Arcosa Brooklyn Plant #1683 Metal/Non-Metal

The EE was desending a ladder on the side of the cooler above the cooler outlet as another EE was removing material from the cooler outlet below in a skid steer. as the injured EE was on the ladder the EE in the skid steer pushed hot material into water creating steam that rose to the level of the ladder causing burns to the injured employees arms.

Dislodging material with metal bar, left hand slipped coming in contact with frame. Lacerated left ring finger, stitches required.

Employees were attempting to dislodge material from crusher when a piece of shale fell, from the feed apron, onto employee's hand smashing thumb.

Employee was moving a sheet of tin to a storage area when a finger pinched between the tin and a vertical support pipe.

Employee was inspecting kiln for potential repairs to be made, while walking in area employee was paying more attention to footing, not noticing overhead obstruction. Employee struck 'material lifter' with hard hat, knocking it off and shoving safety glasses into facial/forehead region and breaking the skin with glasses.

While cleaning a buildup of material in a receiving chute, EE smashed left little finger between the hand tool and chute.

DURING PRE-START UP INSPECTION OF SECO BOTTOM SHAKER SCREEN, EMPLOYEE WAS ON DECK CONDUCTING INSPECTION. THE EMPLOYEE LOST FOOTING DUE TO DAMPNESS OF SCREENS, RESULTING IN A FALL, WHICH INJURED THE EMPLOYEES RIGHT KNEE.

Employee was cleaning beneath a conveyor belt when something fell onto EE's head.

Employee was trying to move a troughing roller on a conveyor, the employee's hand got pinched between the roller and conveyor frame.

Employee was shoveling around shale end area. While being at home employee started to feel pain around the shoulder area.

Employee was cleaning the oil house when employee slip hurting left ankle.

Employee was stepping onto ground level from the lime silos and twisted ankle.

A hydraulic hose on the excavator burst and broke the side window of the machine. The operator had cuts on his face from pieces of glass.

Employee was walking on uneven ground and twisted his right ankle.

A Buchta truck driver was cleaning out the bed of his semi. He lost feeling in his left arm and fell. He had pain in his back and left side.

Employee was picking up a fuel can and twisted wrong, causing an unpleasant pain in his back.

The employee was working on a conveyor near a stockpile, He walked into a piece of steel he had in the JLG basket and cut his lower lip.

The employee was doing inventory on material piles. He lost his footing on the loose material and twisted his knee. He suffered severe pain and was transported to the local hospital.

Employee was poking dust out of a hole in a dust hopper and material/debris fell into his eyes. There was no ill effect until the next day. Employee went to the doctor and had debris removed.

Employee left a hand written note saying he hurt his knee climbing on stiffleg crane bucket to grease it. The employee stated the next day that it was an existing old injury and that he must of aggravated it. He was in the process of having the knee looked at with his doctors before the work incident.

Subject was working on Northwest 180D shovel bucket when he slipped and fell across a block of wood on his back

Our customers contract hauler slipped and fell while climbing on his trailer to remove tarp bows. He fell onto a bracket on the side of the trailer cutting his leg at the thigh area. The cut was about 3in. long X 0.75in. deep. 911 was called and the victim was taken to emergency room. The victim was an employee for Go-Ko trucking federal I.D. #35-1824252.

Employee was helping install a 1200 foot boom cable. We needed about another 6 feet and we decided to pull it by hand and I must have strained my back or pulled a muscle.

Employee was trying to pry a clinker out of #5 kiln cooler when he felt a pull in his lower abdominal area. He didn't say anything about it. Employee went to his personal doctor on Tuesday 7/6/10 but said nothing to any supervision until 6:30pm on 7/9/10.

Employee was welding on coal plant structure and when he started to chip off the slag, a piece went under his welding helmet and under his safety glasses and into his left eye.

Subject was moving scrap metal pieces looking for piece of metal big enough to repair blackjack gear guard. When subject picked up piece of metal he felt sharp pain in right side groin area.

Subject bent over to pick up steel and had muscle spasam in lower back.

Screenplant crew was changing the closing cable on stiffleg. Employee was standing on the stiffleg bucket guiding the cable through the sheave when it flipped out and hit him in the face.

Employee was checking the heater in the #2 Euclid haul truck to see if it was blowing out heat. The employee's foot slipped and to catch himself he grabbed the heater unit with his hand and his finger went into the rotating blower fan and cut his right ring finger.

Employee was disconnecting frozen water lines from stockpile. When he was coming back down the face of the stockpile the employee slipped and slid approxmately 20 feet to the bottom. He called the office with his cell phone to tell that he couldn"t get up he had hurt his back.

Subject was working on the #4 kiln scrubber when he fell into a corner of the ID fan striking his head on a piece of angle iron.

EE was running the dozer and stopped to go to the restroom. When he returned there were some rocks laying next to the dozer and EE tried to move them and strained his back.

Employee was going to the pond pumps to restart after power outage. He drove the Volvo loader out to where the pumps were located and when he got off of the loader he got a sharp pain in his groin area. When he returned to the bathhouse he notified the plant manager of what happened and the manager sent a co-worker to take him to the emergency room.

Subject was getting off of Pettibone crane and struck right hand on machine outrigger.

Subject was welding on the stiffleg bucket when a piece of slag hit him in the eye while he was wire brushing it off. Plant supervisor was notified by telephone at approxmately 8:30A.M. on Saturday 5/31/08.

Individual was climbing up steps of #7 haul truck and pulled something in his shoulder.

Subject was climbing up ladder on 988B front end loader when he slipped and fell off loader. Subject tried to break his fall and hurt his hand when he hit the ground.

Subject was restringing the stiffleg bucket with new cable. While doing this he pulled something in his abdomen.

Employee was changing a screen on the Seco shaker. The catwalk on which he was standing collapsed falling through causing injury to his right hand and arm, head, back and ribs.

Subject was doing service on back pier of #4 kiln. When he came down stairs after servicing back pier he slipped down stairs hurting back.

Subject was working on stiffleg crane when a piece of steel fell from above striking him in the right knee

Subject was carrying a piece of steel down a stairway and fell mashing his right hand middle finger between the piece of steel and stair step.

A Hinkle truck driver delivering coal to this facility was weighing in at office. When he got out of his truck, he slipped off truck and fell on sidewalk.

Sparks from torch while working on Northwest shovel must have gotten into welding truck seat setting it on fire. When employee tried to open the door he lost his balance getting two fingers caught in door handle cutting right middle finger.

Subject was operating stiffleg crane. Started to open window, hand slipped and hit window breaking the window cutting hand.

on aug 13 06 around 10:50pm i was out getting a sample of material to check weights, as i got sample i turned to head back in on the burner floor. when i started to walk back in something caught around my foot and caused me to trip. when i fell i tried to catch myself and hurt my wrist.

had a wrench on the bolt, pipe on the wrench while pulling on the pipe and wrench, wrench slipped off, lost balance and sprained knee.

was trying to take a part off an old machine and reuse it when he struck his hand between thumb and index finger with a hammer causing laceration requiring two stiches.

employee was climbing down from loader, foot slipped off ladder rung, he grabbed hand rail and twisted back

Employee was cutting a piece of chain with a cutting torch when a particle of rust went under his safety glasses into his left eye.

EMPLOYEE WAS CLIMBING STEPS WITH MUDDY FEET. FOOT SLIPPED ON STEP & HE GRABBED FOR HANDRAIL. THE HANDRAIL GAVE WAY CAUSING EE TO FALL TO THE GROUND. THESE STEPS & HANDRAIL WERE UNDER CONSTRUCTION & EE WAS AWARE OF THIS.

He was blackjacking gears on crusher when the gears grabbed the grease paddle causing the paddle to kick back and smack the top of his right hand.

Employee holding a pin to drive another one out. Piece of metal flew off shaft after being struck by hammer. Metal piece struck employee in upper left cheek, cutting cheek.

EMPLOYEE WAS DOING REPAIR WORK ON EUCLID TRUCK THAT WAS PARKED BEHIND THE OFFICE. HE WAS STANDING ON RAIL OF TRUCK UNLOCKING THE HOOK OF BOOM CABLE OF PETTIBONE CRANE EMPLOYEE LOST BALANCE AND TURNED TO CATCH HIMSELF OR REACH FOR HOOK AGAIN. HE COULD NOT SUFFERED MULTIPLE FRACTURES OF RIGHT LEG.

EMPLOYEE WAS REPAIRING DOZER HE PICKED UP A PRY BAR AND PLACED IT INTO DOZER TRACK EMPLOYEE STEPPED BACKWARD AND HURT HIS BACK

EMPLOYEE WAS INSTALLING DRIVE BELTS ON SCREENPLANT. THE LADDER HE WAS STANDING ON SLIPPED OUT FROM UNDER HIM AND HE FELL AND BUMPED HIS RIGHT LEG.

EMP WAS CLEANING UP AREA UNDER #1 KILN HE BENT OVER TO PICK UP A PIECE OF PIPE AND BEFORE HE TOUCHED THE PIPE HE FELT A STAB OF PAIN IN HIS LOWER BACK
