The IP was on the surface retrieving parts for a piping project when they were stung by a bee. The bee stung them on the R Side of their neck and experienced localized swelling. They had no other symptoms. Using the IP reported to the clinic where they were given a OTC antihistamine. The IP returned directly to work.

The IP discovered a minor fire, then extinguished the fire, called an emergency, the mine was evacuated, Mine Rescue team verified the fire was out, the IP was taken from refuge chamber to the surface, cleared medically from the onsite clinic, then released to return to work with no restrictions.

The IP went to work night shift 04/01/2026 while working at around 4:30 in the morning EE was straining with a pry bar to pull a replacement hydraulic hose into place, the IP felt a pop in EE's right shoulder. Then the IP called supervision and went to the clinic to be assessed, then sent to Fairbanks hospital, MRI determined surgery was needed.

A misfire was discovered at the face, the crew was bolting to secure the area to address the misfire. While scaling with the jumbo, the bit contacted the misfire setting it off. The crew failed to follow procedure which directs the bit to be removed from the steel to allow drill water to wash out the hole, had that process failed procedure requires blast crews to reblast the hole.

The IP completed fueling of the mucker. While returning the hose to the fuel cell the hose fell striking the IPs ring finger, causing a cut requiring one stitch to close, the IP went to the local hospital verifying no bone was broken, returning to work the next day. The IP was not at their normal job role after the incident for 1 1/2 shifts verifying the finger was not broken.

Geo Tech teams inspected the same day and ordered the heading shut down after the heading was bolted. On 08/19 mining crew completed ground support. No further access was granted until 09/08 when Geo Tech team entered to inspect, the FOG was discovered, MSHA was notified.

"The accident occurred on 8/29/25, however, the EE was cleared to go back to work the same day. The EE chose to return to place of residence and got an off-site diagnosis, which was provided to the mine operator on 9/23/25."

Operator was driving down the road in a surface loader, when the operator continued to warm the hydraulic oil by raising and lowering the boom and curling the bucket. While doing this the cold hydraulics responded slowly and the bucket kept dumping which drove the teeth of the bucket into the road causing the loader to come to a complete stop. Operator was wearing a seatbelt.

On night shift of September 16th, a NSMS production loader operator was remote mucking the final firing of the BD 1184 100 P1 stope. While retreating with a full bucket, the stope brow failed back to the intersection, partially burying the loader. No personnel were in the vicinity of stope at the time. 800-1000 tons of material fell causing damage to front of mucker.

Employee was changing out G.E.T and was informed to use a steel hammer to remove then a copper hammer to reinstall. EE struck G.E.T with the steel hammer and the third one EE hit had broken a shard of hardened steel and it lodged into EE's finger. Drill Helper was emptying a 10ft NQ inner tube. EE struck the inner tube with a dead blow in an attempt to free core and dislodged a small piece of core from the lifter case. The rock fell along the tube and struck the drill helper on the upper lip leaving a small laceration.

After finishing a run, green offsider was attempting to install a loading chamber. After getting the treads, offsider felt a snap in their wrist and a sharp discomfort that went to EE's elbow. The injured area immediately started to swell. Offsider stated that their pinky went numb. Driller shut down the operation, and contacted walking boss and shifter.

Believed to have occurred at the 0600 firing time; a long-hole stope firing on the X Vein, from the 695 down to the 625, a little over 500 ft away. The 625 is chained off with a No Entry sign, barricade and berm.

EE was working on new basket installation on KBT20, was welding, cutting and grinding using all PPE. EE did not notice irritation to L eye until later in evening. EE was not sure if eye was scratched or irritated. EE sent to town clinic on 7/19. 7/26 received a definitive diagnosis from Dr. that the debris was imbedded and removed easily w/instrument.

A mill employee was mixing copper sulfate within the designated hopper. When EE opened the lids and began placing the copper sulfate into the hopper, free hand was resting on the edge of the hopper when the lid fell down striking and pinching the left hand pinky finger between the lid and hopper. Operator was wearing all required PPE. ***Resulting in three sutures.
