EE fell from the top of the engine, sustaining a nasal fracture and a laceration to EE's forehead.

During the operation of a track drill, one of the drill bits was positioned outside the storage channel. The operator requested EE's coworker to activate the drill changer lever while EE manually held the bit. Upon activation, the operator's right thumb was caught between two drill bits, resulting in an injury.

The worker did not maintain a safe distance from the operating crusher, exposing self to flying rocks, which have contributed to the incident.

The affected worker began workday at 6:30 AM. While loosening a screw on the machine, EE lost control of the tool EE was using, causing EE to hit forehead, specifically on the left side, at the level of the eyebrow. As a result, EE sustained trauma and a wound that required a three-stitch suture.

The accident occurred while the miner was attempting to adjust a piece on a forklift. The piece rolled over, causing minor injury to fingers #2 and #3 on EE's right hand. Finger #3 sustained a small amount of bleeding, with no significant impairment reported.

At the time of the incident, the miner was preparing to replace a wiper on a conveyor. To perform this task, they passed under a different conveyor, unfortunately, they were not wearing their protective helmet. When they lifted their head before passing through, they struck it, causing a laceration that required (7) stitches.

While operating the loader, the miner began experiencing lower back pain when attempting to dismount from the equipment.

Employee was operating the hydraulic hammer to break a rock in the primary crusher and a hydraulic hose failed, being hit with the hydraulic fluid in the chest and splashed in the left eye.

The miner alleges that EE was pumping diesel into equipment and when EE got off a wall EE fell on a stone and sprained EE's ankle.

Miner was operating a loader, and at the time miner wanted to get off the loader the operator started to suffer lower back pain.

After installing a screen cloth, the miner felt back pain illness.

The truck operator was driving from the material extraction point to the crushing plant when apparently lost the vehicle control and impacted the berm and as consequence overturned.

Miner was replacing a conveyor cleaner when EE suddenly raised EE's head and bang into the structure.

When the miner removed a piece of metal from the magnet, the magnet immediately pulled back the metal simultaneously and pricked the thumb of the right hand.

The employee was closing the main entrance gate when it fell and impacted employee's left leg and bruised employee's head.

The Excavator bucket failed and hit the hammer the welder had on his hand and the hammer impacted him.

The miner was pulling the welding cables to the vibrator when the positive and negative wire joined and burned his left hand.

An independent truck driver fell to the ground at the office parking lot. He die from natural cause (Heart Attack). The personnel present at the moment of the incident did not believe this dead was reportable. We called the San Juan District office to ensure we were in compliance with the rules and regulations.

A metal particular was embedded in the left eye, miner was washing his face when he noticed that something entered his eye.

Driving the truck and the seat system failed causing injury to the employees back.

The employee injured his left hand with the angle grinder(polisher), due to the fact that he loos not using the proper hand gear(glove).

Melting screws, a spark occurred and insert into the left ear.

He was mounting the jaw die of the jaw crusher, they were set with a wooden piece, opened the jaw crusher to align the screws, apparently one of the wooden pieces fell causing the jaw die to fall forward and wedge his right foot.

Closing off the ladder, removed the bolts from the stairs it rolled down impacting the employees left foot.

Was working to take out a hydraulic piston bottle with air, the fitting broke hitting him in his middle, left hand finger, and cutting him.

He slipped going up the stairs to the truck and fell. He injured his lower back and his left ankle.

THE LEFT REAR TIRE WENT FLAT. IT WAS REPAIRED AND INSTALLED. AFTERWARD THIS TIRE NUTS BECAME LOOSE AND THE TRACTOR LOST THE WHEEL MAKING THE TRACTOR TO FALL.

EE was trying to position the bulldozer blade vertically with a loader. when the loader went back, the blade fell down and he was thrown out of the loader bucket and fell from an altitude of 10 ft.

WHILE EE WAS WORKING ON A CONCRETE FOUNDATION BENEATH THE SIDE OF A CONVEYOR, THERE WAS A POWER FAILURE. WHEN THE POWER CAME BACK, THE CONVEYOR STARTED TO OPERATE, MAKING SOME ROCKS FALL DOWN, HITTING EE ON HIS BACK.

WHILE THE MINER WAS DRIVING A CAT 769C (DUMP TRUCK) LOADED WITH STONES UP A HILL THE TRUCK STARTED TO LOSE POWER AND HE STOPPED, THEN THE THE DRIVER TRIED AGAIN TO CONTINUE, BUT THE ENGINE OF THE TRUCK STALLED. HE TRIED TO BACK IT UP AGAINST THE EMBANKMENT AND WHILE DOING SO, HE TURNED OVER TO THE SIDE.

THE EE WAS USING A HYDRAULIC JACK TO OPEN THE SIDE BOARD OF THE VIBRATING SCREEN, TO INSTALL A DECK FRAME; WHILE UNDER HYDRAULIC PRESSURE THEY "BANGED" THE SIDE OF THE FRAME WITH A SLEDGE HAM MER. THE IMPACT CAUSED THE HYDRAULIC JACK TO JUMP HITTING THE PIPE HANDLE, CUTTING THE FINGER OF THE EE.

EMPLOYEE WAS TRYING TO INSTALL A TENSION SPRING ON THE EXCAVATOR EX-1-002, THE SPRING BOUNCED CAUSING INJURIES ON ONE OF HIS FINGERS AND ON A TOOTH.

EE WAS DRIVING CO TRUCK WITHOUT PERMISSION. THIS TRUCK HAD TRANSMISSION PROBLEMS AND WAS STOPPED& OUT OF SERVICE IN OUR FACILITY. SUPERVISORS & MECHANICS ADVISED HIM NOT TO DRIVE IT UNTIL THEY FIXED IT. HE DIDN'T CARE AND USED IT CAUSING A FIRE ON THE VEHICLE. WHEN EE TRIED TO COMBAT THE FIRE, STEAM AFFECTED HIS FACE PRODUCING IRRITATION. THE CO HAS SUSPENDED THE EE FOR 2 WEEKS.

EE WAS CHANGING JAW DIES AND ADJUSTING SCREW THE MULTIPLIER CAUGHT PART OF HIS MIDDLE FINGER ON RIGHT HAND CAUSING BLEEDING.

EE COMPLAIN ABOUT A PAIN IN HIS BACK.

THE EE WAS WORKING ON THE HYDRAULIC SYSTEM OF A LOADER WHEN HE LOOSENED UP A UNIT, HE LOST HIS BALANCE FALLING ON HIS BACK HAVING A KNEE INJURY.

EE WAS DRILLING A HOLE INTO METAL TIP OF BATTERY CABLE TO MAKE THE HOLE BIGGER. WHEN DOING SO THE GIMLET GOT STUCK AND THE METAL TIP STARTED TO SPIN. EE WAS HOLDING DOWN THE TIP WITH HIS FINGE R AND IT GOT HIT BY THE TIP AS IT SPUN.

HE WAS STANDING ON THE D-9 TRACKS BECAUSE HE WAS INSTALLING A SCREW ON ONE OF THE DOORS. WHEN HE MOVED HIS LEFT FOOT SLIPPED, & HIS LEG WAS NOW CAUGHT BETWEEN THE TRACKS & THE BLADES SUPPORT. ALL THE BODY WEIGHT WAS ON HIS LEFT LEG & THIS WAS ON HIS LEFT LEG & THIS WAS THE CAUSE OF THE INJURY.

WHILE OPERATING THE TRACK DRILL STARTED TO RAIN HEAVILY, SO HE DECIDED TO MOVE FROM THE SITE GOING BACKWARDS. NOT NOTICING THAT THE RIGHT SIDE OF THE TRACK DRILL WAS OFF THE TRAIL CAUSING THE TACK DRILL TO ROLL OVER.

WHILE PAINTING A HOPPER A FALLING ROCK ABOUT 1 1/2" FROM A CONVEYER HIT HIM AT THE WRIST. THE WORKER HAD A PREVIOUS FRACTURE AT THAT WRIST.

WHILE WORKING WITH A BLASTER, HE FELT A SHARP PAIN IN HIS RIGHT ARM WHICH CAUSED HIM TO STOP WORKING. THE EMPLOYEE COMPLAINED OF PAIN IN HIS RIGHT ARM APPROX ONE MONTH BEFORE THIS ACCIDENT.

THE OPERATOR WAS PREPARING TO MOUNT INTO THE CABOF THE TRACK DRILL AND A STRONG WIND BLEW THE DOOR, PINCHING HIS FINGER AND PRODUCING A CUT.

THE WELDER WAS DISASSEMBLING SOME ROLLERS ON A CONVEYOR TO BE REPLACED IN AN AREA MORE OR LESS 4 FEET IN HEIGHT. HE DIDN'T KNOW ASSISTANT WELDER WAS THERE WHEN HE DROPPED/THREW THE ROLLER HITT ING THE ASSISTANT WELDER.

THERE WAS NO ELECTRICITY AND THE EE TOOK THE BENCH GRINDER AND PLACED IT ON THE BUMPER OF THE WELDING TRUCK NOT SECURING IT. THE BENCH GRINDER FELL AND HE TRIED TO HOLD IT WHILE RUNNING.

THE EMPLOYEE WAS CARRYING A DRINKING WATER COOLER TO PLACE IT IN THE PICK UP WHILE DOING SO HE LOST HIS BALANCE.

EE WAS GETTING OFF HIS TRUCK AND HIS RING FOT CAUGHT IN THE HANDLE OF HIS DOOR.

EE WAS REMOVING A STONE FROM THE FEEDER WHEN THE DRIVER RAISE THE DUMP WITHOUT NOTICING THE EE. THE EE JUMP AND HURT HIS BACK.

THE WELDER REMOVED PART OF THE FLOOR OF A RAMP MINUTES LATER, HE FORGOT HE HAD REMOVED IT AND STEP A LEG IN THE HOLE, CUTTING HIS SHINBONE

THE CRUSHER GOT PLUG WITH A ROCK. THE EMPLOYEE TRYING TO UNPLUG IT WITHOUT USING A CRANE BAR GOT HIT BY THE ROCK.

WHILE PULLING OFF A CABLE THE EE SLIP AND FELL DOWN OVER HIS RIGHT KNEE.

THESUPERVISOR FROM INTERAMERICAN PROTECTION AGENCY, THE CO WE HIRED TO DO THE SECURITY, WENT TO CANTERA SAN ANOTNIO AROUND 1250 PM AND FOUND THE GUARD IN THE FRONT SEAT OF HIS CAR DEATH. THE G UARD HAD A GUN SHOT ON HIS BACK. ALSO THE SUPVERVISOR FOUND 9MM BULLET S CARTRIDGE. THE GUARD S GUN WAS STOLEN.

WHILE COMING DOWN THE STAIRS OF SCREEN PLATFORM THE MAN SLIPPED AND FELL DOWN.

OUTSIDE TRUCKER LOADER "POLIVILLO" ONLY FROM THE BIN, BECAUSE THE CRUSHED ROCK'S BIN IS TO DIFFICULT. BEING LUNCH TIME, HE HAD A TICKET ISSUED FOR WAITING A LOADERHE WENT INTO THE BIN, BECAU SE OF THE HEIGHT OF THE TRUCK HE RUBBED AGAINST THE CRUSHED STONE.

EMPLOYEE WAS INSTALLING A TRANSMISSION TO A TRUCK WHEN IT FELL AND HIT HIS LEFT ANKLE.

WHILE PREPARING TO ENLARGE A FLEXCO HOLE IN A WIPER, INSTEAD OF USING A FLEXCO PUNCHER TOOL, HE PROCEEDED TO HAMMER A CHISEL INTO THE BELT AND A PIECE OF METAL FLEW AND INCRUSTED ON HIS LEFT H AND.

WHILE EMPLOYEE WAS WELDING A PARTICLE FELL ON HIS RIGHT EYE. THE EMPLOYEE WAS WEARING HIS WELDING HERMET.

WHEN EMPLOYEE PUT A PIECE OF METAL ON THE BACK OF HIS TRUCK, HE WAS CUT BY IT ON HIS LEFT HAND.

WHILE EMP WAS TAKING OFF THE MOTORS OIL PAN IT FELL AND HIT HIM IN THE HEAD BECAUSE HE TOOK OUT ALL THE SCREWS AT ONE TIME.

EMPLOYEE WAS OPENING LOADER'S RADIATOR CAP TO POUR WATER WHEN IT EXPLODED AND BURNED HIS LEFT HAND.

EE WAS GETTING OFF FROM THE FRONT SEAT OF A PICK UP. SUDDENLY NOTICED THAT HIS KNEE COLLAPSED. HE WAS ORDERED BY THE DR TO REST AT HOME.

WHILE EMPLOYEE WAS MIXING LIME WITH WATER TO MAKE SOME PAINT FOR THE ROCKS, LIME FELL ON HIS EYES. WHEN HE STARTED ON THE JOB, HE WAS HANDED A PAIR OF GLASSES AS PART OF THE SECURITY EQUIPMENT SUPPLIED BY THE COMPANY.

CRUSHER HEAD WAS PLACE IN TOP OF TWO 55 GALS. DRUMS; THIS HAS BEEN OUR STANDARD PROCEDURE FOR THE PAST 35 YEARS; THE DRUMS COLLIDE AND AS A RESULT OUR EMPLOYEE WAS INJURED.

WHILE WELDING, A PIECE OF HOT IRON FELL INSIDE HIS BOOT.

THE EMPLOYEE WITH HIS HELPER WAS LIFTING THE TRUCK TO REMOVE THE LEAF SPRING. THEY DID NOT PALCE THE JACKING BLOCK ON THE CORRECT PLACE, CAUSING THE TRUCK TO SLIP AND PITCHING THE LITTLE FINGE R OF THE HELPER.

THE EMPLOYEE WAS RELOCATING AN AIR HOSE AND WENT TO PICK IT UP THE HOSE BURSTED WITH THE AIR PRESSURE--SOME ROCK PARTICLE HIT THE LEFT EYE.

THE AREA WAS WET AND SLIP FALLING ON LEFT SHOULDER.

HE WAS HAMMERING IN A BUCKETS TEETH WHEN HE SMASHED HIS FINGER

REPAIRING STRAP ON CONVEYOR,IT BROKE CAUSING HIM TO LOSE BALANCE HIT RIGHT EAR ON CHANNEL STEEL THAT WAS ON HIS RIGHT.
