Employee bent over to pickup a piece of metal off the ground. When EE grabbed the metal, EE cut right middle finger on the sharp edge. While working on the 777 haul truck box.

Lifting side control cabinet door. Door slipped and right wrist twisted. Employee felt tear in forearm and upper arm, followed by a burning sensation.
Employee was welding an I-Beam and the ladder fell out from under them. Employee fell on their left side onto a conduit pipe.

Employee was moving a pedestal for a conveyor. Pedestal was set in place, employee turned and fell on steel chute that was laying behind employee.
Employee was under sump cleaning debris and a piece of debris lodged in EE's right eye.

Employee was placing a pipe at the breaker - Rubber flap caught on pipe flipping up causing debri to enter eyes
Employee was repairing skirting material on the gun loch crusher feed hopper. He twisted and felt a muscle pull across his back.
Employee was pulling on a come along chain to take up slack, and hook slipped off beam. Employee received a laceration on his nose and bruising to forehead.
Employee was lifting a hydraulic filter housing to install on truck. He felt a straining on his upper left leg close to the hip area.
Employee was grinding a piece of metal and a particles of metal got into this eye
While walking to truck at the end of the day, slipped on a patch of ice which was covered by fresh snow, falling backwards and landing on left elbow and shoulder.
Employee was scraping a coal chute clean at the end of the shift. He hit a frozen spot of material causing scraper to stop abruptly, jarring his back and pulling muscles. Employee stated that "it felt like a bunch of needles being stuck in."

Employee was burning impack plate off truck body with torch. Tip flew off causing torch to his employee's upper lip
Employee was swinging a sledge hammer trying to remove a large pin on a DeMag.
Employee was lifting a 100 pound block of wood to put under a loader. Employees belly button popped outward while lifting.
EE was removing rust from a vibrator hose base and debris flew into his left eye.
Employee was cutting a wear plate on a truck with a torch. Plug weld in plate broke hitting torch. Knocked torch out of his hand and hit his right thumb and knuckle causing sprain.

Employee was drilling rivet and his hand slipped puncturing his left index finger.

EE heading up hill-unknown to him the drive coupler tore & truck stopped. Another EE was coming down hill & on CB told him there was a place behind him to drift off the road. Took his foot off brake, truck started to coast backward. Put foot on brake, no brake - no air. Could not steer truck. Panicked & jumped from truck 10' before truck came to rest against berm.
