIS IS A REOCCURRING INJURY. LAST 7000-1 REPORTED ON 9/8/03 AND HE WAS RELEASED ON 10/10/03.

THIS INCIDENT WAS REPORTED TO THE MINE OPERATOR ON 11/25/03. WHILE OPENING A BOXCAR DOOR, THE DOOR GOT STUCK SO THE ASSOCITE PUSHED IT WITH HIS LEG. AS HE APPLIED PRESSURE TO THE DOOR, HE FELT A PAIN IN HIS LOWER LEFT ABDOMEN. LITTLE BY LITTLE THE PAIN IS GROWING. HE SOUGHT MEDICAL TREATMENT ON 11/25/03. HE WAS PLACED ON NO PUSH/PULL RESTRICTIONS UNTIL 12/16/03.

EE WAS MOUNTING A BELT SCALE AND PROPPING UP ROLLERS. HE WAS BENDING OVER AND TWISTING SLIGHTLY. EE EXPERIENCED A PAIN IN HIS LOWER BACK AND HAD A DIFFICULT TIME STRAIGHTENING UP. EE STATED THAT THIS IS A REOCCURRING PROBLEM FOR HIM. HE WENT TO A CHIROPRACTOR.

ASSOCIATE SOUGHT MEDICAL TREATMENT FOR SORE WRIST. NO REPORT OF INJUGY WAS FILED PRIOR TO THE DR APPT.

A CO-WORKER WAS UNLOADING STEEL WITH A FORKLIFT.EE TRIED TO STRAIGHTEN THE STEEL SO IT WOULD STACK EVENLY. WHEN HE SLID THE STEEL HE CUT HIS RT.THIRD FINGER ON THE INSIDE STEEL TUBE ROUGH EDGE . EE RECEIVED 3 STITCHES.

EE WAS WALKING ON CEMENT FLOOR AND WHEN TURNED AROUND FELT A PULL IN HIS RIGHT LEG. HE MENTIONED TO A CO WORKER THAT HIS LEG/KNEE WAS BOTHERING HIM AND TOLD HIS SUPERVISOR EARLY AFTERNOON.

ON NOV. 1, 2001, EMPLOYEE REPORTED HE HAD A SORELEFT SHOULDER AND MAY HAVE TORN MUSCLES. HIS IN-JURY OCCURRED ON OCT. 17, 2001, BUT WAS NOT RE- PORTED AT THAT TIME. HE SAW A SPECIALIST ON 11/7 2001 AND WAS DIAGNOSED WITH LEFT SHOULDER PAIN/ ROTATOR CUFF TEAR. HE WAS PLACED ON LIGHT WORK.

EE WAS EXPERIENCING NECK & LEFT ARM PAIN & STAERTED CHIROPRACTIC CARE ON 11/7/00. AFTER 3 VISITSHE SCHEDULED AN APPT AT KROHN CLINIC. ON 11/21/00 EE WAS DIAGNOSED WITH LEFT CERVICAL RADICULOPA THY & PLACED ON RESTRICTIONS UNTIL RELEASED BY NEUROSURGEON. ON 12/29/00 EE SAW DR & WAS DIAGNOSED WITH CPT FROM WORK RELATED ACTIVITIES

OPERATORS STARTED THE LIQUID PETROLEUM BACKUP SYSTEM FOR DRY PROCESS BURNERS FOR SYSTEM TEST. VAPORIZER STARTED ON FIRE. FIRE EXTINGUISHED WITHIN FIVE MINUTES OR LESS.

ROPE CLAMP BECAME LOOSE WHLE STRINGING CONVEYOR BELT. OPPOSITE INERTA CAUSED EE'S AREM TO SNAP BACK WHEN ROPE CAME UNCLAMPED FROM BELT. EE REOPNED THE CASE ON 11/9/99 BUT DID NOT SEEK MED CHI ROPRACTIC CARE UNTIL 3/24/00. PT STARTED AT THISTIME ON 3/24/00

EE WAS EXPERIENCING BACK PAIN AND SOUGHT MEDICAL ATTENTION ON 1/25/99. AFTER SUBSEQUENT INVESTIGATION AND REFLECTION, EE & HIS DR ON 2/26/99 ATTRIBUTED THE PAIN TO AN INCIDENT INVOLVING A STRA IN AT WORK FROM SLIP ON ICE (NO FALL) ON 1/12/99. EE WAS CHIPPING ICE AND SLIPPED AND CONTINUED TO SHOVEL SNOW THE REMAINDER OF THE DAY.

EE FELT A PAIN WHILE BAGGING, NO SPECIFIC CAUSE COULD BE DETERMINED. EE WENT TO A CHIROPRACTOR,M AND HAD A RIB PUT BACK IN PLACE. HE THEN WENT TO A MEDICAL DR WHO DIAGNOSED HIM WITH THE FOLLOW ING BELOW.

EE WAS PACKAGING SAND BAGS ON SECOND SHIFT WHEN HE FELT A BURNING SENSATION IN HIS RT SHOULDER.

EE WAS WELDING A HOLE IN OVERFLOW OF SOUTH UNIT AT THE FOUNDRY PLANT. WHEN HE WAS DONE WELDING, HE TOOK HIS WELDING HELMET OFF AND STOOD UP TO PUT THE HAT ON AND AT THIS TIME HE HIT HIS HEAD O N THE METAL LIGHT FIXTURE ABOVE THE WORKING AREA.

EE WORKS IN THE PACKAGING DEPT. THE CONTINOUS LIFTING OF 100# BAGS CONTRIBUTED TO THIS INJURY.

EE WAS STEPPING FROM ONE RAIL CAR TO ANOTHER ON THE WET SAND LOADING TACKS. HIS FALL PROTECTION LANYARD CAME TO AJOINT IN THE OVERHEAD CABLE WHERE THE CABLE IS ATTACHED TO THE SUPPORT POLES. W HEN THE LANYARD STOPPED SLIDING ON THE CABLE, IT BECAME TIGHT AND PULLED HIM BACK ONTO THE RAILCAR HE HAD STEPPED OFF OF.

EE WAS LIFTING 100 POUND BAGS OF SAND AND HIS BACK BECAME SORE. THIS INJURY MAY HAVE BEEN CAUSED BECAUSE 100 POUND BAGS MAY BE BEYOND HIS PHYSICAL CAPABILITIES AND THE ASSOCIATE HAS BEEN INJUR ED PREVIOUSLY.

EE WAS PUSHING A WHEEL BARROW OF WET SAND. HE WAS DUMPING THE WET SAND BY PUSHING THE HANDLES UP. THE WHEEL BARROW DID NOT EMPTY THE WET PRODUCT AND PULLED HIM FORWARD. HE NOW HAS A PAIN IN TH E SMALL OF HIS BACK.

EE CUT HIS LEFT HAND WHEN CUTTING OFF TAPE ON ELECRIC WIRES TO MOTOR.

EE STEPPED OFF THE GRADER AND TWISTED HIS RIGHT KNEE.

HOT PLATE SHIFTED AND HIT FINGER AGAINST PLASTIC PIPE WELDING MACHINE WHICH SMASHED FINGER, CHIPPED BONT AND LOST FINGERNAIL.

EE WAS LIFTING AND SETTING PACKAGES BOXES ON THE THIRD LEVEL OF THE MOZZANINE IN THE PARTS ROOM. WHILE BRUING HIS ARM DOWN, HIS ARM CAME DOWN ON THE CORNER OF ANGLE IRON CAUSING A PUNTURE TO H IS RIGHT WRIST. HE RECEIVED THREE STITCHES.

EE WAS FUELING SUBCONTRACTOR'S EQUIPMENT. HE PULLED THE FUEL HOSE AND TWISTED HIS BODY. WHILE CLIMBING THE STEPS, HE FELT A SHARP STABBING PAIN ON THE RIGHT SIDE OF BACK UNDER HIS SHOULER BLAD E. THE INJURY OCCURRED WHILE HE PULLED ON THE HOSE.

EE WAS STACKING A BAG IN THE PACKAING AREA AND FELT HIS WRIST POP.

EMPLOYEE WAS LIFTING BAGS WHEN HE FELT PAIN IN HIS LEFT WRIST.

EMPLOYEE WAS CLEANOING UP AN AREA AND STEPPED DOWN ON AN EXPOSED SPIKED NAIL.

EMPLOYEE WAS CLEANING OUT THE HOPPERS IN THE PIT TUNNEL. HE WAS USING A LARGE SLEDGE HAMMER TO RAP ON THE CHUTE TO GET THE SAND TO DROP (NORMAL PROCEDURE). HE HAD TO CONTINUE THIS FOR SOME TI ME TO GET THE SAND TO DROP ON BOTH THE EAST AND MIDDLE HOPPERS. HE FELT A SHARP PAIN UNDER RIGHTSHOULDER BLADE. EMPLOYEE REPORTED THIS ON 10-28-97. ON NOV. 11 HE SAW DR: MEDICATION & REEVALUAT

EE HAS TENNIS ELBOW IN THE RIGHT ELBOW. APPROX 80 PERCENT OF HER OB IS PERFORMED ON THE COMPUTER. THE REPETITIVE ACTIONS HAS CAUSED TENNIS ELBOW.

EE WAS WORKING ON THE PIPELINE TO UNPLUG THE +30 STOCKPILE PIPE LINE. AS HE WAS REMOVING THE BOLTS, THE PIPE FLANGE JUMPED UP & HIT HIM NEAR HIS RIGHT EYE CAUSING A DEEP INCISION. HE RECEIVED SEVERAL STITCHES. THE TENSION IN THE PLASTIC PIPELINE CAUSED IT TO PULL BACK ENOUGH TO BE ABLE TO PULL UP.

THE ASSOCIATE DOES NOT KNOW HOW INJURY OCCURED. HE CLIMBED STAIRS AND FELT A PULL IN THE BACK AREA.

THE EE WAS BAGGING AND GRABBED A BAG THE WRONG WAY WITH HIS LEFT HAND ON 7/9/97. ON 7/10/97 HIS HAND WAS SORE WHEN HE BAGGED.

EE WAS INJURED ON 6/26/97 BUT DID NOT SEEK MEDICAL TREATMENT. ON 12/23/97 HE WAS SEEING THE DR FOR A NON-WORK RELATED PROBLEM & TOLD HIS DR HE HAS BEEN HAVING PAIN IN HIS LOWER ADBOMEN. THE DR THEN CHECKED THIS AREA & DIAGNOSED A SMALL R INGUINAL HERNIA CAUSED BY LIFTING AT WORK ON 6/26. EE PULLED A MUSCLE IN THE LOWER RIGHT ABDOMEN BY PICKING UP A SHAFT OFF THE PIT HAUL ROAD.

EE WAS WALKING OVER TO CLOSE A VLAVE, WHEN HIS FOOT SLIPPED AND HE TWISTED HIS LEFT KNEE. HE HAS A SPRAINED KNEE AND IS AWAITING FOR MORE TEST S TO BE DONE.

BOLTS ARE TIGHTENED TO 150 LBX OF TORQUE. EE WAS ATTEMTING TO LOOSEN BOLTS WHEN HE FELT THE PULL TO LOWER GROIN AREA. PLEASE REFER BACK TO THE 70001 FORM DATED 8-26-95. HE RETURNED TO WORK ON 9-25-95, WHICH RESULTED IN 12 LOST DAYS. THE DRFELT THIS STRAIN RESULTED FROM THIS PREVIOUS INJURY.

EMPLOYEE CUT DOWN #1 TREE WHICH FELL BACKWARD ONTO A STANDING TREE. EMPLOYEE LEFT TREE #1 AND CUT DOWN TREE #2 WHICH FELL TO THE GROUND WITHOUT INCIDENT. EMPLOYEE STARTED TO REMOVE THE LIMBS F ROM TREE #2 AND WHILE DOING SO WIND CAUSED TREE #1 TO MOVE OFF IT'S PERCH AND FALL TO THE GROUND AND ONTO EMPLOYEE.

CUTTING TREE, TREE SET BACK. WENT ON CUTTING THREE MORE TREES. MARKING & TRIMMING 4TH TREE, GUST OF WIND CAME AND BLEW THE FIRST TREE BACK FORWARD ON TOP OF HIM (A BRANCH HIT HIM IN THE BACK F RACTURING A VERTEBRA).

EE WAS REPLACING A SECTION OF 14" PIPE WITH A TOWNLEY Y. HAD THE Y HANGING FROM A CHAIN HOIST AND LIFTED A SHORT TOWNLEY STUB UP ON THE Y AND PULLED HIS LOWER BACK. THE STUB WEIGHED APPROX. 50 LBS.

EE DEVELOPED PAIN AND STIFFNESS IN HIS RT LEG AND LOWER BACK WHILE WORKING BETWEEN THE DRY PLANT AND LAB. THIS OCCURRED APPROX 11:30 PM ON12-1-96. EE COULD NOT ATTRIBUTE PAIN TO ANY SPECIFIC A CCIDENT OR INCIDENT.

EE WAS GOING TO ADD OIL TO TH HYDRAULIC TANK ON ME-314 PAYHAULER TRUCK. EE SAID HE TURNED HTE CAP ABOUT 1 TURN AND THE CAP BLEW OFF STRIKIGN HIM IN THE LOWER LIP. THE CAP CUT HI LIP AND BROK E ONE TOOTH AND LOOSENED ONE MORE AND BENT IT BACK IN HIS MOUTH.

FINISHED TORCH CUTTING A PIECE OF 6" PIPE. STARTED TO GRIND OFF TO ATTACH A NEW FITTING. GRINDING WAS BEING DONE OVERHEAD FROM AN EXTENSION LADDER, APPROX 8' ABOVE THE GROUND. THE ASSOCIATE MOVED FOR A BETTER POSITION. HAND SLIPPED UP GRNDER AND CUT RIGHT INDEX FIGNER, REQUIRING STITCHES.

EE GOT OF HIS TRUCK IN THE PARKING LOT, IMMEDIATELY FELL ON THE ICE. DOESN'T KNOW IF HE HIT HIS SHOULDER ON THE ICE OR THE VEHICLE PARKED NEXT TO HIM. HIS TRUCK DOOR WAS NOT CLOSED YET.

INJURED WAS IN THE PROCESS OF WELDING 12" PLASTIC PIPE. A CRANE WAS USED TO HOLD, THE HEAVY LENGTH OF PIPE IN PLACE WHILE HE WAS ADJUSTING THE PIPE WELDER TO FIT IN PLACE. THIS WAS DONE BY HAND BY LIFTING THE HEAVY MACHINE WHICH CAUSED THE PAIN IN LEFT GROIN AREA.

THE OPERATOR WAS CLOSING THE COVERS OF A PNEUMATIC TANK TRUCK A FALL PROTECTION DEVICE WAS BEING USED THE DEVICE WHICH SLIDES ON A CABLE WAS NOT POSITIONED OVER THE DRIVER BECAUSE OF THIS BIND ING OCCURED THE MECHANISM WHICH WAS MOMENTARILY RESTRICTED GAVE WAY AS THE OPERATOR BENT FORWARD THE OPERATOR THEN SLIPPED & SLAMMED THE HATCH COVER ON HIS RIGHT HAND

EMP WAS LIFTING AN ELECTRICAL ENCLOSURE WITH AN ELECTRIC HOIST. THE CHAIN HE WAS USING WAS BAD, IT HAD A BAD LINK. SOMEONE HAD DROPPED IT OFF FOR REPAIR BUT DID NOT RED TAG IT. EMP DID NOT NOT ICE BAD LINK,BROKE & DROPPED ENCLOSURE ON FOOT.

EE WAS TYING 3,000# TOTE SACKS ON PALLETS. HE WALKED BETWEEN TWO PALLETS AND TRIPPED ON ONE, FELL FORWRAD, AND TRIED TO CATCH HIMSELF. UPON CATCHING HIMSLEF, BENT BACK LEFT WRIST.

WHILE EE WAS STEPPING DOWN APPROX 28" FROM CATWALK OF LOADOUT TO TOP OF A GBW 6000 RAILCAR, HE LANDED WRONG ON HIS FOOT, TURNING HIS ANKLE.

INJURED WAS USING A CHAIN AND COME ALONG ON A PIPE. AS THE PIPE MOVED, IT DRAGGED THE COME A LONG ACROSS THE TOP OF INJURED'S FOOT.

HE HAD WASHED OUT HIS TRAILER IN THE BMC RAIL WASH-OUT BAY. HE STOOPED UNDER THIS TRAILER TO CLOSE THE BOTTOM VALVES. WATER, WITH CHLORINE IN IT DRIPPED ON HIS NECK WHERE HE HAD A BUG BITE. HE EXPERIENCED IRRITATION BY THE FOLLOWING WEEK AND THEN WENT TO THE DOCTOR. THE DOCTOR GAVE HIM CEPHALEXIN TO HELP THE IRRITATION.

EE WAS GRINDING THE LIP OFF OF A BRAKE DRUM & GRINDING DUST GOT INTO HIS RT EYE ON 08-11-94 NO TE IRRITTATION WAS NOT REALIZED OR REPORTED UNTIL THE NEXT DAY 08-12-94 EE THEN WENT TO THE DOC FOR THE IRRITATION

INJURED WAS BRUSHING THE DERRICK SCREENS. AS HE FINISHED THE TASK, HE STARTED DOWN THE LADDER. THE LADDER TIPPED. HE REACHED FOR A PIPE TO HANG ONTO. HE MISSED THE PIPE AND LANDED ONT HE D ERRICK FRAME, BRUISING HIS LEFT SIDE RIB CAGE.

EE WAS ATTEMPTING TO MOVE 2 STEDMAN CAGE RINGS THAT WERE LEANING AGAINST AN ASSEMBLED CAGE WHILE HE WAS ROLLING THEM TO THE SIDE THEY BEGAN TO FALL EE LET GO & JUMPED BACK TO GET OUT OF THE WA Y WHEN THE RINGS HIT THE FLOOR ONE BOUNCED BACK & STRUCK HIM OVER HIS LEFT EYE

WHILE EE WAS PACKAGING 50# POLY BAGS, HE FELT A STRAIN IN HIS NECK. THE PAIN WOULD COME AND GO UNTIL THE 26TH. WHEN GETTING INTO HIS CAR THE PAIN REUTRNED, SO HE PLANS TO SEE A CHIROPRACTOR FO R THIS INJURY.

HE WENT INTO A STORAGE TRIALER VAN TO CHECK WHAT BAGS WERE ON HAND. TWO STYLES OF BAGS IN STORAGE. HE WAS CLIMBING OVER STACKS OF BAGS AND SLIPPED ON THE POLY SHRINK-WRAP COVERING. INJURED WAS POUNDING WITH A HAMMER ONTHE BACK OF SAND HOPPER TO HELP LOOSEN UP FROZEN SAND. ON THE BACK-SWING, HE HIT HIS FOREFINGER ON AN ANGLE IRON. IT BCAME SWOLLEN AND STIFF. FINGER EVE NTUALLY BECAME INFECTED AND NEEDED MEDICAL ATT .

CONTRACTOR WAS REMOVING A STEEL CYCLONE WITH A CUTTING TORCH. SPARKS FROM THE CUTTING TORCH STARTED A RUBBER LINNING BURNING. WHEN THE UPPER PIPE WAS REMOVED A WIND DRAFT FORCED FLAMES OUT THE LOTTON OF THE CYCLONE BURNING THE WORKERS LEFT HAND AND SOMEWHERE ON THE LEFT SIDE OF HIS FACE.

EE WAS DISMOUNTING FROM A RAIL ENGINE. THE ENGINE WAS MOVING APPROX. TWO MILES PER HOUR. EE STEPPED ON A RAIL BALLAST AND FELT HIS KNEE GAVE OUT.

HE WAS GOING DOWN THE BASEMENT STAIRS AND TURNED DOWN AT THE BOTTOM OF STAIRS WHEN THE BACK OF HIS RIGHT KNEE WENT OUT.

EE WAS CLIMBING DOWN THE LADDER ON A BN RAIL CAR AND WHEN TRANSFERRING TO THE OUTSIDE LADDER ON THE CAR, HE SLIPPED AND FELL TO THE GROUND. HE FELL ABOUT TWO STEP LENGTHS ON HIS BUTT. AT THE T IME OF THIS REPORT, HIS LOWER BACK AND KIDNEY AREA HURTS.

HE WAS ATTEMPTING TO CUT RUBBER CHANNELS WITH A UTILITY KNIFE FOR A BACKUP SCREEN SURFACE THAT PROTECTS THE PIONEER VIBRATING SCREENS. THE KNIFE SLIPPED LACERATING HIS LEFT INDEX FINGER CAUSIN G HIM TO RECEIVE 3 STITCHES. HE IS NOW ON ALTERNATE DUTY- NO LOST TIME.

INJURED WAS FEEDING SAND TO OUR CRUSHER WITH A 988 CAT LOADER WITH THEDOORS OPEN AND WEARING SAFETY GLASSES WITH SIDE SHIELDS. AT THE END OF HIS SCHEDULED WORK SHIFT HE FELT SOMETHING IN HIS RIGHT EYE. 24 HOURS LATER HE HAD METAL DUST REMOVED WITH A NEEDLE AND BUR INSTRUMENT FROM HIS EYE. NOTE: NO TIME LOSS BUT THE METAL DUST WAS IMBEDDED IN HIS EYE.

WHILE EE WAS ASSISTING ANOTHER EE IN MOVING BOX THAT WEIGHTED APPROX. 150 LB HE STRAINED HIS LOWER BACK.

WHILE PULLING ON DRIVE BELTS-I.NAME\S LITTLE FINGER ON HIS LEFT HAND BECAME PINCHED BETWEEN THE BELT & SHEAVE OF THE MOTOR. THE FINGER WAS CUT REQUIRING 4 STITCHES.

THEY WERE CARRYING A 6" BEAM FRAME WORK FROM OUTSIDE OF SHOP TO INSIDE THE SHOP, THE NIGHT SHIFT WELDER COULD WELD TOGETHER BEAM WT APPROX 600 LBS. STEVE STATED AT THIS TIME AND HIS BACK HURT BUT NO OTHER SUPERVISOR CAN REMEBER HIM STATING THIS HE REPORTED THIS TO TOM GONVO II ON THURS. 3-15-84.

MIKE WAS MOVING AN UNLOADING HOSE ON A RAIL CAR.HE WAS ATTEMPTING TO CLIMB THE CAR METAL LADDER WHEN HE SLIPPED ON A STEP PART WAY UP THE CAR LADDER. AS HE FELL BACKWARD HE HIT HIS TAILBONE ON ANOTHER RAIL CAR BEHIND HIM. HE SAID HIS TAILBONE WAS EXTREMELY SORE FROM THE INCIDENT. HE WAS TAKEN TO THE CLINIC WHERE THE DO CTOR DISCOVERED A BROKEN TAIL BONE.

LOADER RUNNING ABOVE NORMAL WATER TEMP,TURNED OFF MACHINE THEN REMOVED RADIATOR CAP AND WAS HIT WITH HOT WATER COMING OUT OF RADIATOR TO CHEST AREA AND UNDER ARMS.

TOM OLSON WAS TRYING TO OPEN THE DOOR OF A RR CAR BY STANDING ON A BAR TO TURN HANDLE OF CAR DOOR HE SLIPPED OFF THE BAR BUMPING HIS KNEE LEFT ON RAILROAD TIE
