Employee was removing nut with a wrench on the classifier at the plant. EE said heard a "pop" in the left wrist.

Employee was at sand & gravel plant and went to pull lever, slipped, and fell. EE put out hand to catch self and the left hand hit a few crushed #2 gravel on deck and cut hand.

Employee was moving steel track about 70 inches long

Employee was breaking up large gravel with sledge hammer and chip flew off and cut his finger.

Employee was grinding metal and piece of metal went into eye. Employee was wearing safety goggles.

He was stepping down off a tractor to refuel and he slipped off the step. He landed on his back on the concrete pad and hit a piece of steel on the pad.

Employee is a mechanic who was assembling a motor in a dozer. He was climbing up and down on stepladder carrying parts and thinks he strained his back and hips.

Employee was raking out gravel from underneath the crusher and one stone came out and hit him in the forehead. He was wearing a hard hat and safety glasses.

Employee was working on crusher deck which is 3 feet off ground; he went to grab a bar, slipped and landed on the ground on his feet. He then stumbled backwards and fell on a piece of steel on the ground.

Employee states he wears full hearing protection and equipment has tested ok for noise levels. He had to be retested due to ear wax build up, which he has had problems all year long with. He is part-time and on off time he farms and cuts wood. We cannot see any work related loss from our investigation.

PINCHED FINGERS BETWEEN FORKS ON FORKLIFT TRUCK AND BEAM.

HE WAS WELDING ALL DAY AND EYES BOTHERED HIM AFTER WORK. HE WAS WEARING SHIELD.

WHILE WELDING RECEIVED FLASH BURN ON ARM.

EE & COWORKER WERE REMOVING ROCKS FROM CRUSHER (SHUT DOWN). COWORKER PICKED UP ROCK, THOUGHT HE WAS LOSING HIS BALANCE, DROPPED ROCK ONTO EE'S THUMB.

EE HAD BEEN LIFTING SEVERAL DIFFERENT ITEMS ALL DAY LONG--DOES NOT KNOW WHICH LIFT STRAINED HIS BACK.

EMP WAS HOOKING UP HOIST ON DUMP TRUCK. WHILE PUTTING PIN IN PLACE, HE CAUGHT HIS FINGER BETWEEN PIN AND RETAINER ON BOX.

HE WAS PREPARING AREA ON PLANT TO INSTALL NEW CRUSHER. EE WAS REMOVING CHALK LINE FROM SCALPER WHEN HE REACHED FOR A SCREEN DECKING WHICH WAS UNBOLTED ONLY MOMENTS BEFORE. THE DECKING MOVED AN D HE FELL.

WALKING ON GROUND NEAR PLANT, SLIPPED ON ICE AND FELL ON ELBOW.

EMPLOYEE STANDING ON RUNNING BOARD TO CHECK LOAD. HE TURNED AND HIT LIP ON EDGE OF DOOR.

EMPLOYEE WAS OPERATING BACKHOE WHICH HE REGULARLY DOES WHEN HE STARTED HAVING BACK PAINS BACK PAIN CAN NOT BE ATTRIBUTED TO LIFTING PULLING ETC UNKNOWN CAUSE.
