While working on maintenance in the plant area, the EE was making a cut on metal beam and allegedly when he blew on the cut, hot cinder entered his left ear.

Our floating employee was releasing belt of conveyor number 7 when allegedly loose material fell from conveyor number 4, which was empty at the time and fell on his helmet.

The employee went to the Workman' s Compensation because he had injured his left wrist while performing his duties at the plant.

THE EMPLOYEE ALLEGES THAT HE HAS BEEN FEELING A SHARP PAIN IN HIS LEFT SHOULDER, LEFT ELBOW AND THE LOWER PART OF HIS BACK. THIS IS DUE TO A FALL HE HAD LAST MAY WHERE UPON GETTING OUT OF THE BULLDOZER, HE FELL ABRUPTLY AND HURT HIMSELF.

THE EE ALLEGES THAT HE FIRST LIFTED SOME PIPES AND WHEN HE FINISHED AND BEGAN TO WALK, HE FELL BECAUSE HE WAS IN A STRONG PAIN, HAVING HURST HIS BACK.

THE EE ALLEDGES THAT WHILE HE WAS CHANGING THE LARGE DRILL, THE COUPLING FELL, HURTING HIS FOREHEAD. HE FELT A STRONG PAIN AND DIZZINESS.

THE EMPLOYEE ALLEDGES THAT WHILE HE WAS POLISHING THE CLARIFIER TANK THE POLISHING MACHINE BOUNCED BACK AND SCRAPED HIS NECK.

THE EMPLOYEE ALLEDGES THAT WHILE HE WAS REMOVING A STONE STUCK IN THE ROLLER OF THE CONVEYOR BELT, THE STONE FLEW OUT HITTING THE LEFT SIDE OF HIS FACE.

THE EE WAS REMOVING A PIN FROM AN HYDRAULIC CYLINDER IN A TRUCK WITH A HAMMER AND IT FELL ON HIS LEFT THUMB.

THE EMPLOYEE ALLEGES THAT THEY WERE INSTALLING A CONVEYOR BELT, AND WHEN THEY WERE TIGHTENING WITH A WRENCH, IT FELL ON HIS SECURITY GLASSES AND SCRATCHED HIS LEFT EYE BROW.

THE EE ALLGES THAT HE WAS GIVING MAINTENANCE IN THE AREA OF THE CONVEYOR #4 WHEN A ROCK FELL ON HIS RIGHT HAND.

THE EE WAS PRYING A BOULDER ROCK THAT WAS STUCK INSIDE THE HOPPER. WHEN HE WAS DOING THIS HE SLIPPED AND THE BOULDER FELL ON HIS LEFT HAND. HIS CO-WORKER HEROICALLY MOVED THE BOULDER ENOUGH TO REMOVE THE HAND.

THE EE WAS CLEANING THE PLANT WITH A WATER HOSE. HIS LEFT FOOT GOT CAUGHT INTO A STEEL BEAM. HE SLIPPED AND FELL DOWN OVER HIS LEFT FOOT.
