Employee was inspecting a Cobra water saw at the end of the day. EE was rotating the drive belt while listening for any obstructions or part failures and EE left EE's hand on the belt and it was pinched between the belt and the pully. This resulted in 5 stiches on the ring finger, it fractured on the tip and the tips fractured on the middle and pointer fingers as well.

Normal day, normal conditions and was working at assigned station.

Stone became stuck on roller table for the hydra splitter. The employee had the skid steer lift the rock from one edge. The skid steer lifted one edge and stopped moving. The employee then pushed on the rock causing it to fall on EE's wrist.
Employee was not aware of surroundings and rested hand on a moving conveyor. A finger was pulled under a guard plate by the conveyor.
Employee showed up late. Was not fully aware of environment. Employee reached to grab a rock off of conveyor as another was coming down onto conveyor and it smashed their finger.

EE was repairing a saw when EE's hand was caught by the belt on it.
EE was making a pallet of stone when a rock fell onto EE's finger. The others were stacking to quickly which led to this.

When they were cooking lunch the hose on propane tank broke/came off & began spewing flames. EE went to turn off the gas flow to keep shop from catching fire & burned hand.

EE was working on the chopper that day, when EE smashed thumb. EE reported it and then went to a pharmacy. The pharmacist instructed EE to apply ice and EE would be fine. On 8/17 EE's thumb was still bothering EE and EE was instructed to go to the Dr where it was discovered EE had a fracture.
Our mechanic was working in the shop on truck FL 104. EE was underneath changing the airbags when one of them popped and caused the truck to come down on thumb and flattening it. EE was taken to the Dr but was back to work the next day.

The employee was operating a skid steer and had brought the forks up into the air when they became loose and swung down fracturing his leg. The forks had not been locked, they were thought to be an automatic lock but was actually a manual lock.
EE was working at a chopper pushing rock through when he became caught in the machine, resulting in his finger being smashed.
Worker was wrapping a pallet of stone in chicken wire and was backed into with a John Deere 624k wheel loader (back-end of loader, not wheels) and fell forward onto/next to the pallet he was working on causing an injury to his hip. Initially the injury was diagnosed as a fractured pelvis and lower back injury. Conditions were clear at the time with a temp. of approx.45 degrees.

Dropped a stone on left foot while working on chopper
Left thumb caught in/under machinery.
Moving stone that was cut to 2 1/4" thick by 2 feet by 4 feet with several in a row when one shifted and caught the very tip of his finger and fractured the middle finger on his right hand. The weather was clear and did not contribute. No machinery was in use - strictly a pinch between 2 stones

Mechanic repaired a dump truck and was attempting to level rock in the dump truck and suffered a fall breaking his pelvis.
