Crusher plugged. Material was spilt onto the ground. Employee was shoveling material when EE slipped on some mud. Employee's knee started hurting. Employee reported the injury but decided not to see a medical professional. On 11/7/23 employee decided to see a medical professional and was diagnosed with a torn meniscus.

Employee slipped on some mud and fell. Employee informed supervisor their shoulder and elbow were sore. Employee did not seek medical attention at the time of the injury. On 1/18/23 employee had a medical exam. On 1/24/24 employee had an MRI. Employee diagnosed with a strained muscle in left shoulder. On 2/7/24 employee had physical therapy.

Employee was using an industrial vacuum hose when lower back started hurting. Employee decided to work with a sore back until 9/11/23 when employee determined the back was not getting better. Medical treatment provided on 9/11/23 at which time the employee was put on restrictive duty.

Employee was coming down stairs. While on the way down the employee's foot caught the third step from the bottom. This caused the employee to fall and land on the right shoulder. This caused an AC joint separation.

Employee was shoveling material when they felt their back pop accompanied with pain. Employee was diagnosed with a lumbar spine strain.

Employee struck knee on a parked rail car while walking around the car. Employee's knee was bruised. Employee saw a medical examiner on 10/24 because of lingering pain in the knee. On 10/24 the employee was put on restricted duty.

The employee was putting on safety glasses when the left eye was scratched by the earpiece. The employee was prescribed an antibiotic.

The employee was lifting 50 lb chemical bags from the pallet to the feeder when the employee felt a pop and sudden pain in the employee's back. The employee's back continued to be sore after the incident. On 8/17/2020 the employee saw a chiropractor and received medical treatment for the injury.

While lifting an empty pallet the employee felt pain in their back. The employee did not want to see a medical professional at the time of the incident. On 6/17/2020 the employee decided to see a chiropractor. The soonest available appointment was scheduled for 6/25/2020 at which date the employee saw a chiropractor and the employee was put on restrictive duty.

Employee was attempting to remove bearings off a shaft. The puller slipped off the bearing housing and hit the employee above the knee. The employee had a sore leg and swelling. The employee did not seek a medical evaluation at the time. On 12/20/2019 the employee received a medical evaluation. On 12/30/19 the employee had a follow up medical evaluation.

Employee was transferring five 50 lb chemical bags from a pallet to a chemical feeder. After the fifth bag employee felt a pain in EE's lower back.

EE was lifting 100 lb. bags off of the ground and restacking them. While doing so, tripped on some bags on the ground possibly straining their back. The repetitive lifting and the trip caused them to have a sore back. The next day their back was having spasms and was in pain. Employee was diagnosed with a back strain on 01/11/2018.

EE was emptying bin into a bulk sack inside plug mud packaging area. Forklift operator approached bulk sack holding frame, placed forks in pallet, and tilted them forward. Left edge of forklift backstop struck EE fingertip, which was resting on metal holding frame.

On 2/17/17 while working on the outside of the Process Elevator the EE was pulling a bearing off from the outside while another employee had cut the shaft with an acetylene torch that the bearing was connected to. EE picked up the bearing by the shaft causing burns on EE's hand.

EE was getting material samples in 16 ounce cups and was returning to the slab when he slipped. EE did not fall. He regained balanced but felt a "twinge" in his back.EE did not want medical attention. On Sept 8 EE said back was still bothering him and made appointment with a PA for Sept 9.PA recommended EE not work until Sept 13 and then on restricted duty for at least one week.

EE needed to open mill door completely. Door was half open. While opening door EE pulled up on door and twisted his body and felt his back pop.

EE was cleaning the grizzly feeder from obstruction with a bar when foot slipped between the grates in the screen and he twisted his knee. Knee injured, put on restricted duty per medical examiner's instructions.

Supervisor observed on 2/4/14 that EE's eye was tearing up. EE said he had gotten dust in it on 2/3/14 while working on the packer. Supervisor asked if he needed medical attention and EE said no. On 2/5/14 EE informed Supervisor he had gone to the doctor and his eye was scratched. EE received prescription eye drops.

EE was cleaning a plugged chemical induction pipe when some chemical (ferric sulfate) came out of the pipe and got in EE's eye. EE was wearing safety glasses. EE had eyes flushed with eye wash solution and received a prescription for eye drops.

Employee stepped into a pot hole and sprained ankle.

EE did not report until 4/16/2013 that he had jarred his back sometime in December 2012 while operating a front end loader. On 4/16/2013 he told his supervisor about the incident and wanted to go to the doctor. EE was placed on restricted duty awaiting the results of an MRI. Upon receiving results all restrictions were removed and EE returned to work at full capacity.

EE informed supervisor 13 hours after his shift ended that he had injured his back while clearing a bag jam in a packaging line. EE did not indicate any accident or injury prior to leaving his shift that day. There were no witnesses or indications of an injury during his shift.

Employee was running power to a new soda ash tank. He was stripping the ground wire and it slipped out of his hand and hit the power bus causing electricity to run to ground. Employee received 1st degree burns to hands and face.

EE was behind the packer bin working to unplug the #2 spout. When he turned the wheel to open the back of the spout, it came open and smashed his fingers between the spout back and the wheel to loosen the spout back. EE fractured the tip of his middle finger.

Employee was checking coal elevator pit/shed to see if a bearing was hot, he dislodged some coal dust that fell onto smolder coal in the bottom of the coal elevator pit. The dust ignited and singed exposed hair on the employee and causing first degree burn on his left hand.

When EE was getting off the forklift to put cardboard on the pallets in the rail car, his radio caught on the forklift arm rest, causing him to turn odd and twist his ankle when he stepped on the ground.

Employee was tightening the T screw on to the top of the filter press cell when her thumb slipped off the T screw. Her right hand was wet at the time from washing fuidloss cells. The employee has torn or strained ligaments in thumb.

Released the latch on the winch, the handle came around and hit his hand.

Employee dismounted forklift to place stretchhooder on bypass, due to a broken bag on a pallet. When walking around the back of the forklift, the employee twisted her right ankle. Employee was examined by a physician who immobilized the ankle and placed her on restricted duty.

Employee was removing a counter weight on a Doosan D-25S forklift to investigate a steering column leak. The counter weight fell on top of employee's legs. Employee recieved severe laceration to the inside left thigh resulting in muscle, tissue, and ligament damage. Employee was transported to Washakie County Hospital for surgical repair.

The wrench the employee was using to loosen a nut on the #3 hot bin auger bracket slipped off causing the employee to catch his finger on the edge of the bracket and cut it open. The cut on the employee's left index finger required 6 sutures.

Employee was sealing a rail car when a wind gust blew dust into his right eye. Employee's eye was flushed at the plant but this did not relieve the pain. Employee was taken to a physician where his eye was reflushed, dialized, and a small particle was removed. Pain drops were administered to the eye and antibiotic cream was given to the employee to insert into his eye.

Employee was climbing down the ladder at the bag house door when she lost her footing, fell, and landed on the frame around the bottom surface of the bag house door. She twisted her right ankle and struck her knee. The accident resulted in an ankle sprain.

While working on a flourescent light from an extension ladder, the ladder came in contact with the light bulb and fixture causing the bulb to break. A shard of glass from the bulb cut the employee's left arm. The employee saw a physician to have the wound cleaned and bandaged.

Employee was climbing over the coupler on the train cars when he slipped and twisted his ankle on the car release lever and fell to the ground. Sprained ankle.

Employee stated that as she was sealing bags of plug mud, her right arm began to hurt just above the wrist.

Ee put his arm into the throat of the #3 mill to clean out mud and cut his left forearm on a sharp piece of metal.

EE WAS PUTTING AN 88 POUND BAG ONTO A PALLET AND CAUGHT HIS LEFT LITTLE FINGER ON BAG AND DISLOCATED.

EMPLOYEE WAS CHECKING THE SOCKS ON THE SOUTH MILL BAGHOUSE WHEN HE SLIPPED AND PUT HIS HAND DOWN TO CATCH HIMSELF. HIS FINGERS WERE THEN CAUGHT IN THE ROTARY VALVE, SEVERING THE TIP OF THE MID DLE FINGER AND CAUSING OPEN FRACTURES OF THE INDEX AND RING FINGERS.

EE WAS WASHING FACE WHEN A PIECE OF METAL ENTERED HIS EYE AND WAS LODGED.

EE HAD ACCESSED THE TOP OF THE BAG PRESS WITH A STEP LADDER TO MAKE ADJUSTMENT. THE PRESS WAS NOT DE-ENERGIZED AND WHEN THE EE WAS DESCENDING THE LADDER HIS HAND CAME IN CONTACT WITH THE BELT ON THE PRESS, RESULTING IN HIS LEFT ARM BEING DRAWN INTO THE PULLEY, TRAPPING AND BREAKING THE EE'S FOREARM.

EE WAS CUTTING SHRINK WRAP OFF OF A PALLET OF EMPTY BAGS WHEN HIS HAND SLIPPED AND WAS CUT BY THE KNIFE.

EMPLOYEE WAS RESTACKING A PALLET OF 100 LB BAGS HE LIFTED A BAG AND TURNED, TWISTING HIS LEFT KNEE.

EMPLOYEE WAS LIFTING A PALLET WHEN HE FELT A PAIN IN HIS BACK.

EMPLOYEE WAS BENT OVER A PIECE OF METAL CUTTING A HOLE IN IT WHEN HE FELT A MUSCLE SPASM IN HIS LOWER RIGHT BACK.

EE WAS CLEANING MUD OFF LOADER, BY THE DOOR, WHEN HE SLIPPED AND FELL TO THE GROUND STRIKING HIS RIGHT ELBOW AND SHOULDER.

EE WAS ASSISTING ANOTHER EE HAULING LUMBER WHEN HE STRUCK HIS HEAD ON AN OVERHANGING PIPE.

EMPLOYEE WAS LIFTING A ROLL OF SHRINK WRAP TO PUT IT ON TO THE STAND, WHEN HE FELT A PAIN IN HISBACK.

EE WAS LIFTING A 100# BAG ONTO A STACK OF BAGS ON A PALLET, WHEN HE FELT A PAIN IN HIS LEFT WRIST. IT WAS DIAGNOSED AS A FRACTURE OF HIS LEFT ULNA.

EE WA STRAIGHTENING A BAG ON PALLET WHEN ANTHER BAG CAME OFF THE BELT & STRUCK HIM ON LEFT LEG SPRAINED LEFT KNEE

WHILE GETTING INTO RAIL CAR TO CHECK INSIDE DOORS, EE STRAINED HIS LEFT KNEE.

EE WAS CUTTING INSULATION WITH A KNIFE WHEN HE CUT HIS RIGHT INDEX FINGER. THE REQUIRED STITCHES.

EE WAS GRINDING A BOLT WITH A DISC GRINDER, WHEN THE DISC HOOKED CAUSING THE GRINDER TO KICK. THE EE LOST CONTROL OF THE GRINDER WHICH ALLOWED THE DISC TO CONTACT THE BACKSIDE OF HIS LEFT HAND . EE UNDERWENT SURGERY 4-1-96 TO REPAIR DAMAGED TENDONS ON HIS LEFT HAND.

EE WAS THROWING BAGS WHEN HE FELT PAIN IN HIS GROIN AREA. INJURY BECAME APPARENT BY DR REVIEW ON MARCH 11 1996.
