Fire was observed on wash tower 2.5 hours following welding on the collar of a feedbox. Fire was extinguished 43 minutes after it was noticed.

On the above date and time, EE was operating a dozer on a dumpsite that sloughed off into an impoundment. EE was trapped inside the dozer for about 2 hr. 17 min under water.

While employee was replacing a guard the employee's finger was pinched between the guard and a hand rail.

Employee pinched right little finger between a scraper and a hopper wall while trying to scrape frozen sand out of hopper. The finger was first treated with first aid. Two days later the employee was seen by a doctor and there was a slight fracture to the finger tip. Employee was able to return to work with no restrictions on next scheduled shift.

Employee had hook from lanyard hooked to the shoulder strap of a harness while not in use. While attempting to remove hook from shoulder strap to put in use the employee removed the hook and hit themselves in the mouth with the hook.

Employee was clearing a frozen block of sand from a conveyor belt. While moving the block of sand it slipped and the employee's finger was pinched between another frozen block of sand.

Employee reported 3 days after date of injury with work restrictions for a sprained right shoulder.

Employee was using a hand grinder to cut a section of chain when chain was pulled into grinder employee lacerated tip of finger.

Employee was walking down from a rail switch and turned ankle on the ballast rock.

Employee was loading on top of rail cars. Employee was tied off to fall protection the entire time. Employee was using a retractable lanyard and let it trail behind while walking on the car and moved too quickly causing the retractable lanyard to lock. Employee lost balance and fell onto the catwalk on the rail car and sustained injury.

The injured miner was assisting an electrician in checking extension cables leading to a stacker. The power circuit being tested was not de-energized before the work was done. When a plug-in was connected the injured miner holding a test wire received an electrical shock and arc flash burns.

Employee was moving a metal box and felt discomfort in left wrist. Employee finished the rest of shift. Employee told supervisor the next day and then informed manager 2 days later that ee wanted to go to the doctor to get out of guard duty. Employee went to the doctor 3 days later and received restrictions.

EE was painting walls in a QC lab. EE stepped off of the step ladder that was being used and unto the counter top in the Lab. EE lost balance and fell from the counter top to the floor. EE hit head on the floor causing a laceration that required 7 sutures to close the laceration. EE was able to return to work that day with restrictions.

Employee was using a self tapping screw to hang a bracket. Metal shaving from the steel that screw was being put into got into employees right eye. Employee had shaving removed at a clinic and was given prescription drops for the eye. No lost time or restrictions were given, and employee returned to work.

EE was lifting on a board that was being used as a form for a small concrete pad that was to be poured. EE noticed discomfort in his shoulder. EE was seen by a doctor on 5-13-2015 and has a follow up appointment on 6-18-2015.

Employee sustained a laceration to right pinky finger from a sharp edge on a steel chute. Employee was treated with first aid. 2 days later laceration was showing signs of infection. Employee was seen by physician and given an antibiotic for infection.

A fatality occurred at this operation between 18:00 on March 25, 2015 and 9:30 on March 26, 2015. An order was issued under 103(j). The order was issued due to a non employee having an ATV accident on the mine property. The accident was not near the active mine or related to mining activity.

On 11-3-14 EE had his foot pinched between a Rotex machine and the floor, at that time there was no injury. He told his direct supervisor that on 11-4 he was checked by a doctor and had no injury or restrictions. He worked for 2 full weeks with no sign of injury. On 11-17 he went back to the doctor and was taken off work, he is claiming it is from the 11-3 incident.

Employee was working on an excavator tooth and a piece of metal came off a pin and lodged in his left thigh.

Employee was moving a pump when it tipped forward and pinched his arm between a guard and the pump. Co-worker took employee to the Hospital where he received three stitches. The Doctor had restrictions on employee.

After cutting banding from wooden stake pallet, EE reached for some of the stakes and cut his finger on the sharp cut edge of the steel band, causing a deep cut. No gloves were worn.

Not recognizing icy conditions while performing shift inspection, causing slip hazard, which resulted in a fall that ended up with a loss of 1 1/2 fingers on right hand.
