Employee went to stand up in work area when employee felt pain in ankle. The employee was taken to the ER and found to have a sprained ankle. Employee has been removed from work as of 2/24/2025 until 3/6/2025 as per doctor.

Employee was opening the door to #2 crusher to clean it out when EE felt pain in the shoulder. The employee went to the doctor on 2/18/25 when they diagnosed EE with a rotator cuff sprain and needs to be on light duty.

Weather created slick conditions in the hopper pit. The employee was resetting camera feed and slipped and fell on the way back to the control room.

Employee was removing an air line from the East blow bottle when they fell from ladder approximately 6 feet. Employee was taken to the hospital for x-rays which confirmed they fractured their right heel.

Employee was lowering a pinion shaft to the floor when EE lost EE's grip. When EE re-positioned quickly, EE felt a pain in EE's shoulder and arm.

Employee was opening gates on rail cars when EE tripped and hit left knee.

Employee was pulling on a pipe wrench trying to rotate a shaft on a screw. The shaft was locked out and tagged. The employee was using excessive effort that caused the shaft twist like a torsion bar. The employee lost his grip on the wrench and stored energy in the twisted shaft caused the wrench to hit the employee's right hand breaking a bone.

An employee was in the process of adjusting the roll of shrink-wrap when one end of the bar holding the shrink-wrap slipped off the stand, this caused the roll weighing approximately 100 pounds to drop the opposite end of the bar raised crushed the employees right little finger between the bar and the stand.

Employee was in the process of connecting a car puller cable to a rail car. The employee did not wait for the railcar to come to a stop before attempting to place the hook on the rail car. The hook on the car puller pinned the employees hand in between the car puller cable and the railcar. This resulted in the employee breaking a bone in his left hand.

An employee was working on loading rail cars when the employees lanyard did not retract the lanyard got caught on a moving railcar and the employee fell about 3feet from a landing onto the top of a rail car. The employee received bruises and cuts to his right side and to his shoulder.

Employee was stacking bags when two bags came at the employee at the same time the employee attempted to stack both bags when he felt a sharp pain in his right shoulder.

The employee put all of his PPE on including the proper fall protection. The employee failed to notify the operators whom normally work in the area of his intentions. The employee began changing the light on the railcars when he felt the cars begin to move. The employee lost his footing and hit his head. The employee received 4 staples in the back of his head.

Employee was attempting to open a door on a railroad box car. The employee was pulling down on the handle and felt a sharp pain in his hip. The pain radiated down to his leg.

Employee was pulling up on a bar when he felt a sharp pain in his lower neck and upper back area.

Employee was in the process of stacking 100# sacks of bentonite. Two bags came down the belt back to back, the employee stacked the first bag and attempted to grab the second when he felt a sharp pain and pressure in his back.

Employee was stacking bags on a pallet, when he felt a muscle cramp in his calf, he then felt and heard a pop in his calf muscle of his left leg. The employee immediately reported the injury to his supervisor.

EE hit his neck while descending the stairs next to #3 Raymond Mill. EE was walking down the stairs when he came to a low overhead clearance pipes; EE did not duck low enough and struck his hard hat on the pipes. This occurred at approximately 11:15am. EE finished his shift that day and in doing so went under the same set of low clearance pipes without another incident.

Employee picked up a roll of shrink wrap. The shrink wrap started to fall out of his hands, the employee tried to catch the roll of shrink wrap and felt a sharp pain in his lower back.

The bag started to fall to the ground, the employee attempted to catch the 100 pound bag of bentonite before it hit the ground. The employee felt a sharp pain in his right shoulder as the weight of the bag pulled on his arms.

While the employee was checking wiring for sensors, he stepped off a step awkwardly and struck the lower part of his left leg on a metal ledge.

Employee was Placing 100# sacks of Bentonite clay on pallets. As he grabbed a full bag and was turning to place the bag on a pallet, he felt severe pain in his lower back.

Employee was in the process of checking on his screens, when he heard the equipment beginning to shut down. The employee immediately turned to go in the opposite direction he was traveling, during this process he heard a popping sound and felt a sharp pain in his knee.

The first Employee put a pallet onto the packing station using a forklift. The second employee was cleaning the rails on the packer. The second employee was between the pallet and the rail. As the first employee was bringing the pallet, he did not raise it high enough, bumping into the empty pallet and pushing into the second employees feet and pinning his feet against the rail.

Employee had connected diengaged car puller to C-6 rail car, then proceeded to push cars with track mobile, car puller became engaged causing cable to stop spolling. Cable became tight then causing cable to become disconnected from clamps, hitting employee on elbow.

Employee was in the process of removing debrie from a clean out system on a bag house. After the employee locked out the system , he proceeds to move the gear on the rotary valve using a pipe wrench while trying to free the object with his other hand. As the object loosens his hand falls forward into the flights cutting his ring and middle finger on his left hand.

EE was assisting another EE spot box rail cars for loading at warehouse. As train came to a stop, EE proceeds to step off of side ladder. While stepping to the ground, he stepped on a large ballast rock and twisted his ankle.

Employee ws preparing work area to load 2500 lb. bags of bentonite when he noticed that the hopper gate was closed. Employee jumped down from deck to ground (approx. 3') landing on his feet. Employee states he felt a "pop".

While Employee was traveling down the stairs between Dryer #1 and #2, the Employee's heel caught on the stairs. This slip caused the Employee to loose his balance and footing. The employee was able to catch himself before he tumbled down the stairs. The employee felt the muscles in his back tighten up

While pounding sign post (steel) on Tater Ridge Road, utilizing a post pounder without handles, EE raised post pounder above level of post. Hands were in gripped position at the bottom edge of pounder. Upon thrusting dowward, hands were pinched and lacerated upon steel post.

Employee was walking to ensure that truck being loaded had ample room to exit. While walking route, employee slipped on muddy area (it had been snowing/raining). Employee's foot slipped. As he slipped his foot caught on ballast (rock). Foot stopped, body continued to travel; employee broke his left ankle.

Employee strained left while operating packing machine, employee was reaching out with left leg to slow bags of bentonite from hitting conveyor belt, keeping bags from breaking open.

Employee went to crusher area to investigate noise coming from the six roll crusher. He proceeded to control panel, switched #14, 15 & 16 from auto to hand (manual) then hit stop buttons. He went back to crusher & removed inspection plate, reached in after spotting rock on top of bottom rollers, catching fingers in bottom roller.

WHILE EE WAS PACKING BAGS OF BENTONITE THE BAG BECAME FULL WHEN THE GLUED SEAMS AT THE TOP OF THE BAG FAILED CAUSING BENTONITE DUST TO BE FORCED ONTO EMPLOYEE'S FACE, HEAD & SHOULDER. EMPLOYEE WAS WEARING SAFETY GLASSES AT THE TIME OF THE INCIDENT.

Employee was striking hopper with 8' rebar to dislodge material from side of hopper. While striking hopper employee claims he felt pain to left shoulder. Not reported until 11-15-05.

Employee was stacking pallets with 100 lb bags of bentonite. Bags were traveling down conveyor to throwing plate, EE was unable to remove quickly. He pressed emergency stop, at which point bags continued to slide down throw plate. He reached to grab bag when it made contact with wrist and throwing plate.

EMPLOYEE WAS STACKING 100 LB BAGS. NOTICED SORENESS IN RIGHT WRIST TOWARDS END OF THE SHIFT. WENT HOME FOR THE EVENING AND WOKE UP WITH SWELLING IN THE MORNING. NOTIFIED SUPERVISOR AND SAW DOCTOR FOR X-RAY. EMPLOYEE WAS GIVEN RESTRICTION OF LIFTING IN EXCESS OF 10 LBS FOR ONE WEEK A BRACE IS ALSO TO BE WORN FOR ONE WEEK.

EMPLOYEE WAS FILLING SACKS OF BENTONITE. THE BENTONITE DUST BECAME LODGED IN HIS EYE. HE WAS TAKEN TO THE DR. TO HAVE HIS EYES FLUSHED.

EE was operating haul truck when it slid off the side of the roadway, tipping on its left side.

Employee was filling 100 bls bags with bentonite when one burst and filled his left eye with bentonite. Employee was taken to the eye dr as a precaution to get his eyes flushed.

EE WAS PLACING 100 LB. SACKS OF BENTONITE ON A PALLET WHEN HE TWISTED WRONG AND PULLED A MUSCLE IN HIS RIB CAGE.

EE WAS STEPPING OFF RAIL TRACK WHEN HIS LEFT KNEE POPPED CAUSING PAIN. EE FINISHED HIS SHIFT BUT WAS UNABLE TO RETURN TO WORK ON 9/19/03.

EMPLOYEE WAS CLEANING INK ROLL WITH A KNIFE, WHEN HE SLIPPED CUTTING HIS RIGHT THUMB. THIS ACCIDENT REQUIRED MEDICAL ATTENTION ONLY.

EE WAS STANDING ON AN OLD PALLET, WHEN THE BOARDS GAVE WAY TWISTING HIS KNEE.

EE WAS PLACING BAGS ON A PALLET WHEN HIS RIGHT FOREARM STARTED TO HURT.

EMPLOYEE WAS FILLING SACKS WHEN ONE EXPLODED INTO RT EYE. HE WAS TAKEN TO HOSPITAL AS PRECAUTION.

EE WAS DESENDING A LADDER WHEN HE TWISTED HIS RTANKLE. HE REPORTED FOR WORK ON 12-10-02. ON 12-11-02 HE WAS SENT HOME MAKING THIS A LOST TIME ACCIDENT.

EMPLOYEE WAS CLEANING PACKER WHEN CYLINDER DISCHARGED AND SMASHED HIS LEFT INDEX FINGER.

EMPLOYEE WAS FILLING BENTONITE SACKS WHEN ONE EXPLODED FILLING HIS EYES WITH BENTONITE.

EE WAS THROWING 100 LBS BENONITE BAGS, WHEN HE MISSED ONE AND IT STRUCK HIM IN THE KNEE.

EMPLOYEE WAS THROWING SACKS OF BENTONITE, WHEN BACK POPPED CAUSING SEVERE PAIN.

WAREHOUSEMAN STEPPED OFF FORKLIFT ON TO A BOARD AND STUCK A NAIL IN RIGHT FOOT, REQUIRING A TETNUS SHOT.

EMPLOYEE WAS SHRINK WRAPPING PALLETTS, WHEN TORCH FELL AND BURNED HIS RIGHT HAND AND FINGERS.

A LABOR WORKER WAS DISENGAGING THE HOPPER CAR PULLER WHEN IT KICKED BACK AND CUT HIS RIGHT THUMB.

EMPLOYEE WAS CLEANING PACKER SPOUT WHEN AIR OPERATED RAM WAS ACTIVATED AND CRUSHED LEFT HAND AND FINGERS.

EE WAS LIFTING BROKEN BAGS OFF PALLET AND HURT LOWER BACK.

EE WAS SHRINK WRAPPING PALLETS OF BENTONITE HE REPLACED TORCH IN THE HOLDER WHEN THE TORCH FELL OUTOF HOLDER & BURNED BACK OF NECK

EMPLOYEE WAS RUNNING 9TM TRACKMOBILE AND ATTEMPTED TO GET OFF WHEN ANKLE BECAME STUCK BETWEEN RAILROAD TIES. HE TWISTED HIS ANKLE AND WAS UNABLE TO CONTINUE HIS SCHEDULED SHIFT.

EE WAS THROWING 100# BENTONITE BAGS AND FELT A POP IN THE LOWER BACK AND WAS UNABLE TOCONTINUE THROWING BAGS.

EE WAS MOVING CARS WITH 9TM TRACKMOBILE. THE CAR HE WAS MOVING COUPLED INTO ANOTHER CAR AND HIS HAND WAS RESTING ON THE WINDOW SILL. THE WINDOW SILL HAD A CRACK IN IT AND HE CUT HIS HAND ON TH E SHARP METAL.

THROWING 100# SACKS OF BENTONITE AND HAND STARTED TO HURT.

EMPLOYEE WAS HELPING ANOTHER MAN LIFT 120# KEG OF GREASE AND THE KEG SLIPPED PINCHING RT THUMB BETWEEN KEG AND STEEL BEAM.
